증례 : 내분비-대사 ; 항갑상선제 치료 중 빈혈과 혈소판감소증으로 발현된 전신성홍반성낭창

저자들은 항갑상선제 치료 중 빈혈과 혈소판감소증으로 발현된 전신성홍반성낭창을 경험하였다. 그레이브스병이 있는 39세 여자 환자가 빈혈과 혈소판감소증으로 입원하였다. 환자는 안티로이드로 치료하다 입원 한 달 전 메티마졸로 변경하였다. 가능한 여러 원인에 대해 검사하였고 환자는 신장 이상, 혈액 이상, 항인지질항체와 항핵항체 양성인 것이 발견되었다. 환자는 그레이브스병에 동반된 전신성홍반성낭창으로 진단되었고 스테로이드 치료에 좋은 반응을 보였다. We encountered a patient with systemic lupus erythematosus that manifested as anemia and thrombocytopenia during antithyroid drug treatment. A 39-year-old woman with Graves` disease was admitted with anemia and thrombocytopenia. She had been treated with propylthiouracil, but had switched to methimazole one month prior to admission. We evaluated many possible causes and found that the patient had renal disorder and hematologic disorder, and was positive for antiphospholipid and antinuclear antibodies. She was diagnosed with systemic lupus erythematosus associated with Graves` disease and was successfully treated with corticosteroids. (Korean J Med 2012;82:232-235)

컬러드 페트리 넷과 사회 관계망 마이닝을 이용한 공급사슬 프로세스 분석의 방법론

Supply chains have become more complex due to globalization, JIT production, outsourcing, single sourcing, and the focus on removing slack from supply chains. While many of these strategies have improved performance, they have also made supply chains more vulnerable to abnormal processes. This paper proposes a methodology for the analysis of supply chain processes using coloured petri-net (CPN) and social network mining. The proposed methodology consists of three phases. In the first phase, the event logs of supply chain processes are collected by using CPN model. In the second phase, the resource social network event frequency matrix and resource social network graph are obtained through social network mining on the event logs. In the third phase, the critical abnormal resource is identified and the process improvement alternatives for it are evaluated. The proposed methodology is validated using a simplified global supply chain of a consumer electronics manufacturing company in Republic of Korea.

AA amyloidosis is one of the most significant complications of rheumatoid arthritis characterized by the deposition of amyloid A (AA) in multiple organs and tissues in the body. This disorder displays variable clinical symptoms depending upon the involved organ and a diagnosis is rendered through a biopsy of the affected organ, followed by staining using Congo-red which reveals an apple-green birefringence. Fundamental disease control is critical in the treatment of AA amyloidosis. Anti-tumor necrosis factor alpha (anti TNF- ) agents are promising in inducing clinical remission by suppressing systemic inflam α - mation in AA amyloidosis. We report a case of AA amyloidosis in a 71 year old woman with rheumatoid arthritis that responded well to infliximab therapy.

책 속 등장인물 관계도 제작 애플리케이션

독서는 독자들의 자아실현과 정보를 습득하는 측면에서 아주 중요한 활동이다. 그러나 최근 들어 개인 독서량은 하락하는 추세이다. 그 원인으로는 도서의 난도와 그에 따른 이해력 부족이 있다. 등장인물이 다양하거나 관계가 복잡한 경우에 독자는 내용을 이해하는 데 어려움을 느낄 수 있다. 이러한 단점을 보완하기 위해 본 논문에서는 독자가 직접 책을 읽으면서 책 속 등장인물 관계도를 제작할 수 있는 애플리케이션을 제안한다. 본 애플리케이션의 주요 기능은 그림판 방식으로 도형을 추가 및 이동하고, 해당 도형들을 조합하여 트리 구조의 등장인물 관계도를 제작한다. 결과적으로 본 애플리케이션은 독자가 복잡한 책을 간편하게 정리함으로써 독서를 편하게 하도록 지원한다. Reading is a very important activity in terms of readers’ self-realization and information acquisition. However, in recent years, personal reading has been on the decline, and the reasons for this are the difficulty of reading and the resulting lack of understanding. When the characters are diverse or the relationship is complicated, the reader may find it difficult to understand the content. To compensate for these shortcomings, this paper proposes a system that allows readers to create relationships diagram between characters in books while reading books themselves. It provides a function to add and move shapes in a drawing board method, and it is a method of creating a tree-structured character relationship diagram by combining the figures. As a result, this application provides readers with an easy way to organize and read complex books.

