      • Device induced neocollagenesis; Profibrotic response or true neocollagenesis?

        Background: Many light and radiofrequency-based rejuvenation devices have claimed to increase the collagen production in skin dermal tissue. Yet there hasn’t been enough scientific evidence to prove whether the result is just profibrotic response or not Objectives: To find the optimal skin rejuvenation device that shows true neocollagenesis Methods: We evaluated dermal collagen thickness and gene expression of procollagen type 1, 3, MMP-3 & TGF- β resulted from different energy based devices in rat model in vivo. The wound healing response was evaluated histologically and by RT-PCR at immediate, 1, 2, 4, 8 & 12 weeks after initial procedure Results: At 12th week, the most relevant changes of dermal thickness were found in specimens after treatment with electrosurgical unit, fractional CO<sub>2</sub> & Q-switched ND:YAG 1064nm. Procollagen 1 & 3 also found highest in electrosurgical unit, fractional CO<sub>2</sub> & microneedle RF. Dramatic change of MMP-3 & TGF-β is remarkable at the early observation but went back to normal level at 12th week. Ratio of procollagen 1/3 found lowest with Q-Switched ND:YAG 1064nm & fractional CO<sub>2</sub> Conclusion: Electrosurgical unit resulted in most significant change but due to irreversible thermal damage and extremely high pro-collagen result, it is considered as a profibrotic response. Fractional CO<sub>2</sub> & Q-Switched ND:YAG 1064nm are applicable to face skin rejuvenation treatment considering thickening of dermal tissue and low procollagen 1/3 ratio similar to neocollagenesis purpose.

      • Histopathologic assessment of needle-free pneumatic injection device with rat model and prospective, single center study on human depressed scars

        Background: The Needle-free pneumatic injection was recently introduced to the field of dermatology However, data on influence of various parameters of pneumatic injection on collagen synthesis is lacking. Objectives: Compare the influence of diameter, pressure, volume, and total work force of pneumatic injection on collagen synthesis in rat model. Methods: Adult rats were shot with different pressure and volume using nozzle of different diameters. The area of collagen synthesis was measured and statistical analysis was performed. Results: The longest diameter, 0.20mm, resulted in significantly wider area of neocollagenesis. Different injection pressure and volume did not yield a significant difference of collagen synthesis. The degree of new collagen fibers increased as total work force became higher. Conclusion: Increasing the diameter can result in higher degree of collagen synthesis. Total work force also correlates with the degree of neocollagenesis.
