        컴퓨터 디자인 기반 모노스키 버킷 사용에 따른 장애인 알파인 스키 선수의 운동학적 특성 변화 연구: 사례 연구

        The purpose of the study was to investigate the effect of CAD (Computer Aided Design) based alpine mono-ski bucket design on disabled ski athletes` kinematic characteristics. Two national team ski athletes with LW11 disabilities (Locomotion Winter Classification) category for sit ski participated in both pre and post experiment. Both of the subjects performed 3 trials of carved turn on a ski slope under two conditions. Where, subject “A” performed pre experiment with personal bucket and post experiment with the newly developed CAD based bucket whereas, Subject “B” as control subject performed both pre and post experiment with his personal bucket. For the experiment, 24 Infrared cameras were positioned on the ski slope which covered the path of the ski turn. Also, motion capture suit with reflective markers were worn by both subjects. In the result, decrement in medial/lateral displacement of COM, anterior/posterior displacement of COM, flexion/extension angle of trunk as well as velocity losing rate of COM was observed in subject “A” when using the newly developed CAD based bucket. In contrast, no larger effect on performance was observed when using personal buckets. In conclusion, the findings obtained from the study indicated effectiveness of newly developed CAD based bucket by reducing excessive movement of hip and trunk which is an important factor to perform an effective turn.

        체감형 스키 시뮬레이터 개발을 위한 플루크보겐 동작의 운동역학적 비교

        The purpose of this study was to compare pflugbogen``s biomechanical characteristics between on the ski simulator and snowed ski slope to develop interactive ski simulator. Nine ski instructors(sex: male, age: 29.6±5.4 yrs, height: 176.0±5.6 cm, body mass: 76.0±14.0 kg) belong to Korean Ski Instructors Association participated in this research. 24 Infrared cameras for snowed ski slope experiment and 13 infrared camera for ski simulator experiment were installed near by path of pflugbogen. The participants did pflugbogen on the snowed ski slope and the ski simulator both. During the experiment, the participants weared motion capture suit with infrared reflective makers on it, and plantar pressure sensors in ski boots, so that ski motion and plantar pressure data were collected together. Displacement of COG(center of gravity) movements, trunk flexion/extension angle, adduction/abduction angle, and plantar pressure data were significantly different between on the simulator and ski slope. However, percentage of time of COG movement in the phases during medial/lateral and anterior/posterior movement were not significantly different. Findings indicate that the difference between two groups occurred because the ski simulator``s drive mechanism is different from ski motion on the slope. In order to develop the ski simulator more interactively for pflugbogen, the ski simulator``s drive mechanism need to be reflected 3D motion data of pflugbogen on the slope that were purposed in this research.

