Expression of Endothelin-1 by Stimulation with CXCL8 in Mouse Peritoneal Macrophages

Endothelin-1 (ET-1) has been characterized as a potent vasoconstrictor secreted by the endothelium, and play a major role in the regulation of vascular tone. It has been also known to participate in inflammatory reactions. The production of ET-1 by macrophages during infection and inflammation is related to tissue perfusion and leukocyte extravasation. The aim of this study is to investigate the role of IL-8/CXCL8, as a major inflammatory chemokine, for ET-1 expression in macrophges. Expression of ET-1 mRNA in mouse peritoneal macrophages ($PeM{\phi}$) was weaker than that in vascular smooth muscle cells (VSMCs) from spontaneously hypertensive rats (SHR) and normotensive Wistar-Kyoto rats (WKY). However, expression of IL-8/CXCL8-induced ET-1 mRNA in $PeM{\phi}$ was much more stronger than that in SHR and WKY VSMCs. Maximum expression of ET-1 mRNA was observed at 50 ng/ml dose of IL-8/CXCL8 and occurred at 2 h after addition of IL-8/CXCL8. Expression of ET-1 by IL-8/CXCL8 was dependent on NF-${\kappa}B$ activation and ERK1/2 phosphorylation. Baicalein, a 12-lipoxygenase (LO) inhibitor, inhibited the expression of IL-8/CXCL8-induced ET-1 mRNA. This inhibitory action of baicalein was mediated via ERK1/2 inactivation. Induction of l2-LO mRNA by IL-8/CXCL8 and expression of ET-1 mRNA by 12-LO metabolite, 12(S)-HETE were also detected. The expression of IL-8/CXCL8-induced ET-1 mRNA was not detected in $PeM{\phi}$ transfected with 12-LO siRNA. These results suggest that IL-8/CXCL8 can act as one of main inducers of ET-1 in vascular inflammatory reactions, and ET-1 expression by IL-8/CXCL8 is related to 12-LO pathway in $PeM{\phi}$.

Object: Recent reports suggest that single-incision laparoscopic surgery, also known as single port surgery, is technically feasible in treating a various disease. But, 25 mm sized skin incision of single incision laparoscopic surgery is very long for cosmesis. So, we developed and applied a simplified two-incision laparoscopic-assised transumbilical extracorporeal adnexal cystectomy using traditional trocar. Methods: For LATEC, Following insufflation with CO2 to a pressure of 10 mm Hg, two 10-mm umbilical trocar were inserted at umbilicus. and 30-degree laparoscope was introduced. Under laparoscopic guidance, needle aspiration of the cyst was performed. The 10-mm trocar was removed, the deflated cyst and adnexa were exteriorized through the umbilicus for either cyst excision with adnexal preservation. Conclusion: Two-incision LATEC is a relatively simple procedure that combines laparoscopy and traditional extracorporeal surgery. and this procedure may have the better advantage of laparoscopy such as rapid recovery and exellent cosmesis, with only umbilical two incision and may offer a better chance for adnexal preservation.
복강경시술 중 발견된 Fitz-Hugh-Curtis증후군의 임상적 고찰

목적: 복강경수술 시 우연히 발견된 FHC증후군 환자들의 임상적 특성, 혈액학적 특성 및 골반 유착정도 및 유착박리술 후 우상복부 동통의 개선되었는지를 알아보고자 하였다. 연구 방법: 2001년 1월 1일부터 2006년 12월 31일까지 영남대학교병원 산부인과에서 1399예의 복강경시술 중 발견된 FHC증후군 40예에 대한 의무기록을 후향적으로 조사하였다. 환자의 과거력, 수술 후 주 진단명, 간기능이상 유무, 우상복부통, 골반강 내 유착의 정도, 간주위 유착의 정도, 과거력 등을 검토하였다. 결과: 환자의 연령 분포는 16세에서 65세였으며 평균 연령은 32.9세로 성적으로 활동적인 가임기여성이 대부분을 차지하였다. 복강경수술 시 주 진단은 난소난관농양을 포함한 골반염이 전체 FHC증후군 중 14예(35%)를 차지하였고, 자궁근종이 9예(22.5%), 양성 난소난관종양이 7예(17.5%), 자궁외임신이 6예(15%), 자궁내막증이 5예(12.5%), 골반결핵이 1예(2.5%)였다. FHC증후군으로 진단된 40예의 과거력을 살펴보면 4예 (10%)에서 자궁외임신의 과거력이 있었고 14예(5%)에서는 골반염이, 3예(7.5%)에서 폐결핵의 병력이 있었고 이 중 1예는 수술 후 AFB 염색 및 배양검사에서 골반 결핵으로 진단받았다. 12예(30%)에서 과거 개복수술이나 복강경수술의 과거력이 있었으며 2예(5%)에서 자궁 내 장치 사용의 경험이 있었다. 결론: 본 연구결과 FHC증후군은 골반 내 염증성 질환 및 자궁외 임신에 이환된 환자에게서 자주 발견되며 자궁근종이나 난소난관종양, 자궁내막증 및 골반결핵 환자에서도 발견되었다. 이는 불분명한 위장관 증상, 우상복부 동통 등의 과거력이 있는 환자 및 부인과질환으로 시행하는 모든 복강경수술 시 우상복부, 특히 주위 공간을 관찰하는 것이 필요하다고 생각되며, 향후 정확한 빈도와 원인에 대한 전향적이고 광범위한 연구가 필요하다고 사료된다. Objective: The aim of this study was to analyse the characteristics, severity of Fitz-Hugh-Curtis syndrome (FHCS), pelvic adhesions and hematologic findings of incidentally founded FHCS in laparoscopy. Methods: We retrospectively reviewed the medical records of 40 patients incidentally found laparoscopically as FHCS at the department of obstetrics and gynecology of Yeungnam University Hospital from Jan. 2001 to Dec. 2006. Severity of FHCS and pelvic adhesions were classified and characteristics of patients, hematologic findings, past medical histories and symptoms after surgery were analysed. Results: The ages of the cases varied between 16 and 65 years old. Mean age of cases was 32.9 years. Indications of surgery were uterine myoma, benign adnexal mass, ectopic pregnancy, endometriosis, chronic pelvic pain and pelvic tuberculosis. From this study we can notice that FHCS can be found out most frequently in the patient with pelvic inflammatory disease (PID). Past medical history were pelvic inflammatory disease, ectopic pregnancy, abdominal surgery and pelvic tuberculosis. 17 patients had been suffered from RUQ pain and 10 patients (58.8%) were in good health after adhesiolysis. Conclusion: Laparoscopy is a confirmative tool in the diagnosis of FHCS. Direct observation of perihepatic space is the most definitive method of diagnosing this syndrome. From our study, we can find that FHCS can be diagnosed frequently in the patient with pelvic inflammatory disease, ectopic pregnancy and pelvic tuberculosis. So, We recommend that investigating the perihepatic space during surgery is a good treatment especially in patients with PID, vague abdominal discomfort and pains.
