        Diastolic Velocity and Future Cardiovascular Events in Asymptomatic High Risk Patients

        Background and Objectives: Prognostic value of additional carotid Doppler evaluations to carotid intima-media thickness (IMT) and plaque has not been completely evaluated. Subjects and Methods: A total of 1119 patients with risk factors for, but without, overt coronary artery disease (CAD), who underwent both carotid ultrasound and Doppler examination were included in the present study. Parameters of interest included peak systolic and end-diastolic velocities, resistive indices of the carotid arteries, IMT, and plaque measurements. The primary end-point was all-cause cerebro-cardiovascular events (CVEs) including acute myocardial infarction, coronary revascularization therapy, heart failure admission, stroke, and cardiovascular death. Model 1 covariates comprised age and sex; Model 2 also included hypertension, diabetes and smoking; Model 3 also had use of aspirin and statin; and Model 4 also included IMT and plaque. Results: The mean follow-up duration was 1386±461 days and the mean age of the study population was 60±12 years. Amongst 1119 participants, 43% were women, 57% had a history of hypertension, and 23% had diabetes. During follow-up, 6.6% of patients experienced CVEs. Among carotid Doppler parameters, average common carotid artery end-diastolic velocity was the independent predictor for future CVEs after adjustments for all models variables (HR 0.95 per cm/s, 95% confident interval 0.91-0.99, p=0.034 inModel 4) and significantly increased the predictive value of Model 4 (global 2=59.0 vs. 62.8, p=0.029). Conclusion: Carotid Doppler measurements in addition to IMT and plaque evaluation are independently associated with future CVEs in asymptomatic patients at risk for CAD.

        Lipoprotein-Associated Phospholipase A2 Is Related to Plaque Stability and Is a Potential Biomarker for Acute Coronary Syndrome

        Purpose: Plasma lipoprotein-associated phospholipase A2 (Lp-PLA2) binds to low-density lipoprotein. The levels of Lp-PLA2 reflect the plaque burden, and are upregulatedin acute coronary syndrome (ACS). We investigated the diagnostic value of Lp-PLA2 levels and found that it might be a potential biomarker for ACS. Materials and Methods: We classified 226 study participants into three groups: patients without significant stenosis (control group), patients with significant stenosis with stable angina(SA group), and patients with ACS (ACS group). Results: Lp-PLA2 and high-sensitivity C-reactive protein (hs-CRP) levels were significantly greater in the ACS group than in the SA group (p=0.044 and p=0.029, respectively). Multivariate logisticregression analysis revealed that Lp-PLA2 levels are significantly associated with ACS (odds ratio=1.047, p=0.013). The addition of Lp-PLA2 to the ACS model significantlyincreased the global χ2 value over traditional risk factors (28.14 to 35.602, p=0.006). The area under the receiver operating characteristic curve for Lp-PLA2 was 0.624 (p=0.004). The addition of Lp-PLA2 level to serum hs-CRP concentration yielded an integrated discrimination improvement of 0.0368 (p=0.0093, standard error:0.0142) and improved the ability to diagnose ACS. Conclusion: Lp-PLA2 levels are related to plaque stability and might be a diagnostic biomarker for ACS.

        Spontaneous Coronary Artery Dissection Mimicking Coronary Spasm Diagnosed by Intravascular Ultrasonography

        Spontaneous coronary artery dissection (SCAD) is a rare and occasionally life-threatening cause of acute coronary syndrome. Patients may present with clinical scenarios ranging from angina pectoris to cardiogenic shock to sudden cardiac death, and it may be a potentially life-threatening condition if not recognized. However, its etiology, pathophysiology and optimal therapeutic strategies have not been well understood. SCAD is diagnosed on the basis of coronary angiography, but complementary techniques as such intravascular ultrasound (IVUS) and optical coherence tomography should be considered for diagnostic clarification where appropriate. Likewise, the selection of treatment strategy depends upon the clinical manifestation, location and the extent of dissection and amount of ischemic myocardium at risk. Herein, we present the case of a 35-year-old woman who presented with acute myocardial infarction. She was diagnosed by IVUS with spontaneous diffuse dissection of the left anterior descending artery without atheroma, treated with percutaneous coronary stenting,and had a favorable clinical course and was discharged on medical therapy.

        Synephrine-containing dietary supplement precipitating apical ballooning syndrome in a young female

        Apical ballooning syndrome (ABS) is a unique reversible cardiomyopathy that is frequently precipitated by emotional or physical stress. In addition, the few drugs reported to precipitate ABS were either illegal or strictly controlled for medical use. This paper reports a case of ABS precipitated by a dietary supplement. Our case accentuates the potential risk of dietary supplements containing synephrine,which is uncontrolled and available to the general public. Therefore, the Korea Food and Drug Administration should regulate these dietary supplements, and warn healthcare workers and the general public of the potential hazards of the indiscriminate abuse of dietary supplements.

