        CASE REPORT : A rare cause of dysphagia: compression of the esophagus by an anterior cervical osteophyte due to ankylosing spondylitis

        Ankylosing spondylitis (AS) is a chronic inflammatory rheumatological disease affecting the axial skeleton with various extra-articular complications. Dysphagia due to a giant anterior osteophyte of the cervical spine in AS is extremely rare. We present a 48-year-old male with AS suffering from progressive dysphagia to soft foods and liquids. Esophagography showed an anterior osteophyte at C5-C6 resulting in esophageal compression. The patient refused surgical resection of the osteophyte and received conservative therapy. However, after 6 months there was no improvement in dysphagia. This case illustrates that a large cervical osteophyte may be the cause of dysphagia in patients with AS and should be included in the diagnostic workup in early stages of the disease.

        Usage of complementary and alternative medicine in women with urinary incontinence at a hospital in Turkey

        Background: Urinary incontinence (UI) is a health problem that is common in women of all ages. Besides pharmacological and surgical treatments, there are lifestyle changes and complementary and alternative medicine (CAM) to relieve UI symptoms. This study aimed to examine lifestyle arrangements and CAM use by women with UI. Methods: We conducted a cross-sectional study using data from 352 women with UI. The study sample consisted of female patients that were aged 18 and above. We asked patients whether they experienced urine leakage, and included all patients with UI in the sample regardless of UI type or severity. We collected the study data by using the personal information form and Incontinence Severity Index (ISI). Results: In this study, 7.1% of women with UI used CAM while 92.9% did not. We found that only the women with mixed incontinence used CAM more (p < 0.05). The CAM techniques commonly preferred by women included prayer (48.0%), hot application (36.0%) and herbal teas (24%). While 52.2% of women stated that they benefited from CAM use, all of them (100%) stated that they experienced no side effects of CAM. The most common lifestyle changes was losing weight. Conclusion: Turkish women with UI had a low rate of CAM use in this study. The use of CAM was related to age and education, and women with mixed UI used CAM more.

        Beneficiation of alunitic kaolin within the floor and wall tile angobe compositions

        Most of the kaolin in Bal kesir-S nd rg (Turkey) region cannot be used in ceramic industry because of its high alunite content. The alunite within the composition of kaolin has to be removed in order them to be used in ceramic industry. Therefore, this research aimed at removing alunite from the kaolins of Balikesir-Sindirgi region and investigating its possible use in ceramic industry. For this purpose, mechanical scrubbing-screening, flotation and calcination methods were applied to the kaolins of the region to remove alunite. The material obtained was then used in the floor and wall tile angobe recipes of a ceramic tile factory. The angobes, prepared using the factory’s standard angobe recipes, were applied to the floor and wall tile bodies of the factory and later they were glazed. After the tests, it was understood that F2 angobe prepared by M2 kaolin, beneficiated through the flotation process and F3 angobe prepared by C3 kaolin, beneficiated through calcination could be alternatives for the commercial angobe currently used by the factory in floor tile production. Similarly, it was proven that W2 and W3 angobes prepared by using M2 and C3 kaolins, respectively, could be seen as alternatives for the angobe currently used by the factory in wall tile production.

        Highly efficient removal from aqueous solution by adsorption of Maxilon Red GRL dye using activated pine sawdust

        Pine sawdust, to which sulfuric acid was applied (APSD), was utilized as an inexpensive adsorbent to perform the batch adsorptive removal from a synthetic dye solution of Maxilon Red GRL (MR GRL). The activated adsorbent was characterized by the points of zero charge, FTIR, N2 adsorption-desorption, SEM, and SEM-EDX analyses. According to the removal results, the highest efficiency of the dye adsorption was reached at 180min. MR GRL removal of 99.35% was achieved using APSD under the optimum conditions (pH=5.7-6.0, temperature=298 K, dye concentration= 250mg/L, and adsorbent dosage=8 g/L). The Langmuir isotherm represents the best explanation model for the experimental data, which has the highest adsorption capacity of 312.5mg/g at 318 K. The compatibility of adsorption with the Langmuir isotherm showed that adsorption was reversible and physical. The other results obtained confirmed this situation. The kinetic research demonstrated that the sorption process was realized in accordance with the pseudo-second- order kinetic model. The thermodynamic parameters revealed that the MR GRL adsorption occurred spontaneously and was exothermic. The findings of the present research confirm that acid-activated pine sawdust may be utilized to remove MR GRL dye from aqueous solutions as a low-cost and efficient adsorbent.

        Circadian Rhythms Characteristics of Nurses Providing Direct Patient Care: An Observational Study

        Background In today's modern world, longer working hours, shift work, and working at night have become major causes of the disruption of our natural circadian rhythms. This study aimed to investigate the effects of the type of shift work (rotating vs. fixed day), duty period (on-duty vs. off-duty), and working period within each shift (nighttime vs. daytime) on the circadian rhythm characteristics of nurses who provide direct patient care. Methods This cross-sectional study used a purposive sampling method. Cosinor analysis was applied to analyze the actigraphy data of nurses providing direct patient care for seven consecutive days. The linear mixed effects model was then used to determine any variances between shift type, duty period, and working period within each shift for the nurses. Results The mesor value did not differ according to nurses' shift type, duty period, and working period within each shift. The amplitude was statistically higher in on-duty nurses and in daytime working hours. The acrophase was significantly delayed in nighttime working hours. As well as nurses in rotating shift had experience. Conclusion Our findings revealed that the peak activity of nurses occurs significantly later at night while working and nurses working during nighttime hours may have a weaker or less distinct circadian rhythm. Thus, this study suggests that limits be placed on the number of rotating nighttime shifts for nurses.         Rare Cause of Hematochezia in Children: Solitary Rectal Ulcer, Single Center Experience

