        관상동맥 질환에서 CD14 유전자형에 따른 세균 및 Heat Shock Protein에 대한 반응의 차이

        Background : CD14 is the receptor for lipopolysaccharides and heat shock protein (HSP), which has been suggested being associated with increased risk of coronary artery disease (CAD). We investigated whether the response to infectious agents or HSP is different according to CD14 polymorphism in Koreans. Materials and Methods : Antibody titers to Helicobacter pylori, Chlamydia pneumoniae, and human HSP60 (hHSP60) were measured in 48 patients with stable CAD and in 41 healthy controls by ELISA. CD14 genotype was determined by PCR and high-sensitivity C-reactive protein (hs-CRP) was measured. Results : Seropositivity to C. pneumoniae and H. pylori, and antibody titer to hHSP60 were not significantly associated with the presence of CAD. CD14 genotype distribution was 31 TT (35%), 43 CT (48%), and 15 CC (17%). To compare the response to the infectious organism and hHSP60, we divided study population into 3 groups; CAD patients with non-TT genotype (group I, n=30), CAD patients with TT genotype (group II, n=18), and normal controls (group III, n=41). Seropositivity to C. pneumoniae and H. pylori, and antibody titer to hHSP60 were not significantly different among 3 groups. Though hs-CRP level was significantly different among 3 groups, post-Hoc analysis showed that hs-CRP level was not significantly different between group I and group II (group I: 1.6[1.1-3.5] mg/L and group II: 0.35[0.1-2.0] mg/L). Conclusions : This study suggests that the inflammatory responses to infectious organisms and HSP do not differ according to the CD14 genotype in Koreans. 목적 : 만성적인 세균 감염이나 자가 면역 반응이 동맥경화와 연관되어 있다는 주장이 제기되어 왔다. CD14은 lipopolysaccharides (LPS)와 heat shock protein(HSP)의 수용체로 C(-260)→T 다형성이 관상동맥 질환의 위험과 관련되어 있다고 제안되어 왔다. 본 연구에서는 한국인에서 CD14 다형성에 따른 LPS와 HSP에 대한 반응성을 염증표지자를 측정하여 살펴보고자 하였다. 재료 및 방법 : 안정형 관상동맥 환자 48명과 정상 대조군 41명을 대상으로 Chlamydia pneumoniae, Helicobacter pylori 항체 및 인체 HSP60 (hHSP60) 항체 역가를 enzyme-linked immunosorbent assay (ELISA) 방법으로 측정하였다. CD14 유전자형은 중합효소연쇄반응을 이용하여 결정하였고 hs-CRP를 측정하였다. 결과 : C. pneumonias 및 H. pylori 항체 양성 여부 및 항체 역가, 그리고 hHSP60에 대한 항체 역가는 안정형 관상동맥 질환의 유무와 유의한 상관관계는 없었다. CD14 유전자형의 분포는 TT 30명(39%), CT 31명(40%), 그리고 CC 16명(21%)이었다. 관상동맥 질환 군에서 TT 유전자형은 38%, 대조군에서는 32%로 TT 유전자형과 관상동맥 질환의 유무와는 유의한 상관관계가 발견되지 않았다(P>0.05). CD14 유전자형에 따라 C. pneumonias와 H.pylori 감염 및 hHSP60에 따른 반응에 차이가 있는지 여 부를 알아보기 위해, 관상동맥 질환 군을 다시 TT 유전자형 군과 non-TT 유전자형 군으로 나누어, 전체적으로 세군에서 항체 양성률과 역가를 비교하였다(I군: CD14 TT 유전자형을 가진 안정형 관상동맥 환자, II군: CD14 TT 유전자형을 가진 안정형 관상동맥 환자, III군: 정상 대조군). 세 군에서 C. pneumonias와 H. pylori 항체 양성률 및 hHSP60 항체 역가에는 유의한 차이가 없었다. 세 군사이에 hs-CRP 값에 유의한 차이가 있었지만, post-Hoc분석에서 II군의 hs-CRP 값이 I군에 비해 유의하게 높지는 않았다(0.35 [0.1-2.0] mg/L in group II vs. 1.6 [1.1-3.5]mg/L in group I, P>0.05). 결론 : 본 연구는 한국인에서 CD14 유전자형에 따라 LPS나 HSP에 대한 반응이 유의하게 다르지 않음을 시사한다.

