        재관류액에 Latamoxef Sodium 첨가가 허혈성 심정지후 흰쥐 적출심장의 기능회복에 미치는 영향

        Recent experimental work indicates latamoxef sodium used as a broadspectrum antibiotics generates oxygen-free radicals. The present study represents an attempt to investigate whether reperfusate containing Shiomarin[85% latamoxef sodium+15% mannitol] might decrease the post-ischemic recovery of cardiac function. In the investigation, twelve isolated rat hearts were subjected to 270 minutes of cold total global ischemia. After the cold total global ischemia, six hearts[KHB group] were reperfused with Krebs-Henseleit buffer solution and the other six hearts[LMS group] with Krebs-Henseleit buffer solution containing latamoxef sodium[200ug /L]. Postischemic recovery rate of heart rate, aortic systolic pressure, aortic flow, coronary flow and cardiac output at 20 minutes reperfusion was 100.66$\pm$10.38, 85.25$\pm$7.61, 78.95$\pm$6. 02, 78.85$\pm$8.86 and 79.11$\pm$6.54 percent respectively in the KHB group and 97.96$\pm$4.19, 87. 72$\pm$4.37, 81.74$\pm$6.80, 82.69$\pm$10.01 and 81.90$\pm$6.67 percent respectively in the LMS group. The hemodynamic data revealed no significant difference in the post-ischemic recovery rate of the two groups. This finding suggests that reperfusate containing Shiomarin[latamoxef sodium, 200ug /L] does not affect the cardiac functional recovery after cold total global ischemia.

        누두흉의 임상적 고찰

        This study is a review of the early postoperative results in fifteen patients who diagnosed funnel chest and underwent the corrective surgical procedure by the sternal turnover[STO] or the sternocostal elevation[SCE] between January 1988 and February 1990. Thirteen among fifteen patients were male. Early eight patients were corrected by the STO and late seven patients by the SCE. Cosmetic result was excellent in one, fair in five, poor in two patients who received the STO and excellent in all patients corrected by the SCE. This unfavorable postoperative result in the STO group was due to the protrusion of inverted xiphisternal joint, the depression along the rejoined costal cartilages and forward tilting of the distal sternum. The lower vertebral indices measured on the lateral chest film were improved postoperatively from 0.42\ulcorner0.08 to 0.30\ulcorner0.05 in the STO group and from 0.31\ulcorner0.05 to 0.24\ulcorner0.03 in the SCE group. There was significant improvement in lower vertebral index in both groups, but it was difficult to compare the postoperative improvement between groups due to the different preoperative status. In the SCE group, the funnel index and the degree of depression measured on the computed tomography were significantly improved from 5.29\ulcorner1.72 and 3.11\ulcorner0.71 to 2.77\ulcorner0.46 and 1.63\ulcorner0.19[P<0.05], but there was no significant change in the degree of flatness postoperatively. This results suggest that sternocostal elevation is more cosmetic procedure than sternal turnover in children and in young adolescents whose funnel index measured on the computed tomography is low.

