뇌경색을 동반한 Haemophilus parainfluenzae에 의한 감염성 심내막염 1예

저자들은 뇌경색을 동반한 H. parainfluenzae에 의한 심내마염을 ampicillin과 gentamicin의 병합요법으로 치료한 1예를 경험하였기에, 이를 문헌고찰과 함께 보고하는 바이다. We experienced a case of infective endocarditis due to Haemophilus parainfluenzae complicated by cerebral infarction. A 40 year-old woman was admitted because of right-sided hemiparesis and dysarthria, which was accompanied by high fever up to 40 C with chills. She had been suffering from low-grade fever and general weakness for one month prior to admission. Blood cultures grew H. parainfluenzae, and the brain CT scan demonstrated cerebral infarction of MCA territory. Transesophageal echocardiogram showed mitral valve prolapse with regurgitation, and no vagetation was found. She was successfully treated with ampicillin and gentamicin for six weeks. To our knowledge, this is the first case of infective endocarditis caused by H. parainfluenzae in Korea.

용융염 합성법으로 Ba-페라이트 미세입자를 합성하기 위하여, 염으로 NaCl과 KCl을 선택하였으며, 이들을 단독 또는 NaCl-KCl의 공정 조성 혼합물로서 시료에 50wt% 첨가하고, 바륨 페라이트의 입자형상을 제어하기 위해 1㏖%의 SiO₂를 첨가 하였다. 보자력 제어를 위한 BaFe_(12-2x)Co_(x)Ti_(x)O_(19) 조성에서 x=0→x=1.0으로 Fe^(3+)를 Co^(3+), Ti^(4+)로 치환량이 증가할수록 H_(c)와 M_(r)값은 큰 폭으로 감소하였고, SiO₂를 1㏖% 첨가한 경우는 첨가하지 않은 경우보다 6각 판상의 c축이 짧아졌고 판상비가 커졌다. In order to synthesize Ba-ferrite fine particles by molten salt method, NaCl and KCl were added to basic composition to 50% by weight, and added 1 mole% of SiO₂to control the shape of the Ba-ferrite particles. H_{c} and M_{r} were decreased when Fe^{3+} was substituted with Co^{2+} and Ti^{4+} from x=0 to x= 1.0 in BaFe_{12-2x}CO_{x}Ti_{x}O_{19}, and 1 mole% SiO₂increased the size but shortened c-axis of hexagonal ferrite.
Helicobacter pylori 감염과 위선암의 발생 사이의 연관성에 관한 최근 연구 : a Review of the Literature

Purpose : Stomach cancer is a major cancer in the world as well as in Korea. Helicobacter pylori infection was suggested causing atropic gastritis, but there has been a debate on the association with the gastric adenocarcinoma. We reviewed recent literatures and meta-analyses on the association between Helicobacter pylori infection and the development of gastric adenocarcinoma. Results : The animal experiments suggested that Helicobacter pylori is a causative agent of gastric adenocarcinoma. Meta-analyses on the relationship between Helicobacter pylori infection and gastric adenocarcinoma showed increased risk in infected person, but the strength of association was varied with study design, characteristic of study participants and the nature of tumors. Conclusions : Recent study results suggested that Helicobacter pylori is a necessary causative agent for development of gastric adenocarcinoma. Assessment of the association between Helicobacter pylori infection and other related with gastric cancer risks and the effects of the eradication therapy of Helicobacter pylori infection on the prevention of gastvic cancer need further evaluation.

The Orbital fracture which is often combined with midface fracture can cause decreased visual acuity, limitation of eyeball movement, diplopia, enophthalmos, etc. Traumatic orbital fracture causes change of orbital volume, results in diplopia and enophthalmos thus, accurate repositioning of displaced bone and reconstruction of orbital defect with autogenous materials(cranial, rib, iliac bone, cartilage and fascia lata) or alloplastic materials(gelatin film, polyglactin mesh, methylmethacrylate, Teflon, silicone, Supermid, hydroxyapatite and metal). The key point of reconstruction of orbit is accurate repositioning of displaced orbital floor, lateral, medial orbital wall and sufficient bonegraft in anatomical defect. As this cases, we obtained good results through transconjunctival and coronal approach who were required orbital reconstruction. Also, we accurately diagnosed orbital fracture with C.T., 3D model and good result for orbital reconstruction with sufficient parietal block bone graft during average 16.8 months follow-up.

Background and Objectives: Mutation or inactivation of the p53 is important mechanism in a wide variety of human tumors. The purpose of this study is to investigate the relationship between expression amount of p53 protein and the grade of astrocytic gliomas. Materials and Methods: This study analyzes 40 paraffin-embedded astrocytic glioma, including 10 cases of low grade, 20 cases of high grade, and 10 cases of maligant glioma (glioblastoma multiforme) were used to analyze the status of abnormal accumulation of wild type of p53 protein. The evaluation of the immunostaining result was based on the percentage of positive neoplastic cells. Results: Average positive cell rate of p53 protein were 4.2%, 35.4% and 16.2% in low grade astrocytoma, high grade astrocytoma and glioblastoma multiforme, respectively. Conclusion: p53 gene mutations were correlated with histologic grade of glial tumors. High grade and glioblastoma multiforme showed higher positive cell rate than low grade astrocytoma. These results suggest that the high positive rates of p53 protein is related to the poor prognosis.

