Objectives: Healthcare organizations that maintain and process Electronic Medical Records are at risk of cyber-attacks,which can lead to breaches of confidentiality, financial harm, and possible interference with medical care. State-of-the-artmethods in cryptography have the potential to offer improved security of medical records; nonetheless, healthcare providersmay be reluctant to adopt and implement them. The objectives of this study were to assess current data management andsecurity procedures; to identify attitudes, knowledge, perceived norms, and self-efficacy regarding the adoption of advancedcryptographic techniques; and to offer guidelines that could help policy-makers and data security professionals work togetherto ensure that patient data are both secure and accessible. Methods: We conducted 12 in-depth semi-structured interviewswith managers and individuals in key cybersecurity positions within Israeli healthcare organizations. The interviews assessedperceptions of the feasibility and benefits of adopting advanced cryptographic techniques for enhancing data security. Qualitativedata analysis was performed using thematic network mapping. Results: Key data security personnel did not perceiveadvanced cybersecurity technologies to be a high priority for funding or adoption within their organizations. We identifiedthree major barriers to the adoption of advanced cryptographic technologies for information security: barriers associatedwith regulators; barriers associated with healthcare providers; and barriers associated with the vendors that develop cybersecuritysystems. Conclusions: We suggest guidelines that may enhance patient data security within the healthcare system andreduce the risk of future data breaches by facilitating cross-sectoral collaboration within the healthcare ecosystem.
USN applied to Smart Cold Chain based on the mesh wireless sensor network

USN, or wireless sensor network, is applied for the real time monitoring for the Supply Chain Industry, starting from the Cold Chain Monitoring (CCM) needs, such as shipments of Fruits & Vegetables, Meat and Medicines. There are a number of different terms related or used in conjunction with the WSN. These are ISM, RFID, GPS, CCM, DASH7 Alliance, ISO18000-7, ZigBee Alliance, IEEE 802.15.4, ZWave WPAN, Wi-Fi, Bluetooth and many, many more. The subject of the WSN is quite a clear, intuitive and simple concept. It goes to the tracing and monitoring of what is needed and possible ?devices, things, living, plants, environment, etc. CartaSense developed the system that enables the customer to take decisions based on: ? Affordable sensors to track and collect ongoing transportation data such as temperature, Humidity and pressure conditions ? Reliable communication with a gateway even in humid and harsh conditions (where regular RF would hardly work) ? Proprietary software to analyze the data and prediction of expected expiration or ripe dates. Major issues and problems in this application which can be accessed by the management science and industrial engineering are analyzed and discussed.

Neutrons elastically scattered from 238U were measured in the neutron energy region from 5 to 120 eV by using a Ta target with the 60-MV electron linear accelerator (LINAC) of RPI (Rensselaer Polytechnic Institute). The neutron energy, having a continuous spectrum, was measured by using the neutron time-of-flight (TOF) method and a 6Li scintillation detector. Two different thickness depleted uranium samples, 169 and 362 g of 7.62 × 7.62 cm2, were used in the experiments. The spectra of scattered neutrons were measured at 25.5 m from the U sample by using a 6Li detector, and the scattering direction was ∼39˚ forward from the direction of the incident neutrons. The peaks and the dips around resonance energy of the measured spectrum were identified and compared to the ENDF/B-VII.0 cross-section of 238U for the same neutron energy. The experimental 36.68- eV resonance was compared with the calculation results. The experimental and the calculation results can be used as reference data for benchmarking neutron scattering models in simulation codes. Columellar reconstruction: a refinement of technique

The nose is an important landmark of the face and its shape and beauty is of significant concern. The columella is the subunit between the two nostrils that provides support and projection to the nasal tip and has functional role in nostrils, as well as aesthetic. Ethiology for columellar absence or deficiency is diverse, and it is one of the most complex nasal subunits to reconstruct because of its narrow horizontal dimension, its tenuous vascularity and limited availability of adjacent tissue. We present a patient with columellar, membranous septum and upper lip defect, due to oncological resection. The lip reconstruction was designed using advancement of two upper lip edges with the technique of webster perialar/nasocheek advancement. However, the perialar/nasocheek tissue which is usually discarded was used as inferiorly based skin flaps to reconstruct the membranous septum, columellar skin and nasal vestibule lining. Rib cage cartilage graft was used as columellar strut for support. At 1-year followup, the patient has good nasal contour and projection. Scaring of the columella is very subtle. This is a versatile way for successful reconstruction of a columella and large central facial defect in one-stage operation. It is a method which provides very satisfactory aesthetic result with minimum patient morbidity and discomfort.

Background/Aims Refractory gastroesophageal reflux disease (GERD) is very common, affecting up to 40% of the patients receiving proton pump inhibitor (PPI) therapy. However, there is not much information about the clinical characteristics of these patients. The aim of the study is to compare the clinical characteristics of PPI responders vs non-responders. Methods Consecutive GERD patients receiving PPI once or twice daily were evaluated by a questionnaire and a personal interview regarding their demographics, habits, clinical characteristics and endoscopic findings. The patients were divided into 3 groups: Patients who fully responded to PPI once daily (Group A, n = 111), patients who failed PPI once daily (Group B, n = 78) and patients who failed PPI twice daily (Group C, n = 56). Results A total of 245 patients (59.3% females, 52 ± 17.2 years of age) were included in this study. Cross-group differences (A vs B vs C) were detected for hiatal hernia (33% vs 51% vs 52%, P = 0.011); erosive esophagitis (19% vs 51% vs 30%, P < 0.0001); cough (24% vs 44% vs 43%, P = 0.007); sleep disturbances (19% vs 30% vs 38%, P = 0.033); chest symptoms (21% vs 35% vs 41%, P = 0.010); Helicobacter pylori status (25% vs 33% vs 48%, P < 0.0001), disease duration (1.6 ± 0.8 vs 1.9 ± 1.0 vs 2.0 ± 1.1 years, P = 0.007), performed lifestyle interventions (68.5% vs 46.7% vs 69.6%, P = 0.043) and compliance (84% vs 55% vs 46%, P < 0.0001). Conclusions PPI failure (either once or twice daily) appears to be significantly associated with atypical GERD symptoms, disease duration and severity, H. pylori status, obesity, performed lifestyle interventions and compliance as compared with PPI responders. Effect of Linear Chirp on Strong Field Photodissociation of H_2^+

We report the experimental findings of a systematic study of the effect of the linear chirp on strong field photodissociation of H_2^+. For vibrational levels around or above the one photon crossing, the effect manifests in terms of a shift in the kinetic energy release (KER) peaks. The peaks shift up for negative chirp whereas they shift down for positive chirp. The measurements are carried out by varying two of the laser pulse characteristics energy, pulse intensity and linear chirp while keeping the third constant. The shifts in the KER peaks are found to be intensity dependent for a given value of chirp. However, in the last two cases, they are found to be independent of the chirp magnitude. The results are understood on the basis of saturation of photodissociation probabilities for these levels.
