        흉부 병소의 세침 흡인 생검

        During the recent 3 years since April 1985 percutaneous fine needle aspiration biopsy(Fn-AB) of 174 chest lesions was performed with Westcott needle (soltted 20G or 22G thin wall needle) in the department of diagnostic radiology of Hanyang University. There were 149 lung lesions(105 malignant tumors 34 benign lesions) and 25 mediastinal lesions. Positive diagn-osis in malignant lung lesions was established in 105 cases (100%) by FNAB, Among them 98 cases (93%) were diagnosed by cytlolgic examination and 94 cases (90%) were diagnosed by histologic examination. Specific diagnoses for benign lung lesions were made in 22 cases (65%) and for mediastinal lesions in 13 cases (52%) The histlolgic specimens obtained from FNAB can make further confirmation of the result of cytology and improvement of diagnostic accuracy. 19 patients developed pneumothorax and 5 of them required treatment,.Minimal hemoptysis was found in 7 patients. FNAB of the chest lesions is prefered diagnostic method because of its safety simplicity and accuracy

        폐동맥의 원발성 악성섬유성조직구종 - 1 예 보고 -

        We recently experienced a case of primary malignant fibrous histiocytoma of the left pulmoary artery occurred in a 27-year-old man who had 5 months history of interittent hemoptysis. Chest X-ray revealed multilobulated mass density in left hilar aream and another small lobulated modular density in left upper lung field, which was intrapulmonary metastatic deposit. CT scan, pulmonary ateriography, and selective left bronchial arteriography after partial resection of the left hilar mass and left upper lobectomy revealed the mass to have the characteristic feature.

      • 염증성 관절염에서 Nitric Oxide(NO)의 역할에 관한 연구

        Nitric Oxide(NO) is a toxid, inorganic, gaseous free redical produced during the metabolism of L-Arginine by NO synthase(NOS). It has been implicated in a rapidly in a rapidly growing number of physiological and pathophysiological processes such as cytotoxic effects against mocrobes and tumor cells, blood vessel dilation and neurotransmitter, NOS exists as several isoenzymes, the major distinction being between inducible forms(iNOS) and constitutive forms(cNOS). Cells which contain the latter enzymes rapidly, but transiently, synthesize small amounts of NO. Cells expressing iNOS, in contrast, synthesize large amount of NO for an extended periods following a lag of several hours during which the enzymes are induced. Recently there is growing evidence implicating NO in immune regulation, inflammation, autoimmunity, and arthritis. This study was performed to determine the role of NO in inflammatory arthritis, especially in rheumatoid arthritis(RA). NO production was measured indirectly as nitrite, a breakdown product of NO, ① in serum and synovial fluid from patients with RA and osteoarthritis(OA) and in serum of controls ② in the supernatant of cultured synovial tissue with RA and OA. And it was investigated whether human chondrocytes and synoviocytes can synthesize NO and if so, how the production is regulated by cytokines and antirheumatic drugs. The results are as follows; 1. Serum nitrite concentrations in patients with RA and OA were higher than in controls. In both disease groups synovial fluid nitrite was higher than serum nitrite. Serum and synovial fluid nitrite concentrations in RA were higher than those in OA. However, those findings are not statistically significant. 2. Although these findings are not statistically significant, the concentration of nitrite in the supernatant of cultured synovial tissue with RA was higher than that in OA. 3. IL-1 β and TNF-a induced the biosynthesis of NO by chondrocytes and synoviocytes. While IGF-1 and TGF-β failed to provoke the production of NO. The biosynthesis of NO required an induction period of approximately 6 hours and was inhibited by L-NMMA and cycloheximide. Dexamethasone, indomethacin, gold sodium thiomalate and methotrexate had no effect on the induction of NO biosynthesis. These results suggest that NO may perform some role as an inflammatory mediator in inflammatory arthritis. Manipulating the physiologic concentration of NO appears to be clinically useful approach to the treatment of inflammatory arthritis in future.