A common form of relapse in adenocarcinoma of the stomach is intraperitoneal dissemination, in fact, among gastric adenocarcinoma patients who have undergone surgery intended to cure, approximately 50% of the patients develope initial recurrence in the peritoneal cavity regardless of the anatomic site of the primary tumor within the stomach. The efficacy of systemic postoperative chemotherapy to prevent peritoneal recurrence of gastrric adcnocarcinoma is not satisfactory. There is still a great need for improved therapeutic strategies on the disseminated microscopic disease and small miliary nodules remaining on the peritoneal surface or lymphatics after operation. The authers have made fluorouracil-polyglycolic acid composite disks(Fu-PGA disks) with fluorouracil and biodegradable polymer: polyglycolic acid for more effective intraperitoneal chemotherapy. We inserted the Fu-PGA disk(s) in the peritoneal cavity of rat and pharmacokinetic study was performed to measure fluorouracil concentration in the peritoneal fluid, plasma, liver, kidney and heart tissue at 24 hour, 72 hour and 168 hour after insertion of Fu-PGA disk(s). Myelosuppressive action of this composite also was determined following its administration. The data of this study suggested that Fu-PGA composite will be a new device releasing drugs in a controlled manner and having targetability to peritoneum, and this device will be improving the efficacy of intraperitoneal chemotherapy for gastric adenocarcinoma.

The jaws of children differ from those of adults in the aspects of architectural and mechanical properties, sites, form and treatment modality. This study reviews the result of treatment of pediatric mandibular fractures between 1994 and 1998 at dept. of Oral & Maxillofacial Surgery, School of dentistry, Wonkwang University by means of the age and sex distribution, mechanism of injury, location of fractures and associated patterns, fracture management, postoperative complications, any associated injuries to other organ systems. Total number of these study was 61 facial bone fractures in 32 patients. The results were as follows. 1.The pediatric mandibular fractures predominated in the age of 6∼12, and had no sexual predilection. 2.The main cause of pediatric mandibular fractures was the traffic related accidents(56.2%) 3.The condylar fracture only was 23 cases (37.7%) in total 61 sites of mandibular fractures and the symphysis fracture was 20 (32.7%) ; the highest incidence of mandibualr fracture was the symphysis with unilateral condyle combined fracture in 61 total mandibular fracture sites. 4.The major postoperative complication was deviation during mouth opening (41.2%) and the facial asymmetry(17.7%) in second. 5.Soft tissue laceration(53%) was the most associated injury with mandibular fracture and the abdominal injury(12.5%), the head injury(12.5%), and the extrimity injury (6.2%) in order.
하악 과두 골절 환자에서 Lag Screw를 이용한 치료시 예후에 관한 임상적 연구

Treatment of condylar fracture is still controversial, but the indication of surgical mangement is increased due to improved fixation methods and surgical techniques. In this article, we report our clinical and radiological results with the use of lag screw osteosyn-thesis for fixation of fractured mandibular condyle. 12 case of condyle fracture treated by open reduction and fixation with lag screw were monitored for an average of 10 months (range from 6 months to 19 months) postoperatively. The obtained results are as follows : 1. Postoperative maximum mouth opening is achieved over 40mm except one patient. 2. Although slight crepitus and deviation of mandible during mouth opening were found, Tempo-romandibular joint problem is satisfactory in all but one patient. 3. Distal migration of screw and resorption of fractured fragment were found in each 2 patients, but good functional recovery was achieved in all. According to the above results, we think that lag screw osteosynthesis is relatively indicated in level III or IV condylar fracture.
Macrophage Inflammatory Protein-1α의 浩血幹細胞 억제 작용에 관한 실험적 연구

The proliferation of bone marrow stem cell compartment is thought to be under both positive and negative controls by cytokines and colony stimulation factors. Macrophage inflammatory protein-1α(MIP-1α) has been assessed for its potential to protect hematopoietic stem cells from cytotoxic effects of a cycle-specific antineoplastic agents. We have tested the ability of MIP-1α to suppress the proliferation of stem cell line Du.528.101 in variety of active status by using [³H]-thymidine incorporation test. The results were as follows. 1. The effect of MIP-1α on steady-state Du.528.101 cell represented the cell growth suppression at the concentration of 10, 50, 100nM of MIP-1α(P<0.001). 2. MIP-α stimulated the proliferation of Du.528.101 cells previously treated with IL-1 at the concentration of 5, 50nM of MIP-1α(P<0.01) 3. The suppression effect of MIP-1 on Du.528.101 cells at the concentration of 5, 50nM was shown when cells were treated with MIP-1α before activation with IL-1β(P<0.01). 4. The growth rate of synchronized cells were slower than that of non-synchronized ones, and MIP-1α represented the similar suppression effect on both synchronized and non-synchronized cells.
