Radiographic Study in Tetralogy of Fallot

Angiocardiography was performed in a total of 150 cases during the period of 11. years from January 1, 1958 to December 31, 1968 at the National Medical Center. 74 cases out of a total 150 cases which were clinically suspected as congenital heart disease were correctly proved to be Tetralogy of Fallot. The results were as follows: 1. The males were 49 and the females, 25 with 5Apatients (73%) before 10 years old. 2. Most of patients complained of exertional dyspnea and cyanosis (100%). 3. Arterial 02 saturation range is decreased as 32-90% (66%). 4. Plain X-ray reveals heart is typical boot shape 58 cases (78.4%) with cardio megaly 63 cases (85.1%) and pulmonary vascularity is decreased 55 cases (74.3%). 5. Angiocardiographic findings are stenosis subdivide as infundibular type 50 cases (67.6%), valvular type 6 cases (8.1%), indefinite type 5 cases (6.8%), and combined type 10 cases (13.5%). 6. Right sided aorta is 15 cases (20%). 7. Angiocardiography reveals pulmonary vascularities are decreased 66 cases (89.9%). 8. Other anomalies are combined 10 cases (13. 5%) such as situs inversus totalis 3 cases, left superior vena cava 3 cases, double superior vena cava 3 cases and polydactylism 1 case. 9. 57 cases (77%) out of a total 74 cases were operation was performed and most of cases are marked improvement after operation. However, not changed 3 cases, and 9 cases are died.

Selective renal angiographies were performed on 15 cases of chronic advanced renal failure, who were admitted to the Hanyang University Hospital during the peiod of May 1976 to September 1977. 1. The cases examined were 10m ale and 5 female patients. Age distribution was broad, most in fourties of age. 2. The main symptoms were dyspnea, generalized edema, general weakness, nausea and vomiting. All of them were anemic. The blood pressure was normal in 1 case, minimally hypertensive in 4 cases, moderately or severely in 10 cases. Chest X-ray revealed cardiomegaly in 9 cases and pulmonary edema in 6 cases. The average values of BUN were 138.2mg/dl, serum creatinine, 20.82mg/dl, and hemoglobin, 6.89gm/dl. 3. Selective renal angiographies revealed abnormalities in all of the cases. The kidneys were shown to be small in 14 cases. With the size of the renal masses and the shape of the renal arteries, the diagnosis of chronic advanced renal disease were possible in these 14 cases. In ne case, one of the kidneys was enlarged, a finding which would have been difficult to prove by other methods. The autopsy of this case revealed renal tuberculosis with hydronephrosis. 4. The diameter of the renal arteries was small in all the kidneys examined. The interlobar arteries were abnormal in shape in all the cases: pruning, 85.2%, tortuosity, 66.7%, spiraling, 96.3%, segmental stenosis, 22.2%, ectasis, 29.6%, and crowding, 81.5%, The arcuate arteries and interlobular arteries were not able to be clearly delineated in most of the cases. The extrarenal vasculature was prominent in 44 per cent of the cases. 6. The selective renal angiography may be a valuable method of screeing out the cases of reversible renal failure.

Since Sept. 1959, a simplified- retrobulbar air insufflations method for orbit examination has been carried out to about 80 patients with the X-ray tomography. As to the technique, about 8-10 cc of the air is injected into- the retrobulbar soft tissue (central space of Charpy) from skin side which is introduced by the 20 gauge needle at the lower part of the external canthus without special anesthesia. The exact contour of the eyeball, optic nerve and muscles are well visualized and it is proved that the condition of newgrowth esp.in retrobulbar space, pathological changes of the optic nerve, the foreign body adjacent to the eyeball can be detected by direct visualization by this method. Also this method has been reliable to decide the site of the operation and the necessity of the enucleation following the extension of the tumor to the ocular tissue from the surrounding parts of the orbit for instance Para nasal sinuses. During about two years, not a single serious complication has been encountered after this examination.
