ISP의 BcN 투자 및 요금체계 평가모형 : 실물옵션과 몬테카를로 시뮬레이션의 응용

모든 정보통신서비스가 All-IP 통합 인프라로 제공되는 BcN 구축은 우리나라의 핵심 국책사업 중의 하나로, 인터넷 강국으로서의 글로벌 입지를 굳히는데 결정적인 역할을 할 것으로 기대된다. 그러나 BcN 기술의 성숙에도 불구하고, 제도적 측면에서 이의 성공을 뒷받침하는 연구는 드물다. 특히 BcN 요금체계와 같은 정책적 문제에 대해서는 별다른 진전이 없다. 본 연구에서는 요금정책이 BcN 투자에 미치는 영향을, 실물옵션과 Monte Carlo 시뮬레이션을 이용하여 분석하였다. 실험결과, BcN 수요의 불확실성뿐만 아니라, 요금체계와 관련된 정책대안이 사업자의 수익구조와 투자동기에 중요하게 작용하였다. 또한 수익과 투자시점은 밀접한 관계를 가지며, 트래픽 규모가 수입에 직접 반영되는 요금체계에서 BcN 투자가 앞당겨짐을 알수 있다. 특히 ISP와 CP간의 요금체계가 투자 의사결정에 보다 크게 작용하는 것으로 파악된다. 이러한 결과는, 사업자의 자발적인 첨단 인프라 및 서비스 개발이 국민경제에 긍정적으로 작용하는 동태적 효과를 보다 전향적으로 검토할 것을 요청한다.
여성노인의 저항운동 시 세트구성 방법의 차이가 Myokine에 미치는 영향

연구목적: 본 연구는 여성노인을 대상으로 저항운동 시 사용되는 set구성 방법에 중 ascending set, descending set, drop set를 각각 적용시켜 Myokine의 변화를 알아보고자 하였다. 연구방법: 연구대상은 노인복지시설을 이용하고 있는 만 65-72세 사이의 여성노인 32명을 대상으로 ascending set, 8명, descending set 8명, drop set 8명, control group 8명으로 구분하였다. 자료처리는 반복측정에 의한 이원변량분석(two-way ANOVA with repeated measure)을 사용하였으며, 유의도는 .05로 하였다. 결론: 운동프로그램은 12주간 주 3회 실시하였으며, 신체구성과 IL-15, BDNF, VEGF, Irisin의 변화를 관찰하였다. 연구결과 신체구성의 체중, 체지방률, 제지방량 모두 유의한 차이가 없었다. IL-15와 BDNF는 운동 전·후의 비교결과 Descending set와 Drop set에서 운동 후 유의하게 증가하였고, 집단 간의 차이에서 Descending set와 Drop set 집단이 대조집단에 비하여 높게 나타났다. VEGF는 운동 전·후의 비교결과 Descending set와 Drop set 집단이 유의하게 증가하였으며, 집단 간의 차이에서 Descending set와 Drop set가 Ascending set와 control group 보다 높게 나타났다. Irisin은 집단 간 차이는 없었지만 Ascending, Descending, Drop set집단에서 운동 전에 비하여 운동 후 유의한 차이로 증가하였다.. Purpose: The purpose of this study is to identify the change of Myokine by applying ascending set, descending set, and drop set which are set composition used during elderly women’s resistance training. Method: 32 participants aged 65 to 72 who were users of welfare facilities for the aged were divided into 4 groups. They were ascending set group of 8, descending set group of 8, drop set group of 8, and control group of 8. The training program was conducted 3 times a week for 12 weeks and the change of body composition, IL-15(interleukin-15), BDNF(brain derived neurotrophic factor), VEGF(Vascular endothelial growth factor) and Irisin was observed. Two-way ANOVA with repeated measure was used for data processing. Result: The result of this study is as follows. Weight, %fat, and LBM(lean body mass) without fat showed no significant change. IL-15 and BDNF increased significantly after training in Descending set group(p<.05; p<.01)and Drop set group(p<.01; p<.05). VEGF significantly increased in descending set group(p<.01) and drop set group(p<.05). Among groups, VEGF was higher in descending set and drop set groups than ascending set and control groups. Irisin showed no difference between groups, but it showed significant increase after training in ascending(p<.05), descending(p<.05), and drop set groups(p<.05).
Long Term Safety and Efficacy of Etanercept in Juvenile Idiopathic Arthritis in a Single Center

