        Interactive Effects of Serum Leptin Levels and Physical Comorbidity on the Pharmacotherapeutic Response of Depressive Disorders

        Objective: To investigate individual and interactive associations of baseline serum leptin levels and physical comorbidity with short- and long-term treatment outcomes in outpatients with depressive disorders who received stepwise antidepressant treatment in a naturalistic prospective study design. Methods: Baseline serum leptin levels were measured, and the number of concurrent physical disorders ascertained from 1,094 patients. These patients received initial antidepressant monotherapy; then, for patients with an insufficient response or who experienced uncomfortable side effects, treatment was administered using alternative strategies every 3 weeks in the acute treatment phase (at 3, 6, 9, and 12 weeks) and every 3 months in the continuation treatment phase (at 6, 9, and 12 months). Then, 12-week and 12-month remission, defined as a Hamilton Depression Rating Scale score of ≤7, was estimated. Results: In multivariable logistic regression analyses, individual effects were found only between higher baseline serum leptin levels and 12-week non-remission. Significant interactive effects between higher leptin levels and fewer physical disorders (< 2 physical disorders) on 12-week non-remission were observed. However, neither individual nor interactive effects between leptin levels and physical comorbidity were associated with 12-month remission. Conclusion: The combination of serum leptin level and number of physical disorders may be a useful predictor of short-term treatment responses in patients with depressive disorders receiving pharmacotherapy.

        Three-Dimensional Myocardial Strain for the Prediction of Clinical Events in Patients With ST-Segment Elevation Myocardial Infarction

        BACKGROUND: Two-dimensional (2D) strain provides more predictive power than ejection fraction (EF) in patients with ST-elevation myocardial infarction (STEMI). 3D strain and EF are also expected to have better clinical usefulness and overcome several inherent limitations of 2D strain. We aimed to clarify the prognostic significance of 3D strain analysis in patients with STEMI. METHODS: Patients who underwent successful revascularization for STEMI were retrospectively recruited. In addition to conventional parameters, 3D EF, global longitudinal strain (GLS), global area strain (GAS), as well as 2D GLS were obtained. We constructed a composite outcome consisting of all-cause death or re-hospitalization for acute heart failure or ventricular arrhythmia. RESULTS: Of 632 STEMI patients, 545 patients (86.2%) had a reliable 3D strain analysis. During median follow-up of 49.5 months, 55 (10.1%) patients experienced the adverse outcome. Left ventricle EF, 2D GLS, 3D EF, 3D GLS, and 3D GAS were significantly associated with poor outcomes. (all, p < 0.001) The maximum likelihood-ratio test was performed to evaluate the additional prognostic value of 2D GLS or 3D GLS over the prognostic model consisting of clinical characteristics and EF, and the likelihood ratio was 15.9 for 2D GLS (p < 0.001) and 1.49 for 3D GLS (p = 0.22). CONCLUSIONS: The predictive power of 3D strain was slightly lower than the 2D strain. Although we can obtain 3D strains, volume, and EF simultaneously in same cycle, the clinical implications of 3D strains in STEMI need to be investigated further.

        Associations of Serum Serotonin Levels with 12-week and 12-month Remission in Patients with Depressive Disorders

        Objective: To investigate associations between baseline serum serotonin levels and short- and long-term treatment outcomes in outpatients with depressive disorders in a naturalistic one-year prospective study design. Methods: Patients were recruited at a University hospital in South Korea from March 2012 to April 2017. At baseline, blood samples were obtained from 1,094 patients who received initial antidepressant monotherapy (Step 1). After the Step 1 treatment, further treatment steps (at least Steps 2−4) could be administered every 3 weeks during the acute treatment phase (3, 6, 9, and 12 weeks; n = 1,086), and every 3 months during the continuation treatment phase (6, 9, and 12 months; n = 884). In cases showing an insufficient response or intolerable side effects, patients were asked to choose whether to remain at the current step or enter the next treatment step, with alternative strategies including switching, augmentation, combination, and a mixture of these approaches. Remission was defined as a Hamilton Depression Rating Scale score of ≤ 7. Results: The remission group had significantly higher baseline serum serotonin levels among patients who received Step 1 monotherapy in both acute and continuation treatment phases. These associations remained significant after adjustment for relevant covariates. No associations were found with any other treatment steps. Conclusion: Baseline serum serotonin levels may be used as a biomarker for predicting short- and long-term treatment outcomes in antidepressant monotherapy-treated patients with depressive disorders in a real-world clinical setting.

