        우울증과 알코올중독간의 상관성에 대한 메타분석

        Objectives : This study was designed to integrate the results of community based studies which assessed a relationship between depression and alcoholism by meta-analysis. Methods : We identified the previons studies and included in meta-analysis by searching MEDLINE. Overall, 21 results of the studies for relationship between depression and alcoholism were selected for quantitative meta-analysis. Before the integra-tion of the each effect size of the relationship between depression and alcoholism, a homogeneity test was conducted. For the publication bias, we also conducted the analyses of funnel plot, normal quantile plot, rank correlation test and the fail-safe n. Results : We used the random effect model to estimate the overall effect size, because the homogeneity of studies was rejected in a fixed effect model. Our quantitative meta-analysis yielded that integrated odds ratio between depression and alcoholism was 2.42 (95% C.I. 1.98-2.97). From the results of analyses of the publication bias, the probability of publication bias is consideredlow. Conclusion : The published evidences suggested that there may be a significant positive relationship between depression and alcoholism.

        Effect and mechanism of Magnolia officinalis pharmacopuncture for treating localized fat via network pharmacology and experimental study

        Background: Recently, for various reasons, the need for non-invasive treatment for localized fat has emerged. This study confirmed whether Magnolia officinalis (MO) pharmacopuncture reduces localized fat by promoting lipolysis and inhibiting adipogenesis. Methods: The network was built using genes related to the active compound of MO and the mode of action of MO was predicted by the functional enrichment analysis. Based on the result from network analysis, 100 µL of 2 mg/mL MO pharmacopuncture was injected into the inguinal fat pad for 6 weeks in obese C57BL/6J mice. Normal saline was injected into the right-side inguinal fat pad as a self-control. Results: It was expected that the AMP-activated protein kinase (AMPK) signaling pathway would be affected by the MO Network. MO pharmacopuncture reduced the weight and size of inguinal fat in HFD-induced obese mice. The phosphorylation of AMPK along with the increases of lipases was significantly increased by MO injection. Also, the expression levels of fatty acid synthesize-related mediators were suppressed by MO injection. Conclusion: Our results demonstrated that MO pharmacopuncture promoted the expression of AMPK, which has beneficial effects on activation of lipolysis and inhibition of lipogenesis. Pharmacopuncture of MO can be a non-surgical alternative therapy in the treatment of local fat tissue.

      • Effects of topical application of <i>Astragalus membranaceus</i> on allergic dermatitis

        <P><I>Astragalus membranaceus</I> (AM) is one of the most popular health-promoting herbs in East Asia, and has been used in traditional medicine for more than 2000 years. This study was performed to examine whether AM suppresses atopic dermatitis (AD)-like skin lesions in BALB/c mice. Seven-week-old female BALB/c mice were sensitized with 1-chloro-2,4-dinitrobenzene (DNCB) to induce allergic dermatitis. Skin sections were stained with hematoxylin and eosin (H&E) to assess epidermal and dermal hyperplasia, which were determined by measuring the thicknesses of the epidermis and dermis, respectively. The serum immunoglobulin G (IgE) concentration was quantified by enzyme-linked immunosorbent assay (ELISA). In addition, the levels of interleukins (IL)-4, -5, -6, and -13 and tissue necrosis factor (TNF)-α were measured in mouse serum. Significance was determined by one-way analysis of variance (ANOVA). Topical AM markedly improved the AD skin lesions in DNCB-induced mice. The AD skin lesions were significantly thinner in the AM treatment group compared with untreated controls, and the hyperkeratosis disappeared. Topical treatment of AM also restored nuclear factor-κB (NF-κB) expression. In addition, the serum IgE level was reduced. AM suppressed the expression of Th2 cytokines (IL-4, -5, -6, and -13) and significantly decreased the TNF-α level. AM is effective for treating AD by regulating cytokines. AM may be an alternative or complementary therapeutic option for treating patients with AD. More in-depth studies are necessary to clarify the mechanisms of AM.</P>

        Tracheoesophageal Fistula as a Complication after Endotracheal Intubation: A Case Report

        Placement of endotracheal tube, even for extremely short periods, can result in injury to laryngeal and tracheal tissue. This may be clinically insignificant, but in rare cases, it could be life threatening and results in permanent disability. Especially, tracheoesophageal fistula (TEF) is a serious and challenging problem because it may contaminate the tracheobronchial tree and interfere with nutrition. This uncommon but lethal complication has been reported to be associated with certain risk factors in tracheally intubated patients, and better knowledge of these factors could reduce the incidence of post-intubation TEF. We report a case of 49-year old male patient who has acquired TEF caused by endotracheal intubation and positive pressure ventilation.

