REVIEW : Current concepts in the management of rheumatoid arthritis

Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized by in-flammation and joint destruction that causes significant morbidity and mortality. However, the combined use of methotrexate, a synthetic disease-modifying antirheumatic drug (DMARD), and biologic DMARD has revolutionized treatment of RA. Clinical remission is now realistic targets, achieved by a large proportion of RA patients, and rapid and appropriate induction of remission by intensive treatment with biological DMARD and methotrexate is prerequisite to halt joint damage and functional disabilities. However, biological DMARD is limited to intravenous or subcutaneous uses and orally available small but strong molecules have been developed. Oral administration of tofacitinib targeting the Janus kinase (JAK) is significantly effective than placebo in active patients with methotrexatenaive, inadequately responsive to methotrexate or tumor necrosis factor (TNF)- inhibitors. The effcacy was rapid and as strong as adalimumab, a TNF-inhibitor. Meanwhile, association of tofacitinib on carcinogenicity and malignancy is under debate and further investigation on post-marketing survey would be warranted. On the other hand, discontinuation of a biological DMARD without disease flare is our next goal and desirable from the standpoint of risk reduction and cost effectiveness, especially for patients with clinical remission. Recent reports indicate that more than half of early RA patients could discontinue TNF-targeted biological DMARD without clinical flare and functional impairment after obtaining clinical remission. Contrarily, for established RA, fewer patients sustained remission after the discontinuation of biological DMARD and “deep remission” at the discontinuation was a key factor to keep the treatment holiday of biological DMARD.

The magnetization of a single-crystalline sample of TbZn2 was measured under high magneticfields up to 180 kOe in the temperature range from 4.2 to 90 K. The metamagnetic transitionswere observed in the directions of all axes at temperatures below TN and along the c-axis attemperatures above TN. The metamagnetic transition field HM along the c-axis at temperaturesabove TN increases with increasing temperature.
Autocorrelation Coefficient for Detecting the Frequency of Bio-Telemetry

A MATLAB program was developed to calculate the half-wavelength of a sine-curve baseband signal with white noise by using an autocorrelation function, a SG filter, and zero-crossing detection. The frequency of the input signal can be estimated from 1) the first zero-crossing (corresponding to ¼λ) and 2) the R value (the Y axis of the correlogram) at the center of the segment. Thereby, the frequency information of the preceding segment can be obtained. If the segment size were optimized, and a portion with a large zero-crossing dynamic range were obtained, the frequency discrimination ability would improve. Furthermore, if the values of the correlogram for each frequency prepared on the CPU side were prepared in a table, the volume of calculations can be reduced by 98%. As background, period detection by autocorrelation coefficients requires an integer multiple of 1/2λ (when using a sine wave as the object of the autocorrelation function), otherwise the correlogram drawn by R value will not exhibit orthogonality. Therefore, it has not been used in bio-telemetry where the frequencies move around.

Interleaved converters are a typical solution to enhance a current capacity and reduce current ripples of input or output ports. Conventional interleaved LLC converters need additional switches and feedback control loop, or some passive components, for active current balancing, increasing the circuit complexity and cost. This paper proposes a novel three-phase interleaved LLC converter with an automatic capacitive current balancing capability. Two flying capacitors contribute to not only the automatic capacitive current balancing but also reduced switch voltage stresses and asymmetric resonant operations to reduce RMS currents. The operation analysis and capacitive current balancing capability are discussed. The experimental verification using a 1-kW prototype demonstrated that the automatic capacitive current balancing could be achieved despite the significant mismatch in transformers’ parameters.

The authors are making a prototype flexible board of a radio-frequency transmitter for measuring an electromyogram (EMG) of a flying moth and plan to apply for an experimental station license from the Ministry of Internal Affairs and Communications of Japan in the summer of 2022. The goal is to create a continuous low-dose exposure standard that incorporates scientific and physiological functional assessments to replace the current standard based on lethal dose 50. This paper describes the technical evaluation of the hardware. The signal of a bipolar EMG electrode is amplified by an operational amplifier. This potential is added to a voltage-controlled crystal oscillator (27 MHz, bandwidth: 4 kHz), frequency-converted, and transmitted from an antenna about 10 cm long (diameter: 0.03 mm). The power source is a 1.55-V wristwatch battery that has a total weight of about 0.3 g (one dry battery and analog circuit) and an expected operating time of 20 minutes. The output power is −7 dBm and the effective isotropic radiated power is −40 dBm. The signal is received by a dual-whip antenna (2.15 dBi) at a distance of about 100 m from the moth. The link margin of the communication circuit is above 30 dB within 100 m. The concepts of this hardware and the measurement data are presented in this paper. This will be the first biological data transmission from a moth with an official license. In future, this telemetry system will improve the detection of physiological abnormalities of moths.
Pressure Effect on the Phase Transition Temperatures of Ni2Mn1−xCuxGa

The martensitic transition temperature TM and the Curie temperature TC of the ferromagneticalloys Ni2Mn1−xCuxGa (0 ≤ x ≤ 0.40) were measured under pressure. We estimated the pressurederivatives dTM/dp and dTC/dp from the results. The values of dTM/dp and dTC/dp for Ni2MnGa(x = 0) were -4.5 K/GPa and +3.9 K/GPa, respectively. The values of dTM/dp increased fromnegative to positive with increasing x in Ni2Mn1−xCuxGa. The values of dTC/dp approached zerowith increasing x.
Competition between Magnetic Ordering and Random Spin Freezing in Dy2PtS3

We report the results for ac and dc susceptibility, magnetization, magnetic relaxation, specific heatand electrical resistivity measurements on a well annealed ternary intermetallic compound Dy2PtSi3. An unusual magnetic phase transition with an apparent ferromagnetic nature was observed at TC= 5.6 K. At temperatures below TC, however, Dy2PtSi3 showed a metastable magnetic behaviorcharacteristic of a typical cluster glass. The classification of Dy2PtSi3 as a large magnetic cluster(extended short-range ferromagnetic ordered) system with competition between magnetic orderingand random spin freezing seems appropriate.
