        안와골절의 외과적 접근에 대한 증례보고

        The Orbital fracture which is often combined with midface fracture can cause decreased visual acuity, limitation of eyeball movement, diplopia, enophthalmos, etc. Traumatic orbital fracture causes change of orbital volume, results in diplopia and enophthalmos thus, accurate repositioning of displaced bone and reconstruction of orbital defect with autogenous materials(cranial, rib, iliac bone, cartilage and fascia lata) or alloplastic materials(gelatin film, polyglactin mesh, methylmethacrylate, Teflon, silicone, Supermid, hydroxyapatite and metal). The key point of reconstruction of orbit is accurate repositioning of displaced orbital floor, lateral, medial orbital wall and sufficient bonegraft in anatomical defect. As this cases, we obtained good results through transconjunctival and coronal approach who were required orbital reconstruction. Also, we accurately diagnosed orbital fracture with C.T., 3D model and good result for orbital reconstruction with sufficient parietal block bone graft during average 16.8 months follow-up.

        상악 제3대구치 발거후 발생한 광범위한 피하기종

        Subcutaneous air emphysema is a relatively uncommon phenomenon. It has been reported in both the medical and dental literature following such various procedures as rectal biopsy, nasogastric intubation, radical neck dissection, tonsillectomy, adenoidectomy, root canal treatment, dental restoration and extraction of maxillary and mandibular teeth. The most common cause involves extraction of mandibular third molar. Subcutaneous emphysema of the head, neck and thorax is a swelling caused by introduction of air into the space of the connective tissue. The clinical signs are local swelling, tenting of the skin, and crepitation on palpation. In extreme casesk, air has been reported to pass through the masticatory space into the parapharyngeal and retropharyngeal space, and penetrate into the mediastinum. If the inflowing air contains bacteria, serious infection may ensue. The first case of subcutaneous emphysema associated with a dental procedure was first reported in 1900. This emphysema is a often limited only to the spaces of head and neck, but also can involve deeper structure. This case is diffuse subcutaneous emphysema following maxillary third molar extraction with a high-speed drill.

        Nowadays, watching sports on TV is becoming a frame in the act of culture. Sports news, sports broadcast, and other programs that has relations with sports are developing not only with the quality of modern people but also with the relations of broadcasting company. This study aims at examining the way of studying TV sports programs and the influence of TV sports programs. It could be said that sports and television programs on sports have not been offered in-depth discussion and empirical exploration in the academic world so for, although their social influence is considerable. We firstly look over the meaning of mass media and then, we go into the study about mass media and sports. TV sports program takes social function, therefore, many theories is being used to study TV sports program. "Cultivation theory" and " Uses and gratification Approaches" are the main theories of the study on TV sports programs. At the first glance, the Cultivation Theory, which empirically analyzed the symbolic function of television as social ideological institution, and the Uses and Gratification Approaches, which contend the media audience are active rather than passive recipients, seem to be contradictory. However, if we look at the relationship between the mass media and audience not as one-way but as interactive process, it could be suggested that combining those two different theoretical worlds will be useful.

