        소아에서 발생한 상지의ㅏ 재골절

        목적: 소아에서 발생하는 상지의 재골절과 관련된 임상적, 해부학적 요인 및 이를 예방할 수 있는 방법을 알아보고자 하였다. 대상 및 방법: 쇄골과 수부를 제외한 상지의 재골절로 내원하였던 만 15세 이하 소아 환자 중 전신적 골질환이 있었던 경우 및 처음 골절부위와 다른 부위에 골절이 발생한 경우를 제외한 18례를 대상으로 하였으며, 처음 골절이 발생한 부위에 따라 3개의 군 (상완골 과상부 골절, 상완골 외과 골절, 전완골 골절)으로 나누어 후향적 분석을 시행하였다. 이들은 다시 재골절의 유형에 따라 미성숙한 가골에서 발생한 조기 재골절과 골의 재형성 후 발생한 후기 재골절로 분류하였으며 골절양상과 동반된 기존 변형 등이 조사되었다. 결과: 상완골 과상부 골절의 경우 9례 중 2례는 과상부에서, 7례는 외과에서 재골절이 발생하였으며 이 중 6례에서 내반변형이 동반되어 있었다. 상완골 외과 골절의 경우 3례 중 1례는 과상부에서, 2례는 외과에서 재골절이 발생하였으며 이 중 1례에서 내반변형이 동반되어 있었다. 이러한 상완골 원위부 재골절의 경우 대부분 골유합이 완전히 이루어진 후 발생한 후기 재골절이었던 반면, 전완골 골절의 경우 총 6례의 재골절 중 4례가 골의 재형이 완전히 이루어지기 전에 발생한 조기 재골절이었다. 또한 이 중 5례는 골간부에서 발생하였으며 재골절 발생 전 모두 경도의 전방 각형성이 선행하였다. 결론: 상완골 원위부 재골절의 경우 내반변형이 가장 중요한 유발인자로서 특히 외과에서 발생하는 경우가 빈번하였고, 전완골 재골절의 경우 전방 각형성이 잔존된 골간부에서 미성숙한 기골을 통한 조기 재골절이 가장 흔하엿다. 이러한 재골절을 방지하기 위해서는 상완골 원위부의 경우 내반변형의 발생 예방을, 전완부의 경우 각형성이 남지 않도록 정확한 정복과 더불어 충분한 기간동안의 고정이 필요할 것으로 사료되었다. Purpose: To investigate the etiologic factors related to refractures of the upper extremity in children. Materials and Methods: 18 refractures of the upper extremity were divided into three groups according to the location of initial fractures: Supracondyle fractures of the humerus, lateral condyle fracture of the humerus, and the forearm bone fractures. They were analyzed in terms of the type of refractures(early refracture occurring at the immature callus and late refracture occurring at the remodeled bone), fracture patterns, and the existence of underlying deformity. Results: Nine supracondyle fractures had refractures at the supracondyle (2 cases) and the lateral condyle (7 cases), in which underlying cubitus varus were present in 6 cases. Three lateral condyle fractures had refractures at the supracondyle(1 case) and the lateral condyle (2 cases), in which one case had underlying cubitus varus. All but one cases in the group of humerus fractures were late refractures and treated operatively except one. Of 6 refractures of forearm, 5 were early refractures and occurred within 9 weeks at the original site: 4 at the diaphysis of both bones of forearm and 1 at the diaphysis of ulna. All cases in the group of forearm fractures had volar angulation before the refracture, and treated conservatively except one. Conclusion: In the humerus, underlying cubitus varus was the most important predisposing factor to refractures and the lateral condyle fractures were common. In the forearm, volar angulation of the diaphysis were related to refractures, and complete and circular consolidation of the primary fracture of forearm was thought to be important to prevent refracture.

      • 유니사이클 로봇의 곡선경로 추종에 대한 퍼지규칙베이스 구축

        Our unicycle has simple mechanical structure. But unicycle's dynamic system is a very sensitive unstable nonlinear system. In this paper, a fuzzy inference control mechanism was established throughout an inquiry into human riding a unicycle, and we developed a fuzzy hybrid controller to control our unicycle robot. This proposed fuzzy controller os consisted with direct fuzzy logic controllers and fuzzy gain scheduled PD controller for attitude stability and wheel's velocity individuslly. Computer simulation results show that our fuzzy controllers have robust control performance to unstable nonlinear unicycle robot system.

