        발열을 동반한 호중구감소증에서 Cefepime 단독투여와 Ceftazidime 및 Tobramycin 병합투여의 효능비교

        목적 : 국내에서 발열을 동반한 호중구감소증이 있는 악성 종양환자에게 경험적 항생제로 베타락탐계 항생제와 아미노배당체의 병합요법의 사용이 일반적이다. Cefepime은 광범위 항균 작용을 가지고 있어, 그람 음성균 뿐만 아니라 그람 양성균에 대해서도 우수한 효과를 나타낸다. 재료 및 방법: 발열을 동반한 호중구감소증이 있는 악성 종양환자를 대상으로 무작위, 공개, 비교 연구를 시행하였다. 대상 환자를 무작위로 cefepime 단독요법군과 ceftazidime 및 tobramycin 병합요법군으로 나누어 투여하고 각각의 임상적 효능과 안전성을 비교하였다. 구강및 인후 점막염이 있는 환자에서 분리된 녹색 연쇄알 구균에 대한 항생제 내성 정도를 조사하였다. 결과 : 대상환자 89명 중 CA 투여군이 48예(53.9%), CT 투여군이 41예(46.1%)이었다. 발열의 유형별로 MDI는 18예(20.2%), CDI는 9예(10.1%), UF는 62예(69.7%)로 두 군 간에 차이가 없었다. CA 투여군과 CT 투여군의 임상적 호전률은 시험약 투여 후 2-4일째 각각 91.7%, 85.4% (P=0.31), 치료 종료 시 각각 91.7%, 100% (P=0.15)로 두 군간에 유의한 차이가 없었다. 치료 종료 시 CA 투여군과 CT 투여군의 세균학적 소실률은 모두 100%로 두 군간의 유의한 차이가 없었다(P=0.78). 점막염이 있는 환자로부터 녹색 연쇄알 구균이 분리된 경우는 25예(28.1%)이었으며, 분리된 녹색 연쇄알 구균은 penicillin, ceftriaxone, cefepime, vancomycin에 모두 감수성을 보였다. 약제 관련 이상 반응의 발생 빈도도 두 군간에 유의한 차이가 없었다. 결론 : 발열을 동반한 호중구감소증이 있는 악성 종양환자의 경험적 항생제로서 cefepime 단독요법은 ceftazidime 및 아미노배당체의 병합요법만큼 효과적이고 안전하였다. Background : Broad-spectrum antibiotic therapy has been recommended as an empirical regimen in cancer patients with febrile neutropenia. Cefepime is a fourth generation cephalosporin with good activity against both gram-positive cocci and gram-negative bacilli. Materials and Methods : To compare the efficacy and safety of cefepime alone with ceftazidime plus tobramycin as empirical regimen for adult cancer patients with febrile neutropenia, a randomized, open label, comparative trial was performed. If the patient showed clinical improvent 72 hours, antibiotic could be changed to oral ciprofloxacin. Clinical and microbiological responses were determined at 72 hours and at the end of therapy. To investigate the antimicrobial resistance of viridans streptococci, swab cultures were obtained from throat in all enrolled patients and antimicrobial susceptibility tests were performed by using microdilution method according to the NCCLS. Results : A total of 89 patients were enrolled. Forty-eight patients received cefepime alone (CA), and 41 patients received ceftazidime plus tobramycin (CT). Demographic and baseline clinical characteristics were similar in both groups (P>0.05). The initial clinical success rate at day 2-4 in group CA (91.7%) was similar with that in CT group (85.4%) (P=0.31). At the end of therapy, the final clinical success rate in CA group (91.7%) was similar to that in CT group (100%) (P=0.15). In 18 patients, with microbiologically defined infections, the eradication rate was 100% in both groups. Adverse events including liver dysfunction (21.3%) and renal dysfunction (2.2%), were similar in both groups (P=0.87). Viridans streptococci were isolated from the throat cultures in 25 cases, and all of these strains were susceptible to penicillin (MIC_(90) 0.12 ㎍/mL), cefepime (1 ㎍/mL), and vancomycin (0.12 ㎍/mL). Conclusion : Efficacy and safety of cefepime monotherapy was comparable to the combination of ceftazidime and tobramycin. It could be used as an alternative empirical regimen for treating cancer patients with febrile neutropenia.

