        The Effect of Group Exercise Program on Cognitive Function of Elderly people

        The goal of this study was to assess the effect of a group exercise program on cognitive function of elderly people. Subjects were chosen to be elders with dementia having minor to moderate degrees of cognitive function. Study was started out by randomly dividing the 16 subjects into two groups, each with 8 people; the group exercise group and the control group. The group exercise group performed 8 weeks of group exercise program and general physiotherapy while the control group only performed general physiotherapy. Cognitive function was measured by Korean version mini-mental state examination. The study group's attention and calculation statistically significantly improved but the control group saw no statistically significant change. The group exercise program affected improvement in cognitive function of elderly people with dementia and in particular, was effective for enhancing their attention and calculation.

        Comparison of Three-dimensional Kinematic Changes of the Lower Extremity between the Two Different Braking Distances of Snowplow in Alpine Skiing

        Objective: The aim of this study was to compare three-dimensional kinematic changes of the lower extremity between the two different braking distances during snowplow in alpine skiing. Method: Six alpine ski instructors (age: $25.3{\pm}1.5yr$, height: $169.3{\pm}2.9cm$, weight: $66.2{\pm}5.9kg$, career: $4.2{\pm}2.9yr$) participated in this study. Each skier was asked to perform snowplow on the two different braking distances (2 and 4 m). Results: Snowplow and edging angles (p = .006 and p = .005), ankle adduction and inversion (p = .033 and p = .002), knee extension (p = .003), and hip abduction and internal rotation (p = .043 and p = .006) were significantly greater in the 2 m than in the 4 m braking distance. Conclusion: Based on our results, we suggest that skiers should make greater snowplow and edging angles on the shorter braking distance. In this situation, ankle joint adduction/inversion angle and hip joint internal-rotation make greater snowplow angle, and hip joint abduction make greater edging angle. In addition, greater knee joint extension angle may lead to more posteriorly positioned center of mass.

        Effect of the Positions of Female Pro-Volleyball Players on the Stability of Shoulder Joints of the Dominant and Non-dominant Arms

        The purpose of this study was to comparatively analyze the active stability of shoulder joints according to the frequency of overhead motions, such as serving and spiking, engaged in by female professional volleyball athletes who play different positions, and to provide the results as the basic data for developing exercise programs to prevent shoulder joint injuries. The subjects of this study were 50 Korean female professional volleyball players and positions were as follows: left and right attackers, centers, setters, and liberos. The external rotation and internal rotation muscle strength and muscle strength ratios of the dominant and non-dominant arms of all subjects were measured using Biodex. The results of this study are as follows: Frist, no significant differences were found in the internal and external rotation muscle strength of the dominant and non-dominant arms between positions. Second, for the shoulder joint muscle strength ratio of the dominant arm, by position, the setter showed significantly greater stability compared to the other positions. Third, for the shoulder joint muscle strength ratio of the non-dominant arm, by position, no significant difference in stability between positions was found. Fourth, it was found that the dominant arm had significantly greater instability of the shoulder joint than the non-dominant arm for attackers and centers, but no significant difference was found for setters and liberos. This study comparatively analyzed the muscle strength ratios of the external/internal rotations and dominant/non-dominant arms, which can determine the stability of the shoulder joints between female professional volleyball playing positions that engage in jumps and spikes using only the dominant hand and positions that do not.

        알파인 스키 회전기술에 따른 인체분절과 스키 간 각운동학 및 상호상관분석

        Objective: The purpose of this study was to investigate the relative angles and cross-correlation between body segments and ski among four alpine ski turning techniques. Method: 19 alpine ski instructors participated in this study. Each skier asked to perform 4- types of turning technique, classified by radius and level. 8 inertial measurement units were used to measure orientation angle of segment and ski on the anteroposterior and vertical axis. Results: Significant differences were found between types of turning in the segments-ski relative angle on the anteroposterior and vertical axis (p<.05). Although, cross-correlation showed a high correlation between angles of segment and ski, there were significant differences between types of turning. Conclusion: Based on our results, the relative movement and timing between each segment and ski is different according to the turning techniques, so the training methods should be applied differently.

