        Two-Track Medical Treatment Strategy According to the Clinical Scoring System for Chronic Rhinosinusitis

        <P><B>Purpose</B></P><P>The previously reported Japanese clinical scoring study (JESREC) suggests that chronic rhinosinusitis (CRS) can be divided into 4 subtypes according to the degree of eosinophilic CRS (ECRS) and offers the information regarding the prognosis of CRS to clinicians. However, this scoring system has not yet been validated by an immunological study and needs to provide treatment guidelines based on underlying immunologic profiles. We investigated the immunologic profile of each CRS subgroup according to the JESREC classification and suggest its clinical application.</P><P><B>Methods</B></P><P>A total of 140 CRS patients and 20 control subjects were enrolled. All patients were classified into 4 groups according to the JESREC (non-, mild, moderate and severe ECRS). Nasal tissues were analyzed for mRNA expression of major cytokines (IL-5, IL-10, IL-13, IL-17A, IL-22, IL-23p19, IFN-γ, periostin, thymic stromal lymphopoietin [TSLP] and ST2), major chemokines (CCL11, CCL24, CXCL1 and CXCL2), transcription factors (T-bet, GATA3, RORC and FOXP3) and COL1A1 for type I collagen. Protein levels of 3 major cytokines (IL-5, IL-17A and IFN-γ) were also measured by multiplex immunoassay. Principal component analysis (PCA) was conducted to investigate the overall profile of multiple mediators.</P><P><B>Results</B></P><P>The moderate/severe ECRS showed up-regulation of type 2-related mediators (IL-5, IL-13, periostin, TSLP and ST-2), whereas INF-γ (type 1 cytokine) and CXCL1 (neutrophil chemokine) expressions were increased in non-/mild ECRS compared with moderate/severe ECRS. The JESREC classification reflected an immunological endotype. In PCA data, PCA1 indicates a relative type 2 profile, whereas PCA2 represents a type 1/type 17-related profile. In this analysis, mild ECRS was indistinguishable from non-ECRS, whereas moderate to severe ECRS showed a distinct distribution compared with non-ECRS. The JESREC classification could be divided into 2 categories, non-/mild vs. moderate/severe ECRS based on underlying immunological analyses.</P><P><B>Conclusions</B></P><P>The CRS clinical scoring system from the JESREC study reflects an inflammatory endotype. However, the immunologic profile of mild ECRS was similar to that of non-ECRS. Therefore, we propose type 2-targeted medical treatment for moderate to severe ECRS and type 1/type 17-targeted for non-ECRS and mild ECRS as the first treatment option.</P>

        Effects of High Cholesterol Diet on Newly Generated Cells in the Dentate Gyrus of C57BL/6N and C3H/HeN Mice

        <P>In this study, we observed and compared the effects of a high cholesterol diet (HCD) on cell proliferation and differentiation in the subgranular zone of the dentate gyrus of C57BL/6N (B6, susceptible strain) and C3H/HeN (C3H, resistant strain) mice. Ki67 (a marker for cell proliferation) positive cells) were significantly decreased in HCD-fed B6 mice compared to those in B6 (49.7%) and C3H mice fed a low cholesterol diet (LCD). In addition, doublecortin (DCX, a marker for cell differentiation or neuroblasts)-immunoreactive cells in HCD-fed B6 mice were significantly decreased compared to those in LCD-fed B6 and C3H mice. These results suggest that B6 strains are sensitive to HCD, which impairs cell proliferation and differentiation.</P>

        농업생명과학대학 교수가 활용하는 교수설계, 방법 및 전략

        이 연구의 목적은 농업생명과학대학 교수들이 과목 지도에서 활용하는 교수전략, 방법 및 전략을 확인하는데 있었다. 연구 목적을 달성하기 위해 총 46개 대학에 근무하는 1,471명의 교수 중 307명을 단순무선표집하여 설문조사를 실시하였다. 주요 연구 결과는 다음과 같다. 첫째, 농학계 대학 교수들은 교수능력을 개발하기 위한 기회가 부족한 편이며, 교육방법에 대한 학습경험이 적은 편이다. 둘째, 농학계 대학 교수들은 사전에 수업운영에 대한 구상은 하지만 실제적인 지도계획 수립이나 구체적인 자료준비는 부족하며, 수업을 위한 교안 작성자는 과반수에 지나지 않았다. 셋째, 농학계 대학 교수들은 강의법, 실험 및 실습 등의 한정된 수업방법을 주로 활용하나 최근에 강조되는 학습자 중심 교수방법들의 활용은 부족한 것으로 나타났고, 실제 수업 단계에서 특히 수업 도입이나 마무리 단계가 미흡한 것으로 나타났다. 넷째, 농학계 대학 교수들은 학생 강의평가 결과를 수업개선에 적극적으로 활용하지 않으며, 강의평가를 제외한 대학차원의 교수개선 지원이 부족하다. 이러한 결과에 기초하여 이 연구에서는 농업 및 농촌 현장과 연계된 교육, 교육목표에 적절한 교수설계, 학습자 중심의 교수방법 활용, 강의부담 경감, 교수 전문성 향상을 위한 기회 및 지원 제공 등의 개선방안을 제안하였다. The purpose of this study was to identify the instructional designs, teaching methods and strategies used by professors in the college of agriculture and life sciences in Korea. The population for this study was 1,471 professors of agriculture and life science colleges. 307 professors were randomly chosen from the population for the study. The survey was carried out by e-mail or in person. The major findings of this study were as follows: First, professors of colleges of agriculture and life sciences do not have many opportunities to improve their own teaching abilities. Also, they do not have many training experiences in taking pedagogy class, and attending seminars and workshops. Second, professors do not make an actual lesson plan or prepare enough learning aids materials on a class period base. Third, the professors rarely used the more learner-centered approaches. In addition, they usually emphasized on the "body stage" of each lesson which resulted in the unbalanced instruction in the "opening" and the "closing stages" of the lesson. Finally, professors execute the course evaluation, but the results of these evaluations are not being used for improvement of teaching and learning. Based on these results, some recommendations were made: the field-related education in agriculture, proper systematic design of the course teaching, use of more learner-centered teaching methods, building the constructive environment for the professors by reducing mandatory lectures and the balanced teaching from the opening to the closing in every instruction.