        골프 스윙 시 선수와 비선수의 두부 분절 움직임 패턴 비교: 골프 헤드업에 관한 해석

        연구목적 본 연구는 골프 샷의 명언 중 하나인 'Do not head up'에 대한 운동학적 분석을 수행하고 그것을 해석하는 것에 목적이 있다. 연구방법 엘리트 골프 선수 8명과 비선수 10명의 5번 아이언 샷을 측정한 뒤, 스윙 구간 전체에서 머리 움직임 변화를 분석하였다. 스윙 구간은 어드레스(AD), 미들 오브 백스윙(MBS), 백스윙 탑(BST), 미들 오브 다운스윙(MDS), 임팩트(IMP), 미들 오브 폴로스루(MF)로 구분하였다. 적외선 기반 3차원 동작분석 시스템을 사용하였다. 머리 분절 좌우 관자놀이에 부착한 2개의 적외선 반사 마커 중심을 머리의 움직임으로 정의하였다. 골프 스윙 시 정의된 머리 분절의 3차원적 움직임을 집단 간 비교하였다. 결과 선수의 경우 머리의 움직임이 백스윙 시 우측(-y 축 방향), 하방(-z 축 방향)으로 움직임이 나타나고, 다운스윙 시 좌측(+y 축 방향), 후방(-x 축 방향), 하방(-z 축 방향) 의 방향으로 3차원 공간에서 머리 움직임이 나타났다. 비선수들은 백스윙 시 이미 머리 위치가 높아지면서 임팩트 시 머리 위치가 충분히 내려오지 못하는 패턴을 보였다. 선수들은 다운스윙 시 일관된 머리 움직임이 타나났으나, 비선수의 경우는 피험자 간 각기 다른 패턴의 머리 움직임을 보였다. 결론 골프 코칭 시 '머리를 들지 마라라는 격언으로는 부족하다. 골프 스윙 동작 시 머리 움직임의 3차원적 피드백이 제공되어야 올바른 머리 움직임을 바탕으로 골프 스윙이 이루어질 수 있을 것이다. Purpose The purpose of this study is to analyze the head movements of elite golfers and non-players during golf swings to interpret the meaning of head-ups kinematically. Method After measuring the 5th iron shot of 8 elite golfers and 10 non-players, the change in head movement was analyzed throughout the swing section. Swing sections were divided into address (AD), middle of back swing (MBS), back swing top (BST), middle of down swing (MDS), impact (IMP), and middle of through (MF). An infrared-based three-dimensional motion analysis system was used. The center of the two infrared reflective markers attached to the left and right temples of the head segment was defined as the movement of the head. The three-dimensional movements of the head segment defined during the golf swing were compared between groups. Result In the case of the eltie golfers, the movement of the head appeared in the right(-y axis direction) and downward directions(-z axis direction) during the backswing, and the movement of the head appeared in the three-dimensional space to the left(+y axis direction), post direction(-x axis direction), and downward(-z axis direction) directions during the downswing. Non-elite golfers showed a pattern in which the head position was not sufficiently lowered during impact as the head position was because of already increased haead position during the backswing. The elite golfers showed consistent head movements during downswing, but in the case of non-athletes, they showed different head movements between subjects. Conclusion When coaching golf, the adage Do not head up is not enough. In the case of a golf swing motion, three-dimensional feedback of the head movement might be provided so that the golf swing can be achieved based on the correct head movement.

        휠체어 탑승 장애인을 위한 이동형 체중측정 자동화 장치 개발

        Wheelchair users have a difficulty in managing their weight because there is lack of measure systems and even though the users have the system, it costs too much. Thus, purpose of this study is for developing the portable weight scale automatic device to help wheelchair users by monitoring their weight on the wheelchairs in real time. Portable weight scale automatic device composed by four strain gauges, and the device was connected with mobile phone so that wheelchair users were able to check their weight in real time. In order to evaluate the device performance, 10 normal subjects and 10 disabled subjects participated in this study. The subjects' truth weight and measured weight by the device are compared. The weigh difference within normal subjects was $0.75{\pm}0.80kg$, and the weigh difference within disabled subjects was $1.02{\pm}0.74kg$. Difference between the truth weigh and the measured weigh were not statistically different. The reason for this error occurred is inaccuracy of the loadcell installation and calibration. If this problems get solved, the measured indicator is expected to help wheelchair users to manage their weight.

