        악성흑색점흑색종 19예에 대한 임상 및 병리 조직학적 연구

        Background: Lentigo maligna melanoma (LMM) is a subtype of melanoma that typically develops on sun-damaged skin. LMM is estimated to comprise 4∼15% of melanomas, but the prevalence is known to be relatively lower in the Korean population than in the Caucasian population. Objective: To review the clinico-pathologic features and treatment outcomes of Korean patients with LMM. Methods: Nineteen patients diagnosed with LMM during 2003∼2015, in the Yonsei University Health System, were included in this study. The age and sex of the patients, lesion location, thickness (Breslow), stage, treatment methods, BRAF, NRAS, and KIT mutation status, and survival rates were analyzed. Results: Among the 19 Korean patients, 11 were male and 8 were female. The median age was 59.2 years. The most common site was the cheek (47.4%), followed by the scalp, eyelid, nose, forehead, lip, and neck. At the time of diagnosis, 13 patients were in localized stages (5 patients, stage 0; 3 patients, stage I; and 5 patients, stage II) and 6 patients were in advanced stages (3 patients, stage III; and 3 patients, stage IV). Patients in the localized stages showed better overall survival (OS) than those in the advanced stages (p=0.012). Nine patients were treated with a wide excision, and 6 using Mohs micrographic surgery. Three patients received high-dose interferon-α therapy; 6, chemotherapy; and 4, radiotherapy. Two patients in stage 0 were treated with topical ingenol mebutate. Two patients had BRAF V600E mutation; 1, NRAS G12R mutation; and 1, KIT mutation. Median OS of the patients was 40.8 months. Conclusion: Our analysis provides additional information about clinical characteristics, treatment, and prognosis of LMM in Korean patients. (Korean J Dermatol 2016;54(10):769∼775)

      • 이동형 생체계측을 위한 임베디드 컴퓨팅

        연구목적: 초소형 정밀 기계 기술로 제작된 2차원 가속도 센서와 916 MH2 단거리 고주파 데이터 통신 기술을 응용하여 인체의 운동상태를 자동으로 감지하고 분류하는 기법을 제안 하고자 하였다. 재료 및 방법: 인체의 신호를 획득하기 위하여 미국 버클리대에서 제작한 무선 센서를 등 산용 조끼의 전면상단에 부착해서, 대상자의 흉골 부위에 위치하도록 하고, 앉은 상태에서 일어서거나 눕거나 하는 동작 및 그 역동작이 일어날 때, 가속도 센서에서 감지되는 4가지 종류의 파형 현화에 해당되는 데이터를 무선센서 네트웍을 통해 획득하였다. 결과: 획득한 신호를 주성분분석(Principal Component Analysis)과 지원벡터머신(Support Vector Machine)을 사용하여, 2차주성분까지를 도표화하고 클러스터링 함으로써, 4개의 영역으로 분할하는 것이 가능함을 말 수 있었다 결론: 소형 센서들이 작동되는 임베디드 컴퓨팅 환경에서 일상생활 중에 발생하는 인체의 4가지 동작을 자동으로 감지하고 분류하는 것이 가능함을 보였다. Purpose: New monitoring and classifying method of human motion contort was pro- posed by using 2-axial MEMS accelerometer and 916 MHz short-range data communica-tion technology. Materials and Methods: The wireless sensor of MICA mote was attached to a woman mountain vest and located close to the subject's sternum. During four types of sub-ject's motion, that is, sit-to-stand, stand-to-sit, sit-to-tying, and tying-to-sit, waveform changes of the accelerometer data were acquired by wireless sensor network. Results: The acquired signal was analyzed by Principal Component Analysis(PCA) and Support Vector Machine(SVM) technique. By clustering the first and second principal components, four regions corresponding to the four types of motion, were clearly segment-ed. Conclusion: Human motion context during daily life could be monitored and classified automatically by using small sensors in embedded computing environment

