치아 우식증은 감염성 질환의 하나로 치아우식의 원인균은 Streptococcus mutans(S. mutansJ와 같은 mutans stepto-cocci로 알려져 있다. S. mutans가 치면에 접착하여 군집을 형성하는 능력은 균독성에 중요한 역할을 하는데. Agl/IT와 같은 세포 표면의 섬유성 단백질을 매개로 한다. Secretory 1gA는 타액이나 누 · 비액, 초유, 그리고 폐나 소화기관의 분비액에서 선택적으로 다량 발견되는데 타액에서 se cretory 19A는 S. mutans의 대사활동을 억제하고 치면으로의 부착을 방해한다. 이전의 몇몇 연구에서 S. mutans에 특이적인 타액 내 IgA와 우식경험도는 역상관관계를 보인다고 발표하였다. 그러나 다른 연구에서 통계적 유의성이 없다고 보고하기도 하였다. 본 연구의 목적은 소아 성인의 치아우식증과 S. mutans의 Ag I/II에 특이적인 타액 내 IgA와의 관계를 알가위한 것이다. 이를 위해 소아(평균6세) 28명, 성인(20-30세) 18명을 대상으로 Group I(deft index ≤ 3). Group II (deft index ≥ 4), Group III (DMFT index ≤ 3), Group IV (DMFT index ≥ 4)로 분류하였다. 그리고 caries resistant group(CR group, deft or DMFT index ≤ 3)과 caries susceptible group(CS group. deft or DMFT index ≥ 4)으로 분류하였다. S. mutans 수와 우식 경험도 간에는 통계적으로 유의 한 상관관계를 나타냈다. Agl/II 특이 salivary IgA titer는 Group III 이 Group II 보다 통계적으로 유의하게 더 컸으며, CR group이 CS group보다 유의하게 크게 나타났다. AgI/II of Streptococcus muiens(S. mutens) is an important virulence factor that contributes to the pathogenesis of S. mutans-induced dental caries. In oral cavity, salivary IgA antibodies act as safeguards against enormous challenges from oral bacteria. IgA antibodies inhibit adherence of cariogenic microorganisms to hard surfaces. Analysis of salivary IgA against AgI/II can be very useful diagnostic and powerful communication tools to the dental caries The purpose of this study was to investigate correlation between salivary AgI/II specific IgA and incidence of dental caries among children and young adults. Subjects consisted of 28 children and 18 adults. They were as¬signed to four groups: Group I (deft index ≤ 3), Group lI (deft index ≥ 4), Group Ill (DMFT index ≤ 3), Group IV (DMFT index ≥ 4) and they was divided two groups into caries resistant group and caries susceptible group. The study group were examined caries activity and their salivary IgA was evaluated by enzyme-linked immunosorbent assay. The results are as follows : 1. There was a positive correlation between the number of S. mutans and caries activity. 2.The titer of salivary IgA against the AgI/II was significantly higher in caries resistant group than caries susceptible group(p<0.01). 3. The titer of salivary IgA against the AgI/II in Group Ill was significantly higher than Group II (p<0.05).

