        광범위 외측 도달법을 이용한 관절내 종골 골절의 수술적 치료

        Purpose: This study was designed to investigate the usefulness of extensile lateral approach for accurate reduction and rigid internal fixation in comminuted intraarticular fractures of calcaneus. Materials and Methods: From October 2002 to May 2007, we managed 55 patients (62 cases) with open reduction and internal fixation using extensile lateral approach. Among these, 38 patients (43 cases) who underwent preoperative and postoperative CT scan were enrolled. All patients were evaluated over 24 months after surgery. Bohler angle and Gissane angle on plain X-ray, displacement and step-off of articular surface of calcaneus on CT scan were measured and we compared the difference between preoperative and postoperative value of them. Clinical results were assessed by using AOFAS Ankle-Hindfoot Scale. Results: The average Bohler angle was restored from $6.8^{\circ}$ to $23.5^{\circ}$ and Gissane angle was improved from $116.4^{\circ}$ to $113.5^{\circ}$ after operation. The average distance of displacement was restored from 4.2 mm to 1.4 mm and step-off of articular surface was recovered from 5.1 mm to 1.3 mm. Clinical results were excellent in 17 cases, good in 18 cases, fair in 3 cases, and poor in 5 cases. 10 cases developed postoperative complications such as skin necrosis, heel pain, limitation of motion of ankle and subtalar arthritis. Conclusion: The extensile lateral approach is valuable for the comminuted intraarticular fractures of calcaneus that enables accurate anatomical reduction and rigid internal fixation by providing direct exposure of subtalar joint.

        국소 스테로이드 반복 주사후 발생한 자발적 아킬레스건 파열 -증례 보고-

        It is known that steroids increase the risk for tendon ruptures. Despite this local steroids are still used in the treatment of achilles tendinitis. 46-year-old women had occurred achilles tendon rupture after repeated local steroid injection. Intraoperatively, necrotic change were seen at both side of ruptured end. Like this, tendon rupture after repeated steroid injection often result in a large defect, which needed surgical repair. After debridement of ruptured end, tenorrhaphy with single Krackow method and plantaris tendon augmentation was done. The patient showed favorable result with return to range in a degree of activity levels.

        족관절 골절 후 발생한 원위 경비 인대의 이소성 골형성

        Purpose: To evaluate the effect on clinical course of heterotopic ossification of distal tibiofibular syndesmosis after ankle fractures. Materials and Methods: From June 2001 to May 2004, we found nine cases of heterotopic ossification of distal tibiofibular syndesmosis after ankle fractures. There were 8 male patients and 1 female patient; their mean age was 42 years old. There were 6 Weber type B and 2 Weber type C fractures, and there is 1 case with posterior malleolus fracture only. Among them, 8 ankle fractures were operated. Follow up period was averaged for 14 months. We were able to review radiographs at initial injury and to review clinical menifestation and radiographs at last follow up. We used an ankle-hindfoot scoring system of AOFAS which combined symptom, function and alignment with maximum score of 100 point. Results: In all cases ankle dorsiflexion and plantarflexion were not significantly different from that of the contralateral side. The patients who had developed heterotopic ossification in distal tibiofibular syndesmosis had a similar functional score. The mean ankle-hind foot score was 94 points. Conclusion: We concluded that the heterotopic ossification of distal tibiafibular syndesmosis after ankle fractures had little effect on clinical course and range of motion of ankle joint.

