      • 만성 C형 간염 환자에서 페그인터페론 알파2a와 리바비린 병합 치료중 발생한 벨마비 1예

        페그인터페론과 리바비린 병합요법은 만성 C형 간염의 일차 치료법이다. 저자들은 만성 C형 간염 환자에서 페그인터페론 과 리바비린 병합 요법 중에 발생한 벨마비 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. 환자는 5년 전부터 만성 C형 간염을 앓아온 48세 남자이며, PEG-IFN α-2a 135μgm 피하주사 주1회와 하루 1200㎎의 리바비린을 투여하였다. 치료시작 후 9개월째 환자는 오른쪽 안면의 근력약화를 호소하였으며 벨마비로 진단되었다. 페그인터페론과 리바비린 병합요법을 지속하면서 관찰하였다. 환자의 벨마비는 페그인터페론 치료를 중단하지 않았음에도 3개월후 증상이 회복되고 이후 벨마비 재발 없이 현재 경과관찰 중이다. 만성 C형 간염에서 페그인터페론과 리바비린 병합 요법시 벨마비의 발생 가능성을 염두에 두어야 하겠다. A Case of Bell's Palsy Associated with Combination Therapy of Pegylated Interferon Alfa-2a (PEG-IFN) and Ribavirin for Chronic Hepatitis C Virus Infection Pegylated interferon alfa(PEG-IFN α) and ribavirin therapy is the first line treatment for chronic hepatitis C. Mild complications of the therapy are common, but more serious complications are rare. We report here a case of Bell's palsy that occurred in a patient with chronic hepatitis C virus infection during combination therapy of PEG-IFN α-2a and ribavirin. The patient was 49-year-old man with chronic hepatitis C (genotype 1b) for 8 years. He had compensated liver cirrhosis with splenomegaly. Therapy with PEG-IFN α- 2a 135mcg/week and ribavirin 1200mg/day was initiated. After 9 months of the therapy, the patient showed a loss of muscular tone on the right side of his face. A diagnosis of Bell's palsy was made. The Bell's palsy resolved over 3 months despite continuation of the combination therapy.

        Although porous concrete is recently used as a pavement at a parking lot, a public square and a bicycle road, we use without enough examining. Most the maintenance of quality for porous concrete is only dealt with strength and color tone. Also, there is not yet enough fundamental data about dynamical properties for strength, rate of void and water permeability, even when it is applied to at the scene, it is been a matter. Accordingly in this study analyzed influences through the dynamical properties, combination, and time to vibration. After the analyzation, they investigated peculiar dynamics which are strength of porous concrete, rate of void, and water permeability follow the rate of increase on an admixture making pozzolan reaction. This is purpose to improve capacity of the sidewalk permeable concrete and development of the economical technique for pavement.

        <P>An all‐solution‐processed flexible piezoelectric nanogenerator, composed of polycrystalline ZnO thin film and functional polymer layers such as P3HT/PCBM and PEDOT:PSS, generates energy through a mechanical rolling and muscle stretching system. On page 6022, Youn Sang Kim, Sang‐Woo Kim, and co‐workers show that this all‐solution‐processed nanogenerator is feasible as a piezoelectric patchable device and is promising for use in future energy harvesters such as wearable human patches and mobile electronics. </P>

        Evaluation of Renal Toxicity by Combination Exposure to Melamine and Cyanuric Acid in Male Sprague-Dawley Rats

        Melamine-induced nephrotoxicity is closely associated with crystal formation in the kidney caused by combined exposure to melamine (Mel) and cyanuric acid (CA). However, there are few dosage-finding studies for toxicological evaluation of chronic co-exposure to Mel and CA. The objective of this study was to investigate the possible mechanism by which a Mel and CA mixture lead to renal toxicity in rats. Mel and CA were co-administered to rats via oral gavage for 50 days. Nephrotoxicity was determined by measuring blood urea nitrogen (BUN) and serum creatinine (sCr) levels. Relative kidney weights were significantly increased in rats after co-exposure to Mel+CA (63/6.3 or 630/6.3 mg/kg) mixtures. BUN and sCr levels were significantly increased after Mel and CA co-exposure. Taken together, significant increase in KIM-1, NGAL, and calbindin levels were observed in the urine of rats exposed to Mel+CA (63/6.3 or 630/6.3 mg/kg) compared with the corresponding control group. Histological analysis revealed epithelial degeneration and necrotic cell death in the proximal tubules of the kidney after co-exposure to Mel+CA (63/6.3 or 630/6.3 mg/kg). Our data suggest that Mel-mediated renal toxicity may be influenced by CA concentrations in Mel-contaminated milk or foods.

        換氣窓이 설치된 中庭形 아트리움의 여름철 熱環境에 관한 測定硏究

        The atrium in a building is a popular design skill due to its aesthetic, open appearance. It is relatively inexpensive to build and maintain atrium, and also we can expect energy saving effect. But its indoor space is thermally different from typical space because of its tall and large shape with top light. The upper part of atrium has the thermal problem of overheating during summer season. As one of the measures to solve this problem, the ventilation windows near the top light can be used. In this study, the vertical temperatures in the atrium were measured and analysed to understand the thermal environment in the atrium and the effect of ventilation window to reduce the overheating at the upper part of atrium. The ventilation windows were proved to have the effect to lower the temperature of the upper part of atrium as 3.8℃ at the condition of outdoor air temperature of 28℃.