        혈액 내 구리, 아연 및 ceruloplasmin 농도에 흡연, 음주 및 신체적 활동이 미치는 영향

        Objectives : To investigate the influence of smoking, alcohol ingestion, and physical activity on copper and zinc in RBC and serum and serum ceruloplasmin, this study was performed in a cross-sectional study in 113 healthy men aged 20 to 40 years who had no symptomatic liver, heart, gastrointestinal, and other chronic diseases. Methods : At the men's entry into the study, blood samples were drawn from each subject and immediately centrifuged for analysis of copper, zinc, iron, ceruloplasmin, total cholesterol, and hematocrit. Each man completed a questionnaire that provided information on smoking, amount of alcohol intake, and physical activity. Partial regression analysis was performed on confounding variables such as age, body mass index, hematocrit, serum cholesterol, and serum iron. Results : In general linear models, adjustment for confounding variables did not show statistical differences, and there was only an increasing tendency in serum copper in heavy smoker(P=0.0678). There was no difference between high physical activity with mild smokers and lower physical with heavy smokers. Conclusions : This study suggested that copper, zinc and ceruloplasmin were not good biomarker for early effect by smoking, alcohol intake and physical activity in young adult. However, selection bias should be considered in evaluation of this result, and a large prospective study will be needed in advance on usefulness of copper, zinc and ceruloplasmin as a marker for risk factors and early change of atherosclerosis.

        仁川地域 産業體 女高生의 貧血과 食習慣에 關한 硏究

        This study is purposed to obtain data for anemia prevalance and eating habits of high school girls who have been working at the industry. 190 subjects residing in Incheon are participated in the study. The blood test results indicate that 40.5% of the subjects belongs to the anemia group and its Hb(hemoglobin) level is 11.3±0.8g/dl and the remaining 59.5%, the normal group, shows 12.9±0.6g/이 Hb level. The eating habit of the subjects do not show any significant differences between these two groups. Their eating habit appears to be equally poor in both groups. the earing habit score of the anemia group is 84.94±10.44 and the normal group, 88.97±14.51. And there is significant correlations between food containing protein and Hb level.

        The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.

        Pseudomonas sp. EL-G527에 의한 환경친화성 생물계면활성제의 생산최적조건

        A biosurfactant-producing microorganism was isolated from activated sludge by enrichment culture when grown on a minimal salt medium containing n-hexadecane as a sole carbon source. This microorganism was identified as Pseudomonas sp. and it was named Pseudomonas sp. EL-G527. It's optimal culture condition is 2% n-hexadecane, 0.2% NH_4NO_3, 0.3% KH_2PO_4, 0.3% K_2HPO_4, 0.02% MgSO_4·7H_2O, 0.0025% CaCl_2·6H_2O, 0.0015% FeSO_4·7H_2O in 1ℓ distilled water and initial pH 7.0. Cultivation was initiated with a 2% inoculum obtained from starter cultures grown in 30 ㎖ of the same medium in 250 ㎖ flask. They were cultivated at 30 ℃ in reciprocal shaking incubator and the highest biosurfactant production was observed after 4 days.