        Cultural Fusion of East and West in Nineteenth-Century Korea: Introduction to and Translation of Ch’oe Han- ki’s Prefaces to the Study of Configurative Energy and Effecting Socio-Political Order among Humans

        이 논문은 19세기 조선의 대표적인 철학자인 최한기의 후기 대표저작인 氣學과 人政 서문에 대한 역사적 철학적 설명을 시도하고 서문 내용을 영어로 번역한 것이다. 최한기는 19세기 서구와 청대 학자들의 서구에 대한 서적을 방대하게 섭렵하고 그 개념을 흡수하여 자신의 유학적 전통의 맥락에서 융합하려 노력한 철학자이다. 이런 관점에서 아직 영문으로 번역된 적 없는 그의 철학 핵심의 영문번역은 19세기의 동서 문화의 교섭과 융합의 한 형태를 널리 알리고 연구하는데 크게 공헌 하리라 기대한다. 최한기는 19세기 동시대인 이규경, 김정호, 이건창의 기록에 의해 도서수집가로서, 또 독특한 철학을 지닌 학자로서 행적이 기록되어 있다. 동시에 그는 수많은 책을 저술했다고 알려져 오고 있으나 현재는 약 100권 정도의 저작이 발견되어 있다. 그는 조선 중, 후반기를 특징짓는 각종 논쟁에서 벗어나 있으면서 서구문명의 새로운 변화를 감지하려고 노력하면서 동시에 유교적 사유 형태로 이를 융합하고자 했다. 번역의 대상이 된 기학과 인정의 서문은 그의 후기 철학을 선명하게 보여주는 짧지만 매우 정련된 텍스트이다. 그의 초기의 저작 氣測體義에서 나타난 경험, 추측 등의 개념들을 계속 사용하면서 이를 새로운 학문체계와 사회정치 철학에 적용하여 범위를 확장시키는 성격을 지닌다. 經驗, 一統, 人政 등의 개념은 유교철학전통에서 익숙한 용어들이었으나 그는 이런 전통적 개념들을 새로운 맥락에서 사용한다는 점에서 전통적 유교철학과 차이가 있다. 동시에 그는 서구의 개념을 포괄하면서 유교적 맥락의 개념들을 사용함으로서 전통과의 단절보다는 變通의 입장에 선다. 저자는 그의 전통의 맥락에 서있으면서도 시대적 요구의 부응하고자 하는 융합철학의 성격을, 번역과정에서 영어 대응어를 선택하면서 세심하게 고려하였다. 말미의 용어해설 부분에서 번역어의 선택의 근거와 주요 개념어의 설명을 통해 서두의 역사적 철학적 맥락을 읽은 독자들이 그의 후기 철학을 입체적으로 이해할 수 있도록 시도하였다. This paper attempts to explicate and translate the prefaces to the Study of Configurative Energy and Effecting Socio-Political Order among Humans by Ch’oe Han-ki, a nineteenth-century Korean Confucian scholar. Ch’oe read widely from Western and Chinese books and documents and endeavored to create a new system of Confucian philosophy reflecting the fusion of East and West. From this standpoint, the first English-language translation of his two prefaces-central and important parts of his late philosophical works-are expected to contribute to the study of his philosophy as well as to the comparative study of philosophy. According to extant accounts by his contemporaries-men like Kim Chŏng-ho, Yi Kyu-kyŏng, and Yi Kŏn-chang-Ch oe Han-ki was a bibliophile and unique philosopher. He committed himself to extensively reading and authoring books whose topics extended from philosophical treatises, techniques of human assessment, and agricultural technology to Western sciences and civilization. Today, merely 100 fascicles survive of his opus that putatively surpassed a thousand works. Ch oe never took up a governmental position.

        We present a robust structural control system design method for a set of the critical excitations. The control system design problem is formulated as a generalized semi-infinite min-max problem and solved by an adaptive approximation algorithm. The critical excitations, which generally account for inherent uncertainties in predicting forthcoming events such as earthquake and wind, are modeled as a parameterized input filter. Decentralized static output feedback is used for fixed structure controller design. A simple numerical example demonstrates the viability of the proposed method.