        Development of Kompetitive Allele Specific PCR Markers for Submergence Tolerant Gene Sub1 in Rice

        The SUBMERGENCE 1 (SUB1) locus, conferring tolerance to complete inundation, was identified, and gel-based DNA markers, AEX1 and GnS2, were previously developed for marker-assisted breeding (MAB). However, a high throughput and high specific method, at low cost, is still required to detect Sub1 alleles. Kompetitive Allele Specific PCR (KASP) markers enable high throughput analysis for a large number of seeds, as well as detection of both alleles, in a single reaction. In this study, we developed KASP markers that can distinguish specific alleles at Sub1A loci based on single nucleotide polymorphisms (SNPs). Marker validations were carried out by genotyping of a segregating population with the developed KASP markers. The results from KASP assay and gel-based marker analysis were consistent for Sub1A alleles. KASP markers developed for Sub1A would be helpful due to high accuracy, low cost, and a high throughput genotyping feature in MAB.

        MN/Road 시험포장 구간내의 공기량 측정 및 결과값 분석을 통한 RAP 및 저온 아스팔트(WMA) 혼합물의 특성 평가

        PURPOSES: This research is to evaluate the mechanical performance of different types of Hot Mix Asphalt (HMA) pavement cells prepared for MN/Road field testing section through an extensive experimental analysis of air voids and simple statistical evaluation tools (i.e. hypothesis test). METHODS: An extensive experimental work was performed to measure air voids in 82 asphalt mixture cores (238 samples in total) obtained from nine different types of road cell located in MN/Road testing field. In order to numerically and quantitatively address the differences in air voids among the different test Cells built in MN/Road, a simple statistical test method (i.e. t-test) with 5% significance was used. RESULTS: Similar trends in air voids content were found among the mixtures including conventional HMA, Reclaimed Asphalt Pavement (RAP) and Warm Mix Asphalt (WMA) combined with taconite aggregate this provides support to the use of RAP and WMA technology in the constructions of asphalt pavement. However, in case of acid modified HMA mixtures, significant differences in air void content were observed between on the wheel path and between wheel path location, which implies negative performances in rutting and thermal cracking resistances. Conclusions : It can be concluded that use of RAP and WMA technology in the construction of conventional asphalt pavement and the use of PPA (Poly Phosphoric Acid) in combinations with SBS (Styrene Butadiene Styrene) in asphalt binder production provide satisfactory performance and, therefore, are highly recommended

        NDP kinases (NDPKs) are multifunctional proteins that regulate a variety of eukaryotic cellular activities, including cell proliferation, development, and differentiation. However, much less is known about the functional significance of NDPKs in plants. We show here that NDPK is associated with H_(2)O_(2)-mediated mitogen-activated protein kinase signaling in plants. H_(2)O_(2)stress strongly induces the expression of the NDPK2 gene in Arabidopsis thaliana (AtNDPK2). Proteins from transgenic plants overexpressing AtNDPK2 showed high levels of autophosphorylation and NDPK activity, and they have lower levels of reactive oxygen species (ROS) than wild-type plants. Mutants lacking AtNDPK2 had higher levels of ROS than wild type. H_(2)O_(2) treatment induced the phosphorylation of two endogenous proteins whose molecular weights suggested they are AtMPK3 and AtMPK6, two H_(2)O_(2)-activated A. thaliana mitogen-activated protein kinses. In the absence of H_(2)O_(2) treatment, phosphorylation of these proteins was slightly elevated in plants overexpressing AtNDPK2 but markedly decreased in the AtNDPK2 deletion mutant. Yeast two-hybrid and in vitro protein pull-down assays revealed that AtNDPK2 specifically interacts with AtMPK3 and AtMPK6. Furthermore, AtNDPK2 also enhances the myelin basic protein phosphorylation activity of AtMPK3 in vitro. Finally, constitutive overexpression of AtNDPK2 in Arabidopsis plants conferred an enhanced tolerance to multiple environmental stresses that elicit ROS accumulation in situ. Thus, AtNDPK2 appears to play a previously uncharacterized regulatory role in H_(2)O_(2)-mediated MAPK signaling in plants.

