        119구급대를 통한 서울지역 외상환자 진료체계에 대한 통계분석

        Backgroud: Despite continous efforts to improve the prehospital trauma care system in Korea, the preventable death rate has been reported to be high. The purpose of this study was to evaluate the prehospital trauma care system in Seoul by analyzing 119 rescue databases. Methods: The 119 rescue data bases of 22,275 trauma patients, who were transported to the secondary and tertiary hospital in Seoul by Seoul 119 rescue services from January 1, 2000 to December 31, 2000, were analyzed. Results: The response time(mean 3.7 min.) showed no time, weekly, and regional variations. However, the transport time from field to hospital showed was high in the morning rush hour(7:00 to 10:00 am), and was gradually decreased and the lowest between 10:00 pm and 7:00 am. There was also a regional variation in the transport time, which was short in the central area and long in the peripheral area of Seoul. Prehospital cares were given to the 10,999 trauma patients(49.4%). Of the 464 unresponsive patients(2%), only 236 patients were identified in transported hospitals. The outcomes of these unresponsive patients were DOA(54%), survival(19%), death in ER(14%), transfer to other hospitals(8%), and death after admission(5%) in order. Conclusion: We suggest that this study may be helpful to the establishment and improvement of the prehospital trauma care system as well as the determination of the adequate numbers and locations of trauma center in Seoul.

        Water soluble components of Persicaria hydropiper have an insecticidal effect. The extraction mechanism of water solubles from Persicaria hydropiper was studied and it turned out to be a diffusion model. The diffusivity was logarithmically increased with respect to extraction temperature (20…70℃) showing the values in 0.7…2.65×10^-7 cm²/min. Some variables which have an effect on extracted quantity of water solubles were also observed. As a result, optimum operatin conditions and empirical equation were presented for the industrial scale extraction.

        The water-soluble extract of Persicaria hydropiper was investigated with respect to its insecticidal effect. Small plant parasitic insects such as Aphis and Perichaeta communissma were wholly killed within 60 min, in the aqueous solution including 1250 ppm of the water-soluble extract. This extract was passed through cation and anion resins, and the non-adsorption parts were fractionated with membrane filters. The lower molecular fraction (MW/<500) exhibited a strong insecticidal effect and a new component having R_f value of 0.24 has been observed on the TLC from this low molecular fraction.

        요인분석법을 이용한 광촉매 산화반응조의 최적 운영인자 도출

        폐수내의 오염물질 제거를 위해 광촉매 산화반응조의 운영인자로 선택한 대상물질의 초기농도, 자외선의 강도, 촉매의 표면적에 대해 실험을 수행하여 각 인자 수준에 따른 페놀농도의 제거율을 조사하고, 분산분석법을 통해 오차에 의한 영향을 평가하고, 요인분석법을 적용하여 각 인자 및 인자 사이의 상호작용 효과를 계산하여 광촉매 산화반응조의 최적운영 조건을 도출하였다. 광촉매 산화반응조의 실험인자로 선택한 페놀의 초기농도, 자외선의 강도, 촉매의 표면적이 페놀의 제거량에 미치는 정도를 조사하기 위하여 각 인자의 수준을 3가지로 변화시키고 반복횟수 3회인 예비실험을 통해, 각 인자의 수준변화가 페놀의 제거에 영향을 미칠 수 있다는 것을 분산분석법으로 검증하였으며, 시간당 페놀 제거농도는 각 인자의 수준을 각각 50 mg/L, 20,000 μW/cm², 2,105 cm²때 가장 높았다. 광촉매 산화반응조의 실험인자로 선택한 페놀의 초기농도, 자외선의 강도, 촉매의 표면적이 각각 페놀의 제거에 미치는 영향과 인자와 인자 사이에 존재하는 상호작용의 영향을 규명하기 위하여 각 인자의 수준이 2가지이고 반복횟수가 3회인 요인분석 실험을 수행하였다. 페놀의 초기농도를 5에서 50 mg/L로, 자외선 강도를 5,000에서 20,000 μW/cm² 촉매의 표면적을 740에서 2,105 cm²증가시킴으로써 얻을 수 있는 페놀농도의 제거율은 각각 1.86 및 1.79와 2.10 mg/L·hr 이었으며, 인자와 인자 사이의 상호작용 효과는 페놀의 초기농도와 촉매의 표면적 사이에 존재하는 상호작용의 영향을 제외 하고는 각 인자들의 주 효과에 비교하여 페놀의 제거농도에 미치는 영향이 상대적으로 적었다. 따라서 각 인자의 수준을 높은 방향으로 운영하는 것이 가장 많은 대상물질을 제거할 수 있지만 광촉매 산화반응조로 유입되는 오염물질의 농도를 조절하기 곤란하고, 자외선의 강도 증가 또한 전력비등으로 한계가 예견되므로, 대상물질의 제거에 가장 큰 영향을 미치는 촉매의 표면적을 증가시키는 것이 광촉매 산화 반응조를 효율적으로 운영하는 방안이라고 판단된다. The objective of this study is to determine the optimum conditions of operational parameters using factorial design for phenol degradation in photocatalytic oxidation reactors. Factorial design is widely used to select the dominant factors and their ranges in experiments involving several factors where it is necessary to study the effect of factors on a response. The effects of initial concentration of phenol, intensity of UV light and surface area of catalyst on phenol degradation were investigated. Two levels were considered in this study so that the experiment was a 2³ factorial design with three replicates. The experimental results show that an increase in initial concentration of phenol from 5 to 50 mg/L, intensity of UV light from 5,000 to 20,000 μW/cm² and surface area of catalyst from 740 to 2,105 cm² enhanced the phenol degradation rate by an average of 1.86, 1.79, and 2.10 mg/L·hr, respectively. Interaction effects do not appear to be as large on the phenol degradation rate as the main effects of single factors. The optimum working condition for photocatalytic oxidation reactors, despite the higher three factors the better removal rate, is the highest surface area of catalyst.