        Study on Design Education Plan Using Microcontroller Board Prototyping Tool

        Unlike in the past, where the expression of the form was given a priority, with the widening of the scopes of the designs, the proportion of design education institution curricula targeting user experience and the application of technology is continually and gradually increasing. Open source microcontroller boards such as Arduino have initiated attracting attention as a countermeasure against these changes. These prototyping tools have many advantages for the diversification of expression and design verification in the design field and therefore have a high likelihood of being introduced into many design education institutes; however, the tools act as high entry barriers for design students who lack engineering knowledge. Although various educational content and tools have been developed to address the issue of barrier, existing solutions remain insufficient as alternatives for the purpose of activation. In this study, we investigated the present state of related education content and conducted a pilot workshop using a prototype microcontroller board with simplified coding. We intend to use the results of this investigation to develop study material for design education. We started by conducting a survey regarding the pre-university education situation. It was observed that engineering education opportunities are insufficient and the problem of mutual application between educations due to course-based education was not realized. We also analyzed the characteristics of simplified training tools using the microcontroller to establish a direction for educational design and conducted a pilot workshop using the microcontroller toolkit with a simplified coding process based on this content. Students who lack a basic knowledge of engineering technology received instruction, and after completing minimum preliminary training, they proceeded to practical exercises that involved utilizing the toolkit. Through this process, we identified the need for a simple-type microcontroller board with low-complexity for use in educating students majoring in design. We also identified some obstacles that serve as barriers to entry of utilizing microcontroller board. Based on these results, we propose several functional requirements and teaching guidelines for prototyping toolkits for design education.

        A Prototyping Tool of Free-Coding-Type Microcontroller Board for Design Education

        As the scope of social expectations and roles in the design field has expanded, the demand for education to cope with changes in the technology environment is increasing in design education. In response to this trend, microcontroller board-type design-prototyping tools have also been introduced into design education, and much educational content is being developed. However, there is the perception that students who are majoring in design without engineering knowledge are still barred from entry. A variety of educational content and tools have been developed to solve these difficulties, although there are several limitations to their practical application. Especially, in the design education courses in universities, the functional expectation level for prototyping is high, but most of the content developed for solving the difficulties has been developed for the lower education levels, and it could be said that a great deal of learning is necessary to solve the problem. In this study, students were asked about microcontroller board utilization and their satisfaction with their design through questionnaires and with the developed microcontroller board development direction via Focus Group Interviews. Based on this, we tested microcontroller boards that eliminate the coding process and which students can use to create and prototype their work as a suggestion to fulfil demand. After using the board, both the usability and improvement of the product were checked. Confirmation of the usefulness of the freecoding- type microcontroller was obtained through this study along with the possibility of responding to various educational demands by applying the application design related to this product.

        A Preliminary Study of the Application of Prototyping Tools for Design Education Plans; Focusing on Open Source Microcontroller Boards

        In the past, design has been recognized as a means to determine colors, shapes, and functions Recently, however, it has been accepted in a wider sense, namely designing the entire service, including product and user experience design. In this way, in addition to creativity and expressive power that designers have as their main competence, according to the expansion of this design object, designers are required to have comprehensive abilities in related fields and accompanying technical professional abilities. If designing as a technology application as a direction toward this situation, design education can be carried out by approaching technology as a method of expression or design subject. It can also be an effective alternative towards improving the understanding of technology. Meanwhile, many small microcontroller board products with advanced functions and multi-functional specialized programming integrated development environments (IDEs) are becoming widespread due to their open source, low cost, and scalable features. However, students in the design department who lack the basic knowledge of science and engineering have difficulties learning, which requires considerable time and is required for practical use. From this point of view, we have made advancements in the technical understanding of design education by conducting fundamental research to the effectiveness of microcontrollerbased prototyping tools as means of expression. We also conducted basic education of microcontroller boards for a certain period of time on students who majored in design in conjunction with the basic survey and investigated the obstacle factors using a questionnaire. Through these surveys, we have confirmed the necessity of designing microcontroller boards of low difficulty, which simplify the coding process that can act as a barrier in difficulty for design students to apply smoothly in design education. In addition, we intend to carry out a basic study on the guideline of microcontroller design for design education and composition of education programs.