        How Women Evaluate Syndromic Recurrent Urinary Tract Infections

        Purpose: To investigate the clinical manifestations of patients with the principal complaint of syndromic recurrent urinary tract infection (UTI), correlate these symptoms with the results of urine cultures, and identify the characteristics that can be used to differentiate UTI from similar diseases.Materials and Methods: A total of 212 consecutive patients with syndromic re-current UTIs over a 24 month period were evaluated. The major symptoms were recorded using the UTISA questionnaire and VAS. The patients were divided into group A (n=98; positive urine and urethral swab cultures) and group B (n=114; negative cultures), and the symptoms were compared. For group B, cystoscopy was used to diagnose 61 patients who complained of pain levels ≥6 on the VAS.Results: The proportion of patients with the classic symptoms of UTI (dysuria, urinary frequency, lower abdominal discomfort during bladder filling, and urgency) was similar in groups A and B. Significantly more patients complained of urethral pain in group B, and significantly fewer patients had gross hematuria, low back pain, a post-voiding sensation of residual urine, and general symptoms compared to group A. Of the 61 patients with a VAS ≥6, 29, 28, and four were diagnosed with bladder pain syndrome, interstitial cystitis, and urethral pain syndrome, respect-ively.Conclusions: In patients with syndromic recurrent UTI, the classic symptoms were not sufficiently characteristic to allow bacterial cystitis to be differentiated from other bladder diseases. Diagnostic cystoscopy and VAS can assist in making a differential diagnosis in patients with non-bacterial syndromic recurrent UTIs.

        <P>The mechanism of drift effect in pH-sensitive silicon nanowire (SiNW) ion sensitive field effect transistor (ISFET) is comprehensively studied by measuring the time-dependent drain current (I-D) and the gate capacitance (C-G) under different liquid-gate biases (V(LG)s) and pH levels. It was revealed that the origin of the current drift can be divided into three different mechanisms; the bulk ionic diffusion in sensing insulator, the chemical modification of insulator surface, and the oxide etch process induced by hydroxide (OH-) ion. Based on the V-LG/pH dependency of current drift and the transient C-G variation, it is clearly recognized that the drift of n-type SiNW (n-SiNW) ISFET results from H+ thorn diffusion in the insulator, whereas that of p-type SiNW (p-SiNW) ISFET is caused by temporal chemical modification (hydration) of the insulator, along with the oxide thickness (t(ox)) reduction by OH- ions.       • Absence of P15^INK⁴^B and p16^INK⁴^A Gene Alterations in Primary Cervical Carcinoma Tissues and Cell Lines with Human Papillomavirus Infection¹

        Alterations of p15□ the and p16□ gene which are separated by 25 kb on chromosome 9p21 have bee reported in various tumor derived cell lines and primary tumors, but the role of these genes in cervical cancer is unknown. To determine the frequency of deletions and point mutations of these genes in hrman cervical cancer, we examined for alterations of the p15□ and p16□ genes in cervical carcinomas. Methods : We examined 57 primary tumors and matched normal tissues, and 3 cervical cancer derived cell lines. All the trmor tissues and cell lines were human papillomavirus (HVP)-positive. Deletions or point mutations of exon 2 of the p15□ gene and exons 1, 2 and 3 of the p16□ gene were examined by polymerase chain reaction (PCR) and direct sequencing, respectively. Results : Our data indicate no evidence for intragenic homozygous deletion or point mutation in the primary cervical cancer tissues or cancer derived cell lines. Conclusion : Deletions or point mutations in the p15 or p16 gene may not be required for the development of HIPV-posetive cervical cancer of for establishment of cervical cancer cell lines.         정상상피세포(HaCat)와 자궁경부 암세포(SiHa)에서 GeneFishing^(TM) PCR technique을 이용한 유전자 발현의 변화