        감염성 승모판 심내막염의 중단기 수술 성적

        Background: Infective endocarditis shows higher operative morbidity and mortality rates than other cardiac diseases. The vast majority of studies on infective endocarditis have been made on aortic endocarditis, with little attention having been paid to infective endocarditis on the mitral valve. This study attempts to investigate the clinical aspects and operative results of infective endocarditis on the mitral valve. Meterial and Method: The subjects of this study consist of 23 patients who underwent operations for infective endocariditis on the mitral valve from June 1995 to May 2003. Among them, 2 patients suffered from prosthetic valvular endocarditis and the other 21 from native valvular endocarditis. The subjects were evenly distributed age-wise with an average age of 44.8$\pm$15.7 (11∼66) years. Emergency operations were performed on seventeen patients (73.9%) due to large vegetation or instable hemodynamic status. In preoperative examinations, twelve patients exhibited congestive heart failure, four patients renal failure, two patients spleen and renal infarction, and two patients temporary neurological defects, while one patient had a brain abscess. Based on the NYHA functional classification, seven patients were determined to be at Grade II, 9 patients at Grade III, and 6 patients at Grade IV. Vegetations were detected in 20 patients while mitral regurgitation was dominant in 19 patients with 4 patients showing up as mitral stenosis dominant on the preoperative echocardiogram. Blood cultures for causative organisms were performed on all patients, and positive results were obtained from ten patients, with five cases of Streptococcus viridance, two cases of methicillin-sensitive Staphylococcus aureus, and one case each of Corynebacteriurn, Haemophillis, and Gernella. Operations were decided according to the AA/AHA guidelines (1988). The mean follow-up period was 27.6 $\pm$23.3 (1 ∼ 97) months. Result: Mitral valve replacements were performed on 43 patients, with mechanical valves being used on 9 patients and tissue valves on the other 4. Several kinds of mitral valve repair or mitral valvuloplasty were carried out on the remaining 10 patients. Associated procedures included six aortic valve replacements, two tricuspid annuloplasty, one modified Maze operation, and one direct closure of a ventricular septal defect. Postoperative complications included two cases of bleeding and one case each of mediastinitis, low cardiac output syndrome, and pneumonia. There were no cases of early deaths, or death within 30 days following the operation. No patient died in the hospital or experienced valve related complications. One patient, however, underwent mitral valvuloplasty 3 months after the operation. Another patient died from intra-cranial hemorrhage in the 31st month after the operation. Therefore, the valve-related death rate was 4.3%, and the valve-related complication rate 8.6% on mid-term follow-up. 1, 3-, and 5-year valve- related event free rates were 90.8%, 79.5%, and 79.5%, respectively, while 1, follow-up. 1, 3-, and 5-year valve- related event free rates were 90.8%, 79.5%, and 79.5%, respectively, while 1, 3-, and 5-year survival rates were 100%, 88.8%, and 88.8%, respectively. Conclusion: The findings suggest that a complete removal of infected tissues is essential in the operative treatment of infectious endocarditis of the mitral valve. It is also suggested that when infected tissues are completely removed, neither type of material nor method of operation has a significant effect on the operation result. The postoperative results also suggest the need for a close follow-up observation of the patients suspected of having brain damage, which is caused by preoperative blood contamination or emboli from vegetation, for a possible cerebral vascular injury such as mycotic aneurysm. 배경: 감염성 심내막염은 다른 심장질환에 비해 유병률과 사망률이 높고, 대부분의 임상연구가 대동맥판에 발생한 심내막염에 편중되어 있어, 승모판 심내막염에 대한 보고는 상대적으로 드문데 이에 대한 임상양상 및 수술 결과를 분석하여 보고하고자 한다. 대상 및 방법: 1995년 6월부터 2003년 5월까지 전남대학교병원에서 감염성 승모판 심내막염으로 단일 술자에 의해서 외과적 치료를 받았던 23 예을 대상으로 하였다. 2예는 인공판막 심내막염이었고 나머지 21예는 자가판막 심내막염이었다. 환자의 평균 연령은 44.8$\pm$15.7 (11∼66)세였고 연령분포에는 큰 차이가 없었다. 17예에서는 혈역학적 불안정이나 거대 우종으로 응급수술을 시행하였다. 수술 전 12예에서 울혈성 심부전, 4예에서 신부전, 2예에서 비장 및 신장경색, 2예에서 일시적 뇌손상, 1예에서 뇌농양 소견을 보였다. NYHA 기능 분류상 Grade II가 7예, Grade III가 9예, Grade IV가 6예이었다. 술 전 심초음파상 승모판 역류우세가 19예, 협착우세가 4예이었으며, 우종은 20예(86.9%)에서 관찰되었다. 혈액배양 검사상 10예(43.4%)에서 원인균이 동종되었는데 Streptococcus viridans 5예, methicillin sensitive Staphylococcus aureus 2예, 그리고 Corynebacterium, Haemophillis, Gernella 각각 1예씩 이었다. 수술 적응증은 ACC/AHA 지침에 근거하였으며, 평균 외래 추적관찰기간은 27.6$\pm$23.3 (1∼97)개월이었다. 결과: 13예에서 승모판치환술을 시행하였는데 9예에서 기계판막을 사용하였고 4예에서 조직판막을 사용하였다. 10예에서 다양한 수기의 승모판성형술을 시행하였다. 동반수기로는 6예에서 대동맥판 치환술, 2예에서 삼첨판 성형술, 1예에서 변형된 미로수술, 1예에서 심실중격결손 낙합술을 시행하였다. 술 후 합병증으로는 출혈에 의한 재수술 2예, 종격동염 1예, 저심박출증 1예, 페렴 1예가 있었다. 술 후 30일 이내의 조기사망이나 병원 내 사망 및 판막 연관성 합병증은 없었다. 1예의 환자가 수술 3개월 후 다시 판막성형술을 받았고, 술 전 우종 전색에 의한 뇌경색을 보인 1예의 환자가 술 후 31개월에 뇌출혈로 사망하여 만기 전 체판막 연관성 사망률은 4.3%, 판막연관성 합병증은 8.6%였다. 1, 3, 5년 valve-related event free rate는 90.8%, 79.5%, 79.5%이었고 1, 3, 5년 생존율은 100%, 88.8%, 88.8% 이었다. 결론: 승모판 심내막염의 외과적 치료 시에는 감염된 조직의 완전 절제가 중요하며, 감염된 조직을 완전히 제거하면 대체물이나 수술 기법상의 차이가 결과에 미치는 영향은 크지 않을 것으로 판단되고, 술 전에 감염이나 우종 전색에 의한 뇌손상이 의심되는 환자에서는 술 후 추적관찰 중에도 뇌혈관 손상 가능성에 대해 주의 깊게 관찰하여야 할 것으로 생각된다.