        Case Report : Acute Patellar Tendon Rupture after Total Knee Arthroplasty Revision

        Patellar tendon rupture is a catastrophic complication following total knee arthroplasty (TKA). Though revision TKA has been suspected of being a predisposing factor for the occurrence of patellar tendon rupture, there are few reports on patellar tendon rupture after revision TKA. Here, we present a case of acute patellar tendon rupture that occurred after TKA revision. In the patient, the patellar tendon was so thin and could not be repaired, and accordingly was sutured end to end. We used the anterior tibialis tendon allograft to augment the poor quality patellar tendon tissue. Fixation of the allograft was done by using the bone tunnel created through tibial tuberosity and suturing the allograft to the patellar tendon and quadriceps tendon. The patient was instructed to wear a full extension knee splint and was kept non-weight bearing for 6 weeks after operation. Full knee extension could be achieved 6 weeks postoperatively.

        Navigation-Assisted Total Knee Arthroplasty for Patients with Extra-Articular Deformity

        Purpose: Since the existence of an extra-articular deformity seriously alters the normal geometry and kinetics around the knee joint, difficulties are often encountered in total knee arthroplasty (TKA) using a standard surgical technique. The purpose of this study was to evaluate the usefulness of surgical navigation system as a treatment option for osteoarthritic knees with extra-articular deformity. Materials and Methods: The authors retrospectively reviewed medical records of the patients who underwent primary TKA between 2007 and 2012. Knees with preoperative radiography showing an angular deformity within the region from the middle third of the femur to the middle third of the tibia in the ipsilateral limb of the arthritic knees were considered as cases having extra-articular deformity. Thirteen knees of the 13 patients were found to have undergone TKA using a navigation system for osteoarthritis with ipsilateral extra-articular deformity. The hip-knee-ankle angle, Knee Society score (KSS), and range of motion were measured before and after the operation to evaluate the improvement. Results: The mean hip-knee-ankle angle in the coronal plane was improved to 0.2o±4.5o in valgus alignment postoperatively. The KSS was improved to 89.6±4.6 points postoperatively at the last follow-up, with over 90% of good and excellent results. The range of motion was improved to 118.5o±10.5o postoperatively. Conclusions: Navigation-assisted TKA is a good treatment option of osteoarthritic knees with extra-articular deformity.