Objective. Our aim was to investigate the long term safety and efficacy of etanercept in children with juvenile idiopathic arthritis (JIA). Methods. The study subjects were the 90 JIA patients treated with etanercept in the Department of Pediatrics, Hallym University Medical Center between January 2004 and December 2017. We retrospectively reviewed their medical records for age at diagnosis, duration of etanercept treatment, number of active joints, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and adverse events during treatment. Results. Among the 90 patients, 38 (42.0%) were male and 52 (58.0%) were female; 15 (16.7%) had systemic onset, 41 (45.6%) had extended oligoarticular, 14 (15.6%) had rheumatoid factor-positive polyarticular, 18 (20.0%) had rheumatoid factor-negative polyarticular, and 2 (2.1%) had enthesitis-related arthritis. The median age at the start of etanercept treatment was 9 years (range, 3∼18 years), and the median duration of etanercept treatment was 6 years (range, 0.5∼13 years). The median number of active joints decreased from 9 to 0 after 6 months of etanercept treatment. The median CRP and ESR were within normal range after 3 months of treatment. Six patients experienced recurrence, 9 switched to other medications and 3 discontinued etanercept. Of the 14 reported adverse events, 1 was serious, and there were no tuberculosis infections or malignancies. Conclusion. Long-term treatment with etanercept is efficacious and safe for children with JIA. However, those with the systemic onset subtype appear to have low drug survival rate compared to those with other types of JIA. (J Rheum Dis 2019;26:200-205)

<P><B>Abstract</B></P> <P>We present a highly sensitive and selective NO<SUB>2</SUB> sensor based on Bi<SUB>2</SUB>O<SUB>3</SUB> branched SnO<SUB>2</SUB> nanowires (NWs). SnO<SUB>2</SUB> NWs were first synthesized by a vapor-liquid-solid method, were coated with an Au layer, and Bi<SUB>2</SUB>O<SUB>3</SUB> branches were grown on their stems by the same procedure used for pure Bi powders. The fabricated sensor showed a high response (R<SUB>g</SUB>/R<SUB>a</SUB>) of 56.92 to 2 ppm of NO<SUB>2</SUB> gas at an optimal temperature. Furthermore, its response to other interfering gases such as ethanol, acetone, toluene, and benzene, was less than 1.55, which demonstrated excellent selectivity of the sensor towards NO<SUB>2</SUB> gas. For comparison and to better understand the sensing mechanism, a pristine SnO<SUB>2</SUB> NWs sensor was also tested. The superior sensing properties of the branched NW sensor relative to the pristine sensor were mainly attributed to the high surface area of the sensor resulting from Bi<SUB>2</SUB>O<SUB>3</SUB> branching, as well as the formation of homo-and heterojunctions (Bi<SUB>2</SUB>O<SUB>3</SUB>-SnO<SUB>2</SUB>). In addition, several factors including the presence of Au contributed to the excellent selectivity to NO<SUB>2</SUB> gas. <P><B>Abstract</B></P> <P>We report the electrical and optical characteristics of SnO<SUB>2</SUB> thin films irradiated by microwaves (MWs) and grown using atomic layer deposition in a commercial MW oven operating at a frequency of 2.45 GHz. The properties of the MW-irradiated SnO<SUB>2</SUB> thin films were compared with those of the as-deposited SnO<SUB>2</SUB> thin films. After MW irradiation, the conductivity and transparency of the thin films were enhanced. In addition, the samples irradiated for 5 min showed optimal carrier concentration, Hall mobility, resistivity, and transmittance values of 1.5 × 10<SUP>20</SUP> cm<SUP>−3</SUP>, 4.6 cm<SUP>2</SUP>/V s, 8 × 10<SUP>−3</SUP> Ω cm, and 95.77%, respectively. The improved properties of the MW-irradiated samples were attributed mainly to the formation of an oxygen vacancy in the SnO<SUB>2</SUB> lattice during MW irradiation. Our results can be applied for the fabrication of pure SnO<SUB>2</SUB>-based transparent conductive oxides; these oxides are generally doped with other elements.</P>