        수지의 원위부 피판에서의 감각기능 회복에 대한 임상적 고찰

        A consecutive series of 271 patients(285 digits) underwent the distant flap reconstruction for digital injury between January 1985 and June 1991. The cases were reviewed and the sensory recovery of the finger tip was assessed by the two point discrimination test. It usually depends on the degree and the extent of the injury and involved digit. The results were as follows : 1. Younger patients showed better recovery of 2-point discrimination. 2. Better recovery of 2-point discrimination was related to the lapse of time. 3. The recovered 2-point discrimination resembled that of the recipient site more than that of the donor site. 4. The smaller size of the distant flap showed better recovery of 2-point discrimination 5. The cross arm flap showed better recovery of 2-point discrimination than abdomen, groin&buttock flaps. 6. The pedicled flap showed better recovery of 2-point discrimination than tubed flap. 7. Sensory recovery increased with the thinning of the flap with debulking 8. Reconstruction with the neurovascular island flap is recommended in the thumb and index where heat, cold, touch-pressure& vibration sense are required.

        가토의 골수내 식염수 주입이 말초혈액 제변수에 미치는 영향

        Study Objective: To examine the effects of intraosseous saline infusion on hematologic parameters. Design and Setting: Eight New Zealand White rabbits were anesthized intramuscular ketamine injection, An initial blood sample was withdrawn from an earlobe artery under introduction of anesthesia. An 18-gauge intraosseous(10) needle was inserted into proximal tibia of rabbits, and 10 mL/kg of normal saline was infused over two minutes after the intraosseus needle insertion. Measurements: Complete blood counts were performed on the blood sampled. The light microscopic examination of the peripheral blood smear was perfomed by clinical pathologic staff. Results: No statistically significant differences were found in hematologic paramenters from samples drawn before and after intraosseous infusion. Conclusion: We advocate less caution in the interpretation of hematologic studies performed on blood samples obtained soon after an intraosseous infusion.

        Terfenadine과 Ketoconazole 복용 후 발생된 Torsades de pointes 심실 빈맥증의 치험 1례

        Torsades de pointes is a form of polymorphic ventricular tachycardia that is associated with prolongation of the QT interval, characterized by QRS complexes of progressively changing amplitude and contour that seem to revolve around the isoelectric line. The most common cause of Torsades de pointes induced by antiarrhythmic agents, occupying in certain drugs or toxin, electrolyte disturbance, congenital QT prolongation syndrome. Cardiotoxicity terfenadine, in overdose a widely used, nonsedating antihistamine, has been reported. Case after administration of terfenadine and ketoconazole causing syncope and torsades de pointes have been reported in recommended dose. We report a case of QT prolongation with torsades de pointes after therapeutic administration of terfenadine, a long acting histamine H1 receptor antagonist, together with ketoconazole, an antifungal preparation. The patient is a 43-year-old female, who experienced palpitation, dizziness and syncopal episoders after coadministration of terfenadine and ketoconazole, for a fungal infection of the skin. The ECG showing typical episodes of torsades de pointes with QT prolongation. Magnesium sulfate was given, torsades de pointes did not recur. The patient was discharged with improved condition.

