        ‘Outspoken’ Oral Hygienists Promote Oral Health Care Awareness at Public Events for (Pregnant) Women and Young Mothers

        Background: The aim of these two (pre COVID-19 pandemic) public health studies in Amsterdam were to promote the profession of the oral hygienist and to increase oral health care awareness among (pregnant) women and young mothers, who were visitors of public consumer’s exhibitions. Methods: Two cross-sectional studies were carried out. In 2018, during the first study, 1,765 visitors of the Household Fair completed a questionnaire. In 2020, during Study 2, 304 visitors of the Nine-Month Fair completed a semi-structured online questionnaire. At both fairs a team of ‘outspoken’ oral hygienists, i.e., members of the Foundation ‘Goed Gebekt’, provided the visitors with tailored advices about general health related to adults, mothers’, and their children’ oral health, using natural routine interviewing, that was based on their own professional daily practical experience. Results: In Study 1, just over half (53.5%) of a total of 1,742 visitors, (85.7% female) was younger than 45 years old. In Study 2, 210 visitors with a mean age of 30.7 years were included. Most women reported their self-perceived oral health as ‘good’ (mean=8.0, standard deviation=1.3; range 3∼10). Qualified (self) employed oral hygienists seem to be known among the visitors, but still only visited upon referral of the dentists. Up to the age of 18 reimbursements by the basic insurance is still unknown to the public. Conclusion: The Foundation ‘Goed Gebekt’ encourages other oral hygienists to make their own initiatives and experiences public and to share it. Public awareness is needed for oral health promotion and the development of optimally targeted interventions among (pregnant) women and young mothers by oral hygienists.

        Collaborating in a Flipped Translation Classroom: The Student Perception

        The development of technology has revolutionized the translation process and the workflow of the translator. Translators are no longer working alone, but are expected to work in a team and join in discussions. The skills required of a contemporary translator is not the same as the skills required of a translator ten years earlier, but has this affected the way translators are trained? In order to prepare students for the job market, teachers should find ways to encourage students to play a more active role in acquiring the skills and competencies required of a professional translator. This is where flipped translation training comes into play. This study investigates the effects of group discussions and peer revisions on students in a translation class using the flipped classroom approach and observes how comments from their peers help them in translation revision. This study also analyses the effects of these discussions and reviews on the revisions. The research questionnaire included student perceptions of the effects of group discussions and peer revisions on their revision and adoption rate of feedback from these discussions and reviews.

        Business Cycle Synchronization and Core-Periphery Patterns in the East African Community: A Wavelet Approach

        Optimum currency area theory suggests that various characteristics are needed for a successful monetary union, including similarities in economic structures for both shocks and business cycles. Accordingly, this study uses continuous wavelets to investigate business cycle synchronization among countries of the East African Community, which is, a region working toward the establishment of a monetary union by 2024. Wavelet decomposition is an alternative and powerful tool for analyzing the comovement of business cycles. Crosswavelet coherency suggests that the business cycles of Tanzania and Uganda were in phase with that of Kenya’s at high and medium frequencies in the early 1990s and after the establishment of the customs union in 2005. Wavelet spectra clustering shows that Kenya, Tanzania, and Uganda form the core of the monetary union, whereas Burundi and Rwanda form the periphery. Overall, the wavelet analysis highlights the significance of asymmetric shocks and the prevalence of core-periphery patterns, which casts doubts on the eventual viability of the East African Monetary Union.

        Hepatocellular carcinoma surveillance: Eastern and Western perspectives

        Eastern and Western guidelines for the management of hepatocellular carcinoma (HCC) are known to significantly differ on many points, because they reflect different diagnostic and therapeutic approaches to this cancer. Importantly, these guidelines are primarily consensusdriven when it comes to surveillance, both in term of the tests used and surveillance program design. The main difference between East and West lies in clinical practice, as several Eastern countries implement coordinated and systematic surveillance programs, while most Western countries rely on individual adherence to surveillance recommendations. This review article presents an overview of the evidence supporting surveillance programs for HCC, with a particular focus on the efficacy, cost-effectiveness, and consequences of this approach for patient survival. Western and Eastern guideline recommendations are discussed.

        The Effects of Fatigue on Cognitive Performance in Police Officers and Staff During a Forward Rotating Shift Pattern

        Background: Few studies have examined the effects of a forward rotating shift pattern on police employee performance and well-being. This study sought to compare sleep duration, cognitive performance, and vigilance at the start and end of each shift within a three-shift, forward rotating shift pattern, common in United Kingdom police forces. Methods: Twenty-three police employee participants were recruited from North Yorkshire Police (mean age, 43 years). The participants were all working the same, 10-day, forward rotating shift pattern. No other exclusion criteria were stipulated. Sleep data were gathered using both actigraphy and selfreported methods; cognitive performance and vigilance were assessed using a customized test battery, comprising five tests: motor praxis task, visual object learning task, NBACK, digital symbol substitution task, and psychomotor vigilance test. Statistical comparisons were conducted, taking into account the shift type, shift number, and the start and end of each shift worked. Results: Sleep duration was found to be significantly reduced after night shifts. Results showed a significant main effect of shift type in the visual object learning task and NBACK task and also a significant main effect of start/end in the digital symbol substitution task, along with a number of significant interactions. Conclusion: The results of the tests indicated that learning and practice effects may have an effect on results of some of the tests. However, it is also possible that due to the fast rotating nature of the shift pattern, participants did not adjust to any particular shift; hence, their performance in the cognitive and vigilance tests did not suffer significantly as a result of this particular shift pattern.

        ENPP1-Fc prevents neointima formation in generalized arterial calcification of infancy through the generation of AMP

        Generalized arterial calcification of infancy (GACI) is associated with widespread arterial calcification and stenoses and is caused by mutations in ENPP1. ENPP1 encodes for ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1), which cleaves ATP to generate inorganic pyrophosphate (PPi) and adenosine monophosphate (AMP) extracellularly. The current study was designed to define the prevalence of arterial stenoses in GACI individuals and to identify the mechanism through which ENPP1 deficiency causes intimal proliferation. Furthermore, we aimed to effectively prevent and treat neointima formation in an animal model of GACI through the systemic administration of recombinant human (rh)ENPP1-Fc protein. Based on a literature review, we report that arterial stenoses are present in at least 72.4% of GACI cases. We evaluated the effect of rhENPP1-Fc on ENPP1-silenced human vascular smooth muscle cells (VSMCs) and on induced intimal proliferation in Enpp1-deficient ttw/ttw mice treated with carotid ligation. We demonstrate that silencing ENPP1 in VSMCs resulted in a tenfold increase in proliferation relative to that of cells transfected with negative control siRNA. The addition of rhENPP1-Fc, AMP or adenosine restored the silenced ENPP1- associated proliferation. In contrast, neither PPi nor etidronate, a current off-label treatment for GACI, had an effect on VSMC proliferation. Furthermore, subcutaneous rhENPP1-Fc protein replacement was effective in preventing and treating intimal hyperplasia induced by carotid ligation in an animal model of GACI. We conclude that ENPP1 inhibits neointima formation by generating AMP. RhENPP1-Fc may serve as an approach for the effective prevention and treatment of arterial stenoses in GACI.
