      • In vitro maturation of cynomolgus ovarian follicles

        목적: In vitro ovarian follicular growth (IVFG) is an important issue from the perspective of artificial ovary engineering and fertility preservation. Most of currently known IVFG conditions have been established using mice models, however, they have significantly different reproductive physiology compared to human. Therefore, it is necessary to use non-human primate models, which possess more similar characteristics of reproductive physiology to those of human. In this study, we tried to establish an optimal in vitro growth condition of cynomolgus monkey ovarian pre-antral follicles using different combination of gonadotropins. 방법: Under general anesthesia, ovaries of 3-year-old female cynomolgus monkey (Macaca fascicularis) were excised and collected. Pre-antral follicles were mechanically separated and the follicles containing layers of granulosa cells with a centrally located, visible oocyte, were selected for the experiments. These pre-antral follicles were cultured in droplets of 300 μL media containing nine types of FSH and LH combination and incubated for 14 days. Media was changed by refreshing half of the medium every other day. Morphological growth of the cultured follicles was evaluated under an inverted microscope and oocytes were matured further in vitro for 2 days in the presence of human chorionic gonadotropin (hCG). 결과: The characteristics of follicular growth were compared among the tested FSH and LH combination groups. In vitro grown follicles demonstrated visible expansion of granulosa and theca cells during culture period. The addition of 100-200 mIU FSH was superior to 10 mIU FSH. LH co-treatment showed better IVFG outcomes. From these groups, mature oocytes were successfully retrieved. 결론: Conclusively, we demonstrated cynomolgus monkey pre-antral follicles can be grown into mature follicles in vitro using specific FSH and LH combinations (HI14C2289).

        Added prognostic value of CT characteristics and IASLC/ATS/ERS histologic subtype in surgically resected lung adenocarcinomas

        <P><B>Abstract</B></P> <P><B>Objectives</B></P> <P>Our study investigates the added value of computed tomography (CT) characteristics, histologic subtype classification of the International Association for the Study of Lung Cancer (IASLC)/the American Thoracic Society (ATS)/the European Respiratory Society (ERS), and genetic mutation for predicting postoperative prognoses of patients who received curative surgical resections for lung adenocarcinoma.</P> <P><B>Materials and methods</B></P> <P>We retrospectively enrolled 988 patients who underwent curative resection for invasive lung adenocarcinoma between October 2007 and December 2013. Cox’s proportional hazard model was used to explore the risk of recurrence-free survival, based on the combination of conventional prognostic factors, CT characteristics, IASLC/ATS/ERS histologic subtype, and epidermal growth factor receptor (EGFR) mutations. Incremental prognostic values of CT characteristics, histologic subtype, and EGFR mutations over conventional risk factors were measured by C-statistics.</P> <P><B>Results</B></P> <P>During median follow-up period of 44.7 months (25th to 75th percentile 24.6-59.7 months), postoperative recurrence occurred in 248 patients (25.1%). In univariate Cox proportion hazard model, female sex, tumor size and stage, CT characteristics, and predominant histologic subtype were associated with tumor recurrence (P < 0.05). In multivariate Cox regression model adjusted for tumor size and stage, both CT characteristics and histologic subtype were independent tumor recurrence predictors (P < 0.05). Cox proportion hazard models combining CT characteristics or histologic subtype with size and tumor stage showed higher C-indices (0.763 and 0.767, respectively) than size and stage-only models (C-index 0.759, P > 0.05).</P> <P><B>Conclusion</B></P> <P>CT characteristics and histologic subtype have relatively limited added prognostic values over tumor size and stage in surgically resected lung adenocarcinomas.</P> <P><B>Highlights</B></P> <P> <UL> <LI> In surgically resected adenocarcinomas, prognostic factors were investigated. </LI> <LI> EGFR mutation were not associated with tumor recurrence. </LI> <LI> CT characteristics and histologic subtype were independent predictors of recurrence. </LI> <LI> Both had added prognostic values over tumor size and stage. </LI> </UL> </P>

        Comparison of Sorafenib versus Hepatic Arterial Infusion Chemotherapy-Based Treatment for Advanced Hepatocellular Carcinoma with Portal Vein Tumor Thrombosis

