        Serum leptin, adiponectin and endometrial cancer risk in Chinese women

        Objective: To investigate the relationship between serum concentrations of leptin or adiponectin, and endometrial carcinoma in Chinese women. Methods: We conducted a case-control study of a total of 516 Chinese women to detect the relationships between serum concentrations of leptin or adiponectin, and endometrial carcinoma in Chinese women. The study subject constituted 206 cases of endometrial cancer and 310 normal controls. Results: Patients with endometrial carcinoma had higher serum leptin concentrations than controls (28.8±2.2 ug/L vs. 19.8±1.4 ug/L; p<0.001). The adiponectin levels in patients were lower than in controls with borderline statistical significance (2,330.7±180.5 ug/L vs. 2,583.9±147.2 ug/L; p=0.078). Logistic regression analysis confirmed the associations between leptin or adiponectin, and endometrial carcinoma after adjustment for age, body mass index, fasting insulin, serum glucose, cholesterol, triglycerides, and high-density lipoprotein cholesterol (odds ratio for the top tertile vs. the bottom tertile: leptin 2.05; 95% confidence interval [CI], 1.28 to 3.29; p<0.001; adiponectin 0.52; 95% CI, 0.32 to 0.83; p<0.001). Conclusion: Increased leptin or decreased adiponectin levels are associated with endometrial carcinoma.

        Different failure rates of insertion of 10/12-Fr ureteral access sheaths during retrograde intrarenal surgery in patients with and without stones

        Purpose: The aim of this study was to compare the failure rates of insertion of a 10/12-Fr ureteral access sheath (UAS) during retrograde intrarenal surgery (RIRS) in cases with and without stones and to analyze the risk factors for UAS insertion failure. Materials and Methods: A total of 640 RIRS cases (538 with and 102 without stones) were evaluated. The primary outcome of interest was the failure rate of insertion of a 10/12-Fr UAS. Associated risk factors were assessed using univariate and multivariate logistic regression analyses. Propensity score (PS) matching and inverse probability of treatment weighting (IPTW) were used to ensure the robustness of the results. Results: The overall failure rate of 10/12-Fr UAS insertion in the cases without stones was significantly higher than that in the cases with stones (39.2% vs. 7.2%; p<0.001), and was approximately 2.5 to 4 times higher after PS matching and IPTW. Multivariate logistic analyses showed that being in the group without stones and younger age were independent significant risk factors for insertion failure in both the PS-matched cohort (odds ratio [OR], 5.43; 95% confidence interval [CI], 2.16–13.6; and OR, 1.04; 95% CI, 1.01–1.07) and the IPTW-adjusted cohort (OR, 1.82; 95% CI, 1.14–2.90; and OR, 1.03; 95% CI, 1.01–1.04). Conclusions: The incidence of 10/12-Fr UAS insertion failure during RIRS was higher in cases without stones than in those with stones. These results provide valuable information for surgeons to use during informed consent discussions with patients undergoing RIRS, especially patients without stones.

        Awake Surgery for Angiocentric Glioma in the Eloquent Area in an Adolescent: A Case Report

        Angiocentric glioma (AG) is an extremely rare tumor that often develops in adolescents. Awake surgery for AG occurring in the eloquent area has not been reported to date. We report a case involving a righthanded 15-year-old boy with AG. He presented with a first-time generalized tonic-clonic seizure and was rushed to the local hospital. CT of the head indicated a left frontal low-density mass with no calcification. He was subsequently referred to our hospital. Comparison with a CT scan obtained two years prior due to mild head trauma indicated that the lesion showed a trend toward enlargement. The lesion was located in the anterior and lateral portions of the primary motor cortex, and MRI showed homogenous hypointensity on T1-weighted and hyperintensity on T2-weighted images. Contrast-enhanced MRI showed a linear contrast effect. The patient underwent awake surgery with successful intraoperative brain mapping and total resection, and brain function was preserved. Pathological analysis revealed AG. He returned to his normal life and has shown no recurrence without additional treatment for 2 years. Thus, awake surgery for complete tumor resection while preserving brain function is effective and safe even in adolescents with AGs.