        흉부손상 401례에 대한 임상적 관찰

        A clinical evaluation was performed on 402 cases of chest trauma those were admitted and treated at the department of chest surgery, Chonnam National University Hospital, during the past 13 years and 7 months from Jan. 1968 to July 1980. 1. The ratio of male to female was 5.5:1 in 1968 and 3.4:1 in 1980 with progressive increase of female patient and age from 20 to 59 years is 81.0%. 2. The most common cause of chest trauma was stab wound in penetrating trauma and traffic accidents in nonpenetrating trauma. 3. The most common injury from chest trauma was hemothorax in penetrating trauma and rib fracture in nonpenetrating trauma. 4. Hemothorax was observed in 76 cases [18.9%] of total cases and rib fracture was observed in 163 cases [40.5%] of total cases. 5. Rib fracture was prevalent from 4th to 8th rib, and 1st and 2nd rib fractures were associated with major thoracic injuries and with other organ injuries. 6. Open thoracotomy was performed in 72 cases [17.9%] and others were treated with conservative nonoperative treatment. 7. Overall mortality was 4.5% [19 cases], and common causes of the death were shock and respiratory insufficiency.

        단관상동맥에 발생한 관상동정맥루 -치험1례-

        Single coronary artery is rare congenital anomaly with incidence about 0.04%. Above half of single coronary artery is symptomless, and about 41% of cases are associated with another congenital cardiac anomalies . Single coronary artery is sometimes important for cardiac operation to avoid fatal result from cutting of coronary artery during the right ventriculotomy. Among the associated anomaly of the single coronary artery, coronary arteriovenous fistula is very rare. We experienced congenital single coronary artery with coronary arteriovenous fistula drained into the right ventricle, and so we report this case with literatures.

        노걸대(老乞大)와 그 언해서(諺解書)의 이본(異本)

        Nogoldae (老?大), namely Laoch``ita was the most basic textbook used at the Bureau of Interpreters of Foreign Language in the Choson Dynasty. Therefore Nogoldae, and its Korean translation, Nogldae Onhae(老?大諺解) have respectively many different versions. The purpose of this paper is to research these different versions. The oldest version of Nogoldae existing is the wood-cut reprint of Kabin-ja(movable types minted in 1434). Through the frame of version, this may have been published in the beginning of sixteenth century There are two different wood-cut editions which were from the same text in the late sixteenth century or in the early seventeenth century. These three editions respectively have wood-cut editions consisting of one volume. There is a wood-cut edition consisting of two volumes published in 1703. However its text is not different from other editions. There exist two editions; one is Nogoldae Sink(老?大新?), revised version of text in 1763, and the other is Chunggan Nogoldae (重刊老?大) which was re-revised version of text, published in 1795. This is the last edition of Nogldae. Among Korean translations of Nogoldae, Ponyok Nogoldae is the oldest edition. It may have been published in the middle of sixteenth century It is a wood-cut reprint of the Uhae-ja(movable types minted in 1445) edition and was translated by Ch``oe Se-jin(崔世珍) in the 1610``s. The second oldest edition we have is the Musin-ja(movable types minted in 1668) edition. The translation of this version is different from Ulhae-ja edition. The third translated edition which was published at Pyong``yang in 1745 has the modified transcription of Chinese pronunciation from the Musin-ja edition. The fourth and fifth of translations of Nogoldae respectively were published as a translations of Nogoldae Sinsk in 1763 and Chunggan Nogoldae in 1795. Their translations and transcriptions of Chinese pronunciation were different from the previous editions. The translated editions consist of two volumes, however, only the fourth edition consists of three volumes.

        內賜本의 한 연구

        According to records of successive dynasties, there have been the tradition that a king bestowed books published by the government to officials and government divisions since the middle eleventh century of the Kory? dynasty. Many samples concerning such records as well as books circulated by the Chosen government have been survived to this day. In general these books have a record called as naesa-gi (內賜記), which present when and to whom the books were distributed, and royal seals that reads s?nsaji-gi (宣賜之記) or kyujangji-bo (奎章之寶). Those are designated to naesa-bon (內賜本), which means books bestowed and distributed by a king, because the sentence of naesa-gi begins with the two syllables, naesa (內賜). The date of distribution is important for bibliographic studies because it let us know when the book was published. Such books are thought to be valuable as antiques because they were made of good paper, in good printing, and were well mounted when compared to other contemporary prints. In text-criticism, they are highly phrased because they were scrutinized and proofread after publication. This paper deals with several topics such as stages of printing and distribution, writing modes of naesa-gi, and places of royal seals on the text, that have not been fully discussed before. Following to this, the present paper explores the meaning of naesa-bon by investigating them as historical documents in terms of history of Korean linguistics, that is supposed to play an important role for its historical approaches.