        목적 : 본 실험의 목적은 정상상피 세포와 자궁경부 암세포 사이에서 유전자의 발현 차이를 조사하였다. 연구 방법 : 정상상피 세포(HaCat)와 자궁암 세포(SiHa)를 사용하였으며, 두 세포 간에 유전자 발현 차이를 GeneFish^(TM) PCR을 이용하여 알아보았으며, BLAST serach를 통해 분석하였다. 결과 : 정상상피 세포와 자궁암 세포 비교 결과, 자궁암 세포에서 S1-2-2와 S5-1을 포함한 25개의 유전자가 발현이 증가하였고, 24개의 유전자가 감소하였다. 결론 : GeneFishing^(TM) PCR기법은 유전자의 발현 변화를 확인하는데 있어서 아주 민감하고 효과적인 방법이다. 우리는 정상상피 세포와 자궁경부 암세포에서 다르게 발현하는 유전자를 찾을 수 있었고, 앞으로는, 종양의 발생과 진행과정에 관여하는 유전자를 더 탐지하고 해당 유전자의 기능을 연구할 필요가 있다고 생각된다. Objective : The purpose of this study is investigated the differentially expressed genes between normal and cervical cancer cell line. Methods : We used normal human keratinocyte (HaCaT) as a control and HPV-16 positive cervical cancer (SiHa) cell line. Two cell lines were studied differential expressed genes by using GeneFishing^(TM) PCR and analyzed with BLAST search. Results : As compared with normal, cervical cancer cell line was showed 25 up-regulated genes including the S1-2-2, S5-1 and 24 down-regulated genes. Conclusion : GeneFish^(TM) PCR test is very sensitive and effective method for detection of changed gene expression. We could search differentially expressed genes between normal and cervical cancer cell line. In the future, we need to research various genes function to participate in the process of tumor development and progression.

        Ischemic necrosis of the cirrhotic liver is rare because the liver has dual blood supply and an extensive collateral system. However, it may occur in the condition of sudden reduction of hepatic arterial flow such as acute blood loss or shock Recently, we experienced two cases of ischemic necrosis of the liver assoicated with cirrhosis. Those were presented as space-occupying lesions on the radiological study. In contrast with hepatocellular carcinoma, these lesions showed neither hypervascularity nor deposition of injected lipiodol. Repeated percutaneous fine needle cytology revealed no evidence of malignant cells, Furthermore, short-term follow-up examination showed decrease in size and/or extent of the lesions, It is important to differentiate these lesions from tumorous condition because interventional trial such as transarterial chemoembolization may further aggravate the ischemic lesion and unnecessary surgery under false impression of hepatocellular carcinoma in cirrhotic patients should be avoided.

        Original Articles : Roles of Serotonergic and Adrenergic Receptors in the Antinociception of Selective Cyclooxygenase-2 Inhibitor in the Rat Spinal Cord

        Background: The analgesic mechanisms of cyclooxygenase (COX)-2 inhibitors have been explained mainly on the basis of the inhibition of prostaglandin biosynthesis. However, several lines of evidence suggest that their analgesic effects are mediated through serotonergic or adrenergic transmissions. We investigated the roles of these neurotransmitters in the antinociception of a selective COX-2 inhibitor at the spinal level. Methods: DUP-697, a selective COX-2 inhibitor, was delivered through an intrathecal catheter to male Sprague-Dawley rats to examine its effect on the flinching responses evoked by formalin injection into the hindpaw. Subsequently, the effects of intrathecal pretreatment with dihydroergocristine, prazosin, and yohimbine, which are serotonergic, a1 adrenergic and a2 adrenergic receptor antagonists, respectively, on the analgesia induced by DUP-697 were assessed. Results: Intrathecal DUP-697 reduced the flinching response evoked by formalin injection during phase 1 and 2. But, intrathecal dihydroergocristine, prazosin, and yohimbine had little effect on the antinociception of intrathecal DUP-697 during both phases of the formalin test. Conclusions: Intrathecal DUP-697, a selective COX-2 inhibitor, effectively relieved inflammatory pain in rats. Either the serotonergic or adrenergic transmissions might not be involved in the analgesic activity of COX-2 inhibitors at the spinal level.
