        십이지장 평활근 육종 1예

        Malignant growth of the small bowel accounts for approximately 1 to 3 percent of malignant tumors of the gastrointestinal tract. Leiomyosarcomas are the second most common primary tumor of the small bowel, their frequency being one-third to one-half of adenocarcinoma. The common clinical symptoms of leiomyosarcoma of the small intestine are hemorrhage and abdominal pain. Accurate diagnosis cannot be based on solely on the roentgenographic finding, although in certain situations the dignosis of leiomyosarcoma may be suggested strongly. We report a case of bleeding leiomyosarcoma located in the second portion of the duodenum which was first recognized by endoscopic examination and confirmed by explolaparotomy.

        지방재정 운용의 자구노력 제고 방안 - 지방교부세 인센티브제도 -

        General Local Share Tax (GLST) may discourage willingness for local governments to make their efforts to enlarge revenues and reduce expenditures in that it is distributed based on the policy goal of intergovernmental financial equity. To prevent such negative probabilities, Incentive system, where each local government’s financial self-supporting efforts(SSE) for revenue enlargements and expenditure reductions are evaluated and a part of GLST is discriminately alloted based on its accomplishment, has been introduced and executed since 1997. The incentive system needs to now be analyzed whether it actually encourages SSE in local financial activities. Futhermore, the determinants, which influence SSE of local governments, are need be discerned to provide policy tools to strengthen the SSE. The findings of this paper show that SSE of local governments are overall so weak that they suffer from cutback of GLST, which is more frequently observed among comparatively higher governments in local financial self-sufficiency degree. And in regression analysis to figure out determinants to affect the SSE by the local government type, the values of R² show low level, which mean that local governments are negligent of their SSE in carrying out the revenue and expenditure activities composed of discretional items. Therefore, practical policy means is demanded to more activate incentive system for enforcing the SSE in local financial activities.

        Intergovernmental Distribution of VAT Revenue in Korea: Local Consumption Tax

        In Korea, local consumption tax (LCT) has been established as a form of VAT revenueshare in 2010. LCT takes 5% of the national VAT revenue as the financial base anddistributes it to the local governments based on the regional sharing ratio of private finalconsumption expenditures (vertical equity) and weighting system to mitigate regionalfinancial disparity (horizontal equity). This paper first evaluates current LCT distributionin terms of both equities, suggesting that new distribution indices be needed to satisfy bothgoals. Before producing them, by reassessing the status and the goal of LCT, this papersuggests LCT as a new local transfer, not a local tax, should pursue another goal, thecorrespondence between production and incidence of regional value-added which is VATresource and ultimately LCT base, because its interregional migration is substantial inKorea. Based on such status and goals, new distribution indices are suggested and thefindings are evaluated.

        수도권 기초단체의 인구분포와 인구이동 현황과 정책적 함의: 의정부시를 중심으로

        의정부시는 서울과 경기북부의 중간지대에 속하는 전이지역이다. 실제로 의정부시는 인구가 서울시에서 경기북부 혹은 경기북부에서 서울시로 이동하는 과정에 있어 가교 및 중계 역할을 담당하는 지역이다. 의정부시는 기본적으로 2012∼2016 기간동안 매년 2,000여명의 인구가 증가하고 있고, 특히 2015∼2016 기간동안에는 총 4,520명이 증가했고 이중에서 3,368명이 역외 유입되었고 나머지 1,152명이 출생에서 사망을 제외한 자체 증가분이다. 이와 같이 의정부시의 역외유입 규모가 자체 증가의 세 배에 이르고 있다는 점은 주변의 서울시와 경기북부 지역에 대해 경쟁력을 확보하고 있다는 점을 보여준다. 특히 서울시 노원구와 도봉구에서 순유입되는 규모(4,962명)가 전체 순유입(3,368명)보다 더 많으며 이러한 현상은 서울시의 높은 주거비용이라는 배출요인과 의정부시의 주택을 비롯한 낮은 주거비용이 흡입요인으로 작용했기 때문이다. 반면에 인구 유출요인으로는 경제요인의 영향력이 크며 실제로 직업 선택을 이유로 경기남부로 유출되는 인구가 많다. 따라서 의정부시는 기본적으로 향후 경기 남부지역으로의 인구유출을 방지하기 위해서는 고용기회의 확충이 요구되는 상황이다. 그리고 인접한 노원구와 도봉구로부터 유입이 유출보다는 높지만 유출 또한 2016년 경우 유출이 2,226명에 달할 정도로 그 규모가 크다. 이는 주로 자녀교육과 관련이 높은 40대의 유출에 의해서 비롯된 바가 크기 때문에 교육을 중심으로 한 정주여건의 확충이 요구되는 상황이다. Uijeongbu is considered a transit region which plays a role of bridge though which population moves from Gyeonggi Northern regions to Seoul, and vice verse. The population of Uijeongbu has increased by 2,000 a year during the period from 2012 to 2016. Specially a total of 4,520 has increased during term from 2015 to 2016, 3,368 of the total is in-migration from outside, and the rest, 1,152, is its own net increase which results form difference between birth and death. The fact that in-migration is three times larger than its own increase indicates that Uijeongbu has advantage in competition in attracting population over the neighboring regions, Seoul and Gyeonggi Northern regions. The magnitude of 4,962, a net increase which is calculated from difference in-migration and out-migration where arises in only Nowon and Dobong District of Seoul adjoining to Uijeongbu exceeds the size of 3,368, a total net migration from all over the country. That is due to both push effect of Seoul arising from the high cost of residence and pull effect of Uijeongbu resulted from the low cost. However, population of Uijeongbu decreases due to economic factor. A large part of population depart to gain employment opportunities in Gyeonggi Southern area, therefore, Uijeongbu is required to expand jobs to retain them. And the size of 2,226, although it was less than in-migration number, 4,962, moved to Nowon and Dobong District of Seoul, which was largely composed of resident of forties who are concerned about education of their kids. Therefore, Uijeongbu is demanded to improve residence condition based on education.

        Jeungsan(甑山) carried out the Universal Redesign(天地公事) through which the former world full of a mutual opposition(相克) can be peacefully transferred into the latter world composed of a mutual beneficence(相生). Although the Universal Redesign consists of such three factors heaven, earth, and human, it converges towards Human Redesign in that humans are the main agents for heaven and earth. Human as Redesign object is characterized as a member of community, not an anarchistic individual. The Universal Resign of Jeungsan is eventually connected with the construction of a country comprising a mutual beneficence in that it is the most typical strain among all the human communities. His Universal Resign for foundering the foremost country does not target all the countries, but only one country upon and through which the world with a mutual beneficence can be formed. He executed the Universal Redesign based on such three principles as Haewon sangsaeng(解冤相生), Boeunsangsaeng(報恩相生), and Wonsibanbon (原始返本), Judging from such principles, Korea can be naturally selected as the target of the Universal Redesign for constructing the foremost country. It is approaching when the Universal Redesign in order to transform Korea into the foremost country comes true. The first step for Korea to be made as the foremost country is the construction of DMZ World Peace Park (DWPP) which was proposed by Bak Geunhye, the Korean President, to Obama, the President of USA, in 2013 she visited there. DWPP can be expected to be a center for corporation and interchange between South and North Korea under the interest and support of the world including UN, furthermore, the Capital area of reunified Korea, and finally the world hub of inter national political and economic relation. DWPP is planned to be built at the core of the best place in Korea which is a propitious spot all over the world, upon which a new world capitalism order armed with mutual beneficence can spread.
