시각구조분석에 의한 음성 김주태 가옥의 지각된 환경지원성에 관한 연구

본 연구는 건축공간의 현상학적 논의로 물리적 환경과 심리적 행태의 관계성에 주목하며, 특정한 관계에 의해 분화 구성된 김주태 가옥을 대상으로 공간의 구성 방법과 행태적 의미를 시각 구조적 관점에서 분석하였다. 김주태 가옥은 사용자 의 사회문화적 관계를 반영하여 명확한 방법으로 분화되어 구성되었으며, 채와 담으로 에워싸인 물리적 조건으로 관계적이 며 독립적인 영역성을 부여하고 있다. 각각 독립적으로 한정된 영역은 순차적으로 접근할 수 있도록 공간의 열림을 제안하고 있으며, 이와 같은 과정적 공간 구성방식은 유도된 공간경험 과정에서 상대적 차이를 지각하게 되고 점증적으로 강한 폐쇄성 을 인지하게 된다. 가옥의 중심부에 위치하고 있는 사이마당은 사랑채와 안채 사이의 공간깊이를 더욱 강조하는 입체적 층을 형성하고 있으며, 물리적 크기는 상대적으로 작은 공간이지만 .관찰자 시점의 위치설정(공간의 원근감), 짧은 주기의 반복적 방향전환, 켜(layer)의 중첩 등의 환경적 조건에 의해 유도된 심리적 행태는 실제적 크기에 비해 상대적으로 멀게 혹은 깊게 인지하게 된다. 이는 ‘일반화된 타자’의 시각적 관점에서 대상을 어떻게 보이게 의도했는가에 대한 관계적 접근 방법이며, 사용자에게 지각된 환경지원성으로 심리적 태도와 행동을 유도하는 실행적 요소로 이해할 수 있다. This study discusses the architectural space's phenomenology, focusing on the physical environment and psychological behavior. Kim, Ju-Tae House, which is divided by special relationship, was analyzed by method of constructing space, and behavioral meaning with the perspective of visual structure. Kim, Ju-Tae House was constructed in a clear way by reflecting the users social, and cultural relationship. It has a relational and independent territories by physically surrounding Chae and wall. Each independent limited space is suggesting the space being opened by accessing the space stage by stage. This method of constructing the space makes the user aware of the relative difference from the induced space experience, and recognize the incremental strong isolation. Between the yard, which is located in the center of the house, forms a three-dimensional layer between Sarangchae and Anchae, emphasizing the spacial depth. The physical space is relatively small but because of the environmental conditions, such as the positioning of the observer's perspective, short cycle reorientation, and overlapped layer, it is recognized as far or deep compared to the actual size. This is the relative method as a visual perspective that is intended, and is understood as an active element that induces the psychological attitude by the recognized environment affordability of the user.

Vagal reflex during laryngosopy and tracheal intubation may result in cardiac arrhythmia such as bradyarrhythmia and asystole. A 66-year-old woman, scheduled for coronary artery bypass surgery, received intravenous bolus of midazolam 2 mg, sufentanil 50 μg, and vecuronium 10 mg for induction of general anesthesia. After two minutes of manual ventilation, tracheal intubation was attempted and the patient became asystolic during laryngoscopic manipulation. The laryngoscope was immediately withdrawn, and the patient returned to normal sinus rhythm. Ten minutes later, more experienced practitioner performed the second laryngoscopic intubation, but it eventually induced asystole again. External cardiac massage was commenced and normal sinus rhythm retuned at a rate of 60 beats/min after 1-2 minute later. (Korean J Anesthesiol 2009;57:503∼6)

Intraoperative transesophageal echocardiography (IOTEE) is an invaluable diagnostic method for management of cardiac surgical patients, including patients undergoing valve replacement surgery. We report a patient who underwent reoperation for mitral valve replacement due to intravalvular regurgitation following mitral valve replacement with a bioprosthetic valve. The condition was detected by IOTEE and caused by suture entrapment. (Korean J Anesthesiol 2009; 56: 709~12)

한탄바이러스에 대한 면역억제제 및 항혈액응고제의 작용

In 1976, Lee and Lee discovered the etiologic agent of Korean hemorrhage fever(KHF), Hantaan virus, although similar disease to KHF had been reported as hemorrhagic nephrosonephritis in Russia, as nephropathia epidemica in hemorrhagic fever in China. In 1981, French et al. reported a successful growth of Hactaan virus in A549 cells which originated from alveolar epithelial cells of a human lung cancer. The growth of Hantaan virus and the appearence of antibody complex in lung tissues of Apodemus agrarius following intramuscular inoculation of the virus and cyclophosphoamide and also the effect of anticoagulants to infectivity of Hantaan virus in Apodemus agraius were studied. The following results were obtained. 1. After inoculation of Hantaan virus in Apodemus agrarius, the formation of immune complex was effectively suppressed by consecutive injection of procytox intramusculary per 3~6 ㎎ three times at certain intervals. 2. The effect of anticoagulants, EDTA, Heparin and Sod. citrate to infectivity of Hantaan virus in Apodemus agrarius was not demonstrated. Among these, EDTA was the most safe anticoagulant in Hantaan virus experiment.

The differentiated space in the traditional space was the limited space that should sort the using area on account of the distinction of a social standing and sex by Confucian ideas and was the processing space that followed the circulation. Comparing the limited organization way and the processing organization way, urban residence has diminished the limited exterior space for a traditional residence and the degree of closure. The processing organization way have the many linking ways of space and the access from the inside to exterior space is easier than the access from the outside to exterior space. That fact shows that the differentiated space become the current situation that is adopted on a realistic situation

감염성 심내막염 환자에서 증상 없이 발견된 뇌동맥류 1예

30세 남자 환자로 심내막염 진단 후 발견된 무증상의 감염성 뇌동맥류로 항생제 치료 후 단계적인 수술로 심장판막 대치술과 뇌동맥류 결찰술을 시행하여 심각한 뇌신경학적 후유증이나 합병증이 발생하기 전에 성공적으로 치료한 경험을 하였기에 문헌 고찰과 함께 보고하는 바이다. Cerebral mycotic aneurysm is an uncommon complication of infective endocarditis. In spite of low incidence, mycotic aneurysm is a serious complication because of the high risk of its rupture. Therefore, early diagnosis and management of cerebral mycotic aneurysm are imperative before the rupture. We experienced a case of asymptomatic cerebral mycotic aneurysm in a 30-year old man with infective endocarditis. He was admitted for splenic and renal infarction due to systemic embolization. The echocardiography showed severe mitral regurgitation, mitral valve prolapse and vegetation on the mitral valve. Streptococcus mitis was identified from blood cultures. In the course of antibiotics therapy, brain magnetic resonance image and 4-vessel cerebral angiography were performed, which revealed multiple unruptured cerebral mycotic aneurysms. Considering the high risk of rupture we performed mitral valve replacement with bioprosthesis. On follow-up cerebral angiography after the valve replacement surgery, one of aneurysms was enlarged, and successfully treated by surgical intervention without complication. We report one case of mycotic aneurysm without neurologic symptom in an infective endocarditis patient, who was treated without serious sequelae of the aneurysm. (Korean J Med 64:477-481, 2003)