        Background: Extramammary Paget’s disease(EMPD) is a rare intraepithelial neoplasm of the skin with high local recurrence rates due to clinically indeterminate margins and diffuse spread. Mohs micrographic surgery(MMS) has been proposed to reduce the rate of local recurrence but application of MMS can be difficult because of the large size of EMPD lesions. Objectives: To evaluate the efficacy of modified peripheral MMS in the treatment of EMPD. Methods: one week before surgery, preoperative multiplescout biopsy was done to determine appropriate resection margin. Mohs technique was performed by taking a rim of tissue around the lesion and processing it with Mohs frozen analysis. After the margins were cleared, central specimen was removed, and then repair for final surgical defect was performed. The presence of residual Paget’s cells in the central specimen also was checked by postoperative histologic analysis. Twelve patients received this modified MMS for EMPD were included in the study. Results: Most patients were male (11 males and 1 female) and the locations of lesion were pubic region. The number of Mohs stage and frozen section was lesser comparing with classic MMS performed previously in our hospital, and therefore the time required in operation was saving. No severe complications or recurrences were observed during follow-up period. Conclusion: Modified peripheral MMS is an excellent surgical option for EMPD, and more practical and time-saving than classic MMS technique.

        Iron–nitrogen-doped mesoporous tungsten carbide nanostructures as oxygen reduction electrocatalysts

        <P>Since Pt-based catalysts have the disadvantages of high cost, large overpotential loss, and limited long-term stability, there have been various promising alternatives to Pt-based catalysts to improve the catalytic activity towards the oxygen reduction reaction (ORR). We have synthesized iron–nitrogen-doped mesoporous tungsten carbide catalysts (WC-m-FT) by pyrolysis of well-ordered mesoporous tungsten carbides with iron porphyrin. WC-m-FT exhibits excellent ORR catalytic activity in an alkaline medium, <I>i.e.</I> a high electron-transfer number as well as superior stability and methanol tolerance. The improved activity and stability of WC-m-FT are ascribed to iron-containing catalytic active sites surrounded by nitrogen species and the well-defined mesoporous tungsten carbide structure.</P> <P>Graphic Abstract</P><P>WC-m-FT shows excellent electrochemical properties towards the oxygen reduction reaction (ORR) due to the iron and nitrogen species acting as catalytic sites and the mesoporous structure. <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c4cp01157k'> </P>

        Case Reports : Lichen Striatus Occurring after Allogenic Peripheral Blood Stem Cell Transplantation in an Adult with Aplastic Anemia

        Lichens striatus (LS) is an acquired, self-limiting inflammatory dermatosis that follows the lines of Blaschko. The etiology of the eruption is unknown, but several theories have been proposed with focus on environmental factors, viral infection, cutaneous injury, hypersensitivity, and genetic predisposition. We describe a 19-year-old woman who developed a unilateral linear eruption 17 months after allogenic peripheral blood stem cell transplantation. Histopathology revealed features, which were consistent with LS. To the best of our knowledge, our patient is the first case describing the appearance of LS occurring after allogenic stem cell transplantation. We speculate that this condition represents an unusual form of localized, chronic graft- versus-host disease. (Ann Dermatol 24(1) 87∼89, 2012)

        Background: Dermoscopy has been proven to increase diagnostic accuracy of malignant melanomas. However, dermoscopic features of melanomas in Asians have been scarcely reported. Objectives: To investigate dermoscopic patterns and evaluate diagnostic algorithmic approach in Korean patients with melanomas in trunk and extremities. Methods: Dermoscopic evaluation of histopathologically confirmed melanomas at 2 university hospitals from 2007 to 2014 was performed. Dermoscopic scores were evaluated using various dermoscopic algorithmic methods. Results: Sixteen patients with 16 primary melanomas and 5 patients with 15 metastatic melanomas were included. In primary melanomas, several dermoscopic melanoma-associated features were found: blue-white veil (76%), blotches (62%), atypical dots/globules (52%), ulcer (48%), atypical pigment network (43%), irregular peripheral streaks (43%), atypical vessels (38%), shiny white lines (29%), and regression (10%). Mean scores of 7-point checklist, revised 7-point checklist, 3-point checklist, ABCD rule, and CASH algorithm were 6.3, 4.1, 2.6, 7.6, and 10.9. In metastatic melanomas, the algorithmic approaches were not accurate as the mean scores were 1.7, 0.9, 0.8, 2.0, 4.1. Conclusion: Dermoscopy can be useful in diagnosingmalignant melanomas in trunk and extremities in Koreans. Dermoscopic algorithmic approaches were useful in detecting primary melanomas; however, they were not precise in identifying metastatic melanomas.