        몇 가지 항진균제가 Aspergillus fumigatus에 의한 대식세포의 Cytokine 생산에 미치는 영향

        배경 : 현재 임상에서 사용되는 서로 다른 계열의 항진균제가 면역세포인 폐 대식세포(alveolar macrophage)에서의 cytokine 생산과 nuclear factor- κB (NF- κB)활성에 미치는 영향을 시험관내에서 파악하고 그 의미를 추론해 보고자 하였다. 재료 및 방법 : AmB, MF, ITZ 등 세 가지 항진균제가 쥐의 폐 대식세포인 RAW264.7 세포의 생존에 미치는 영향과, 이들 항진균제의 전처치가 Aspergillus fumigatus conidia 자극에 의한 RAW264.7 세포에서의 TNF-α, IL-10 생산, NF-κB 활성에 미치는 영향, 그리고 GMCSF와의 상승 작용 등을 WST-1, ELISA, western blot assay를 이용해 통해 측정하였다. 결과 : AmB, MF, ITZ 모두 농도 의존적으로 RAW264.7 세포의 생존을 감소시켰다. A. fumigatus conidia 자극으로 RAW264.7 세포에서 TNF-α 생산이 증가하였으며, 세포 생존에 영향을 미치지 않는 농도의 AmB를 전처치하고 conidia로 자극한 경우 TNP-α 생산에는 차이를 보이지 않았으나, MF와 ITZ을 전처치한 경우에는 TNF-α 생산이 감소하였다. GM-CSF는 AmB 전처치 및 A. fumigatus conidia 자극에 의한 TNF-α 생산에 상승 효과를 보였으나, MF, ITZ은 GM-CSF에 의한 상승 효과를 보이지 않았다. IL-10은 AmB에 의해 생산량이 증가하였으며 GM-CSF와 동시 투여시 IL-10 생산이 유의하게 상승하였다. MF와 ITZ 역시 RAW264.7 세포에서 IL-10 생산을 유도하였으나 AmB에 비해 상승폭이 적었으며 GM-CSF와의 상승 효과는 관찰되지 않았다. A. fumigatus conidia는 NF- κB를 활성화하였으며 항진균제와 GM-CSF 등 약제 처치에 따른 NF-κB의 활성 정도는 분비된TNF-α와 IL-10의 생산 정도와 상관관계를 보였다. 결론 : AmB, MF, ITZ 등 본 연구에 사용된 항진균제는 시험관내에서 A. fumigatus conida로 자극하는 RAW264.7 세포에 전처치함으로써 염증성 cytokine인 TNF-α의 생산을 감소시키고 항염증성 cytokine인 IL-10의 생산을 증가시키는 면역 조절 효과를 나타낼 수 있음이 관찰되었다. 추후 약제간의 면역조절 기능 차이가 항진균효과에 미치는 영향 및 약제에 따른 면역 조절 경로 등에 대한 연구가 필요할 것으로 사료된다. Background : We wanted to evaluate the immunomodulatory effect of three antifungal agents, amphotericin B (AmB), micafungin (MF), itraconazole (ITZ), in the aspect of cytokine production and expression of nuclear factor-κB (NF-κB) in the Aspergillus fumigatus conidia treated-RAW264.7 cells, a murine alveolar macrophage cell line. Materials and Methods : We evaluated cytotoxic effect of antifungal agents using commercial cell proliferation assay. Tumor necrosis factor (TNF)-α and interleukin (IL)-10 production according to stimulation (control, A. fumigatus conidia only, conidia + antifungal drug, conidia + antifungal drug + GM-CSF) was evaluated and compared each using commercial ELISA method. NF-κB activation was evaluated by western blot analysis. Results : AmB, MF, ITZ showed dose-dependent cytotoxic effect on the tested cells. Stimulation of cells by A. fumigatus conidia induced TNF-α production. Pretreatment of AmB at concentration not affecting cellular survival did not change the production of TNF-α compared to conidia treated cells, but pretreament of MF or ITZ showed reduced amount of TNF-α production compared to conidia treated cells. AmB also showed synergistic effect on TNF-α production when simultaneously treated with GM-CSF. IL-10 production was markedly increased when the cells were treated with AmB with conidia. MF and ITZ induced less increase of IL-10 production than AmB. AmB also showed synergistic effect on the production of IL-10 when treated with GM-CSF simultaneously. A. fumigatus conidia enhanced expression of NF-κB. The degree of NF-κB expression was associated with the amount of TNF-α and IL-10 produced. Conclusion : The antifungal agents we used in this experiment showed that decreased TNF-α production and increased IL-10 production from the RAW264.7 cells stimulated by A. fumigatus conidia after pretreatment of antifungal agents. But more studies, such as the association between immunomodulatory effect, antifungal activity and difference of signal pathway of cellular activation according to drugs should be performed.