목 적 : 수술적 방법 및 보존적 방법 으로 치료한 상완골 근위부 골절에서 견관절의 해부학적 정복 및 기능적 회복 정도와 치료 방법에 따른 결과의 차이를 알아 보고자 하였다. 대상 및 방법 : 12개월 이상 추시 된 42예를 보존적 치료군(1군, 20예), 도수정복후 Rush pin 또는 경피적 핀 삽입군(2군, 11예), 근위 골수강내 교합정 고정군(3군, 11예)으로 나누었다. 기능적 평가 에는 설문지를 이용한 modified neer 방법을 이용하였고 방사선 사진상 경간각을 이용한 Paavolainen 방법으로 해부학적 정복 정도를 분석 하였다.각 군의 결과를 비교한 통계 처리는 Generalized Liner Model을 이용한 분산분석과 Fisher's exact test을 이용하였다. 결 과 : 방사선사진에 의한 해부학적 정복정도의 결과는 3군에서 가장 우수하였다.(p<0.05). 골 유합 시기는 평균 10.33 주였다. 기능 점수 또한 3군에서 가장 높았으며 합병증도 9례 에서 발견되었는데 3군에는 1례만 존재하였다. 전체적 결과에서 3군이 다른군과 비교하여 우수한 결과를 보였다. 결 론 : 근위 골수강내 교합정 고정술은 전위형 상완골 근위부 골절에서 여러방향 으로 고정 나사의 삽입이 가능하여 골편의 정복이 용이하므로 해부학적 정복 정도가 다른 방법에 비해 좋고(p<0.05) 합병증을 최소화하면서 조기 재활 및 골 유합에 충분한 고정력을 얻을 수 있는 좋은 방법으로 사료 된다. Purpose : To evalute the anatomical and functional outcome after treatment for proximal humerus fracture according to surgical and conservative treatment and assess the final results according to treatment methods. Materials and Methods : Forty-two cases with follow-up over 12 months were divided into three groups: conservative treatment (Group 1, 20 cases), closed reduction with Rush or percutaneous pin fixation (Group 2, 11 cases), proximal intramedullary interlocking nailing(Group 3, 11 cases). The functional outcome was obtained by modified neer method using self-assessed score paper and the anatomical outcome was obtained by paavolainen method using radiologic film score. Statistics in comparing with the result of each group was analized by variance analysis using Generalized Liner Model and Fisher?s exact test. Result : Anatomical reduction was best obtained in group 3 (p< 0.05) and mean duration of bone union was 10.33 weeks. Functional score was also best obtained in group 3 (p<0.05). Complication was noted in 9 cases. There was only 1 case in group 3. In studing of overall outcome, the group 3 has good result than other groups. Conclusion : Proximal intramedullary interlocking nailing in displaced proximal humerus fracture can be demonstrated as better method for anatomical reduction(p<0.05) than other methods because it can make insertion of locking screw ease with multiple direction for anatomical reduction and can provides a sufficient fixation for early rehabilitation and union , while minimizing complication.
PLLA/HA Composite Scaffold와 골수 줄기세포를 이용한 조직공학적 골재생에 대한 연구

Aim of the study: Scaffolds are crucial to tissue engineering/regeneration. Biodegradable polymer/ceramic composite scaffolds can overcome the limitations of conventional ceramic bone substitutes such as brittleness and difficulty in shaping. In this study, poly(L-lactide)/hydroxyapatite(PLLA/HA) composite scaffolds were fabricated for in vivo bone tissue engineering. Material & methods: In this study, PLLA/HA composite microspheres were prepared by double emulsion-solvent evaporation method, and were evaluated in vivo bone tissue engineering. Bone marrow mesenchymal stem cell from rat iliac crest was differentiated to osteoblast by adding osteogenic medium, and was mixed with PLLA/HA composite scaffold in fibrin gel and was injected immediately into rat cranial bone critical size defect(CSD:8mm in diameter). At 1. 2, 4, 8 weeks after implantation, histological analysis by H-E staining, histomorphometric analysis and radiolographic analysis were done. Results: BMP-2 loaded PLLA/HA composite scaffolds in fibrin gel delivered with osteoblasts differentiated from bone marrow mesenchymal stem cells showed rapid and much more bone regeneration in rat cranial bone defects than control group. Conclusion: This results suggest the feasibility and usefulness of this type of scaffold in bone tissue engineering.

Pleiotrophin or osteoblast-specific factor 1(HOSF-1) is a growth-associated protein present in bone matrix. This study was designed to study pleiotrophin expression in osteoblastic cells. Pleiotrophin was expressed by osteoblast-like cell line. Pleiotrophin expression increased following the proliferative phase and was minimal at the terminal phases of the induced differentiation of cultured MC3T3-E1 cells. Pleiotrophin expression represents another autocrine factor that may contribute to the physiologic control of induced bone formation. In this study, induced osteogenesis will be examined in the context of the osteoblast expression of and regulation by PTN. I hypothesized that PDGF-BB stimulation of PTN expression represents an important paracrine signal during the induced osteogenesis associated with periodontal and implant surgeries. The possible mediation by PTN of anabolic effects attributed to PDGF-BB stimulation was examined in cell culture models of osteoblast differentiation. These studies will contribute fundamental insights to osteoblast biology and insights regarding the potential use of factors such as PTN in the clinical environment.

광양항 3단계 1차 컨테이너터미널 부지 연약지반개량의 경우 광양지역의 특성상 재하용 토사확보 곤란, 시공기간 단축 및 환경훼손 최소화를 위해 진공압밀공법이 적용되었다. 본 사례연구에서는 진공압밀공법의 시공에 따른 문제점 및 개선사항을 제시하였고, 현장계측 및 진공 압밀완료 후에 실시된 지반조사 결과 등을 분석하여 준설매립지반에서의 진공압밀공법의 적용성을 평가하였다.
