생애 말 연명의료에 대한 중환자실 간호사의 인식: 내용 분석 연구

Purpose: The purpose of this study was to describe what critical care nurses perceived about life-sustaining treatment at end of life. Methods: A qualitative content analysis method was utilized. The unit of analysis was interview text obtained from fifty critical care nurses of a general hospital. Results: Seven categories in two content areas were abstracted. In the negative perception area, the following five categories were abstracted: patients' suffering, dying with damaged dignity, patients' isolation from family members, regret about choosing life-sustaining treatment, and family members' burden. In the positive perception area, the following two categories were abstracted: willingness to sustain life and duty as family members. Conclusions: Nurses have better competencies pertaining to understanding patients' responses and suffering than any other health care professions do. Nurses should play an important role in advocating for patients and their family in the process of end-of-life care decision making.

Purpose: The purpose of this literature review was to describe the roles of advanced practice nurses related to advance directives. Methods: A comprehensive literature review was conducted utilizing Pubmed, Ovid, and CINAHL. The key words for search were 'advance directives', 'living wills', 'durable power of attorney', 'advanced practice nursing', 'advanced practice nurse', 'nurse practitioner', and 'nurse specialist'. Research articles published between January 1990 and September 2010 and written in English were included. Review papers and expert's opinion were excluded. Results: Five articles were yielded and analyzed. Among them, two focused on the advanced practice nurses' role during the process of advance directives. The other three presented the outcomes depending on the type of nurses' involvement in each health care delivery system. Conclusion: Recently, Korea established advance directives and expanded to diverse sites. Advanced practice nurses' role in the advance directive decision-making process should be examined and developed in research and practice.

Purpose: In Korea, about 30,000 people die in intensive care units annually. However, their quality of life at the end-of-life seems very low. The purpose of this study was to describe palliative care models that could be applied in intensive care units and examine nurses' roles in the models. Methods: A conventional literature review was performed focusing on palliative care in intensive care units and nursing roles in the approaches identified. Results: There are two different models regarding the implementation of palliative care approaches in the intensive care unit. Those are the consultative model and the integrative model. Each model has advantages and disadvantages. Therefore, an appropriate model must be chosen according to the setting. Nurses' roles could vary depending on the setting. Conclusion: Palliative care in the intensive care unit is important to improve patients' quality of life. Moreover, nursing roles are important in providing comprehensive palliative care in intensive care units.
재난 피해자의 피해 체감 규모, 외상 후 스트레스, 재난 이후 주관적 건강상태의 관계: 심리지원에 대한 인식의 조절효과를 중심으로

Background: This study examined the relationship between perceived disaster damage, posttraumatic stress, and subjective health status after disasters and the moderating effect of perceived psychological support among disaster victims. Methods: Data from 1,821 participants in the fourth ‘Follow-up Survey on Change of Disaster Victims’ conducted by the National Disaster Management Research Institute in 2019 were analyzed. Results: The results showed that the perceived degree of disaster damage significantly impacted subjective health status after disasters; this effect was fully mediated by posttraumatic stress. Furthermore, perceptions of psychological support were found to moderate the mediation effect of posttraumatic stress. Conclusions: Based on these findings, a discussion was suggested regarding future improvements in disaster-supporting policies. 본 연구는 재난 피해자의 피해 체감 규모와 외상 후 스트레스, 재난 이후 주관적 건강상태의 관계 및 심리지원에 대한 인식의 조절효과를 확인하고자 하였다. 이에 국립재난안전연구원에서 2019년 시행한 4차 ‘재난피해자 삶의 변화 추적조사’ 중, 총 1,821명의 결과를 통계 분석하였다. 분석 결과, 피해 체감 규모는 재난 이후 주관적 건강상태에 유의미한 영향을 미쳤으며 이 경로는 외상 후 스트레스에 의해 완전 매개됨을 확인하였다. 또한 재난 피해자의 심리지원에 대한 인식은 해당 매개효과를 유의하게 조절하는 것으로 나타났다. 이와 같은 결과를 바탕으로 재난 지원 정책의 추후 개선 방향을 논의하였다.
의료처치 및 환자 비밀유지에 대한 중환자실 간호사와 일반병동 간호사의 윤리적 갈등 경험 비교연구

Purpose : This study compares experiences of ethical dilemma between nurses working in intensive care units and those in general units under specific situations of treatment decisions and confidentiality. Method : This cross-sectional descriptive study utilizes the self-report survey method. The survey questionnaires were completed by 50 and 52 nurses working in intensive care units and general units, respectively. The instrument, which consisted of 16 items of ethical dilemma situations about treatment decision and confidentiality, was used. The mean scores for each item were compared between the two groups. Results : The study found no differences in terms of age, gender, education level, clinical experience in years, and being educated on healthcare ethics. For 9 out of 16 items, the mean scores of nurses in intensive care units were significantly higher than those of nurses in general units. Conclusions : Nurses in intensive care units experienced ethical dilemmas regarding treatment decisions and confidentiality more often than those in general units. This study emphasizes the need to establish strategies for improving the ethical competence of critical care nurses.
