한약투여 및 식이, 운동지도를 통한 소아성장치료의 임상적 관찰

Objectives : The aim of this study was to observe and analysis the effectiveness of clinical treatments on the growth of children treated with Korean herbal medicine (KHM). Methods : Forty-two children of growth retardation were evaluated from January 2007 to May 2008. All children were administered KHM with diet and exercise guidance. Height percentile and predictive height were calculated following the General Growth Statistics Result (2007), and compared those of pre-treatment with post-treatment. Results : Thirty-one of them were improved (73.8%) and eleven were not (26.2%). The mean growth rate was 7.2cm/yr for male and 6.8cm/yr for female. The mean growth percentile of male showed 7.6% improvement (p=0.003), female 4.4% respectively (p=0.036). The mean predictive height of male showed 2.2cm improvement (p=0.005), female 1.0cm respectively (p=0.007). Change of growth percentile and predictive height were significantly higher on period of prepuberty, and in small children below 50% of height percentile. About inter-relations between growth improvement and compliance of diet, exercise or medication, meaningful statistics were shown in diet control than the others (p=0.077). Diet compliance was in close connection with exercise, but others were not shown close correlations. Conclusions : These results showed that KHM showed promising results on children growth and healthy diet would be an important factor for growth of children. And it was recommended that KHM should be applied on period of prepuberty and smaller than average height.

Objectives : A rare case of dural tear ensuing after a cervical Chuna Manipulation Treatment leading to cerebrospinal fluid (CSF) leakage in the lower cervical and upper thoracic spine was found, so we report it. Methods : A 32-year-old woman presented with back and neck pain in 1 days earlier. The patient undertook a cervical Chuna Manipulation Treatment. After this maneuver, the patient complained of an orthostatic headache with nausea. The patient's headache worsened, and lying down gave the only measure of limited relief. In Brain CT and MRI study, nonspecific finding was detected. In Cistemography study, CSF leakage at lower cervical or upper thoracic area was detected. Results and Conclusions : It is supposed that this patient suffered a dural tear and CSF leakage secondarily due to a cervical Chuna Manipulation Treatment. From this case, we can understand the etiology of dural tear to some extent and consider the complication of Chuna Manipulation Treatment. In the future, more study, research and prospective trial for complications of a cervical Chuna Manipulation Treatment is needed.

Objectives: The aim of this review was to investigate studies on fire needle and to propose for the better method of studies in the future. Methods: Studies related to fire needle were searched with electric database for this study, and 10 domestic and overseas databases were included. Results: 19 clinical studies were analyzed in terms of condition, time of heating, number and frequency of treatment, number of heating, safety procedure or device and reports of adverse events. Also 3 review studies were summarized. The number of studies was increasing and the kind of diseases to treat with fire needle were various. Especially musculoskeletal diseases like ligament lesion and lumbago were noticeable. Heating-after-insertion method was recently proposed but the use of term was confusing. The report of adverse events or safety procedure was rare. Also there was an effort to develop more convenient and safer device. Conclusions: High quality clinical trial on fire needle is needed especially to compare 2 methods, heating-after-insertion and heating-before-insertion. And studies need to report safety procedures and adverse events to evaluate the safety of fire needle therapy.

Objectives:The aim of this study was to investigate the safety, injury mechanism, complications and adverse reactions associated with lumbar manipulation treatment (LMT), to report newly reported three cases of adverse reactions with LMT.Methods:We searched relevant documents associated with safety of LMT using internet based search (through medline in international field, relavant institute in domestic field) and bibliography, combed adverse cases in clinical field.Results:The relative-to-absolute contraindications were classified into vascular disorders, articular derangement, trauma, bone-weakening and destructive disorders, neurological disorder or psychological factors.The majority of adverse reactions were mild to moderate in about 90%, began within 4 h and generally disappeared within the next 24 h.The most common were local discomfort (15.2-53.0%), stiffness (19.5%), fatigue or tiredness (11.0-12.1%), or radiating discomfort (10.0-12.1%).All three newly reported cases were generation or aggravation of herniated lumbar disc.Conclusions:We have to go ahead of inspecting patient`s past history, going through with physical, neurological, and diagnostic radiological examination.We should be fully aware of the relative and absolute contraindications to Chuna manipulation for preventing adverse reactions before treating patients with LMT.In the future, we need sufficient education and systematic discipline through curriculum or related institute to decrease complications and adverse reactions of Chuna manipulation.