        북한이탈주민의 외상 후 스트레스 장애와 자연살해세포 활성

        Objectives : The objective of this study was to evaluate an association between posttraumatic stress disorder (PTSD) and altered immunity m North Korean defectors who were more likely to develop medical conditions and other stress-related psychiatric disorders. Methods : Twenty-four North Korean defectors with PTSD and twenty-two controls without PTSD were recruited from the resettlement and training center for North Korean defectors in South Korea. Peripheral T lymphocyte subsets, natural killer(NK) cells, NK cell activity and plasma hormones (ACTH and cortisol) were obtained from all subjects. We also applied the Hamilton Rating Scale for Depression (HAM-D) and Hamilton Rating Scale for Anxiety (HAM-A) for all subjects. Results : We found NK cell activity was relatively lower than number of NK cells in North Korean defectors with PTSD. Subjects with PTSD had higher HAM-D and HAM-A scores than controls. However, there were no statistical differences in ACTH. cortisol, CD3, CD4, CD8, CD4/CD8 ratio, CD19 and CD56 between the two groups. Conclusion : These findings suggest North Korean defectors with PTSD show higher levels of depression, anxiety and impaired or weak immune function in NK cells.

      • 열린 과학 교육을 위한 구체적 실행 방안

        열린 과학 교육의 필요성에 대해서는 많은 사람들이 공감하고 있다. 그러나 아직 열린 과학교육에 대한 구체적인 정의와 지도 방법이 마련되어 있지 않다(Choi,K.H., 1994). 본 연구에서는 열린 과학 교육을 위한 연수 자료를 개발하고 전국 초, 중, 고 연수 담당자 및 과학 전문직을 대상으로 열린 과학 교육 연수를 실시하였다. 연수 후 열린 과학 교육의 문제점과 지도 방법에 대한 의견을 수렴하였다. 충분한 토의와 협의를 통해 학교 현장에서 실시할 수 있는 열린 과학 교육 방안을 마련하였다. 연수에 참여한 모든 과학 교육 담당자는 열린 과학 교육에 대한 의견을 제시하였다. 본 연구에서는 설문 결과를 바탕으로 열린 과학 교육에 대한 바람직한 방안을 마련하였다. 현재의 학교 환경을 고려하여 4가지 형태의 열린 과학 교육 방법을 제시하였다. 열린 과학 학습 형태 1은 예측, 실험 수행, 결과 토의 등의 단계를 거치는 직소형 학습 방법이다. 열린 과학 학습 형태 2는 학습자의 특성에 따라 교사의 지도 방법이 달라지는 학습 방법이다. 열린 과학 학습 형태 3은 탐구 단계별로 학습하는 학습 방법이다. 열린 과학 학습 형태 4는 학습자 스스로 자신의 학습 수준을 파악하고 수준에 맞는 학습 방법대로 학습하는 방법이다. Many peoples recognize the need of open science education by students' cognitive level. But identification and teaching method of open science education by students' cognitive level are not prepared. We developed teachers' training program for open science education by students' cognitive level and trained science teachers' trainers and inspectors. After the training program was completed the problem and the direction of open science education by students' cognitive level were presented. By this workshop's results we suggest the desirable plan of open science education by students' cognitive level.

        Green Building is a sustainable building preserving the environment and saving energy and aims to provide the comfort indoor environment. KIER Green Building is the first Korean green building which was adopted the technologies of energy and resource conservation and in and outdoor environment preservation. In order to verify the luminous environment of green building and to convince the effect of daylight through atrium, the lighting environment element of KIER Green Building such as illuminance was measured and analyzed.

      • Small Cell Lung Cancer Cell Line을 이용한 Xenoplanted nude mice에서 방서선 치료후 종양의 변화 관찰에 관한 연구

        Recently, combination of ionizing radiation with inhibitors of angiogenesis has been reported to improve tumor eradication compared to treatment with irradiation alone. However, the mechanism of this effect have not been defined. For this pupose we established a non-small cell lung cancer model in nude mice. Tumor vascularization was visualized in vivo by MRI using gadolinium-DTPA as contrast agent. Further, cryosections were produced exactly in the MRI slice positions. Since we were interested to examine formation of recurrent tumor irradiation was performed with a single fraction of 6 Gy. This dose caused a partial remission followed by recurrent tumor growth 25 to 35 days after therapy. The process of partial remission as well as formation of the recurrent tumor was examined in 35 nude mice analysing the following parameters: (1) contrast agent enhancement using high-resolution MRI, (2) proliferation of tumor cells and fibroblast using Ki-67 immunohistochemistry, (3) formation of microvessels using CD31 immunohistochemistry. The latter analyses lead to differentiation of three stages. Stage 1(day 1 to 15 after irradiation) was characterized by increasing area of dead cell mass in hematoxylin-eosin stained slides that corresponded to a decrease in tumor cellproliferation as well as contrast agent enhancement. The percentage of Ki-67 positive tumor cells decreased from initially 45.1 ±6.0 to 1.4 %±1.2 % on day 15. Stage 2(days 6 to 20 after irradiation; overlapping with stage 1) was characterized by proliferation of fibroblast leading to formation of fibrotic septae with abundant microvessels. Already during late stage 2 MRI identified new contrast agent enhancing areas. Stage 3(day 20 to 40 after irradiation) was characterized by new tumor cell proliferation. Interestingly, tumor cells almost exclusively proliferated in the direct neighbourhood of the fibroblasts and blood vessels was a condition prior to foramtion of recurrent tumor tissue. Thus our results are in contrast with the view that tumors or recurrent tumors begin as avascular masses that later induce neovascularization. With respect to clinical practice our results suggest that (1) adjuvant anti-angiogenic therapy should not be limited to the day of irradiation but should cover a critical period until day 5 to day 20 after radiotherapy, (2) adjuvant therapy should also include inhibition of fibroblast proliferation, (3) MRI can identify a recurrent tumor 10 to 15 days before occurrence of new tumor growth.