        Quantitative Light-induced Fluorescence를 이용한 법랑질 탈회 속도에 관한 비교 연구

        치아 우식증을 조기에 발견하여 기존의 외과적 와동 형성을 피하고 건전한 치질을 최대한 보존하고자 하는 예방적 시술 개념에 대한 관심과 연구가 최근 유럽을 중심으로 확산되고 있다. 치아 우식증의 임상적 조기 진단을 위해 새로운 장비들이 개발되고 있으며, 이미 실험실 연구를 통해 성능의 우수성이 입증된 바 있다. 본 연구의 목적은 치아 우식증의 조기 진단을 목적으로 새로 개발된 Quantitative light induced fluorescence analysis (QLF) 시스템을 이용하여 우치, 사람의 유치 및 영구치의 법랑질 탈회 속도를 비교 평가하고자 하였다. 각 군당 25개씩 배정된 실험 대상 치아들은 표면을 약 50~100㎛정도 제거한 후 활택하여 탈회 용액(0.1mol/L 젖산, 0.2% 카보폴 907 그리고 50%로 농축된 수산화 인회석으로 제조)에서 1, 2, 4 그리고 8일 동안 탈회시켰다. 모든 시편들은 QLF분석을 시행하였으며, 실험 일정에 따라 5개의 시편들을 선택하여 gold standard로 사용하기 위해 Transverse Microradiography analysis (TMR)를 시행하였다. TMR분석을 통하여 무기질의 탈회량(IML)과 병소 깊이(LD)를 측정하였다. QLF 분석에서 측정된 정상 법랑질의 fluorescence radiance (FR<TEX>$$\_(S)$$</TEX>) 값에 대한 탈회 법랑질의 fluorescence radiance(FR<TEX>$$\_(D)$$</TEX>)의 비율인 FR ratio (FR<TEX>$$\_(D)$$</TEX>/FR<TEX>$$\_(S)$$</TEX>)를 산출하였다. 우치에서는 FR Ratio와 병소 깊이 간에 통계적 유의성이 있었으며(p<0.05), 유치에서는 FR Ratio와 광물질 소실 및 병소깊이 간에는 유의성이 없었다(p>0.05). 영구치에서는 FR Ratio와 광물질 소실 및 병소 깊이 간에 통계적 유의성이 있었다(p<0.05). 탈회 시간에 대한 FR Ratio의 회귀 분석 결과에서 얻은 회귀 계수 값은 우치에서는 -4.643(p<0.05), 유치에서는-5.421 (p<0,05), 영구치에서 -4.435(p<0.05)로 나타났다. The objective of this study was to compare the rate of in vitro demineralization of bovine permanent (BP), human deciduous (HD) and human permanent (HP) enamel. Twenty five flattened and polished enamel samples for each group (BP, HD, HP) were immersed in a demineralizing solution (0.1 mol/L lactic acid 0.2% Carbopol 907, and 50% saturated hydroxyapatite) for 1, 2, 4 or 8 days. All 25 samples from each group were subjected to quantitative light induced fluorescence analysis(QLF) and 5 samples from each group were randomly selected for Transverse Microradiography analysis (TMR). Integrated mineral loss (IML) and lesion depth (LD) were determined by TMR. The fluorescence radiance (FR) of sound enamel (FR<TEX>$$\_(S)$$</TEX>), demineralized enamel (FR<TEX>$$\_(D)$$</TEX>) were determined by QLF and FR ratio (FR<TEX>$$\_(D)$$</TEX>/FR<TEX>$$\_(S)$$</TEX>) was calculated. Bovine enamel samples showed significant correlation between FR ratio and lesion depth(p<0.05), and deciduous enamel samples does not showed significant correlation between FR ratio and lesion depth(p>0.05). Permanent enamel samples showed significant correlation between FR ratio and lesion depth(p<0.05). The constant of demineralization time between FR ratio from regression analysis were as follows : bovine enamel was -4.643 (p<0.05), deciduous enamel was -5.421(p<0.05) and permanent enamel was -4.435(p<0.05).