        구순열 및 구개열 환자에 대한 Toxoplasma항체의 혈청학적 연구

        The etiology of the cleft lip and palate can be explained with a multifarious theory. According to articles, there are some relationships between congenital toxoplasmosis and the cleft lip&palate, but the direct relationships is not obvious. Therefore, this research aims to find the difference of the serum toxoplasma antibody titer between 172 cleft patients who have been treated at National Medical Center and their mother group of 81 cases, healthy control group of 150 cases. The antibody titer was identified by the indirect fluorescent antibody test(>16)and the enzyme-linked immunosorbent assay(>0.3). The results are as follow : 1. By ELISA, the positive rate of the cleft patients(8.1%, 14cases)and that of the mother group(9.9%, 8cases)is higher than the healthy control group(2.7%, 4cases)(p<0.05). 2. By IFAT, the positive rate of the cleft patients(10.5%, 18cases)is higher than the healthy control group(5.3%, 8cases), but the difference is not significant(p>0.05). And the positive rate of the mother group(13.6%, 11cases)is higher than the healthy control group(5.3%, 8cases)(p<0.05). 3. By ELISA and IFAT, the positive rate of the mother group is higher than the cleft patient but the difference is not significant. 4. By ELISA and IFAT, the positive rate of the cleft patients according to the clincal group is not significant. 5. By ELISA and IFAT, the positive rate of the chronic disease group(17.3% & 27.7%)is higher than the healthy control group(2.7% & 5.3%)(p<0.05).

        The basin of the river Bosung is more abounds in water resources than the other basins. Domestic and industrial water supply in Kwangju, Yosu, Sunchon, and Kwangyang area will be depends on water resources in this basin. This study, therefore, was conducted to predict the changes of weather and the structure of riverbed in this basin, which was caused by contruction of Ju―Am Dam. Results obtained were as follows. 1. The changes of weather(e.g. abandunce fog, drop in temperature) will be arise. 2. Because of low runoff, there is little changes of riverbed and transportation of quicksand. 3. From the results of continous changes of weather and structure of riverbed, it requires futher research.

      • 1995년 및 1996년에 국내에서 분리된 풍진바이러스의 염기서열 및 계통분석

        목적: 유소아에서 경한 발진성 질환을 일으키는 풍진바이러스는 토가바이러스과에 속하며 일회 감염으로 평생면역을 획득하는 것으로 알려져 왔다. 그러나 1960년대 후반 풍진예방접종이 시행된 이후 선진국의 경우 풍진 호발 연령이 유소아에서 청소년 층으로 변화하는 양상을 나타내었으며 우리 나라의 경우에도 1995년에서 1996년도 사이에 발생한 풍진대유행기에 주로 중 고등학생층에서 풍진이 호발하였다. 주기적인 풍진의 유행에도 불구하고 우리 나라에서는 최근까지도 풍진 바이러스의 분리나 유전정보에 대한 연구보고가 없어 이에 대한 연구를 시행하였다. 방법: 1995년에서 1996년에 발생한 풍진 환자의 인후분비물을 채취하여 세포배양법으로 풍진바이러스 분리를 시도하였으며, 분리된 풍진바이러스의 RNA를 추출하여 역전사 중합효소연쇄반응법으로 증폭한 후 염기서열 및 아미노산 서열을 결정 분석하였다. 결과: 세포배양을 통해 7주의 풍진바이러스를 분리하였으며 국내 풍진바이러스주들은 염기서열상 92.5-100%의 동질성을 나타내었고, 백신주 RA27/3과는 2.2-6.4%의 차이를 보였다. 아미노산서열상에서는 국내에서 분리된 풍진바이러스주들은 98.3-100%의 동질성을 나타내었고, 백신주 RA27/3과는 1.7%이하의 차이를 보였다. 계통분석 결과 국내에서는 최소한 2가지 유형의 풍진바이러스가 유행하였다. 결론: 국내에서 유행한 풍진바이러스주의 E1 gene 부위를 기존에 알려진 외국의 분리주나 백신주와 비교시 의미있는 차이는 관찰되지 않았으며 Vero E6 세포는 풍진바이러스의 분리배양에 유용함을 알수 있었다. 그러나 풍진바이러스의 지속적인 변이 양상이 관찰된 사실은 체계적이고 지속적인 풍진바이러스 감시체계의 필요성을 제시하였다. Background: The age group of Rubella virus (RV) infection in most industrialized nations located in temperate climates, has in large part shifted from children to young adults since introducing of the RV vaccine in late 1960's. Interestingly, there were rubella outbreakes from 1995 through 1996 in Korea, and middle and high school students were mostly affected during that time. Although continued of genetic information of rubella viruses circulated in Korea. Method: To isolate RV circulated in Korea, and determine the phylogenetic relationship between RV strains in Korea and RV isolates from other geographic regions including vaccine strains, we inoculated nasopharyngeal secretion samples from clinically diagnosed rubella patients to Vero E6 cells, and sequenced corresponding region of the 5' EL encoding genomic regions of RV isolates. Result: Seven RV strains isolated from Korea showed 93.6 to 97.8% and 98.3 to 100% sequence homologies in nucleotide and amino acid levels, respectively, compared to RA27/3 vaccine strain. Phylogenetic tree based on 359bp of RV indicated that at least two different groups of RV circulated in Korea during 1995-1996 epidemics. Conclusion: Our data suggested that mutant RV strains were possiblely not the cause of recent rubella epidemics in Korea.