        갑상선과 부갑상선 수술 후 발생한 오심 및 구토 연관인자들에 대한 분석

        목적: 수술 후 발생한 오심 및 구토는 갑상선과 부갑상선 절제술을 시행받은 환자들에게 비교적 흔한 문제이다. PONV는 환자, 수술, 마취등과 관련된 다양한 인자들에 의해서 영향받는다. 이에 저자들은 갑상선과 부갑상선 수술을 시행받은 환자들을 대상으로 하여 PONV 발생율과 환자특성을 조사하는 데에 그 의의를 두고 있다. 방법: 2011년 3월 1일부터 5월 20일 사이에 서울대병원에서 갑상선 및 부갑상선 수술을 시행받은 192명의 환자 중 PONV가 발생한 22명의 환자를 대상으로 분석했다. 나이와 성별, BMI, 수술 후 조직검사결과, 수술시간 및 마취시간, 수술방법 및 마취방법들을 후향적으로 분석했다. 결과: PONV는 192명의 환자중 11.5%인 22명의 환자에서 발생하였다. 단변량분석을 시행했을때에 연령, 수술시간 및 마취시간, 수술방법 그리고 수술 후 재원일수가 통계적으로 유의하게 PONV와 연관성을 보였다. 하지만 다변량분석을 시행했을때에 위의 연관인자들 중 어느 것도 통계적으로 유의한 결과를 보이지 않았다. 결론: 본 연구에서 갑상선과 부갑상선 수술을 시행받은 환자들은 상대적으로 낮은 빈도의 PONV를 보였다. 하지만, PONV는 수술후 재원일수의 상승, 출혈위험의 증가등을 유발할 수 있으므로 PONV가 발생할 위험이 높은 환자들에 대한 관심이 필요하다. Purpose Postoperative nausea and vomiting (PONV) is a common problem in patients undergoing thyroidectomy and parathyroidectomy. PONV is influenced by various factors that involve the characteristics of the patients, surgery, and anesthesia. This study aimed to evaluate the incidence of PONV in patients undergoing thyroid and parathyroid surgery and to analyze the clinicopathological characteristics. Materials and Methods Between March 1st and May 20th 2011, 22 patients with PONV were enrolled from 192 patients who underwent elective thyroid or parathyroid surgery at Seoul National University Hospital. Clinicopathologic characteristics such as age, sex, BMI, pathology, operation/anesthesia time and methods were retrospectively reviewed. Results PONV occurred in 22 patients (11.4%) of 192 patients. Age, operation/anesthesia time, operation method and postoperative hospital stay were associated with PONV in univariate analysis. However, there were no statistically significant factors in multivariate analysis. Conclusion Patients undergoing thyroid or parathyroid surgery have a relatively low risk of developing PONV. However, we should take interest to high risk patients considering that PONV is related to longer hospital stay.

      • Biomechanical Analysis of Sliding Board for Developing Augmented Reality Based Feedback Skating Simulator

        Track sports such as “Short track” and “Speed skating” are some of the winter sporting discipline dominated by South Korea which have high hopes for the upcoming 2018 Pyeongchang Olympics. Despite the winter sports being in the limelight, training of the seasonal sports are not only problematic due to accessibility but also because of financial reasons due to which, dry land training such as slide board exercise has been considered by coaches to enhance endurance, agility, balance, body composition, muscular strength, proprioception, as well as cardiorespiratory fitness. Although the exercise was initially developed for sports specific off the ice training, slide board training has been also effective for recreational purposes, conditioning, functional rehabilitation for ACL injuries as well as activities involving lateral movement. Similarly, an increased demand of sports oriented fitness equipment’s and simulators have been seen in the field of sports however, without substantial biomechanical evidence as well as interactive visual feedback of performance to the user. Hence, the purpose of the study was to provide quantitative data of skating maneuver performed on sliding board for developing augmented reality based feedback skating simulator.

        손보조하복강경부신절제술 초기경험

        Purpose: Laparoscopic adrenalectomy has been a standard method for adrenal gland tumors. On the other hand, hand-assisted laparoscopic adrenalectomy is an operative method which enables operator to use a hand to facilitate dissection and retraction. The aim of this study was to report initial experience of hand-assisted laparoscopic adrenalectomy and to evaluate potentialities for surgical application. Methods: Seven patients who underwent hand-assisted adrenalectomy from September 2010 to July 2013 were enrolled. Clinicopathological characteristics, intraoperative hemodynamic status, and parameters associated with postoperative recovery were reviewed. Surgical indications for hand-assisted laparoscopic adrenalectomy were right side adrenal tumors larger than 4 cm on preoperative computed tomography (CT) scan. Mean age was 36.9 years (range, 23 to 51 years) and there were 3 men and 4 women. Results: The mean size of adrenal tumor was 6.1±2.4 cm. Operation time was 119.4±29.7 minutes in average and mean blood loss was 142.9±109.7 mL. There were 4 patients with pheochromocytoma, one patient for each adrenocortical neoplasm, paraganglioma and lymphoma. Intraoperative hypertensive crisis were observed in 2 patients with pheochromocytoma and there was no intraoperative tachycardia crisis. Patients had first oral intake at 1.4±0.8 days in average and mean postoperative hospital stay was 3.4±1.3 days. There was no postoperative complication and mortality. The median follow-up was 12 months and there was no case with evidence of recurrence during the periods. Conclusion: In our initial experience, hand-assisted laparoscopic adrenalectomy was performed safely and effectively. Hand-assisted laparoscopic adrenalectomy might be an alternative to laparoscopic adrenalectomy for moderate sized right side adrenal tumors.