Bacterial nanocellulose (BNC) which is generally synthesized by several species of bacteria has a wide variety of industrial uses, particularly in the food and material industries. However, the low levels of BNC production during the fermentation process should be overcome to reduce its production cost. Therefore, in this study, we screened and identified a new cellulose-producing bacterium, optimized production of the cellulose, and investigated the morphological properties of the cellulosic materials. Out of 147 bacterial isolates from ripened fruits and traditional vinegars, strain SFCB22-18 showed the highest capacity for BNC production and was identified as Komagataeibacter sp. based on 16S rRNA sequence analysis. During 6-week fermentation of the strain using an optimized medium containing 3.0% glucose, 2.5% yeast extract, 0.24% acetic acid, 0.27% Na<sub>2</sub>HPO<sub>4</sub>, and 0.5% ethanol at 30°C, about 5 g/l of cellulosic material was produced. Both imaging and IR analysis proved that the produced cellulose would be nanoscale bacterial cellulose.
Room-temperature NO<sub>2</sub> sensor based on electrochemically etched porous silicon

<P><B>Abstract</B></P> <P>With high-performance room-temperature gas sensors being in great demand from an energy-saving standpoint, in this study, we fabricated porous silicon (PS) sensors by electrochemically etching at different times (30, 60, and 90 min). The porous nature of the etched PSs was studied using scanning electron microscopy, and subsequently gas sensors were fabricated. NO<SUB>2</SUB> sensing studies showed that the highest gas performance can be obtained at room temperature (30 °C). Furthermore, the PS sensor etched for 60 min had the best performance among the sensors, which is related to its higher surface area and high enough initial resistance. In particular for the PS sensor etched for 60 min, the response (R<SUB>a</SUB>/R<SUB>g</SUB>) to 10 ppm NO<SUB>2</SUB> was 9.56, which was much higher than other interfering gases, demonstrating its high selectivity towards NO<SUB>2</SUB> gas. The explanation of isolatedorgan tuberculosis rests on the assumption that in the course of the lymphatic or hematogenous dissemination of bacilli, organisms may be rapidly destroyed in all other sites save for the particular tissue involved in the isolated tuberculous process. Tuberculosis can arise in all tissues having lymphatics or blood supply, but the disease causing biliary tract obstruction has been known to be rare. Recently, we experienced a case of isolated-organ tuberculosis causing common bile duct obstruction and periductal lymph node enlargement in a 46-year-old Korean male. An ultrasonographyguided percutaneous needle biopsy revealed a granulomatous inflammation of the lymph node. After 7 months of antituberculous medication, the common bile duct obstruction and periductal lymph node enlargement disappeared completely in a follow up abdominal CT and ERCP.
The clinical significance of arterial stiffness in the differential diagnosis of vertigo

Objectives: Pulse wave velocity (PWV) is a known indicator of arterial stiffness reflecting vascular damage. However, there are few reports of the relationship between PWV and dizziness/vertigo of a vascular origin. We investigated whether the PWV value could be useful as an ancillary measurement for identifying the etiology of dizziness/vertigo.Methods: We enrolled dizzy patients from March 2016 to December 2018. The patients with posterior circulation stroke presenting with dizziness/vertigo were categorized as having central vertigo. The patients with vertigo due to benign paroxysmal positional vertigo, Ménière disease, or acute unilateral vestibulopathy were categorized as having peripheral vertigo. The PWV value, ankle-brachial index (ABI), and traditional vascular risk factors were collected.Results: We consecutively enrolled a total of 93 participants. The PWV values were higher in the central vertigo group (p<0.001), but ABI did not differ between the groups. Among the vascular risk factors, the number of patients with diabetes mellitus, dyslipidemia, and male patients was significantly higher in the central vertigo group. The brachial-ankle PWV (baPWV) values were higher in the central vertigo group, even after adjusting for confounding factors (p<0.01). The receiver operating characteristic curve showed that the sensitivity was 74% and specificity was 81% when the mean baPWV value was 14.78 m/sec.Conclusions: Increased baPWV values were observed in patients with dizziness/vertigo due to stroke. Measurement of the baPWV value could be an ancillary test for evaluating the cause of dizziness/vertigo, even though more convincing evidence is still required.
