        혈청이 마우스 간 세포주 BNL CL.2의 Nitric Oxide 생성에 미치는 영향

        마우스 간 세포주인 BNL CL.2의 시험관내 배양에서 혈청과 IFN-γ가 세포주의 nitric oxide(NO) 생성과 세포 손상에 미치는 영향을 알아보기 위한 실험을 하였다. 혈청이 공급된 배양에서 IFN-γ에 의한 세포 생존율은 거의 변동이 없었으나, 혈청을 제거한 배양에서는 약 65%의 생존율이 유지되었으며, NO 생성 억제제인 N -monomethy-L-arginine (NMA)의 첨가는 농도 의존적으로 세포의 생존율을 감소시켰다. 혈청이 제거된 BNL CL.2세포주는 IFN-γ단독 처리에서도 NO 를 생성할 수 있었으며, IFN-γ와 lipopolysaccharide (LPS)의 복합 처리는 세포주의 NO 생성을 상승적으로 증가시켰다. 또한 protein tyrosine kinase (PTK) inhibitor인 herbimycin A와 genistein에 의해서 NO 생성이 억제되어 PTK의 활성이 혈청이 고갈된 BNL CL.2 세포에서 NO의 생성에 중요한 역할을 담당하고 있기 때문으로 판단된다. IFN-γ의 독성은 혈청을 제거시킬 때 NO 생성 억제제에 상승적으로 간 세포를 손상시키며, 이때 NO가 IFN-γ에 의해 유도된 손상을 어느 정도 억제시키는 것을 알 수 있었다. Nitric oxide (NO) plays an important role in immunologic defense, and influences upon the functioning of secretory tissues and cells. It also exhibits cytotoxic/cytostatic activity as one of major operating effectors of the cellular immunity system. We investigated the effects of serum on the cell damages and NO production in the mouse liver cell line BNL CL.2 to establish the role of NO. We observed that, when BNL CL.2 cells were cultured in serum-free medium, they were induced to cell damage by the stimulation of IFN-γalone or IFN-γplus LPS. Serumstarved cells showed large amount of nitrite accumulation and NO synthase (NOS) expression in response to IFN-γalone in dose-and time-dependent manners, but serum-supplied cells did not. The production of NO was blocked by protein tyrosine kinase (PTK) inhibitors, genistein and herbimycin. These results suggest that the deprivation of serum in the BNL CL.2 cell culture medium might primed with the cells to produce NO when the cells are triggered by IFN-γ and the involvement of PTK signal transduction pathway in the expression of NOS gene in murine hepatocytes.

        조혈모세포이식 환자에서 침습성 진균 감염에 대한 Micafungin의 예방 효과 및 안전성

        Background: Micafungin, a potent inhibitor of 1,3-β-D-glucan synthase, is a novel antifungal agent of the echinocandin class. In vitro study showed that micafungin was effective against Aspergillus species as well as Candida species, but clinical data on the prophylactic efficacy against invasive fungal infections (IFIs) other than candidiasis are still lacking. Materials and Methods: We identified 60 consecutive adult hematopoietic stem cell transplantation (HSCT) recipients who received at least 3 doses of micafungin during neutropenic period. Micafungin was started as an alternative in patients who were intolerant or had adverse events (AEs) to primary prophylactic antifungal agents. We retrospectively reviewed the medical records and analyzed the efficacy and safety of micafungin for prophylaxis against IFIs. Results: The patients either had autologous (n=9) or allogeneic (n=51: 1 syngeneic, 24 sibling, 26 unrelated donor) HSCT. Itraconazole oral solution (n=58) was the most frequently used first line antifungal agent for prophylaxis and was administered for median 11 days. The most frequent cause of switch to micafungin was vomiting (n=42). The duration of neutropenia and micafungin administration was median 13 and 12 days, respectively. A successful outcome was achieved in 45 (75%) patients. Empirical antifungal therapy was initiated in 13 (22%) patients. There were 2 cases (3.3%) of breakthrough fungal infections which comprised a probable invasive pulmonary aspergillosis and a possible invasive fungal sinusitis. There was no case of invasive candidiasis. A total of 53 (88%) patients experienced at least one AE regardless of causality during micafungin administration. The most frequent AEs were hypokalemia, vomiting, diarrhea, and elevated serum aspartate aminotransferase or alanine aminotransferase. Among the aforementioned AEs, only 1 case of diarrhea could be classified as a probable relation with micafungin when causality was assessed. There was no AEs that caused discontinuation of micafungin. Conclusions: Micafungin seems to be a safe and effective agent for prophylaxis of IFIs including aspergillosis as well as candidiasis in HSCT recipients. However, further large, prospective, and randomized comparative studies are warranted for aspergillosis.

