        Media Optimization for Laccase Production by Trichoderma harzianum ZF-2 Using Response Surface Methodology

        Trichoderma harzianum ZF-2 producing laccase was isolated from decaying samples from Shandong, China, and showed dye decolorization activities. The objective of this study was to optimize its culture conditions using a statistical analysis of its laccase production. The interactions between different fermentation parameters for laccase production were characterized using a Plackett-Burman design and the response surface methodology. The different media components were initially optimized using the conventional one-factor-at-atime method and an orthogonal test design, and a Plackett-Burman experiment was then performed to evaluate the effects on laccase production. Wheat straw powder, soybean meal, and CuSO4 were all found to have a significant influence on laccase production, and the optimal concentrations of these three factors were then sequentially investigated using the response surface methodology with a central composite design. The resulting optimal medium components for laccase production were determined as follows: wheat straw powder 7.63 g/l, soybean meal 23.07 g/l, (NH4)2SO4 1 g/l, CuSO4 0.51 g/l, Tween-20 1 g/l, MgSO4 1 g/l, and KH2PO4 0.6 g/l. Using this optimized fermentation method, the yield of laccase was increased 59.68 times to 67.258 U/ml compared with the laccase production with an unoptimized medium. This is the first report on the statistical optimization of laccase production by Trichoderma harzianum ZF-2.

      • A case of granular cell tumor on the tongue

        Granular cell tumors (GCTs) are rare and mostly benign neoplasms. They are commonly seen in the skin, tongue, breasts, and larynx. Histogenesis of GCT is uncertain, but they are reported to be neural crest origin. A 65-year-old woman presented with an asymptomatic and well-demarcated skin-colored mass on tongue. About 10 years ago, the mass developed and has gradually grown. Physical examination showed the presence of a non-tender, firm, 1 X 1cm sized dome shaped nodule. Histopathologic examination revealed an ill-defined dermal nodule composed of large cells with abundant eosinophilic granular cytoplasm. The overlying squamous epithelium showedpseudoepitheliomatous hyperplasia. Immunohistochemical stain revealed positivity for S-100 protein. Based on these findings, we diagnosed this neoplasm as GCT on the tongue.

        Background: Viral wart is a common disorder and conventional therapies are cryotherapy, CO2 laser ablation, bleomycin intralesional injection. Cryotherapy has been regarded as mainstay in treatment. However the cryotherapy causes unbearable pain and requires repeated treatment sessions during uncertain period.Objectives: The aim of this study was to evaluate the efficacy of punch excision prior to cryotherapy in viral wart treatment, as a synergistic method in reducing cryotherapy sessions. Methods: A retrospective chart and photographic review was performed on 39 patients with viral warts who received punch reduction prior to cryotherapy and compared with other patients who received only cryptherapy on similar site and size. Results: In 39 patients who received punch reduction, the average number of cryotherapy sessions after punch reduction was 1.1±2.57. Compared 15 patients who received punch reduction with other patients who received only cryotherapy, having similar size and site of wart, there was significant difference between them (p<0.01). Conclusion: Compared to ordinary treatment sessions of cryotherapy, punch reduction showed significant decrease in the duration of treatment. Punch excision may be an effective and synergistic method in reducing repeated cryptherapy sessions.

      • A case of osteochondroma masquerading as viral wart

        Osteochondroma is one of the most common benign bone tumors. It is easy for dermatologists to misdiagnose osteochondroma on the fingers or toes as other cutaneous tumors including pilomatricoma, epidermal cyst, and glomus tumor. We report a case of a 42-year-old woman presenting with a palpable mass on the left thumb. Skin biopsy was performed in the other hospital a year ago and she heard the result as a viral wart. In our hospital, the tumor was checked by X-ray, which showed a calcified nodule on the distal phalanx. Bizarre parosteal osteochondromatosis proliferation was suspected on MRI but bony connection between a bony spur and the distal phalanx was obscure. We additionally performed a computed tomography, which revealed a 5mm-sized bony spur connected with cortical bone. Specimen after orthopedic surgery showed fibrous tissue and cartilage cap composed of hyaline cartilage. Based on these findings, we concluded her tumor is an osteochondroma masquerading as viral wart.