        Background/Aims: Sorafenib is the first approved systemic treatment for advanced hepatocellular carcinoma (HCC). However, its clinical utility is limited, especially in Asian countries. Several reports have suggested the survival benefits of hepatic arterial infusion chemotherapy (HAIC) for advanced HCC with main portal vein tumor thrombosis (PVTT). This study aimed to compare the efficacy of sorafenib-based therapy with that of HAIC-based therapy for advanced HCC with main PVTT. Methods: Advanced HCC patients with main PVTT treated with sorafenib or HAIC between 2008 and 2016 at Korea University Medical Center were included. We evaluated overall survival (OS), time-to-progression (TTP), and the disease control rate (DCR). Results: Seventy-three patients were treated with sorafenib (n=35) or HAIC (n=38). Baseline characteristics were not significantly different between groups, except the presence of solid organ metastasis (46% vs 5.3%, p<0.001). The median OS time was not significantly different between the groups (6.4 months vs 10.0 months, p=0.139). TTP was longer in the HAIC group than in the sorafenib group (2.1 months vs 6.2 months, p=0.006). The DCR was also better in the HAIC group than in the sorafenib group (37% vs 76%, p=0.001). Subgroup analysis, which excluded patients with extrahepatic solid organ metastasis, showed the same trends for the median OS time (8.8 months vs 11.1 months, p=0.097), TTP (1.9 months vs 6.0 months, p<0.001), and DCR (53% vs 81%, p=0.030). Conclusions: HAIC-based therapy may be an alternative to sorafenib for advanced HCC with main PVTT by providing longer TTP and a better DCR. (Gut Liver 2021;15:284-294)

        Molecular and Cellular Characteristics of the Colonic Pseudo-obstruction in Patients With Intractable Constipation

        Background/Aims Chronic intestinal pseudo-obstruction (CIPO) is a disorder characterized by recurrent symptoms suggestive of obstruction such as abdominal pain, proximal distension with extremely suppressed motility in the absence of lumen-occluding lesion, whose etiology/ pathophysiology is poorly understood. In this study we investigated a functionally obstructive lesion that could underlie symptoms of CIPO. Methods We studied colons surgically removed from 13 patients exhibiting clinical/pathological features of pseudo-obstruction but were unresponsive to standard medical treatments. The colons were characterized morphologically, functionally and molecularly, which were compared between regions and to 28 region-matched controls obtained from colon cancer patients. Results The colons with pseudo-obstruction exhibited persistent luminal distension proximally, where the smooth muscle was hypertrophied with changes in the cell phenotypes. Distinct luminal narrowing was observed near the distal end of the dilated region, close to the splenic flexure, previously referred to as the “transition zone (TZ)” between the dilated and non-dilated loops. Circular muscles from the TZ responded less to depolarization and cholinergic stimulation, which was associated with downregulation of L-type calcium channel expression. Smooth muscle contractile protein was also downregulated. Myenteric ganglia and neuronal nitric oxide synthase (nNOS) positive cells were deficient, more severely in the TZ region. Interstitial cells of Cajal was relatively less affected. Conclusions The TZ may be the principal site of functional obstruction, leading to proximal distension and smooth muscle hypertrophy, in which partial nNOS depletion could play a key role. The neuromuscular abnormalities probably synergistically contributed to the extremely suppressed motility observed in the colonic pseudo-obstruction. (J Neurogastroenterol Motil 2015;21:560-570)

        Development of 3-D Stereotactic Localization System and Rediation Measurement for Stereotactic Radiosurgery

        본 연구는 방사선 수술시 필수적으로 이용되는 환자의 정위적 데이터 및 방사선 측정 데이터를 효율적으로 처리하여 종래의 시행착오적 방식을 통한 수술 계획상의 오류 및 부정확성을 지양하고 능률적이고 효율적인 수술이 될 수 있도록 하는데 목적이 있다. 정위시스템 개발을 위하여 Fisher 프레임이 사용되었으며, 뇌혈관 촬영 및 CT의 정위적 방법에 의하여 병변의 위치를 3 차원적으로 정확히 얻을 수 있는 알고리듬을 개발하였다. 정위시스템 과정은 컴퓨터에 의해 쉽게 운열될 수 있도록 프로그램화하였으며, 환자 영상을 컴퓨터에 입력하는 과정, 입력된 영상으로부터 병소 및 위치측정 보조기구 (localizer)에 대한 정보를 추출하는 과정, 입력된 영상의 위치 정보로부터 병소의 3 차원적 정위좌표로 변환하는 과정 등으로 나누었다. 좌표 변환 계산은 실시산에 구해졌으며, CT 및 혈관촬영술의 정위적 방법에 의하여 얻어진 병소의 3차원 좌표를 서로 비교한 결과 오차는 통상적으로 인정되는 2 mm 이내였다. NEC 6 MVX 선형 가속기를 사용하여 소조사면 선량을 측정하였다. 사용된 측정기로는 전리함(ion chamber), 필름, 다이오드 등이며, 측정된 값은 출력 계수, 깊이선량률 (PDD), 최대조직선량비(TMR), 축선량비 (OAR) 등이다. 측정기의 종류에 따라, 측정치가 조금씩 상이했으나, 전반적으로 같은 경향을 보여 줌으로 써 측정치의 신뢰도를 확인할 수 있었다. 제작된 구형 물팬톰과 필름을 이용하여 표준 방사선 치료시 예측했던 선량 분포가 실제 나오는가를 실험을 통하여 확인하였으며, 예측한 선량 값을 얻을 수 있었다. 본 연구를 통하여 개인용컴퓨터에 의해 병소의 정위좌표를 정확히 얻을 수 있는 프로그램이 개발되었고, 소조사면을 간단하고 편리하게 잴 수 있는 방법을 개발함으로써 방사선 수술에 많은 도움이 될 것으로 사료된다. The purpose of this research is to develop stereotactic localization and radiation measurement system for the efficient and precise radiosurgery. The algorithm to obtain a 3-D stereotactic coordinates of the target has been developed using a Fisher CT or angio localization. The procedure of stereotactic localization was programmed with PC computer, and consists of three steps: (1) transferring patient images into PC; (2) marking the position of target and reference points of the localizaer from the patient image; (3) computing the stereotactic 3-D coordinates of target associated with position information of localizer. Coordinate transformation was quickly done on a real time base. The difference of coordinates computed from between Angio and CT localization method was within 2 mm, which could be generally accepted for the reliability of the localization system developed. We measured dose distribution in small fields of NEC 6 MVX linear accelerator using various detector; ion chamber, film, diode. Specific quantities measured include output factor, percent depth dose (PDD), tissue maximum ratio (TMR), off-axis ratio (OAR). There was small variation of measured data according to the different kinds of detectors used. The overall trends of measured beam data were similar enough to rely on our measurement. The measurement was performed with the use of hand-made spherical water phantom and film for standard arc set-up. We obtained the dose distribution as we expected. In conclusion, PC-based 3-D stereotactic localization system was developed to determine the stereotactic coordinate of the target. A convenient technique for the small field measurement was demonstrated. Those methods will be much helpful for the stereotactic radiosurgery.