        Recent reports have proposed that Ephedrae Herba may modulate the immune response on allergy or asthma. Human nasal mucosal fibroblasts are a rich source cytokines, inflammatory mediators, and chemokines. Chemokines are important for the recruitment of leukocytes to sites of infection, which is essential in host defense. Objectives : The objective of this study was to investigate the effect of Ephedrae Herba on the release of the chemokines such as monocyte chemotactic protein(MCP)-1, MCP-2, and MCP-3 in human nasal mucosal fibroblasts after stimulation with cytokines like tumor necrosis factor -α(TNF-α), interferon-γ(IFN-γ), and interleukin -1β(IL-1β).

        We have encountered a rare case of symmetrical peripheral gangrene complicating Escherichia coli sepsis in a 47-years-old male. He was successfully treated with antibiotics, anticoagulants, and vasodilator. To our knowledge, this is the first report on symmetrical peripheral gangrene complicating E. coli sepsis in Korea.

        Primary ciliary dyskinesia is a genetic disorder of abnormal ciliary structure and function that leads to defective mucociliary clearance, resulting in sinopulmonary infection and infertility. Kartagener's syndrome is a subclass of primary ciliary dyskinesia and which is clinically distinguishable by the presence of situs inversus. Electron microscopy of cilia reveals ultrastructural defects in the ciliary axoneme. In the respiratory tract, this inability presumably causes impaired clearance of mucus and inhaled particles and results in the chronic infections of sinuses and bronchial trees that are characterized of the disease. Early diagnosis of the disease may be important for prevention of bronchiectasis and pulmonary insufficiency. We report one case of primary ciliary dyskinesia associated with Kartagener's syndrome in a 14 year-old young man who was presented as bronchiectasis, chronic and situs inversus, with review of literature.

        조혈모세포이식 환자에서 발생한 Cytomegalovirus 질환의 특징 : 일개 대학변원에서 최근 10년간의 경험

        Background : Studies on cytomegalovirus (CMV) diseases in Korean hematopoietic stem cell transplant (HSCT) recipients are lacking and do not reflect the recent trends of advances and changes. Therefore, we tried to analyze the clinical features of CMV diseases in HSCT recipients over the past 10 years at a tertiary university hospital in Korea. Methods : Retrospective review of medical records was done for all adult HSCT patients who received transplant at the Catholic HSCT Center from January 1998 to January 2008. Results : Forty-four cases (2.2%) of CMV diseases were identified. CMV pneumonia was diagnosed in 17 patients, retinitis in 16 patients, enterocolitis in 7 patients, esophagitis 1 patient, gastritis in 1 patient, duodenitis in 1 patient, and hepatitis in 1 patient. The median onset of symptom was 90 days after transplantation. Late CMV diseases accounted for 47.7%. CMV related death varied from 0 to 58.8% according to the involved organ. CMV retinitis was diagnosed relatively later in the course of transplantation mostly in patients who had chronic graft versus host disease (GVHD). On the contrary, CMV enterocolitis mainly occurred in patients who suffered from acute GVHD. The overall concurrent CMV reactivation was documented to be 63.6%: the concurrent CMV reactivation was observed only in 37.5% of patients with retinitis. Conclusions : We observed some differences in the pattern of CMV disease manifestation according to the involved organ and reconfirmed the fact that CMV pneumonia is the most common and fatal disease in HSCT recipients. Additionally, CMV retinitis was not uncommon in HSCT recipients. Since specific marker does not exist in predicting retinitis, regular ocular examination should be done thoroughly, especially in patients with chronic GVHD.