        Background/Aims: Solitary rectal ulcer syndrome (SRUS) can be overlooked, diagnosed late, or misdiagnosed, particularly in childhood. This study reviewed the 13-year experience of the authors’ institution to increase clinicians' awareness of SRUS in the presence of symptoms. This paper reports the endoscopic and histopathological findings in children presenting with hematochezia. Methods: The clinical and laboratory findings of 22 patients diagnosed with biopsy-proven SRUS in the authors’ clinic between 2007 and 2020 were evaluated retrospectively. Results: The mean age at diagnosis was 12.5±2.6 years, and 59.1% of the patients were male. The median time of diagnosis was 24 months. A single ulcer lesion was found by colonoscopy in 18 patients (81.8%), two ulcers in two patients (9%), and more than two ulcers in two patients (9%). The pathology reports of all biopsies taken from the lesions were consistent with a solitary rectal ulcer. In the first stage, the treatment was started with toilet training, a high-fiber diet, and laxatives. In 11 patients (50%) who did not respond to the initial treatment, a 5-ASA enema was added. A glucocorticoid enema was added to treatment in five patients (22%) whose complaints did not regress despite this treatment. Clinical remission was achieved in five of the patients (18.1%). The time to diagnosis was significantly shorter in those in remission than those not in remission (p=0.04). Conclusions: This study is the first large series on Turkish children. An increased awareness of SRUS in children will increase the rate of early diagnosis and treatment, allowing remission in more patients.

        Preparation and Electrochemical Performances of Graphene Oxide/PEDOT and Reduced Graphene Oxide/PEDOT Nanofibers and Nanocomposites

        In this work, conductive polymer poly(3,4-ethylenedioxythiophene) (PEDOT) was synthesized on the surface of graphene oxide (GO) and reduced graphene oxide (rGO) sheets via in-situ microemulsion polymerization process to achieve graphene oxide-poly(3,4-ethylenedioxythiophene) (GO-PEDOT) and reduced graphene oxide-poly(3,4-ethylenedioxythiophene) (rGO-PEDOT) nanocomposites. Nanofibers of PEDOT, GO-PEDOT, rGO-PEDOT and GO on poly(acrylonitrile-costyrene) (P(AN-co-St)) matrice have also been fabricated due to their high potential in using as electrodes for flexible supercapacitors. As demonstrated by scanning electron microscope, atomic force microscope and spectroscopic techniques, PEDOT has been succesfully synthesized on the surface of the GO and rGO sheets. The SEM results demonstrate that nanofibers have beadless structure with a diameter range less than 300 nm. The electrochemical capacitive properties of the nanofibers were investigated by using electrochemical impedance spectroscopy. The electrochemical measurements reveal that GO enhances capacitive behavior of PEDOT nanofibers more than rGO, but both GO and rGO improve the electrochemical performance of the nanofibers.

        Expression of matrix metalloproteinase-2 and survivin in endometrioid and nonendometrioid endometrial cancers and clinicopathologic significance

        Objective: To determine matrix metalloproteinase-2 and survivin expressions in endometrial cancers, their relation to clinical and histologic parameters and to investigate any difference in the expression of these markers between endometrioid and nonendometrioid cancers. Methods: Ninety-five patients with endometrial cancer, were included. Matrix metalloproteinase-2 and survivin expressions were analyzed immunohistochemically from paraffin-embedded tissues by using specific monoclonal antibodies. Results: Survivin nuclear expression was higher in endometrioid cancer as compared to nonendometrioid cancer (p=0.040), but there was no difference for cytoplasmic survivin and matrix metalloproteinase-2 expressions between type I and type II carcinomas. Survivin cytoplasmic staining was significantly lower in patients with deep myometrial invasion (p=0.038). Nuclear expression of survivin is decreased in histologic grade 3 tumors compared to grade 1 and 2 tumors (p=0.013), but there is no difference between grade 1 and 2. We did not find any statistically significant difference between survivin or matrix metalloproteinase-2 expressions and survival. Conclusion: Survivin and matrix metalloproteinase-2 are present in endometrioid and nonendometrioid cancers. Grade 1 and 2 tumors and carcinomas having myometrial invasion less than 50% have higher survivin expression. These results supports that, survivin may play an important role in early stage tumors and early phases of tumor development. We did not find any association between matrix metalloproteinase-2 expression and classical prognostic factors in endometrial cancer and both proteins were not associated with survival.         Impact of root canal curvature and instrument type on the amount of extruded debris during retreatment

        Objectives: The aim of the current study was to assess whether the amount of extruded debris differs for straight and severely curved root canals during retreatment using H-files, R-Endo, Reciproc and ProTaper Universal Retreatment (PTU-R) files. Additionally, the area of residual filling material was evaluated. Materials and Methods: Severely curved (n = 104) and straight (n = 104) root canals of maxillary molar teeth were prepared with WaveOne Primary file and obturated with gutta-percha and AH Plus sealer. Root canal filling materials were removed with one of the preparation techniques: group 1: H-file; group 2: R-Endo; group 3: Reciproc; group 4: PTU-R (n = 26). The amount of extruded material and the area of the residual filling material was measured. The data were analyzed with 2-way analysis of variance (ANOVA) and 1-way ANOVA at the 0.05 significance level. Results: Except for Reciproc group (p > 0.05), PTU-R, R-Endo, and H-file systems extruded significantly more debris in severely curved canals (p < 0.05). Each file system caused more residual filling material in severely curved canals than in straight ones (p < 0.05). Conclusions: All instruments used in this study caused apical debris extrusion. Root canal curvature had an effect on extruded debris, except for Reciproc system. Clinicians should be aware that the difficult morphology of the severely curved root canals is a factor increasing the amount of extruded debris during the retreatment procedure.