        멀티코어 시스템에서 흐름 수준 병렬처리에 기반한 리눅스 TCP/IP 스택의 성능 개선

        최근 멀티코어가 장착된 시스템이 증가하면서 이를 통한 애플리케이션 성능향상에 대한 노력이 계속 되어왔다. 하나의 시스템에 다수의 처리장치가 존재함으로 인해 프로세싱 파워는 기존보다 증가했지만 기존의 소프트웨어나 하드웨어들은 싱글코어 시스템에 적합하게 설계된 경우가 많아 멀티코어의 이점을 충분히 활용하지 못하고 있는 경우가 많다. 기존의 많은 소프트웨어들은 멀티코어 상에서 공유 자원에 대한 병목현상과 비효율적인 캐시 메모리 사용으로 인하여 충분한 성능향상을 기대하기 어려우며 이러한 문제점들로 인하여 기존 소프트웨어는 코어의 개수에 비례한 성능을 얻지 못하며, 최악의 경우 오히려 감소될 수 있다. 본 논문에서는 TCP/IP를 사용하는 기존의 네트워크 애플리케이션과 운영체제에 흐름 수준 병렬처리 기법을 적용하여 성능을 증가 시킬 수 있는 방법을 제안한다. 제안된 방식은 개별 코어단위로 네트워크 애플리케이션, 운영체제의 TCP/IP 스택, 디바이스 드라이버, 네트워크 인터페이스가 서로 간섭 없이 작동할 수 있는 환경을 구성하며, L2 스위치를 통해 각 코어 단위로 트래픽을 분산하는 방법을 적용하였다. 이를 통해 각 코어 간에 애플리케이션의 데이터 및 자료구조, 소켓, 디바이스 드라이버, 네트워크 인터페이스의 공유를 최소화하여, 각 코어간의 자원을 차지하기 위한 경쟁을 최소화하고 캐시 히트율을 증가시킨다. 이를 통하여 8개의 멀티코어를 사용하였을 경우 네트워크 접속속도와 대역폭이 코어의 개수에 따라 선형적으로 증가함을 실험을 통해 입증하였다. With increasing multicore system, much effort has been put on the performance improvement of its application. Because multicore system has multiple processing devices in one system, its processing power increases compared to the single core system. However in many cases the advantages of multicore can not be exploited fully because the existing software and hardware were designed to be suitable for single core. When the existing software runs on multicore, its performance improvement is limited by the bottleneck of sharing resources and the inefficient use of cache memory on multicore. Therefore, according as the number of core increases, it doesn't show performance improvement and shows performance drop in the worst case. In this paper we propose a method of performance improvement of multicore system by applying Flow-Level Parallelism to the existing TCP/IP network application and operating system. The proposed method sets up the execution environment so that each core unit operates independently as much as possible in network application, TCP/IP stack on operating system, device driver, and network interface. Moreover it distributes network traffics to each core unit through L2 switch. The proposed method allows to minimize the sharing of application data, data structure, socket, device driver, and network interface between each core. Also it allows to minimize the competition among cores to take resources and increase the hit ratio of cache. We implemented the proposed methods with 8 core system and performed experiment. Experimental results show that network access speed and bandwidth increase linearly according to the number of core.