Objectives : The aim of the present study was to report the safety of cervical Chuna manipulation treatment (CCMT). We investigated the mechanism and complications of adverse reactions associated with CCMT, and newly demonstrated three cases in litigation. Methods : We searched relevant case reports, surveys, and review articles using a comprehensive databases of online and bibliography, combed adverse cases in clinical field. Results : A relative-to-absolute contraindication to cervical Chuna manipulation was circulatory and haematological disorders, neurological disorders, bone-weakening and destructive disorders, articular derangement or psychological factors. The majority of adverse reactions were mild or moderate in 85~90%, began within 4 h and generally disappeared within the next 24 h, at least one of them was reported by 30.0~60.9% of the patients after cervical manipulation. The most common were local discomfort (15.2~53.0%), headache (10.0~19.8%), stiffness (19.5~25.0%), tiredness (10.0~12.1%), or radiating discomfort (10.0~23.0%). The two of reported clinical cases were the occurrence of retropharngeal hematoma and cervical herniated disc disease after CCMT. The one was not a fault of practitioner, but complain for compensation. Conclusions : Manipulation of the cervical spine may also be associated with higher complication rates than previously reported. Although It is difficult to estimate the incidence of cervical manipulation complications, it is important to differentiate those patients who are susceptible to adverse reactions in order to inform them correctly.

Objectives : This study aims to educate clinical doctors of the valuable practice of Manipulative Therapy(MT) as an opportunity for evidence-based medicine. Consequently it also serves to review the effectiveness and safety of MT. Methods : The literature studies of overseas were done by Cochrane Library and Medline website; those of domestic researches were completed by utilizing the sources which are gained from KERIS, KISS, DBpia, Kisti, and domestic institutes related with MT. Results : Eight papers related to the effectiveness of MT were published in South Korea, which concluded with the positive effects of MT. None of them are, however, well-designed randomized controlled trials(RCT). On the other hand, fifteen cases of nine articles indicate the adverse reaction of MT, and numbers of the researches in overseas revealed the side effects of MT in order of cervical, lumbar, and thoracic vertebrae. A vascular adverse reaction such as vertebral and carotid artery dissection was a highly reported cervical adverse reaction; in the thoracic and lumbar regions, neurological adverse reaction in terms of disc herniation was frequently discovered. Conclusions : Henceforth, highly qualitative studies are required developing the effective outcomes and preventing any possible complications of MT. Therefore, systemic curriculums in institutions and sufficient clinical training in the filed are strongly recommended.
고지방 식이 유도 비만에 대한 대시호탕 추출물의 억제 효과

Objectives : The aim of this study is to investigate Daesiho-tang(Dachaihu-tang) water extracts (DSTE) have potent anti-obesity activities in a high-fat diet-induced obesity mouse model. Methods : In this study, we designed three groups (normal diet group, high-fat diet group, high-fat diet plus DSTE group for 7-weeks oral administration). Results : Increases in body weight were inhibited by 7-weeks oral administration of DSTE at a 500 mg/kg concentration in this animal model. Results from blood lipid analysis showed that the levels of triglyceride, total cholesterol and LDL-cholesterol were significantly lowered by DSTE administration, also HDL-cholesterol was increased more than high-fat diet-induced obese mouse. To understand the underlying mechanism at the molecular level, the effects of DSTE were examined on the expression of the genes involved in lipogenesis by real-time PCR. In epididymal fat and liver of DSTE-treated mice, the mRNA level of lipogenic genes such as sterol regulatory element binding protein 1 and fatty acid synthase were decreased, which was well correlated with the reduction of the tissues weight. Conclusions : These results suggest that DSTE may have great potential as a novel anti-obesity agent.

Objectives :The purpose of this review is to investigate clinical studies of Hominis placenta pharmacopuncture and to find out the effectiveness of Hominis placenta pharmacopuncture in order to suggest a better research method in the future. Methods :We retrieved numbers of clinical studies about Hominis placenta pharmacopuncture from 7 Korean web databases, using key words such as ‘Hominis Placenta`, ‘Hominis Placenta AND Pharmacopuncture`. This study had been conducted from 1st February 2012 to 30th April 2012. Controlled studies and case studies were only used for this study. Clinical studies that we picked from the databases were classified according to the diseases that those studies are about, and from these clinical studies, we are to research what has to be improved generally in clinical researches. Results :21 case studies, 12 controlled studies had been under research. Hominis placenta pharmacopuncture has a therapeutic effect in various diseases ranging from dermatological disorders, neurological disorders, gynecological disorders, male reproductive disorders, musculoskeletal disorders, respiratory disorders, psychiatric disorders including stroke and cancer. However objectivity and reliability of the Hominis placenta pharmacopuncture studies still remains controversial. Conclusions :It has been proved that there are various effects of Hominis placenta pharmacopuncture therapy in treating wide-ranging diseases. However, in order to put Hominis placenta pharmacopuncture therapy to use for many kinds of diseases in more reasonable ways, it is essential to build well-designed clinical research tools. In the future, abundunt case studies, follow-up trials and RCTs should be done to use Hominis placenta pharmacopuncture for a clinical purpose.