Package design is a popular promoting skill in the current promoting trend. With the development of technology, the beauty industry has grown exceedingly and the beauty market is now overflowing with a variety of cosmetic products. It is hard to differ each product from another without a distinctive characteristic of it’s own. Therefore, package design has grown as one of the biggest role in promoting a product. Limited edition is a marketing strategy that cooperates with the package design, which establishes a higher brand value by emphasizing the product’s deficiency. Recently, the retro trend is being seen in most of the fields such as fashion, design and culture. The 1960s art trend can be named as a main trend in the retro trend. Therefore in this research we studied the 1960s art trend and picked up four main trends; pop-art, minimalism, futurism, and optical art. The target of the research is to specify each art trend and analyze the art trends that have been used in the limited edition package designed cosmetic products. The data of this study were collected from research papers, thesis, art books and various internet sources. 17 cosmetics brands that continuously release a limited edition product were chosen (3 local, 14 international). Each product has been categorized with one of the art trend and analyzed. The qualities of the 1960s art trend shown in the limited edition package design had similar characteristics matching the trend 1) Pop art: illustration, symbols, cartoons were used with vivid colors 2) Minimalism: minimal use of material, color, lines were used 3) Futurism: strong and geometrical shape with a use of metallic material 4) Optical art: various pattern that gives visual illusion.

증례 : 호흡기 ; 급성 섬유소성 기질화 폐렴 2예

AFOP은 임상적으로는 급성 또는 아급성으로 폐 손상의 악화 증세를 보이는 급성 간질성 폐렴의 한 형태이다. 임상양상은 미만성 폐포손상과 유사하나 기계환기의 필요성에 따라 다양한 예후가 보고되고 있다. 저자들은 최근 급성으로 진행하는 폐 손상에서 조직 검사상 AFOP가 확인된 예후가 좋지 않았던 1예와 아급성으로 진행하는 폐렴에서 조직 검사상 AFOP가 확인된 예후가 좋았던 1예를 거의 같은 시기에 경험하였기에 이를 문헌고찰과 함께 보고하는 바이다. 결론적으로 적극적인 병리 조직학적 확진을 통하여 향후 더 많은 경험을 통한 연구가 필요할 것으로 생각된다. Acute fibrinous and organizing pneumonia (AFOP) is a histological pattern consisting of intra-alveolar fibrin in the form of fibrin "balls" and organizing pneumonia, without hyaline membranes or prominent eosinophil infiltration. Some reports suggest that the clinical course and pathological findings of AFOP are different from typical findings of bronchiolitis obliterans organizing pneumonia (BOOP) or eosinophilic pneumonia (EP), and its prognosis can be better or similar to that of diffuse alveolar damage (DAD). We report two cases of pathologically demonstrated AFOP experienced recently at our institute. One fatal case revealed a rapid development of respiratory failure and the need for mechanical ventilation. Another nonfatal case revealed subacute diffuse bilateral lung infiltration without the need for mechanical ventilation. Judging from our experience, the patient who required a mechanical ventilator had a poorer prognosis than the one who did not need a mechanical ventilator. (Korean J Med 77:764-770, 2009)

Di-(2-ethylhexyl) phthalate (DEHP)를 처리한 흰쥐 호르몬 및 정소 미세구조 변화

Di-(2-ethy1hexyl) phthalate (DEHP) has been commonly used as a plasticizer in polyvinyl chloride resins for manufacturing flexible vinyl products. The toxicity of DEHP has been the topic of extensive research, especially concerning endocrine disrupting properties. This study was carried out to investigate the effects of DEHP on the ultrastructural changes of the spermatogenesis in the rat. And also, some hormones in serum were measured to evaluate the physiological effects of DEHP. The rats were orally intubated with either corn oil (experimental control) or 0.5㎎, 1.5㎎ and 4.5 ㎎ DEHP/㎏/day in 0.5㎖ of corn oil for 15 days before sacrificing and sampling. DEHP reduced the growth of the testes, inhibited spermatogenesis and altered ultrastructural features of most cell types of the rat testis depending on the dosage treated. Briefly, Differentiations of the Leydig cells, the Sertoli cells and the developing germ cells seemed to be impaired. DEHP increased the level of LH(ICSH), but lowered the level of testosterone. The detrimental effect of DEHP on spermatogenesis seems to be derived from the decreased level of testosterone by the Leydig cells, followed by the weak activity of the Sertoli cells and the germ cells, In conclusion, DEHP inhibited the spermatogenesis via lowering testosterone in the rats. The results suggest that DEHP could show properties of endocrine disrupter impairing the spermatogenesis and has effects on the ultrastructures in rat

절식에 의한 흰쥐 췌장의 기능 및 구조 변화

Ultrastructural characteristics of the pancreatic acinus and the islet of Langerhans of Sprague-Dawley rats after 4-day fasting were observed. Food-restricted rats showed a low glucose concentration of blood. On transmission electron microscopy, acinar cells had well developed rough endoplasmic reticulum, Golgi complex and zymogen granules in control groups. In case of experimental groups, ecocytosis of zymogen material into the lumen obviously decreased, distribution of zymogen granules in cytoplasm was eccentric, and swellings of rough endoplasmic reticulum and nuclear envelope were observed. And, in control groups, many capillaries were present near islet of Langerhans and insulin secreting B cells or glucagon secreting A cells were observed. Meanwhile, ultrastructural changes including the swellings of nuclear envelope and rough endoplasmic reticulum and the decrease of secretory granules of endocrine cells in experimental groups. These results suggest that food deprivation altered the ultrastructure and function of pancreas in rat.