        이온보조반응법에 의한 Polyethylene(PE) 표면의 친수성 증가

        고분자 Polyethylene(PE) film 표면을 1 keV의 산소 이온 빔만을 사용하여, 또는 산소 분위기에서 1 keV의 아르곤 이온 빔으로 조사하는 이온보조반응(ion assisted reaction)법을 이용하여 개질을 하였다. 개질된 polyethylene 표면의 친수성(wettability)과 표면 에너지를 측정하기 위해 접촉각 측정기(contact angle micrometer)를 사용하였으며, 개질 된 표면의 화학적 변화를 측정하기 위해 x-ray photoelectron spectroscopy(XPS)를 이용하였다. 표면 개질을 위한 이온 조사량은 1×10¹⁴-1×10^(17) ions/㎠, 산소 가스는 0~4 sccm(ml/min.) 까지 변화 시켰다. 산소 이온 빔만으로 조사 후 polyethylene 표면과 물과의 접촉각은 95˚에서 최대 62˚까지 변화하였으며, 산소를 4 sccm(ml/min.) 주입하면서 아르곤 이온 빔으로 조사하면 물과의 접촉각이 최대 28˚까지 감소하였으며, 이 때 이온 조사량은 1×10^(17) ions/㎠ 이었다. 또한 polyethylene을 산소 이온 빔으로만 표면 개질시 표면 에너지는 작은 증가를 보였으며, 산소 분위기에서 아르곤 이온 빔으로 표면 개질 하였을 때에는 표면 에너지가 2배에 가까운 증가를 하였다. 이와 같은 표면의 친수성과 표면 에너지의 증가는 이온보조반응법에 의해 polyethylene의 표면을 산소 분위기에서 아르곤 이온 빔으로 조사한 시료의 XPS의 spectra 결과로 보아 PE의 표면에 C-O 또는 C=O와 관련된 결합의 증가로 인한 친수성 작용기가 polyethylene 표면에 형성되었기 때문이라고 사료된다. Surface of polyethylene film was modified by ion assisted reaction in which ion beam is irradiated on polymer in reactive gas environments. Ion (argon and oxygen) beam energy was 1 keV, doses were varied from 1×10¹⁴-1×10^(17) ions/㎠, and amount of blowing oxygen from 0 to 4 sccm(ml/min). Wettability was measured by water contact angle measurement, and the surface functionality was analyzed by x-ray photoelectron spectroscopy. The contact angles of water to polyethylene modified by oxygen ion beam only decrease from 95 to degrees, and surface energy was not changed much. The contact angles remarkably decrease to 28 degrees and surface energy increase to 67 erg/㎠ when the films were modified by argon ion with various ion doses with blowing oxygen gases near the polyethylene surface. Improvement of wettability and surface energy are mainly due to the new functional group formation such as C-O or C=O, which are known as hydrophilic groups from the XPS analysis, and the ion assisted reaction is very effective to attach oxygen atoms to form functional groups on C-C bond chains of polyethylene.

        족부와 족관절의 재발한 총상 신경초종

        신경초종은 Schwann 세포에서 기원하는 양성 종양이다. 총상 신경초종은 총상형이나 다결정성의 성장 형태를 보이는 신경초종의 드문 아형이다. 총상 신경초종은 대부분은 피부와 피하조직에 고립성 병변(solitary lesion)을 보이지만 드물게 심부조직 내에 위치하기도 한다. 저자들은 매우 드물게 후경골 신경 및 원위부 분지 신경에서 발생하였으며, 심부조직에 위치하여 골 변형을 동반한 다발성 총상 신경초종이 재발한 1예를 경험하였기에 이를 보고하고자 한다. Schwannomas are benign neoplasms with a Schwann cell origin. A plexiform schwannoma is a rare variant of a schwannoma with a plexiform or multinodular growth pattern. The condition occurs mostly as a solitary lesion in the skin or subcutaneous tissue, or uncommonly located in the deep soft tissue. We report a rare case of recurred multiple plexiform schwannomas arising from the posterior tibial nerve and its branch, which was located in a deep anatomic location and accompanied by a bony deformity.

      • 오리엔트종 연초의 생육 및 화학특성에 미치는 이식시기의 영향

        Oriental tobacco (KA 101) was transplanted from Mar. 25 to May 5 with 10 days interval in 1984 and 1985, and its agronomic characteristics, chemical properties ware compared to determine the Proper transplanting time of aromatic tobacco In Korea. As the transplanting was delayed, days to flowering of plant was shortened, length and width of largest leaf, leaf area index tended to decrease. Yield was highest for the Apr. 5th transplanting followed by Mar. 25th planting, then decreased as the planting date was delayed. Quality by price decreased as the transplanting was delayed later than Apr. 25. Delaying transplanting increased nicotine, total nitrogen and ash content, but decreased reducing sugar and petroleum ether extract of cured leaves, The content of volatile organic acids such as 3-methyl pentanoic acid was lower when it was transplanted later than Apr. 25th. Neophytadiene content increased as the transplanting was delayed, but there were no trends with the content of alcohols, aldehydes esters and ketones. Several quality indices including the ratio between the content of volatile organic acids plus petroleum ether extract and ash content plus pH was higher for the Apr. 5th transplanting.