        Hypotrichosis simplex is characterized by scanty or absent hair, which is not associated with ectodermal defects or systemic abnormalities. It is classified according to hereditary transmission, extent of affected area, and degrees of absence of hair. We report a case of an 18-year-old man presenting decreased hair density and woolly hair on the scalp. He recognized decreased hair density when he was 7 years old. There was no hair on the arms, legs, and axillary area. There were scanty hair on the eyebrow and pubic area. There was no family history of hereditary disorders affecting hair. Serum analysis revealed the increased level of dehydroepiandrosterone sulfate and progesterone. Low dose dexamethasone suppression test was negative and abdominopelvic computed tomography showed no adrenal incidentaloma or hyperplasia. The histologic examination of the scalp showed decreased number of hair follicles and mild perifollicular lymphohistiocytic and eosinophilic infiltration. Herein, we report a rare case of non-familial generalized hypotrichosis simplex.

Objectives: The purpose of this study is to identify the clinical efficacy and safety of picosecond Nd:YAG laser in treating skin pigmentary disorders in Koreans. Methods: A retrospective photographic and chart review of patients treated with 532-nm or 1,064-nm picosecond laser was performed. Clinical improvement was assessed by two blinded physicians using 5-point global assessment scale (GAS). Results: Within 8-months, total 48 patients with Fitzpatrick skin types II-V, 79.2% women, mean age 31.8±21.2 years, were included. The pigmentary disorders consist of nevus of Ota (20.8%), melasma (18.8%), lentigines (16.7%), postinflammatory hyperpigmentation (PIH), cafe-au-lait macules (CALM, 10.4%), freckles (6.3%), congenital melanocytic nevus, melanonychia (4.3%), blue nevus, nevus spilus, Becker’s nevus, and argyria (2.1%). Patients with CALM showed the best improvement (GAS 3.8±0.76). In contrast, a patient with blue nevus showed the least improvement, which was worse than before. The highest number of treatment sessions was observed in patients with PIH (6.6±3.4 times). Duration of follow-up was the longest in patients with nevus of Ota (22.5±5.4 weeks). There was no persistent complication after treatment. Conclusion: The picosecond Nd:YAG laser is effective and safe modality for treating benign pigmentary disorders.

        Lupus panniculitis usually manifests deep nodules and plaques involving the skin of trunk and proximal extremities, such as the lateral aspects of the upper arms, thighs, and buttocks. Hereditary character and sunlight are well-known predisposing factors. Rarely, lupus panniculitis was exacerbated following anti-hepatitis B vaccination and a lupus profundus like nodule at the site of numerous vaccinations was reported. Also, lupus erythematosus is probably related to trauma induced process, even though the correlation is poor or questionable. We report a case of a 25-year-old woman presenting erythematous patches on the both upper arms and left buttock. She recognized a palpable nodule on an erythematous base on the left upper arm occurred exactly at the site of the first human papillomavirus (HPV) quadrivalent vaccine injection (Gardasil^{®}). Thirty two months later, erythematous to brownish patches were observed on the right upper arm into which the third HPV vaccine was injected and the left buttock. Serum analysis revealed the presence of antinuclear autoantibodies of 1/640. The histologic examination of three cutaneous lesions showed lymphocytic infiltration in the fat lobules and perivascular area, a lymphoid follicle, and loose edematous stroma around fatty lobules consistent with lupus panniculitis. Herein, we report a rare case of lupus panniculitis triggered by HPV vaccination.