        이 연구는 중등학교 교실 역사수업에서 어떠한 수업목적과 수업목표들이 설정되고 있는지 분석하고자 했다. 이를 위해 현재까지 역사교육 연구들에서 논의되어 온 역사수업의 목적들과 목표들에 대한 연구 성과를 검토하여 분석 기준을 마련했다. 그리고 이러한 기준에 의하여 역사수업 사례들에 나타난 수업의 목적들과 목표들에 대한 분석을 시도했다. 여기에서 분석 대상이 된 것은 전국역사교사모임에서 정기적으로 발행하는 『역사교육』지에 소개된 최근 10여 년간의 역사수업 사례들이다. 이 사례들이 실린 『역사교육』지는 전국의 역사교시들이 다른 어떤 단체보다도 가장 많은 회원으로 가입하고 있는 곳에서 발간한 정기 간행물이다. 따라서 학교현장 역사수업의 흐름과 분위기를 파악하기에 매우 유용한 자료라 할 수 있다. 분석 결과, 현재의 역사수업 사례들에서는 그동안 논의되어 온 다양한 역사수업의 목적과 목표들이 고르게 제시되지 못하고 있음을 볼 수 있었다. 즉 특정한 수업목적과 목표에 집중하고 있는 경향을 나타냈다. 또한 현재의 수업 목적과 목표 설정은 학생들의 활동 보다는 수업 내용의 성격이나 주제에 크게 의존하고 있음을 볼 수 있었다. 따라서 수업의 내용뿐만이 아니라 수업의 형태나 학생들의 활동 방식을 바꾸는 수업을 운영한다면 다양한 수업 목적과 목표들을 적용할 수 있을 것으로 본다. This article attempts to investigate into that hidden meaning the goal and object of history learning by analyzing the goal and object of learning that appeared in history classes. The research about the goal and object of history learning are very inactive in history education comparatively. Thus The goal and object of the history education have applied the general goal of education. And those have not reflected the characteristic of subject-based history education. We try to grasp that how determined the goal and object of learning in history classes and offer the foundation of research for goal and object of learning in history education. It was reviewed the history teaching cases that is in the 『History Education』 which published by history teachers for this research. We could find these significance and limitations in this research. (1) The most of history class have a goal and object that was focusing a knowledge acquisition and understanding. (2) The main object of history learning was in understanding of facts and concepts of history. (3) A lesson theme was an important factor which determined a goal and object of history learning. (4) A goal and object of history learning was also altered by teaching and learning methods. These result have such meaning. In spite of same lesson theme, the goal and object of history learning was changed by methods in history teaching and learning. therefore, the research of methods in history teaching and learning should active various classroom activity which can apply to same lesson theme for utilizing in history practice.

      • 엘리트 알파인 스키 선수와 대학서클 알파인 선수의 등속성 근력 발현 특성에 대한 비교 연구

        The purpose of this study was to compare isokinetic muscle strength between elite players and college players on alpine skiing and to offer foundamental information on characteristics of thigh muscle strength for elite players and effective training method for college club players The subjects of this study were 10 national ski players and 10 college club players. Also, the experimental instruments used were Martin's anthropometer(PKS-100, Japan), weight meter(TBF-202, Japan), and isokinetic equipment(CYBEX NORM, USA). To analyze the data, the statistical method used in this study were mean, standard deviation, and t-test using SPSS for Windows(Version 10.0.7). Based on aforementioned research method, the results of this study are as follows. 1. In peak torque, elite group was higher than college club group. Especially, right extension(p〈.05) and right flexion(p〈.01) were significant high in 240˚/sec. 2. In peak torque % of body weight, elite group was significant high on riggtㆍleft extension and flexion in 60˚/sec & 240˚/sec. 3. In angle of peak torque, elite group was higher than college club group. Especially, left extension(p〈.01) was significant high in 60˚/sec. 4. In average power, elite group was higher than college club group. Especially, right extension(p〈.05) and right flexion(p〈.01) were significant high in 240˚/sec. 5. In total work, elite group was significant high on rightㆍleft extension and flexion. However, on the muscle endurance ratio, there was no significant difference. 6. In peak torque for flexion/extension, there was no significant difference between elite group and college club group.