        증거기반 실제를 위한 국내 감각통합중재 단일대상연구 메타분석

        연구목적: 국내 감각통합중재를 단일대상연구로 실시한 선행연구를 대상으로 연구참여자 특성 및 중재구성 요소에 따른 효과크기를 분석하여 감각통합중재가 증거기반 실제(Evidence Based Practice)로 자리매김하기 위한 기초자료를 마련한다. 연구방법: 감각통합중재가 국내에 활성화되기 시작한 2000년부터 2019년까지 국내 학술지에 게재된 감각통합중재 단일대상연구 24편을 분석하였다. 분석대상 연구의 일반적 특성을 분석하고, 연구의 효과크기는 연구대상자 변인 및 중재구성 변인으로 구분하여 비중복비율(PND)과 개선율차이(IRD)지표로 산출하여 분석하였다. 연구결과: 첫째, 연구대상자 변인에 따른 효과크기 분석 결과 감각통합중재는 4세 이하와 5-6세인 학령기 이전 아동 연령대에서 효과크기가 매우 높았으며, 진단명과 관련하여 자폐성장애, 중도중복장애, 감각처리에 어려움을 갖는 아동, 뇌성마비, 발달지체 등 대부분의 장애영역에서 매우 효과적인 중재로 나타났다. 둘째, 감각통합중재 구성에 따른 효과크기는 중재회기가 10회기 미만이거나, 16-20회기, 21회기 이상일 경우 매우 효과적인 중재로 나타났고, 연구설계 유형에서는 ABA 설계를 제외한 모든 설계 유형에서 효과크기가 매우 높게 나타났다. 종속변인에 따른 효과크기 PND는 행동, 구강, 감각처리, 작업수행, 운동, 학업(기초학업기술) 모두에서 효과적인 것으로 산출되었는데, 이는 국내 감각통합중재가 연구대상자의 개별화된 중재목표 달성에 효과가 있음을 의미한다. 결론: 감각통합중재 단일대상연구의 증거기반 실제 수준을 제고하기 위한 세부 요소를 검토하고 연구의 시사점과 연구의 제한점에 따른 후속연구 필요성을 논의하였다. Purpose: To provide basic data to establish sensory integration(SI) intervention as evidence-based practice, this study analyzed the effect size according to the characteristics of the participants and the components of the SI-intervention, targeting the preceding studies conducted as a single subject study in the domestic SI-intervention. Method: From 2000 to 2019, when SI-intervention began to be activated in Korea, 24 single subject studies on SI-intervention were published in domestic journals. These studies were collectively analyzed. The general characteristics of the studies were classified into characteristics of the subject and methodology of each variable. The effect size was classified into the subject and the intervention composition variables, and calculated and analyzed as Percentage of Non-overlapping Data (PND) and Improvement Rate Difference (IRD) indicators. Result: First, the SI-intervention was very highly effective in the before-school age group of children. It was found to be very effective in most areas of disability, including autism, severe disabilities, cerebral palsy, and developmental delays. Second, the effect size was found to be very effective when the intervention involved less than 10, 16-20, or more than 21 sessions. In the study design type, the effect size was very high in all designs except the ABA design. The effect size PND according to the dependent variable was calculated to be effective in behavior, oral cavity, sensory processing, work performance, exercise, and study (basic academic skills), which meant that domestic SI-intervention was effective in achieving individualized intervention goals of the children. Conclusion: Detailed factors to improve the evidence-based actual level were reviewed and the need for follow-up research according to the implications and limitations of this study was discussed.

        인삼 사포닌이 백서 장점막 $Na^+,K^+$-ATPase에 미치는 영향에 관한 연구

        We have studied the effect of ouabain, tool ginseng saponin, panax saponin C (protopanaxatriol derivative) and ginsenoside $Rb_{1}$ (protopanaxadiol derivative) on $Na^+,K^+$-ATPase and $Mg^{++}$-ATPase activities were determined by the method of Robinson and ATPase activities were determined by the method of King. The $Na^+,K^+$-ATPase activities were inhibitied by 90.1% and 51.1% respectively at the concentration of $10^{-3}M$ and $10^{-4}M$ ouabain. The results are consistent with those of Robinson. The $Na^+,K^+$-ATPase activities were increased by 14.3% and 10.0% respectively at the concentration of $10^{-4}$g/ml and $10^{-5}$g/ml total ginseng saponin. Panax saponin C obtained by the method of Han and ginsenoside $Rb_{1}$ obtained by the method of Shibata were used. The $Na^+,K^+$-ATPase activities were increased in the presence of panax saponin C and the increased activity with panax saponin C was greater than that with total ginseng saponin. On the other hand ginsenoside $Rb_{1}$ showed an inhibitory effect on $Na^+,K^+$-ATPase. Total ginseng saponin, panax saponin C and ginsenoside $Rb_{1}$ had no effect on $Mg^{++}$-ATPase. Therefore, it may be concluded that total ginseng saponin has dual effects on microsomal $Na^+,K^+$-ATPase, that is, panax saponin C exhibits stimulatory action, whereas ginsenoside $Rb_{1}$ shows inhibitory action.

        자궁근종과 자궁선근증 : 초음파와 자기공명영상 소견

        Leiomyoma and adenomyosis of the uterus are the most common gynecologic disorders in an enlarged uterus. The characteristic US and MR findings in differentiation between both lesions were prospectively evaluated in 30 lpatients. Of 30 patients, 15 were leiomyomas, 6 were adenomyosises, 8 were leiomyomas and adenomyosises, and 1 was a normal pregnancy, histoligically. The total number of leiomyom nodules were 49 while adenomyosises were 14 (9 diffuse and 5 focal). Among 49 myomas nodules, 36 were correctly diagnosed by sonography. The characteristic US findings of uterine leiomyoma were well defined nodules (36), hypoechoic peripheral rim (16), and whorl-like internal echoes(13). Forty four of the 49 myoma nodules were correctly ciagnosed by MRI. The characteristic MR findings of myoma were well defined nodules (43), peripheral low signal intensity rim on T1WI(13) and T2WI (9), and peripheral high signal intensity rim on T2WI (5). Among 14 adenomyosises, 9 were correctly diagno ed by sonography. The characteristic US findings of adenomyosis were diffuse uterine hypertrophy more than 5.5cm in AP diameter with endometrial displacement and no significant echo change in myometrium All 14 adenomyosis as were correctly diagnosed from MRI. On T2WI, adenomyosis appeared as ill defined localized or diffuse thickening of the junctional zone more than 1cm in thickness. It was our conclusion that to differentiate between leiomyoma and adenomyosis focused on should be the detection of existence of nodule in leiomyoma, the primary sign, not on the secondary indirect sign.

        우측폐의 형성 부전과 " Scimitar Sign " 을 보이나 좌심방으로 유입되는 우측 폐정맥을 보인 Scimitar Syndrome 1 예

        The scimitar syndrome is a rare, complex, variable and congenital malformation of the right lung. These anomalies consist of anomalus pulmonary venous drainage of the right lung to the IVC, hypoplasia of the right lung, hypoplastic right pulmonary artery and anomalies of the right hemidiaphragm. But we recently experienced a case of typical scimitar syndrome except pulmonary venous drainage of the right lung to the IVC. This case shows drainage not to the IVC, but to the left atrium. We present this case with the review of literature.