        Predicting Peri-Device Leakage of Left Atrial Appendage Device Closure Using Novel Three-Dimensional Geometric CT Analysis

        Background: After left atrial appendage (LAA) device closure, peri-device leakage into the LAA persists due to incompleteocclusion. We hypothesized that pre-procedural three-dimensional (3D) geometric analysis of the interatrial septum (IAS) and LAAorifice can predict this leakage. We investigated the predictive parameters of LAA device closure obtained from baseline cardiaccomputerized tomography (CT) using a novel 3D analysis system. Methods: We conducted a retrospective study of 22 patients who underwent LAA device closure. We defined peri-device leakageas the presence of a Doppler signal inside the LAA after device deployment (group 2, n = 5) compared with patients withoutperi-device leakage (group 1, n = 17). Conventional parameters were measured by cardiac CT. Angles θ and φ were defined betweenthe IAS plane and the line, linking the LAA orifice center and foramen ovale. Results: Group 2 exhibited significantly better left atrial (LA) function than group 1 (p = 0.031). Pre-procedural θ was alsolarger in this group (41.9° vs. 52.3°, p = 0.019). The LAA cauliflower-type morphology was more common in group 2. Overall,the patients’ LA reserve significantly decreased after the procedure (21.7 mm3 vs. 17.8 mm3, p = 0.035). However, we observed nosignificant interval changes in pre- and post-procedural values of θ and φ in either group (all p > 0.05). Conclusion: Angles between the IAS and LAA orifice might be a novel anatomical parameter for predicting peri-device leakageafter LAA device closure. In addition, 3D CT analysis of the LA and LAA orifice could be used to identify clinically favorable candidatesfor LAA device closure.

      • 얼굴 표정의 카툰 렌더링

        일반적인 물체와 달리 사람의 얼굴은 표정이라는 중요한 시각적 요소를 포함하고 있다. 이러한 이유로 만화가들은 빛에 의한 음영 이외에도, 장면의 분위기와 캐릭터의 특징을 효과적으로 전달하기 위해 특별한 음영을 얼굴에 그려 넣는다. 이러한 음영은 물리적으로 타당하지는 않지만 얼굴의 형태와 표정을 강조하기 때문에 얼굴 표정의 카툰 렌더링에서 반드시 고려해야 할 사항이다. 이 논문에서는 실제 만화가의 음영 표현방식에 착안하여 캐릭터 얼굴을 위한 새로운 카툰 렌더링 방법을 제안한다. 이를 위해 먼저 다양한 만화 이미지를 수집하여 얼굴의 음영표현을 분석하고, 캐릭터의 표정에 따라 카툰 렌더링을 달리 하기 위해서 음영표현 템플릿을 정의하였다. 그리고 이러한 만화가들의 음영표현방식을 바탕으로 기존의 카툰 렌더링 방법을 발전시켜 효과적으로 캐릭터의 표정을 강조하는 캐릭터 얼굴 카툰 렌더링 시스템을 구현하였다. 마지막으로 사용자평가를 통해 논문에서 제안한 방법의 유용성을 증명한다. The human face has "expression" as an important visual factor in contrast with general objects. For this reason, cartoonists draw shadow that emphasizes facial shape and facial expression in order to convey atmosphere of scene and trait of character. This shadow should be considered when doing cartoon rendering for facial expression although it is not an physical shading. This paper proposes a cartoon rendering system for facial expression based on shading techniques of real cartoonist. First of all, we searched such techniques of cartoonist through variety of collected cartoon images and defined shadow templates according to character's facial expression to do cartoon rendering diffently. After that, we demonstrated cartoon rendering system of facial expression on the basis of survey result that effectively emphasizes facial shape and facial expression. Finally, we showed the usefulness through the user questionnaire.

      • 3차원 텍스처를 이용한 카툰 렌더링의 지역적인 음영편집

        본 논문에서는 음영을 단순화시켜 3차원 물체를 만화적으로 표현하면서도, 물체의 형태적 특성을 강조할 수 있는 카툰 렌더링 방법론을 연구한다. 이를 위해 물체의 기하학적인 중요도를 정의하는 척도인 세일리언시(Saliency)를 물체의 음영을 결정하는 요소로 사용하며, 이에 따라 각 정점의 음영 단계 및 색상을 차등적으로 적용하였다. 또한 카툰 렌더링 방법에서 음영 결정요소를 추가하기 위해 기존 텍스처를 3차원으로 확장한다. 나아가 확장된 텍스처를 이용한 카툰렌더링에서 만화가들의 주관적인 음영표현 스타일을 반영하기 위해, 2D 애니메이션 제작과정과 같이 물체 위에서 직접 음영을 편집하는 직관적인 방법을 제시한다.