      • 고령토로부터 알루미나 추출에 관한 연구(Ⅱ) : 질산에 의한 추출

        The preparation conditions of alumina powder extracted from domestic San-Cheong Kaolin by nitric acid were investigated. The experimental conditions which have an effect on the yield of alumina were as follows ; the calcination temperature and time, the acid concentration, the reaction temperature and time, pH of the extracted solution, and particle size of the raw material. The aggolomeration of pseudo-boehmite precipitated by NH₄OH at pH 8 was controlled by various drying methods. α-Al₂O₃powder prepared by heating of the dried powder for 1 hour at 1200℃. Under the optimum condition, the yield and purity of α-Al₂O₃powder were 73.6 percent and 95.5 percent, respectively. The alumina particles prepared by the freeze drying method revealed narrower size distribution and less agglomerate than others. And impurities of the raw material which have an effect on the extraction of alumina were Kyanite and Anorthite.

        Lymphangioma에 의한 거설증을 지닌 환자의 개교합 증례보고 : A CASE REPORT

        Lymphangioma는 림프관에 나타나는 양성종양으로 림프관의 증식을 특징으로 한다. 이 종양의 대부분은 출생 시에 나타나며 종양의 95%정도가 10세 이전에 나타난다. 구강 내에 발생하는 경우에는 혀에서 가장 호발하며 거설증을 야기한다. 대개 양측성으로 혀의 전방 2/3부위에 나타나며 불규칙한 회색과 핑크빛을 띤 소절형태를 보인다. 본 증례 모두에서 환아들은 출생당시 혀에 lymphangioma를 지니고 태어나 거설증을 보였으며 이로 인해 개교합과 하악과성장 양상을 나타내었다. Lymphangioma에 의한 비정상적인 혀의 비대에 의해 환아의 구강과 두개안면부의 주위조직에 대한 적응 결과로 나타난 소견을 관찰하고 관련문헌을 고찰하여 다소의 지견을 얻었기에 보고하는 바이다. Lymphangioma is a benign tumor of lymphatic vessels. The majority of cases are present at birth and 95% of the tumors aroused before the age of 10 years. Oral lymphangioma may occur at various sites but are most frequent on the anterior two thirds of the tongue, where they often result in macroglossia. The lesions present nodularity with gray and pink projections. In the present cases, all the patients who had the macroglossia combined with lymphangioma showed openbite and mandibular prognathism. The purpose of this paper was to report the dental and craniofacial findings of macroglossia combined with lymphangioma and review the pertinent literature through the cases.

      • S-162 : Anomalous origin of left coronary artery from right sinus of valsalva presenting with syncope

        Introduction: Congenital anomalies of coronary artery have various symptoms, depending on the adjacent or surrounded structure with its origin and pathway. Anomalous left coronary artery (LCA) arising from right coronary valsalva sinus is one of coronary artery anomalies can lead to ischemia, but most of patients with anomalous LCA from opposite sinus have no symptoms and diagnosed by autopsy in young age. About 18-30% of patients with anomalous coronary artery have symptoms, who could have early diagnosis and undergoing CABG surgery for risk reduction. Case report: A 62-year-old patient came to hospital by syncope. He had underlying disease as hypertension, diabetes mellitus and dyslipidemia. In coronary angiography, LCA originated from right sinus of valsalva without stenosis. Following coronary CT angiogram showed Left main was originated from right sinus of valsalva and its pathway was interarterial type. This patient keeps doing well with medical treatment without any fatal events until now. Conclusion: Anomalous coronary artery is rare case, so it is easily missed until fatal symptoms occur. Congenital anomalies of coronary artery have various symptoms depending on the adjacent surrounded structure with origin and pathway of coronary artery. We report a male patient with anomalous LCA arising from right coronary valsalva sinus presenting syncope. This patient does not have exertional coronary symptoms, and keeps 1 year follow up with medications without undergoing CABG surgery or any of fatal events.