      • A study on Korean women`s skin types by baumann skin type questionnaire

        Background: Skin types are various, therefore it is hard to find out them at a glance. To understand them effectively, analysis based on questionnaire could be helpful. Objectives: The purpose of this study is to identify the accuracy and effectiveness of Baumann skin type questionnaire (BSTQ) as compared with an interview with dermatologic specialist. In addition, we aim to find out the difference of skin type proportions according to age and invent a modified BSTQ. Methods: Two-hundred-two women who visited the dermatologic clinic of Kangbuk Samsung hospital, aged 19-64, were enrolled as subjects. They performed both BSTQ and an interview with dermatologic specialist. Modified BSTQ was invented by removing similar and racial questions and adjusting grading scores. Results: The agreement between skin type proportions analyzed by BSTQ and an interview is not reasonable (リ =0.428, 95% CI: 0.363-0.493). There is a weak correlation between the proportion of dryness and age (r=0.208, p<0.01). However, other skin type proportions (sensitivity, pigmentation, and wrinkle) are not significantly correlated with age. Modified BSTQ showed substantial agreement with BSTQ in skin type proportions (リ=0.691, 95% CI: 0.641-0.740). Conclusion: We identified the skin types of Korean women of various ages by BSTQ and an interview. Furthermore, the modified BSTQ might be helpful to recognize skin types concisely.

        The effect of life satisfaction on the silver generation’s spa recognition and preferences

        Background: Although the spa industry is becoming increasingly popular as it is integrated and combined with the aesthetic, cosmetic, medical, food, apparel, and amusement park industries, research and development on spas for the silver generation is still lacking. Objective: The present study aimed to analyze satisfaction with life (SwL), spa perception, and spa program preferences of the silver generation with the objective of using the findings to develop spa programs that are actually needed by the growing elderly population, and provide basic data for the revitalization of the spa market. Methods: The study population consisted of people aged 60 years and older—also known as the silver generation—who were visitors at tourist sites in Damyang-gun, Jeollanam-do (South Korea). Results: The results showed that the silver generation’s self-esteem was highly correlated with their spa perception, while those with high SwL reported a high number of responses for “body massage/body treatment.” Regarding the appropriate cost of a single program, “Korean Won (KRW) 30,000-49,999” was the most common response among respondents with high SwL and “less than KRW 30,000” was the most common response among respondents with low SwL. Regarding the ideal combined program services, “special facial care+massage+care using water” was the most common response among respondents with high SwL, while “massage+care using water+total relaxation care” was the most common response among respondents with SwL. Conclusion: The silver generation’s SwL was found to be mutually associated with spa perception and spa program preferences. Therefore, the study’s findings could be used as basic data for the development of programs tailored for the silver generation and increase the positive perception about spas and the development of spa programs that consider the characteristics of people with high or low SwL.

        Background: According to the increasing incidence of malignant cutaneous neoplasm and life expectancy, evaluating metastasis is getting important factor in treatment. Computed tomography (CT) is the most common initial imaging technique and is useful for evaluating tumor extent, depth of invasion and distant metastasis. Objectives: The aim of this study was to compare the efficacy of CT in detecting metastasis for the non-melanomatous cutaneous malignancies. Methods: We carried out regional CT for evaluation of non-melanomatous cutaneous malignancies in the patients who had been diagnosed in the Kangbuk Samsung hospital from January 2009 to July 2014. Results: CT was performed on 129 patients and there were 3 cases of metastasis. 2 cases were high risk squamous cell carcinoma (SCC) among 37 SCC cases. One case was a patient showing multiple basal cell carcinomas (BCC) on face among 74 BCC cases and there was no metastasis risk factor including large tumor size, deep structure invasion, et cetera. All metastatic lesions were regional lymph nodes. Conclusion: This study revealed that CT is recommended for the high risk SCC patients and BCC patients who show multiple lesions. There is no gold standard radiologic study for evaluating metastasis in non-melanomatous cutaneous malignancies. However, considering low distant metastasis rate of those, CT can be the best initial radiologic study.
