소아치과에서 국소마취는 공포의 대상이며, 특히 행동조절이 힘든 소아의 경우 더욱 그러하다. 국소마취에 대한 공포는 주사침의 자입시나 주사액의 주입시 느끼는 동통 만큼이나 주사침 자체의 공포도 크게 차지한다. 주사침 자입시의 동통을 완화하기 위하여, 도포마취제 등이 개발되었으나 이들은 국소마취의 동통을 완화시키는 면은 있으나 주사침 자체에 대한 공포를 없애지는 못하였다. Syrijet Mark Ⅱ??(Mizzy Inc, USA)는 주사침이 없는 치과용 분사식 주사기(jet injection device)로서 소아에서 주사침에 대한 공포를 성공적으로 감소시킬 수 있다고 알려져 있다. 저자들은 여러 행동등급의 소아를 대상으로 Syrijet을 사용하여 국소마취를 시행한 후 동통평가의 척도로서 VAS(visual analogue scale)를 소아에게 적합하게 색생화한 CAS(color analogue scale)을 이용하여 국소마취시의 동통수준을 측정한 결과, 기존 주사침에 의한 국소마취시 보다 국소마취의 동통이 감소된 것을 알 수 있었다. Syrijet의 장점으로는 주사침이 없다는 점, 기존의 술식에서 일회용이었던 미취용 카트리지를 재사용할 수 있다는 점, 신경 및 혈관에 안전하다는 점 등이 있었으며, 단점으로 부피가 크며, 가격이 비싸며, 사용시 숙련성을 요구한다는 점 등이 있었다. Local anesthesia procedure in pediatric dentisty using needle-tipped syringes is stressful and painful for the child patients. The Syrijet Mark Ⅱ (Mizzy Inc. USA) is a jet injection instrument, which uses high pressure to propel fluids into soft tissue without the use of a hollow neeldle, so it seems to be able to reduce the injection phobia of the child patients. The authors compared the Syrijet with the conventional syringe by assessing the pain level after local anesthetic procedures using CAS(color analogue scale) which was developed from VAS(visual analogue scale). The result showed that the pain was reduced by the use of syrijet. The advantages of syrijet were no use of neeldes, the reuse of the anesthetic cartridge, and the safety to nerves and vessels. The disadvantages were the large size, the high price, and the need of skill to use.
심장 이식 환자에서 발생한 Cryptococcus neoformans에 의한 뇌막염 2예 : The report of two cases

최근 장기 이식과 관련된 면역억제제의 사용 빈도의 증가 및 후천성면역결핍증 환자의 증가 등으로 인해 기회감염이 증가되고 있으며, C. neoformans에 의한 감염도 증가되고 있다. 저자들은 심장이식 후 6개월이상 지난 환자에서 발생한 크립토코쿠스 뇌막염 2예를 경험하였는데, 한 예의 경우는 거대세포 바이러스 감염과 함께 C. neoformans의 전신적 파급성 감염의 형태로 나타났었고, 폐에 A. fermigatus에 의한 감염이 같이 병발된 경우였다. 이들은 모두 경미한 증상만을 호소하였고, 뇌척수액 검사를 통해 진단받았으며 liposomal amphotericin B와 경구용 fluconazole을 사용하여 완치되었기에 문헌고찰과 함께 보고하는 바이다. Cryptococcal meningitis is a subacute or chronic mycotic infection caused by Cryptococcus neoformans and it is a second most common central nervous system (CNS) infection in organ transplant patients. The recent rise in incidence of the disease can be attributable to the wide use of immunosuppressive agents in organ transplant patients, the advent of acquired immunodeficiency syndrome (AIDS) patients and increased use of chemotherapeutic agents. We report two cases of cryptococcal meningitis after cardiac transplantation. Their symptoms were not typical and neurologic examination was nonfocal. One patient had disseminated cryptococcal infection and invasive pulmonary aspergillosis, it might be assodated with cytomegalovirus infection. They were diagnosed by cryptococcus latex agglutinin test and culture study with cerebrospinal fluid and treated with liposomal amphotericin B and oral fluconazole without any neurologic defects.

Age and growth of Glyptocephalus stelleri in the East Sea of Korea were determined, from monthly samples of commercial catches, caught by the eastern sea Danish seine fishery in 2007. The annuli of G. stelleri are formed once a year, with the boundary between opaque and translucent zones forming in September. Relationships between total length (TL) and total weight (TW) were TW=0.002TL3.392 (r2=0.970) for females and TW=0.002TL3.335 (r2=0.961) for males. TLs at annuli formation in otoliths were backcalculated from the otolith-length relationship and were adjusted to von Bertalanffy growth curves to Lt=39.71(1 - exp (-0.152(t+1.156)) for females and Lt=32.16(1 - exp (-0.213(t+0.879))) for males. From the age of 3yerars, females grew faster than males (P<0.05).

The average total length of L. ochotensis was 63.4 cm and 64.1 cm for females and males, respectively. Gonadosomatic index (GSI) began to increase in July, reaching a maximum in November, then began to decrease in December. Monthly changes in the GSI values of L. ochotensis were similar for females and males. Annual reproductive cycle of this species can be divided into five successive stages in females: early growing stage (May), late growing stage (June to July), mature stage (August to September), ripe and spent stage (September to December), and recovery and resting stage (January to April). Males show four successive stages; growing stage (May to July), mature stage (August to September), ripe and spent stage (September to December), and recovery and resting stage (January to April). Relationship between total length (TL) and fecundity (F) was F=$0.00003TL^{4.002}$ ($R^2$ = 0.703), and F increased with TL. Total length at 50% group maturity was estimated to be 55.8 cm. Our findings suggest that the spawning period takes place from September to December, with the main spawning period occurring from October to December.