        Congenital anomalies of the coronary artery are associated with various symptoms including syncope, myocardial ischemia, and sudden cardiac death. The abnormality depends on the adjacent structure and pathway of the coronary artery. Most patients with an anomalous left coronary artery that arises from a right coronary sinus of the valsalva have no symptoms and are usually diagnosed at autopsy. Therefore, their first symptom might present as sudden death, particularly when the left coronary arterial course is between the aorta and the pulmonary trunk. Symptomatic patients could be diagnosed early with an anomalous coronary artery, and the risk of fatal events could be decreased by surgical correction. Here, we report the case of 62-year-old male who experienced a first episode of syncope with an anomalous left coronary artery arising from the right sinus of the valsalva with a separate orifice from the right coronary artery. He is alive and in good health receiving medical treatment, and has had no medical events for over 2 years. (Korean J Med 2014,87:722-727)

        Paired Ig-Like Type 2 Receptor-Derived Agonist Ligands Ameliorate Inflammatory Reactions by Downregulating beta1 Integrin Activity

        The paired immunoglobulin-like type 2 receptor (PILR) family consists of two functionally opposite members, inhibitory PILR and activating PILR receptors. PILRs are widely expressed in various immune cells and inter-act with their ligands, especially CD99 expressed on activated T cells, to participate in immune responses. Here we investigated whether PILR-derived agonists inhibit 1 integrin activity as ligands for CD99. PILR-derived peptides as well as PILR-Fc fusion proteins prevented cell adhesion to fibronectin through the regu-lation of 1 integrin activity. Especially, PILRpep3, a representative 3-mer peptide covering the conserved motifs of the PILR extracellular domain, prevented the clustering and activation of 1 integrin by dephosphory-lating FAK and vinculin, which are major components of focal adhesion. In addition, PILRpep3 inhibited transen-dothelial migration of monocytes as well as endothelial cell tube formation. Furthermore, upon intraperitoneal injection of PILRpep3 into mice with collagen-induced arthritis, the inflammatory response of rheumatoid arthritis was strongly suppressed. Taken together, these results suggest that PILR-derived agonist ligands may prevent the inflammatory reactions of rheumatoid arthri-tis by activating CD99.

        반응성 이온 증착법에 의한 타이타늄 질화물 피막의 구조 및 성질에 미치는 기판온도와 질소압력의 영향

        Titanium nitride films were deposited onto the high speed steel substrates by reactive ion plating method The substrate temperature and ;partial pressure of the N₂gas were controlled as process variables. The nitride phases in the films were identified by X-Ray diffraction technique and were observed in a scanning electron microscope. The hardnesses of deposited films were also measured. The phases presented in the coatings could be predicted from nitrogen gas pressure. And the structure modification by ion bombardment during deposition was also observed. The maximum hardness values of 2800-3300 KHN were obtained at the nitrogen pressure of 0.8×10_(-3) torr.

        BARE법에 의한 TiN 증착막의 미세조직과 절삭 공구의 수명에 관한 연구

        TiN has been applied to wear resistant coatings in cutting tools and bearings by PECVD, Sputtering and Ion Plating. Most of the TiN depositing processes take advantage of plasma assistance in lowering required substrate temperature for commercial tool materials like toos steels and high speed steels. BARE(Biased Activated Reactive Evaporation) has wide applications in compound film deposition at low substrate temperature. The plasma density is the main factor to activate the compound formation reaction and to enhance the adhesion between TiN film and the substrate material. In this study, the direct comparison of coatings' performance by screech machining test was carried out for various discharge conditions including the substrate bias, the anode current, and the partial flow rate of less active gas, N₂. The machining life was more dependent on the adhesion than films' microhardness. The maximum increase of machining life was 15 times as the uncoated drills. On the average, 5 times increase was achived with gold color TiN coated high speed steel drills.