        High-Flexion Total Knee Arthroplasty Using NexGen LPS-Flex System: Minimum 5-year Follow-up Results

        Purpose: This study is to report clinical and radiological results of high-flexion total knee arthroplasty (TKA) using NexGen LPS-flex system at a minimum 5-year follow-up, and to analyze the implant survivorship based on the results. Materials and Methods: A total of 80 patients (118 knees) who underwent patellar preserving TKA using NexGen LPS-flex implant between February 2007 and February 2008 and could be followed for minimum 5 years were reviewed. The range of motion (ROM), hip-knee-ankle angle, Knee Society Knee score (KSKS), and Knee Society Function score (KSFS) were assessed preoperatively and at the last follow-up and analyzed. Implant position of the femoral and tibial components on the immediate postoperative and last follow-up X-rays were compared. Results: The mean ROM was 110.2o±14.5o (range, 60o to 140o) preoperatively and 132.4o±5.2o (range, 90o to 145o) at the last follow-up. KSKS was 36.9o±6.4o preoperatively and 94.2o±3.2o at the last follow-up. KSFS was 30.5o±5.7o preoperatively and 93.7o±4.1o at the last follow-up. There was no statistically significant change in the implant position measured as α, β, γ, and δ angles at the last follow-up compared to the immediate postoperative values. Radiolucent lines were observed in 13 knees (11%) on the last follow-up X-rays. Revision TKA was performed due to aseptic implant loosening in 1 knee (0.84%), and the survival rate at the 5th postoperative year was 99.2%. Conclusions: The clinical and radiological outcomes of high-flexion TKA using NexGen LPS-Flex implant design were satisfactory with 99.2% implant survival rate after 5 years of protected activities of daily living.

        Comparison of Precision between Optical and Electromagnetic Navigation Systems in Total Knee Arthroplasty

        purpose: The purpose of this study is to compare and analyze the precision of optical and electromagnetic navigation systems in total knee arthroplasty (TKA). materials and methods: We retrospectively reviewed 60 patients who underwent TKA using an optical navigation system and 60 patients who underwent TKA using an electromagnetic navigation system from June 2010 to March 2012. The mechanical axis that was measured on preoperative radiographs and by the intraoperative navigation systems were compared between the groups. The postoperative positions of the femoral and tibial components in the sagittal and coronal plane were assessed. results: The difference of the mechanical axis measured on the preoperative radiograph and by the intraoperative navigation systems was 0.6 degrees more varus in the electromagnetic navigation system group than in the optical navigation system group, but showed no statistically significant difference between the two groups (p>0.05). The positions of the femoral and tibial components in the sagittal and coronal planes on the postoperative radiographs also showed no statistically significant difference between the two groups (p>0.05). conclusions: In TKA, both optical and electromagnetic navigation systems showed high accuracy and reproducibility, and the measurements from the postoperative radiographs showed no significant difference between the two groups.

        Femtosecond characterization of vibrational optical activity of chiral molecules

        Optical activity is the result of chiral molecules interacting differently with left versus right circularly polarized light. Because of this intrinsic link to molecular structure, the determination of optical activity through circular dichroism (CD) spectroscopy has long served as a routine method for obtaining structural information about chemical and biological systems in condensed phases. A recent development is time-resolved CD spectroscopy, which can in principle map the structural changes associated with biomolecular function and thus lead to mechanistic insights into fundamental biological processes. But implementing time-resolved CD measurements is experimentally challenging because CD is a notoriously weak effect (a factor of 10<SUP>-4</SUP>–10<SUP>-6</SUP> smaller than absorption). In fact, this problem has so far prevented time-resolved vibrational CD experiments. Here we show that vibrational CD spectroscopy with femtosecond time resolution can be realized when using heterodyned spectral interferometry to detect the phase and amplitude of the infrared optical activity free-induction-decay field in time (much like in a pulsed NMR experiment). We show that we can detect extremely weak signals in the presence of large achiral background contributions, by simultaneously measuring with a femtosecond laser pulse the vibrational CD and optical rotatory dispersion spectra of dissolved chiral limonene molecules. We have so far only targeted molecules in equilibrium, but it would be straightforward to extend the method for the observation of ultrafast structural changes such as those occurring during protein folding or asymmetric chemical reactions. That is, we should now be in a position to produce ‘molecular motion pictures’ of fundamental molecular processes from a chiral perspective.