      • Mind bomb-1 Is Essential for Intraembryonic Hematopoiesis in the Aortic Endothelium and the Subaortic Patches

        <B>ABSTRACT</B><P>Intraembryonic hematopoiesis occurs at two different sites, the floor of the aorta and subaortic patches (SAPs) of the para-aortic splanchnopleura (P-Sp)/aorta-gonad-mesonephros (AGM) region. Notch1 and RBP-jκ are critical for the specification of hematopoietic stem cells (HSCs) in Notch signal-receiving cells. However, the mechanism by which Notch signaling is triggered from the Notch signal-sending cells to support embryonic hematopoiesis remains to be determined. We previously reported that Mind bomb-1 (Mib1) regulates Notch ligands in the Notch signal-sending cells (B. K. Koo, M. J. Yoon, K. J. Yoon, S. K. Im, Y. Y. Kim, C. H. Kim, P. G. Suh, Y. N. Jan, and Y. Y. Kong, PLoS ONE 2:e1221, 2007). Here, we show that intraembryonic hematopoietic progenitors were absent in the P-Sp of <I>Mib1</I><SUP>−/−</SUP> embryos, whereas they were partly preserved in the <I>Tie2-cre</I>; <I>Mib1</I><SUP><I>f</I></SUP><SUP>/<I>f</I></SUP> P-Sps, suggesting that Mib1 plays a role in the endothelium and the SAPs. Interestingly, <I>dll1</I> and <I>dll4</I>/<I>Jag1</I> are expressed in the SAPs and the endothelium of the AGM, respectively, where <I>mib1</I> is detected. Indeed, Notch signaling was activated in the nascent HSCs at both sites. In the P-Sp explant culture, the overexpression of Dll1 in OP9 stromal cells rescued the failed production of hematopoietic progenitors in the <I>Mib1</I><SUP>−/−</SUP> P-Sp, while its activity was abolished by Mib1 knockdown. These results suggest that Mib1 is important for intraembryonic hematopoiesis not only in the aortic endothelium but also in the SAPs.</P>

      • Optimal conditions for in vitro maturation of marmoset oocytes

        Objective: Non-human primate, common marmoset, has many advantages as an animal model for studying reproductive biomedicine because of its short gestation period and similar reproductive physiology to human. The investigators attempted to search for the optimal conditions of in vitro maturation for marmoset oocytes. Methods: Ovaries of 34-month-old female common marmoset (Callithrix jacchus) were excised and oocytes were retrieved. The oocytes were in vitro matured either in MEM (Invitrogen) universal or IVF media (Orgio) in the presence of epidermal growth factor and human chorionic gonadotropin (hCG) for up to 48 hours. Maturity of oocytes was graded and analyzed among the groups of various concentrations. RNAs were extracted from matured oocytes, and oocyte-specific gene expression was assessed by single cell qRT-PCR. Results: The maturation rate was highest in the universal media group treated with 100 IU/L hCG. Quantitative RT-PCR profiling demonstrated that the expression of bmp15 and zp1 gene was regulated in this group (P<0.05). Conclusion: In conclusion, the investigators elucidated the optimal conditions for in vitro maturation of marmoset monkey oocytes (2016R1E1A1A01943455).
