우육지방질의 산화에 미치는 간장의 항산화작용에 관한 연구

牛肉 지방질의 산화에 미치는 간장의 抗酸化作用을 살펴보기 위하여 GCM(ground and cooked meat), GCM-water, GCM-brine, GCM-soybeab sauce system들을 6℃에서 5주간 저장하면서 TBA 값, 과산화물값, 지방산조성 및 관능검사를 행하였다. 저장 기간 중 TBA값 및 과산화물값의 경우 GCM-soybean sauce system에서는 거의 변화가 없이 낮았으나 기타 system들에서는 현저한 증가 현상이 있었으며 특히 GCM-brine system에서 심하였다. 각 system들의 지방산 조성에서는 모노엔지방산 및 폴리엔 지방산의 함량이 변화하였으며, 저장 후 중성지방질 획분에서는 리놀레산이, 또한 인지방질 획분에서는 아라키돈산의 함량이 상대적으로 감소하여 저장 중 산패가 상당히 진행됨을 알 수 있었다. 그러나 GCM-soybean sauce system에서 가장 그 변화가 적었다. 그리고 관능검사에서 나타난 변패취는 GCM-water 및 GCM-system에서 2주 후에 각각 나타났으며 5주후에는 system 모두에서 나타났으나 GCM-soybean sauce system에서 가장 낮은 변패취 생성 결과를 보였다. 그러므로 이와 같은 결과들을 종합해 볼 때 간장은 牛肉 지방질의 산화 억제 효과가 현저한 것으로 판단되었다.
연축성 발성장애(Spasmodic Dysphonia)에 대한 보톡스 주입치료

Spasmodic dysphonia is a chronic, focal, movement-induced, action-specific dystonia of the laryngeal musculature during speech. It can have a profound effect on quality of life, severely limiting people's communication, especially via telephone and in noisy backgrounds. Spasmodic dysphonia (SD) is usually of the adductor type characterized by glottic contractions causing tightness and voice breaks with forced-strangled voice, but it may also be abductor type or, much less commonly, mixed. Treatment options for adductor spasmodic dysphonia (ADSD) include voice therapy, surgical procedures, and botulinum toxin injections (Botox). The use of Botox injected into the laryngeal muscles remains the "gold standard" treatment for reducing the vocal symptoms of ADSD and Botox induces a temporary paresis of the laryngeal muscles and provides short-term relief of symptoms. Repeated injections of the laryngeal muscles, generally every 3-4 months, are required for continuous relief of symptoms. Improvement in vocal function has been reported after use of Botox injections, though a completely normal voice is rarely achieved. In this hospital, 1,030 patients have been enrolled for Botox injection therapy so far (May, 2012). In this review article, I'd like to present my personal experience of management of spasmodic dysphonia mainly by Botox injection.

Unilateral vocal cord palsy which is associated with laryngeal trauma is not uncommon event. In a 42-year-old male, a cricoid cartilage fracure had been developed after blunt trauma. The endoscopic findings showed contusion and diffuse swelling around the left arytenoid and false cord. During phonation, the mobility of left side true vocal cord was decreased. There were no level difference and displacement of the left side arytenoid. We used the laryngeal electromyography (LEMG) to make a differential diagnosis between the cricoarytenoid joint dislocation and the injury of recurrent laryngeal nerve. At the right thyroarytenoid muscle and cricothyroid muscle, the findings of LEMG were normal. But the amplitude and frequency during phonation were decreased (partial denervation) at the left thyroarytenoid muscle. LEMG is a very useful method to predict the diagnosis of vocal cord palsy.

Denture-type Electrolarynx is being watched recently because it is easy to control the pitch and volume and it is not exposed on the outside. This system consists of three pars. The first is the Oral-Unit part, which contains a receiver, a loud speaker, and a rechargeable battery. The others are the Transmitter St Control-Unit Part, and the Charging-Unit part. We have newly developed the Korean Denture-type Electrolarynx : NeoVox. That system is designed considering low-power consumption, wireless charging system, small size and the speaker emphasizing low-frequency. So that the laryngectomees feel comfortable to use it and speak naturally.
과대비성을 동반한 점막하 구개열 환자에 대한 Double Opposing Z-plasty 3례

Several kinds of surgical methods have been used to treat the submucous cleft palate with hypernasality. The purpose of this study was to evaluate the double opposing Z-plasty which is one of the newly established surgical methods by Dr. Furlow in three cases of submucous cleft palate with hypernasality operated in this department. There were a number of advantage of double opposing Z-plasty. The surgery is adaptable for use in early patient. Midfacial growth retardation and complication were not seen in all cases. Hypernasality was improved subjectively. Using the nasometer, the nasalance score of the postoperative period was compared with that of before surgery, much improvement were noted in 'Ah', 'PaPa'. And fiberoptic endoscope was used to evaluate the velopharyngeal closure pattern. Velopharyngeal closure pattern with velum was improved dramatically, Double opposing Z-plasty seem to be a good procedure for the treatment of submucous cleft palate with hypernasality.
설소대단축증의 수술적 치료로서의 Z-plasty 술식 1례

Ankyloglossia is the presence of a lingual frenulum, which can range from a mucous membrane band to a short and thick band and, in extreme cases, to fusion of the tongue to the floor of the mouth. The effects of such a condition, in addition to speech defects and occasionally restriction of sucking, including dental deformities, such as open bite, or even prognathism. Treatment is surgical. The preferred treatment is horizontal sectioning of the frenulum down to the lingual septum and then suturing of the mucosa. The main problem after the healing of surgical wound is adhesion and contracture. Adhesion restrict the movement of tongue like tongue-tie. Z-plasty at the site of incision can solve this problem by changing the direction of scar. We have experienced a patient with ankyloglossia with speech defect, who underwent frenuloomy by Z-plasty. So we present a surgical treatment of Ankyloglossia using Z-plasty and discuss the treatment with a review of literature.