        Overlay 기반의 WiBro와 WLAN 이종망 연동을 위한 Vertical 핸드오버 구현 및 성능 측정에 관한 연구

        유비쿼터스 사회를 위한 미래 네트워크는 다양한 이종 무선망이 혼합된 형태가 될 것으로 예상되고 있다. 본 논문에서는 개별 무선망의 특징을 최대한 활용하여 1,270,000㎡ 면적의 KAIST(Korea Advanced Institute of Science and Technology) 대학교 캠퍼스에 IEEE 802.16e/WiBro와 IEEE 802.11/WLAN 를 복합적으로 설치하여 건물외부에서는 WiBro, 건물내부에서는 WLAN으로 자동 스위칭 되는 Vertical 핸드오버 System을 구축하고 이동 성능을 분석하였다. 이종망 통합 과정에서 제조사 및 통신사업자의 사정으로 인하여 Mobile IP를 지원하지 않는 라우터들이 존재하였고, 특히 현재 WiBro에 사용되는 ACR(Access Control Router)은 Mobile IP를 지원하지 않기 때문에 이를 해결하기 위하여 Overlay 기반의 Mobility 기법을 적용하였다. 본 연구에서는 실제로 대규모 통신망을 구현하고, 이에 대한 성능시험을 시행한 후 handover delay 시간 분석을 통한 현장 적용 가능성 및 개선 필요사항을 도출하였다. 그 결과 이종망간 핸드오버 시간은 평균 2.58초로써 일반 데이터 서비스는 이종망간 연속성이 보장됨을 확인하였고, 음성∙영상과 같은 실시간 서비스는 중간에 지연시간이 발생하나 응용서비스 세션 자체는 유지됨을 확인하였다. The future network is forecast with the fact that the form where the various heterogeneous wireless networks. In this paper we build both IEEE 802.16e/WiBro and IEEE 802.11/WLAN network on the KAIST(Korea Advanced Institute of Science and Technology) campus(1,270,000m2) as well as established the vertical handover system which supported WLAN in the building and WiBro outside the building. A Mobile IP is considered for the efficient vertical mobility function in IEEE 802.16/WiBro and IEEE 802.11/WLAN environment. However, during the deployment of WiBro, only some of the ACRs(Access Control Router) can support the Mobile IP function. Therefore, WiBro network can not support the mobility function for the mobile node. In this paper, we implement an overlay-based Mobile IP function and measure the performance through a field trial. Moreover, we evaluate the possibility of commercial deployment and suggest areas that need to be improved. The average handover time between WiBro and WLAN was 2.58 seconds. As a result, the user application session maintained but real time applications were delayed during the handover time.

        우수 이용을 위한 포집재료별 포집수량과 수질에 관한 연구

        In this study, quantity and quality of collected rainwater by sand, gravel, soil, lawn and concrete surface, as collection materials were investigated and Rainwater Collection Prediction Model was developed to predict the amount of collected rainwater. The quantity of collected rainwater in concrete surface, gravel, sand, soil and lawn collection system was 1,067L(93.2%), 1,006L(87.8%), 902L(78.8%), 800L(69.9%), 788.5L(68.8%) for 8 months period, respectively. The average turbidity of collected rainwater in concrete surface, gravel, sand, soil and lawn collection system was 3.2NTU, 2.2NTU, 1.9NTU, 1.7NTU, 1.5NTU for 8 months period, respectively. For sand collection material, predicted amount by the Model and actual collected amount were 931.5L and 902L, which were very closed. For gravel collection material, predicted amount by Model and actual collected amount were 1,028.21. and 1,006L, which were very closed. To simulate the optimal rainwater storage volume, the rainfall and evaporation data in Dae-jeon city were used. For sand collection system with 30m2 area, the maximum storage volume was 17㎥ and 62% of the year was secured for use of 240L/day.

        하악지 시상 분할술 후 하치조 신경의 감각회복에 관한 연구

        Dysfunction of the inferior alveolar nerve was indicated by various degree of numbness on the lower lip. The chin is considered as one of the few drawbacks of mandibular osteotomy, especially bilateral sagittal split ramus osteotomy(BSSRO) and genioplasty. In this study, 53 consecutive patients under class Ⅲ malocclusion impression who were taken with BSSRO only 30 cases and BSSRO with genioplasty 23 cases were carried out inferior alveolar nerve dysfunction test according to time at 1, 2, 4, 8 weeks and 6 months, 1 year postoperatively. The tinnel sign was checked by patient's self-evaluation. 20 members without trauma and nerve damage history, as control group nerve test was taken. Majority of patiens returned to normal sensation during post operative 6 months. Although immediate nerve deficit were 82.86%, 74.29%, 75%, these were recovered to 16.22%, 21.62%, 13.51% at 6 months and 0%, 31.2%, 3.12% at 1 year. Nerve recovery tendency increased predominantly between post-op 4 weeks and 6 months. There was no statistically difference neurosensory deficit among the chin area. Neurosensory deficit was more severe when the BSSRO with genioplasty group than the BSSRO only group at post-op. But 1 year later, the differences between two groups was no significant. There was no significant difference between left side and right side in neurosensory distubance. Static light touch was more sensitive method of neurosensory deficit than two point discrimination and pin pressure nociception. Tinnel sign was almost appeared at between 1 week and 3 weeks after operation.