        The Effects of NEES on PARP Expression and Cell Death in Rat Cerebral Cortex After Ischemic Injury

        The majority of strokes are caused by ischemia and result in brain tissue damage, leading to problems of the central nervous system including hemiparesis, dysfunction of language and consciousness, and dysfunction of perception. The purpose of this study was to investigate the effects of Poly(ADP-ribose) polymerase(PARP) on necrosis in neuronal cells that have undergone needle electrode electrical stimulation(NEES) prior to induction of ischemia. Ischemia was induced in male SD rats(body weight 300g) by occlusion of the common carotid artery for 5 min, after which the blood was reperfused. After induction of brain ischemia, NEES was applied to Zusanli(ST 36), at 12, 24 and 48 hours. Protein expression was investigated using immuno-reactive cells, which react to PARP antibodies in cerebral nerve cells, and Western blotting. The results were as follows: In the cerebral cortex, the number of PARP reactive cells after 24 hours significantly decreased(p<.05) in the NEES group compared to the GI group. PARP expression after 24 hours significantly decreased(p<.05) in the NEES group compared to the GI group. As a result, NEES showed the greatest effect on necrosis-related PARP immuno-reactive cells 24 hours after ischemia, indicating necrosis inhibition, blocking of neural cell death, and protection of neural cells. Based on the results of this study, NEES can be an effective method of treating dysfunction and improving function of neuronal cells in brain damage caused by ischemia.

      • 내시경적 점막절제술로 치험한 식도 과립상 세포종 1례

        과립상 세포종은 Schwann 세포 기원으로 생각되며 인체에 비교적 드물게 발생한다. 과립상 세포종은 전신 어느 곳에서나 발견될 수 있으나 주로 혀, 구강, 피부 혹은 유방 등에서 호발하며 드물게 위장관에서 발견된다. 위장관에서는 식도에서 가장 호발하며 다음으로 위, 대장 순이다. 과립상 세포종은 대부분, 특히 위장관에서는 양성이며 소수의 악성 병변이 보고되었다. 이러한 이유와 함께 수술 전의 진단이 어렵기 때문에 과립상 세포종에 대한 근본적인 치료는 현재까지 외과적 절제술이다. 최근에 시도되는 치료방법들로는 내시경적 레이저 치료, 용종절제술, 내시경적 점막 절제술 등이 있다. 저자들은 상부 소화관 내시경검사를 시행하여 식도 과립상 세포종을 진단하고 내시경적 점막 절제술을 시행하여 합병증 없이 퇴원하여 현재 재발없이 경과 관찰중인 1례를 경험하였기에 보고하는 바이다. Granular cell tumors, which occur infrequently, are probably of Schwann cell origin. They can occur almost anywhere in the body but usually affect the tongue, oral cavity, skin, or breasts and are rarely found in the gastrointestinal tracts. The esophagus is the most frequent gastrointestinal site, followed by the stomach and the colon. Granular cell tumors are generally benign, especially in the gastrointestinal tract, some malignant lesions have been reported. For this reason, and also because preoperative diagnosis is difficult, the standard treatment for granular cell tumor has until now been surgical excision. In recent years, other therapeutic methods is endoscopic laser therapy (ELT), polypectiomy, endoscopic mucosal resection (EMR). We report a case of esophageal granular cell tumor which was diagnosed by an endoscopy and managed using an endoscopic mucosal resection without complication.

        평창동계올림픽 알파인 스키 활강ㆍ슈퍼대회전 코스의 지형 특성 및 경기력 요인에 미치는 영향

        본 연구의 목적은 평창 동계올림픽이 열린 정선 알파인 경기장의 활강(downhill, DH) 및 슈퍼대회전(Super giant slalom, SG) 종목 코스에 대한 특성을 정량화하고, 그 특성과 스키어의 속력과의 관계에 대하여 조사하는 것이다. 이를 위해 정선 알파인 스키 경기장에서 지속적인 훈련을 실시하고 있는 올림픽 대표선수 중 평창 동계올림픽 출전권을 확보한 상위수준 선수가 본 연구에 참여하였다. 위성항법시스템을 이용하여 각 종목별 기문의 위치를 측정하고, 알파인 스키어에게 착용하여 각 종목 수행 시 나타나는 위치데이터를 수집하였다. 수집된 위도, 경도, 고도 데이터는 미터단위로 환산하여, 기문 간 거리ㆍ기문변화각ㆍ경사도를 산출하고 스키어의 속력을 계산하여 상관관계를 분석하였다. 기문 간 거리는 DH에서 89.79±37.07 m, SG에서 55.85±20.40 m, 기문변화각은 DH에서 136.83±19.45°, SG에서 147.26±15.54°, 경사도는 DH에서 -18.71±5.93°, SG에서 -18.90±7.78°로 나타났다. 또한 슈퍼대회전에서는 기문 간의 거리가 스키어의 속력에 유의한 상관관계가 있음을 나타냈다. The purpose of this study is to quantify the characteristics of the downhill (DH) and super giant slalom (SG) course of Jeongseon Alpine Stadium where the PyeongChang Winter Olympic Games are held, and to investigate the correlation between the characteristics and the skier’s speed. A high level racer who are continuously training at Jeongseon alpine centre and who secured the right to participate in the Olympic Winter Games in PyeongChang participated in the study. The Global Positioning System (GPS) was used to measure the position of the gates of DH and SG, and to collect the skier’s trajectory when he was skiing. The collected latitude, longitude, and altitude data were converted into meter units, and the gate distance, gate shift angle, and gradient were calculated and the correlation between the skier’s speed and course characteristic parameters was calculated. The gate distance was 89.79±37.07 m in DH and 55.85±20.40 m in SG. The gate shift angle was 136.83±19.45° in DH and 147.26±15.54° in SG. The gradient was -18.71±5.93° in DH and -18.90±7.78° in SG. In addition, in the super giant slalom, there was a significant correlation between gate distance and skier’s speed.