      • Development and reproduction of a Japanese Tetranychus takafujii population and six foreign T. evansi populations

        Tetranychus evansi Baker & Pritchard occurs on solanaceous plants, and causes serious damage to various crops in Africa and Europe. Tetranychus takafujii Ehara & Ohashi was found on Solanum nigrum L. in Japan in 2001 and described as a new species in 2002. However, because T. takafujii is very similar to T. evansi in morphology, doubt has arisen about the difference between these two species, although there is no report discussing their taxonomic status. Tetranychus takafujii is specialized on solanaceous plants and it is possible that this species gets out of hand after invasion of solanaceous agricultural crops. To evaluate its potential severity as a pest, it is necessary to assess its performance on a variety of food plants in a carefully controlled laboratory set-up. The present study determined the development on 21 crops and weeds, and the reproduction on Solanum nigrum of a Japanese T. takafujii population and six T. evansi populations from abroad. Four plants (potato, horsenettle, pear and cucumber) served as host plant for all seven populations and most larvae successfully reached adulthood. Life-history parameters, such as the rate of development and the intrinsic rate of natural increase (rm), became higher with temperature increasing from 15 to 35°C. The rm-value at 25°C was similar among all populations tested and it was close to the value obtained in T. takafujii [Kotsubo et al. (2004) J. Acarol. Soc. Jpn. 13: 71-76]. This indicates that T. takafujii has the potential to become a serious pest on solanaceous crops, just the same as T. evansi in Africa and Europe.

        Second-Look Arthroscopic Findings and Clinical Outcomes after Adipose-Derived Regenerative Cell Injection in Knee Osteoarthritis

        Background: To evaluate the clinical outcomes and second-look arthroscopic findings after intra-articular adipose-derived regenerative cell (ADRC) injection as treatment for knee osteoarthritis (OA). Methods: ADRCs were administered to 11 patients (19 knees; mean age, 61.7 years) with knee OA. Subcutaneous adipose tissue was harvested by liposuction from both thighs, and arthroscopic lavage was performed, followed by ADRC injection (mean dose, 1.40 × 107 cells) into the synovial fluid. Outcome measures included the Knee Injury and Osteoarthritis Outcome Score, Lysholm score, and visual analog scale score. Arthroscopic examinations were performed to assess the International Cartilage Repair Society cartilage injury grade preoperatively and overall repair postoperatively. Noninvasive assessments were performed at baseline and at 1-, 3-, and 6-month follow-ups; arthroscopic assessments were performed at baseline and at 6 months. Results: All outcome measures significantly improved after treatment. This improvement was evident 1 month after treatment and was sustained until the 6-month follow-up. Data from second-look arthroscopy showed better repair in low-grade cartilage lesions than in lesions with a greater degree of damage. No patients demonstrated worsening of Kellgren-Lawrence grade, and none underwent total knee arthroplasty during this period. Conclusions: Clinical outcomes were improved in patients with knee OA after ADRC administration. Cartilage regeneration was more effective in smaller damaged lesions than in bigger lesions.

      • Effect of the relative rotation axis position of the stretching machine and ankle

        At the onset of a stroke, paralysis of the lower leg typically causes a foot deformity called the foot drop. Stretching is an effective technique for physical therapy. An automated stretching machine provides the option to incorporate long-term stretching at home. We have remodeled a commercial foot exerciser to function as a stretching machine for ankle dorsiflexion [1]. It is believed that the rotation axis of such machines should be aligned with the biomechanical axis of the human body, which is the design principle our prototype is based on. However, there have been no studies that have investigated the best position of the axis required to achieve effective stretching. In this study, we evaluated several positions of the machine’s rotation axis in the sagittal plane with respect to the stretching effect and physical burden (safety). The force applied on the foot and ankle dorsiflexion angle were measured during the stretching experiments. We computed the work involved in ankle dorsiflexion and the force not contributing to the dorsiflexion movement, which served as indicators for the stretching effect and potential physical burden, respectively. It was found that the best position of the machine’s rotation axis can be above the ankle.

      • An enhanced incompressible SPH method for simulation of fluid flow interactions with saturated/unsaturated porous media of variable porosity

        A refined projection-based purely Lagrangian meshfree method is presented towards reliable numerical analysis of fluid flow interactions with saturated/unsaturated porous media of uniform/spatially-varying porosities. The governing equations are reformulated on the basis of two-phase mixture theory with incorporation of volume fraction. These principal equations of mixture are discretized in the context of Incompressible SPH (Smoothed Particle Hydrodynamics) method. Associated with the consideration of governing equations of mixture, a new term arises in the source term of PPE (Poisson Pressure Equation), resulting in modified source term. The linear and nonlinear force terms are included in momentum equation to represent the resistance from porous media. Volume increase of fluid particles are taken into consideration on account of the presence of porous media, and hence multi-resolution ISPH framework is also incorporated. The stability and accuracy of the proposed method are thoroughly examined by reproducing several numerical examples including the interactions between fluid flow and saturated/unsaturated porous media of uniform/spatially-varying porosities. The method shows continuous pressure field, smooth variations of particle volumes and regular distributions of particles at the interface between fluid and porous media.
