난소적출이 혈중 FSH 및 LH치에 미치는 영향에 관한 연구

폐경전에 양축난소를 모두 적출하였을 때 혈중 FSH와 LH치의 변동을 적출직후, 적출후 단기간 및 장기간 동안 관찰하고 자연 폐경시의 경우와 비교 검토한 결과 다음과 같은 결론을 얻었다. 1. 난소 적출직후의 변화를 관찰하기 위하여 수술전일부터 수술후 1주일까지의 성선자극호르몬치를 측정한 결과 FSH치는 수술전에 15.3mIU/ml이었으나 수술 24시간부터 급격히 증가하기 시작하여 수술후 제7일에는 51.7mIU/ml까지 계속 증가하였고 LH치는 수술전에 21.7mIU/ml이었으나 수술후에는 비교적 완만한 증가를 보여 수술후 제7일에는 40.8mIU/ml를 나타내었다. 2. 난소적출후 1개월간의 변동추세를 보면 FSH치는 수술후 2주에는 81.8mIU/ml로 급상승하였다가 그 후에는 비교적 증가가 완만하였으며 LH치도 수술후 2주에는 73.4mIU/ml로서 FSH와 비슷하였다. 3. 난소적출후 장기적인 변동을 보면 FSH치는 수술후 1년에 88.3mIU/ml로서 증가추세이나 그 후는 계속적인 감소를 나타내었으며 LH치는 수술후 3개월에 71.6mIU/ml였고 그 후부터는 감소경향을 나타내었다. 4. 자연폐경 1년후의 FSH치는 44.3mIU/ml이고 3년후에는 63.5mIU/ml로 최고치에 도달하였다가 그 후에는 감소하는 추세를 보였고 LH치도 같은 추세로서 폐경 1년후에는 43.4mIU/ml, 3년에는 68.5mIU/ml로 최고치에 도달하였다가 그 후로는 차츰 감소되었다. 이상의 성적으로 보아서 난소적출시에는 성선자극호르몬이 급상승하여 수술 2주후부터 1년후사이에 최고치에 도달하였고 자연폐경시에는 서서히 상승하여 폐경후 3년만에 최고치를 나타냈으나 그 최고치는 난소 적출시에 더욱 높았으며 10년 후에는 모두 비슷한 혈중치 즉 45mIU/ml 전후치를 나타내었다. Immunologic study of gonadotropins in serum was performed in order to determine the effect of oophorectomy in the premenopausal women upon the serum concentrations of gonadotropins and to compare the hormonal levels with those of spontaneous postmenopausal women. The concentrations of gonadotropins were measured by methods based upon the principles of radioimmunoassay. The results of the comparative study of the gonadotropin concentrations before and after oophorectomy in premenopausal women versus spontaneous postmenopausal women were summarized as follows: 1. The gonadotropin concentrations were increased abruptly in immediate postoperative stage to the levels of 51.7mIU/ml in FSH and 40.8mIU/ml in LH in 7th postoperative day comparing with the levels of 15.3mIU/ml and 21.7mIU/ml respectively prior to the operation. 2. In two weeks after the operation, FSH concentration reached to the level of 81.8mIU/ml and LH, of 73.4mIU/ml, which was considered as a peak through 10 years after oophorectomy. 3. In one year after the operation, FSH concentration showed 88.3mIU/ml and LH, 50.5mIU/ml, followed by the gradual decrease to 46.5mIU/ml of FSH and 45.1mIU/ml of LH in 10 years after oophorectomy. 4. In the spontaneous menopause, gonadotropin concentrations were gradually increased to the levels of 63.5mIU/ml of FSH and 68.5mIU/ml of LH at 3 years after the menopause, which were considered as a peak. And the levels of FSH and LH showed gradual decrease to 44.7mIU/ml and 47.2mIU/ml respectively in 10 years after the monopause. In conclusion, gonadotropin concentrations of premenopausal oophorectomized women were increased abruptly and the maximal concentrations were observed in 2 weeks after the operation whereas maximal concentrations in the spontaneous menopause, in 3 years. And their maximal values were significantly higher in oophorectomized women than in the spontaneous menopausal women, but those values after 10 years showed the same levels in both groups.

Interstital pregnancy following homolateral salpingectomy is a rare but well-documented entity. As a second case in Korea a 33-year-old Korean housewife was presented. She had had a left salpingectomy for a ruprured rubal pregnancy two years before this incidence. Pregnancy had been uneventful until seventeenth week of estation when it was spontaneously terminated through cavum uteri resulting in "Tubeneckenplazenta". Immediate hysterectomy with blood replacement was neccessary to save the patient.