        알파인 스킹 시 스키딩과 카빙턴 간 무릎의 과사용 상해를 유발할 수 있는 잠재적 위험 요소

        무릎의 과사용 상해는 알파인 스키 선수들 사이에서 흔히 발생하는 부상이다. 특히 바깥 다리는 회전하는 다리의 기능으로 인해 더 큰 힘과 모멘트가 발생한다. 알파인 스키 선수들은 국제스키연맹규칙과 코스의 환경적 요인에 따라 스키길이 (또는 스키반경)에 따라 스키딩턴과 카빙턴을 선택적으로 사용한다. 따라서 본 연구는 스키딩턴 및 카빙턴 수행 시 무릎의 3차원 각도를 설명하고, 알파인 스키에서 무릎의 과용 부상을 유발할 수 있는 잠재적 인 위험 요인이 될 수 있다는 가설의 타당성을 평가하기 위해 조사되었다. 한국 알파인 스키 선수 10명이 본 연구에 참여하였다. 8개의 관성센서를 사용하여 200Hz의 샘플링 주파수에서 분절의 운동학적 데이터를 수집했다. 관성센서는 허벅지, 정강이 및 발 부분 (스키)에 양측으로 부착되고 알파인 스키 수트 안쪽의 골반과 하부 척추에 부착했다. 각 스키어는 정설된 15°슬로프에서 각각 10번의 스키딩턴과 카빙턴을 수행하였다. 최대 외반 각도는 전후축에서 대퇴각도와 관련된 하퇴 각도를 계산했다. 카빙턴과 스키딩턴의 차이는 paired t-test (유의수준 : α<.05)를 사용하여 결정하고 효과크기를 계산했다. 결과는 카빙턴에서 바깥 다리 무릎의 최고 외반각(33.96±2.49°)이 스키딩턴에서 바깥 다리 무릎의 최고 외반각(26.36±0.44°)보다 유의하게 더 컸음을 나타냈다(t=9.35, p<.05). 두 개의 서로 다른 회전기술 사이의 외반각도에 대해 관찰된 효과크기는 0.8보다 컸다(r=0.957). Overuse injuries of the knee are a common injuries among alpine ski racers. Especially, outer leg might experience higher force and moment due to its function as steering leg. Racers selectively use skidding turn and carving turn according to the ski length (or ski radius) specified by International Ski Federation rules and the environmental factors of the course. Therefore, This study was investigated to describe 3-dimensional angle of the knee during carving and skidding turns and to assess the plausibility of the hypothesis that a valgus angle might be a potential risk factor leading to overuse injuries of the knee in alpine ski. Ten Korean alpine ski racers were participated in this study. Eight inertial measurement units (myoMOTION, Noraxon, USA) were used to collect segment kinematic data at a sampling frequency of 200 Hz. Inertial measurement units were attached bilaterally to the thigh, shank and foot segments (ski), and to the pelvis and lower-spine under the alpine ski racing suit. Each skier was asked to perform turns including ten carving turns and ten skidding turns on the groomed 15° slope, respectively. The peak valgus angle was calculated the shank angle related to the thigh angle on the anteroposterior axis. The differences between the carving turn and skidding turn were determined using paired t-test (significance level : α<.05), and the effect sizes (Cohen’s d) were calculated. The obtained results indicate that the peak valgus angle of the knee on the outer leg in carving turn (33.96±2.49°) was significantly greater than the peak valgus angle of the knee on the outer leg in skidding turn (26.36±0.44°; t=9.35, p<.05). For the valgus angle between two different turning techniques, the observed power was greater than 0.8 (r=0.957).
