Background: In most solid tumors, the CD34 antigen has not been detected, so positive selection of CD34+ cells may reduce tumor cell contamination and the CD34+ cells are capable of reconstituting hematopoiesis. We tried CD34+ cell transplantation in two patients. Method: CD34+ cells from chemotherapy + G-CSF mobilized PBPCs or bone marrow were positively selected with an avidin-biotin immunoadsorption column (CEPRATE SC system). Case 1. One course of chemotherapy using cyclophosphamide(200㎎/㎡) and etoposide (4.2g/㎡), combined with G-CSF(5㎍/㎏) S.C. was used in a relapsed lymphoma patient. This patient responded to the induction chemotherapy. CD34+ cells from harvested bone marrow were selected by the CellPro immunoadsorption column. The total number of mononuclear cells loaded onto the CellPro was 2.4×10^(8)/㎏, with 1.1% CD34+ cells. After column separation, the total number of positively selected cells was 5.16×10^(6)/㎏. The number of CFU-GM was 76.8×10⁴/㎏. This patient was treated with melphalan (140㎎/㎡) and TBI (1200cGy) and the positively selected CD34+ cells were infused. The time to neutrophil recovery greater than 0.5×10^(9)/L was 19 days and the time to platelet recovery greater than 50×10^(9)/L was 21 days. Case 2. Two courses of mobilizing chemotherapy were given 4 weeks apart using taxol(210㎎/m2) and adriamycin(60㎎/m2), combined with G-CSF(5㎍/㎏) S.C. in a breast cancer patients with 7 axillary node metastasis. CD34+ cells from each single leukapheresis product were selected by the CellPro immunoadsorption column. In the first collection, the total number of nucleated cells was 4.4×10^(8)/kg, with 0.42% CD34+ cells. In the second collection, the total number of nucleated cell was 2.8× 10^(8)/㎏ with 0.43% CD34+ cells. After colum separation, the total numbers of collected cells were 4.0×106/kg and 4.8×10^(6)/kg, the total number of CD34+ cells were 1.2×10^(6)/㎏ and 0.82×10^(6)/㎏. Colonogenic assays of positively selected CD34+ cells gave rise to myeloid erythroid, and multilineage colonies, with a median of 190 CFU-GM, 190 BFU-E, and 164 CFU-GEMM per 1×10³ adsorbed cells, respectively. High-dose chemotherapy with cumulative doses of 40mg/㎡ mitoxantrone, 750mg/㎡ thioptepa, and 1000mg/㎡ carboplatin was administered. Positively selected CD34+ cells were rapidly infused 24 hours after the end of high-dose chemotherapy. The time to neutrophil recovery greater than 0.5×10^(9)/L was 16 days and the time to platelet recovery greater than 50×10^(9)/L was 20 days.

저자들은 척수압박으로 하지마비를 보인 환자에서 수술 후에 급성 백혈병에 동반된 과립구성 육종으로 진단된 환자 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Granulocytic sarcoma(GS) is an uncommon tumor composed of immature cells of the granulocytic series. Most case of GS occur in the course of acute leukemia and the blast crisis of chronic leukemia, Rarely, however, it may present before leukemia becomes clinically apparent. It may also occur in patients with myeloproliferative disoders. GS has been reported that it occurs in 3% to 9% of patients with acute myelogenous leukemia(AML) and the incidence of GS is reported to be higher in patients with t(8;21). GS occurs relatively commonly in africa and has been reported to affect 10-25% of black children presenting with AML. This is very rare case of granulocytic sarcoma with AML(FAB M5) presented with spinal cord compression which was supported by decompression laminectomy.

식도암의 임상적 고찰

A Clinical observation was made in 52 cases of patients with esophageal cancer at Chosun University Hospital during about 5years from january 1981 until may 1986. The following results were obtained: 1. The peak age of patients was fifth decade. The average age of patients is 55.7 years old. The sex ratio is 7.6 : 1 with a greater prevalence male. 2. Duration of the disease before admission was relatively long, about 4~6 months is 48% of all case. 3. The most common initial symptoms were swallowing difficulty, weight loss, substernal pain. 4. On the majority of patients, Heavy s.noking was manifested. 5. In blood type, A type was predominent. 6. In location of tumor, 69.2% of total cases were located in mid esophagus. 7. In histopathologic finding, 73% of total cases was squamous cell ca.

