Case Report : Spontaneous pneumothorax during Laparoscopy-assisted Billroth-1 gastrectomy

Pneumothorax associated with a pneumoperitonium in laparoscopic surgery is rare but can cause life-threatening complications. A 62-year-old man was scheduled for a laparoscopy-assisted Billroth-I gastrectomy under general anesthesia. Approximately 70 minutes after insufflating carbon dioxide into the intraabdominal cavity at a pressure of 12 mmHg, the peak inspiratory pressure increased, while the oxygen saturation decreased. The pneumothorax of the left lung was evident on the intraoperative chest radiograph. The pneumothorax improved after inserting a catheter into the affected area. The cause of the pneumothorax was unknown but an anatomical defect is believed responsible. This report shows that pneumothorax developed under an intraabdominal pressure in the conventional safety range. Careful monitoring and immediate treatment is necessary to prevent the condition from worsening. (Korean J Anesthesiol 2010; 58: 405-408)

Natural antisense transcripts (NATs) constitute a class of non-coding RNAs that have emerged as important regulators of gene expression in an epigenetic manner. Tumor suppressor P53 plays an important role in the cell-intrinsic responses to genome instability, including a transient cell cycle arrest, senescence and apoptosis. A Wrap53α (WD40-encoding RNA anti-sense to p53) that transcription initiated from exon 1α is required for p53 induction upon DNA damage. However, involvement of Wrap53α in lung cancer has not been reported to date. We determined the methylation status of Wrap53α genes in primary lung tissues using a methylation-specific PCR (MSP) and evaluated their relationship with clinicopatholgial features and sur-vival in patients with non-small cell lung cancer (NSCLC). Wrap53α promoter was unmethylated in most malignant and corre-sponding nonmalignant tissues. Wrap53α methylation was detected in 19 (13.3%) of 146 tumor tissues and 12 (8.2%) of 146 normal tissues. In addition, Wrap53α methylation in normal and non-malignant tissues was frequently detected in p53 muta-tion-negative patients and in patients with stages II-IV. Patients with Wrap53α methylation in normal tissues exhibited sig-nificantly worse overall survival, while methylation in tumor tissues did not affect the survival outcome of patients. These findings suggest that Wrap53α methylation play a role in lung carcinogenesis. However, further studies with a large number of cases are needed to confirm this.

The term of Green, from the viewpoint of various agreements and concerns at home and abroad, forms a certain paradigm. In building industry, many approaches such as energy-zero house have been evaluated to accomplish the greenhouse gas reduction goal. Many countries of Europe introduced the concept of the Passive House to reduce greenhouse gas emission in the construction sector. Passive House makes building more airtight and install lower U-value walls and windows. And most important purpose is to prevent the decline of indoor air quality. To improve indoor thermal comfort is the most important purpose. In this study, Europe’s Passive House design and ventilation standard is summarized.
Methylation of TMEFF2 Gene in Tissue and Serum DNA from Patients with Non-Small Cell Lung Cancer

Lung cancer remains a global health problem with a high mortality rate. CpG island methylation is a common aberration frequently associated with gene silencing in multiple tumor types, emerging as a highly promising biomarker. The trans-membrane protein with a single EGF-like and two follistatin domains (TMEFF2) is epigenetically silenced in numerous tumor types, suggesting a potential role as a potential tumor suppressor. However, the role of TMEFF2 in lung cancer remains to be fully elucidated. We explored the methylation status of the TMEFF2 gene in 139 patients with non-small cell lung cancer (NSCLC) and the feasibility of detecting circulating methylated DNA as a screening tool for NSCLC using methylation-specific PCR (MSP). TMEFF2 methylation in tumor tissues was found in 73 of the 139 NSCLCs (52.5%) and was related to gene expression. In adenocarcinomas, TMEFF2 methylation was significantly more frequent in tumors without EGFR mutation than those with EGFR mutation (adjusted hazard ratio=, 95% confidence interval=, P=0.002). In addition, the frequency of TMEFF2 methylation was also higher in females and never-smokers than in males and smokers with borderline significance (65.8% vs 47.8%, p=0.06; 65.7% vs 48.1%, p=0.07). Furthermore, TMEFF2 methylation was exclusively detected in the serum of NSCLC patients at a frequency of 9.2% (29/316). These findings suggest that the methylation-associated down-regulation of the TMEFF2 gene may be involved in early lung tumorigenesis and TMEFF2 methylation can serve as a specific blood-based biomarker for NSCLC.