        일차성, 이차성 그리고 삼차성 부갑상선 기능 항진증 환자의 수술 후 임상결과: 서울대학교 병원에서의 14년 경험

        목적: 일차성, 이차성, 삼차성 부갑상선 기능 항진증은 각각 원인과 치료방법 그리고 임상 경과가 서로 다르지만 부갑상선 절제술이 표준치료로 알려져 있다. 본 연구에서는 각각의 부갑상선 항진증의 수술 전후의 임상적 변화를 알아보고 부갑상선 절제술의 의미를 재고 해보고자 하였다. 방법: 1996년부터 2009년까지 총 126명이 서울대학교 병원에서 부갑상선 절제술을 시행 받았으며 각각 일차성 96명, 이차성 24명, 삼차성 6명이었다. 환자들의 나이, 성별, 생화학적 검사, 수술방법, 병리학적 검사 결과를 전자 의무기록을 통해 후향적으로 분석하였다. 결과: 모든 세 군의 환자에서 혈청 칼슘, 부갑상선 호르몬, 이온화 칼슘이 수술 전보다 호전을 보였다. 이차성 환자군은 다른 두 군에 비해 수술 전, 후의 부갑상선 호르몬이 높았으며(p<0.001, p=0.036), 수술 후 지속적인 부갑상선 기능 항진증(30.4%) 및 일과성 저칼슘혈증(87.5%)도 다른 두 군에 비해 많이 발생하였다.(p< 0.001) 일과성 저칼슘혈증의 대부분(90.4%) 은 6개월 이내 회복 되었다. 이차성 환자에서 부갑상선 아전 및 전 절제술을 시행 받은 경우 수술 후 일과성 저칼슘혈증이 많이 나타났으나(71.4%), 제한적 절제술을 시행 받은 경우 지속적 부갑상선 기능항진증이 더 많이 나타났다.(50%) 결론: 부갑상선 절제술은 생화학 검사상의 호전을 위한 치료 방법으로 추천될 수 있으며, 이차성에서는 제한적 절제를 하는 경우 지속적 부갑상선 항진증의 빈도가 더 높으므로 부갑상선 아전 및 전 절제술을 시행해야 한다. Purpose: Primary hyperparathyroidism(PHPT), secondary hyperparathyroidism(SHPT) and tertiary hyperparathyroidism(THPT) are different in the cause, treatment and prognosis. However the parathyroidectomy has been an efficient treatment in all hyperparathyroidism groups. A single institution`s 14 year experience of surgical treatment was analyzed to investigate perioperative changes of clinical characteristics and reconsider the value of parathyroidectomy as the treatment option. Materials and Methods: From 1996 to 2009, 126 patients underwent parathyroidectomy at single institute and the number of patients with PHPT, SHPT and THPT were 96, 24 and 6 retrospectively. The electronic medical records of age, sex, biochemical analysis, operative method, and pathologic results were reviewed retrospectively. Results: Postoperative calcium (Ca), parathyroid hormone (PTH), ionized calcium (iCa) levels were improved definitely than preoperative Ca, PTH, iCa level in all three groups. Pre and postoperative PTH level in SHPT was higher than in PHPT and THPT(p<0.001, p=0.036) and postoperative persistent PTH increased status were more common in SHPT.(30.4%, p<0.001) Postoperative temporary hypocalcemia was more common in SHPT(87.5%, p<0.001), almost of them (90.4%) were recovered in 6 month. In SHPT group, temporary hypocalcemia were more common in subtotal or total parathyroidectomy group than in limited resection group (94.1%) but persistent iPTH increase were more common in limited resection group (50%). . Conclusion: Parathyroidectomy is highly recommended to improve biochemical laboratory findings in patients with hyperparathyroidism. And in SHPT, subtotal or total parathyroidectomy is more appropriate surgical method for reducing the high incidence of persistent hyperparathyroidism.