고비중 척추마취시 필첨바늘의 출구방향이 신경차단범위와 차단기간에 미치는 영향

Background: The endogenous and exogenous factors influence the distribution of local anesthetic solutions within the subarachnoid space. This study was designed to examine the influence of direction of pencil-point tip needle sideport using a 25 gauge Whitacre needle(Vygon, France). Methods: Thirty adult male patients undergoing orthopedic, urologic or anal surgery were divided randomly into two groups according to the direction of the needle sideport(cephalad or caudad). Intrathecal anesthesia were performed with 10mg of 0.5% hyperbaric bupivacaine. The technique was typically performed using a midline approach at L4-5 interspace in the lateral decubitus position and the patients were turned to supine position immediately after the block. After the injection, we registered the segmental spread of analgesia(pin-prick). Duration of block(two segment regression time) and degree of motor block(Bromage scale) were measured. Results: There were no significant differences in onset time to maximal level of sensory and motor block, two segment regression time between groups. Mean maximal sensory level was T6.1± 1.6 for the cephalad group vs T7.0±1.8 for the caudad group, but no statistical significance was found. Conclusions: This study suggests that needle sideport direction of Whitacre needles does not significantly influence extent and duration of hyperbaric spinal anesthesia.

톨루엔 취급 근로자의 ALDH2 Genetic Polymorphism에 따른 뇨중 마뇨산 배설량

In this study we evaluated the effects of the genetic polymorphism of aldehyde dehydrogenase2 (ALDH2) on the toluene metabolism and determined biological exposure indices(BEIs) for toluene by the genotypes of ALDH2. The study subject were 77 men workers who are handling toluene in a video tape manufacturing factory and a textile company. Through the face-to-face interview, the information about smoking and drinking behavior was obtained. For determination of ALDH2 polymorphism, 5ml of venous blood sample was obtained from each subject after informed consent. DNA was extracted from the buffy coat and ALDH2 genotyping were performed using a polymerase chain reaction(PCR) method. The genotype of ALDH2 was classified into the homozygous genotype of normal ALDH2(NN), and the heterozygous genotype of normal and inactive ALDH2(ND), and homozygous genotype of an inactive ALDH2(DD). The concentration of hippuric acid (HA), the main metabolite of toluene, was determined in urine specimens collected at the end of shift, corrected with creatinine(HA/C), and compared with BEI for toluene, which is 2.5 g/g creatinine. The personal exposure level of toluene were monitored, using personal air sampler and analyzed by gas chromatography. The frequencies of the three genotypes in this study subject were, NN : 45 (58.4%), ND : 26 (33.8%) and DD : 6 (7.8%), and frequencies of the genotypes in the middle or heavy toluene exposure workers were not significantly different from those in the mild toluene exposure workers. The frequencies of the DD type of ALDH2 was lower among alcohol drinkers than among non-drinkers. The urinary HA concentration of DD group was significantly lower than that of the NN or ND group, which suggests that the HA formation from toluene decreased in DD group. Regression lines were used to estimate the BEIs of the NN, ND, and DD groups. NN : y = 0.0085x + 0.23, r = 0.90 ND : y = 0.0074x + 0.21, r = 0.85 DD : y = 0.0041x + 0.82, r = 0.83 The three regression lines revealed that the estimated urinary HA concentration of NN, ND, and DD groups at 377 mg/㎥ toluene (TLV-TWA) exposure were 3.43, 3.00, and 2.37g/g than that of the ACGIH BEI (2.5 g/g creatinine) ; however, the HA level of DD group was lower than the BEI. These results suggest that the ACGIH BEI is not adequate to estimate the toluene exposure of the NN, ND and DD groups at the same time. Based upon these results, a new BEI for ALDH2 deficient workers may be necessary.

The purpose of this paper is to decide the ratio mixture between microbes and the most suitable material in order to immobilize white rot fungus. It shows that white rot fungus favorably lives, regarding to respiration speed and activity when pellet is immobilized by PVA. In addition, it survives better with sodium alginate, concerning about the rate of swelling. When PVA-frozen method is applied, methylene blue is decolorized 100% by immobilized Trametes versicolor and 90% by immobilized Irpex lacteus after 44hr. As a result, pellet immobilized by PVA, which indicates better mechanical stability and declororization rate, is superior to sodium alginate. The mixture rate between microorganisms and PVA is 1:1.

To find the prevalence and related factors of low back pain experienced by some beauticians. a survey was made toward 252 beauticians in Kwangju City from March 1st to April 1st, 1998. The results were as follows; 1. The prevalence rate of low back pain among the respondents for the last week was 59.5%. 2. The aid group and staff group with low back Pain had significantly longer duration of work than those without low back pain. Managers who had bad posture with twisted or too much extended back showed a significantly higher prevalence of low back pain than those with good posture during work(P<05). 3. Aid group with low back pain showed significantly higher points at total stress score, depression score, general health score and vitality score than those without low back Pain. The staff group with low back pain showed significantly higher points for total stress score, sleep disorder and anxiety score, depression score, general well-being and vitality score than those without low back pain. Managers with low back Pain showed significantly higher points of stress at social performance and self confidence (P<05) than those without low back pain. The above results suggest that low back pain is related to different jobs and work environments for beauticians. So effective measures and further studies against risk factors must be made considering their jobs. To prevent or reduce low back pain, appropriate management of stress for beauticians is necessary.
