Objective: This study seeks to establish the benefits of Upper Thoracic Manipulation on chronic neck pain patients with forward head posture by examining positive changes in their degree of neck pain, physical functioning, posture, pulmonary function and chest mobility. Method: 30 women with chronic neck pain patients with forward head posture who were receiving physical therapy in H hospital were divided into a Upper Thoracjc Manipulation Group(TMG) and a Upper Thoracic self Exercise Group(TEG). One group was treated by upper thoracjc manipulation and the other group by upper thoracic self exercise 3 times a week for 8 weeks. Evaluation were made on Visual Analogue Scale(VAS), Neck Dysfunction Index(NDI), Cranio Vertebral Angle(CVA), Pulmonary Function(PF) and Chest Expansion(CE). Repeated measure ANOVA was used to compare the evaluations of results before the experiment, results after 4 weeks, and results after 8 weeks. ANCOVA was used to compare between the groups. Results: In VAS, NDI, CVA, PF and CE both TMG and TEG showed weekly differences(p<.05). TMG showed more reduction in VAS, NDI more increase in CVA, PF and CE. Conclusion: In this study it showed that TMG is more effective than TEG on chronic neck pain patients with forward head posture in VAS, NDI, CVA, PF and CE.

Ossification of the posterior longitudinal ligament (OPLL) is a rare but potentially devastating cause of degenerative cervical myelopathy (DCM). Decompressive surgery is the standard of care for OPLL and can be achieved through anterior, posterior, or combined approaches to the cervical spine. Surgical correction of OPLL via any approach is associated with higher rates of complications and the presence of OPLL is considered a significant risk factor for perioperative complications in DCM surgeries. Potential complications include dural tear (DT) and subsequent cerebrospinal fluid leak, C5 palsy, hematoma, hardware failure, surgical site infections, and other neurological deficits. Anterior approaches are technically more demanding and associated with higher rates of DT but offer greater access to ventral OPLL pathology. Posterior approaches are associated with lower rates of complications but may allow for continued disease progression. Therefore, the decision to pursue either an anterior or posterior approach to surgical decompression may be critically influenced by complications associated with each procedure. The authors critically review anterior and posterior approaches to surgical decompression of OPLL with particular focus on the complications associated with each approach. We also review the recent work in developing new surgical treatments for OPLL that aim to reduce complication incidence.

The Mediterranean Basin is the largest of five regions around the world that constitute, in aggregate, the Mediterranean Woodlands, Forests, and Scrub Biome under a commonly-used global ecological classification system. All of these regions – the Mediterranean Basin itself as well as the similar ecological regions in California, Chile, South Africa, and Australia – face severe ecological degradation, largely because of agricultural practices. Traditional nation-states cannot address this ecological crisis adequately. A new form of political organization – an “Eco-State” – can and should be established for this purpose. Doing so will require a reorientation of the centuries-old notion of sovereignty, a reorientation that is already underway in some respects. The Mediterranean Biome Eco-State would build on this momentum. It would hold binding authority over all ecological and agricultural aspects of the territories falling within its boundaries, thus exercising a form of blended sovereignty that it would share with other authorities. This essay summarizes some key aspects of such a new Mediterranean Biome Eco-State.
