      • How to increase young OB/GYN doctors? Trials of JSOG “Project Plus One”

        The membership of JSOG is about 16,000 people. Sixty percent of the new membership are female doctors. In order to maintain the QOL of Ob/Gyn doctors, it is considered necessary to ensure an annual 500 new members to the society. However, the number of new Ob/Gyn candidates and maternity hospitals have decreased remarkably due to some events, such as the new training programs for junior resident begun in 2004, the incident at Ohno Hospital in 2006 and the maternal acceptance denial incidents in Nara and Tokyo. For this reason we went recruiting. Efforts included 1) an exchange program of young doctors between JSOG and ACOG, SOGC, KSOG, TAOG, DGGG, 2) promotional DVD creation, 3) revision of the Society home page, 4) summer school, 5)publication of newsletters “Reason for your choice,”6)publication of the free paper “Anetis,”7) a spring forum, 8) a resident education program in the annual congress, and 9) scholarships. Some administrative changes were also conducted, such as childbirth allowance, the addition of a management fee for high-risk pregnant women, and high-risk delivery, and an obstetric medical compensation system. These various innovations have increased the number of newly admitted doctors since 2004, steadily increasing until 2010, when 491 people were enrolled. However, since then the number has begun to decrease again. The future vision committee in JSOG launched “Project Plus One,” involving 11 new businesses that specialize in recruiting in two years. These businesses include, 1) Project "Plus One" seminar (ALSO, endoscopic or ultrasound hands-on seminars), 2) Directors’ of meeting, 3) Student forum at the annual meeting of JSOG, 4) establishment of young doctors’ meeting (JTOG), 5) effective use of free paper “Anetis,” 6) creation of a new promotional video, 7) JSOG established five information graphics showing the world-class quality of Japan’s obstetric and gynecological medicine, 8) new health handbook " HUMAN + : dictionary for woman and man ", 9) new health handbook" Baby + : text book of pregnancy and childbirth made by doctors ", 10) Exhibitions at private medical office “Rejinabi” , 11) creation of a JSOG mascot character. By these numerous attempts that have been conducted by the JSOG, we were able to put the brakes on the decrease in number of new candidates, and it has been demanded that further efforts be taken in the future.

        First Clinical Trial of a Newly Developed, Low Menaquinone-7, Fermented Soybean Natto in Warfarin-Dependent Patients

        Bacillus subtilis fermented soybeans (natto) contain high vitamin K2 levels, mostly as menaquinone-7 (MK-7), and must be avoided by warfarin-dependent patients. This is the first report which demonstrates the characteristics and clinical relevance of a low MK-7 natto for such patients. We generated a novel, mutant B. subtilis strain TTCC2051 with short-term fermentation and reduced MK-7 production, yielding 19–24% of the normal MK-7 content. After functional assessments and a preclinical trial, 10 warfarin-dependent patients underwent a clinical trial with a 7-day ingestion test of the low MK-7 natto. Functional assessments were satisfactory, and the preclinical trial showed no increases in plasma MK-7 levels after 7 days of ingestion. In the clinical trial, 20 g/day of the low MK-7 natto significantly increased plasma MK-7 levels while 10 g/day did not. However, neither dose of low MK-7 natto changed international normalized ratio of prothrombin time (PT-INR) values in either group. The low MK-7 natto neither changed PT-INR values nor precipitated adverse events if ingested with a once-daily maximum of 20 g (46 μg of MK-7). Thus, this novel food product has potential for consumption by warfarin-dependent patients.