        Our unicycle has simple mechanical structure.But unicycle's dynamical system is a very sensitive unstable nonlinear system. equations of motion for a simple unicycle robot were derived using Lagrage's method. In this paper, a human fuzzy influence control mechanism was established throught an inquiry into human riding a unicycle, and we developed a hybrid controller to control our unicycle robot. This proposed hybrid controller is consisted with a pure PD and two fuzzy gain scheduled controllers and a direct fuzzy logic controller. Computer simulation results show that our hybrid controller has robust control performnance to unstable nnonlinear unicycle robot system.

        This paper describes the application of dynamic programming(DP) method to solve the problem for optimal load dispatching of thermal power systems. A model system that is similar to the system of Korea electric power corporation is composed of 26 thermal generating units. In operating a model system, total generating fuel costs of two cases were analysied; one was the case of operating all units depending on the formula of equal incremental fuel costs, another was the case of the application of dynamic programming methods with proper starting-up and shutting down units. The computation results showed that the application of dynamic programming methods could reduce remarkably fuel costs(up to 3%) by using of practical data.

        ACOS5 is Required for Primexine Formation and Exine Pattern Formation During Microsporogenesis in Arabidopsis

        Pollen exine, mainly composed of sporopollenin,plays important roles during microspore development. It hasbeen reported that Acyl-CoA Synthetase5 (ACOS5) is requiredfor sporopollenin biosynthesis in Arabidopsis. Here we showthat ACOS5 is essential for primexine formation duringArabidopsis microspore development. Through genetic screen,we identified a point mutation of ACOS5 allele, acos5-2,showing abnormal microspore development. Its microsporeswere degenerated and aborted after released from the tetrads. Transmission electron microscopy showed that primexineformation was reduced in acos5-2 mutant as compared tothat of the wild-type. Consequently, sporopollenin wasaggregated and randomly deposited on the microspores. Insitu hybridization indicated that the key regulators of tapetumdevelopment, DYT1 and TDF1, are required for the expressionof ACOS5 in tapetum. Furthermore, the GUS reporter showedthat the 593-bp promoter sequence was sufficient for theexpression of ACOS5 in the anther. Our data provide evidencethat ACOS5 is required for primexine formation andsporopollenin deposition during microspore development.

      • Effect of Diallyl Trisulfide on Human Ovarian Cancer SKOV-3/DDP Cell Apoptosis

        Aim: To investigate the effects of diallyl trisulfide (DT) on apoptosis of cisplatin (DDP)-resistant human epithelial ovarian cancer SKOV-3 cells (SKOV-3/DDP), and the role of p53 upregulated modulator of apoptosis (PUMA). Methods: SKOV-3/DDP cells were randomly divided into control, DT, DPP and DPP+DT groups, which were treated with DT or combined DT and DDP. All cells were incubated for 48 h. and apoptosis rates were assessed by flow cytometry. mRNA and protein expression of PUMA, Bax and Bcl-2 was determined by RT-PCR and Western blot assays, respectively. Results: Compared with control group, the apoptosis rates of SKOV-3/DDP cells in DT groups were obviously increased, with dose-dependence (P < 0.05), the mRNA and protein expressions of PUMA, Bax also being up-regulated (P < 0.05), while those of Bcl-2 were down-regulated (P < 0.05). Compared with DT groups, the apoptosis rate in the DDP+DT group was significantly increased (P < 0.05). After knockdown of PUMA with specific siRNA, the apoptosis rate of SKOV-3/DDP cells was obviously decreased (P < 0.05). Conclusion: DT can promote the apoptosis of SKOV-3/DDP cells with PUMA playing a critical role.

        A facile enzymatic synthesis of partial glycerides in an up-flow column reactor

        A facile solvent-free synthesis of partial glycerol caprylates/caprates was studied using an enzyme packed bed reactor (EPBR). In the up-flow EPBR, without addition of molecular sieves or vacuum, the highest conversion of caprylic acid reached 90.2% in 4 h. As a comparison, a same reaction executed in an enzyme batch reactor (EBR) present the highest conversion of caprylic acid of 88% in 9 h with similar reaction conditions. In addition, the EPBR represented a good operating stability than that in EBR. The conversion decrement was only 6.5% for caprylic acid and 4.5% for capric acid after 30 runs (180 h),respectively.

        증례보고 : 전신마취하 제왕절개술 중 발생한 뇌실내 출혈

        An intracranial hemorrhage is a fatal complication associated with general anesthesia. It can occur in patients with an intracranial aneurysm, hypertension, cerebral vascular malformation, and blood dyscrasia, etc. A sudden hemodynamic change during intubation and extubation in general anesthesia is dangerous, particularly in these patients. We encountered an intraventricular hemorrhage in a 42 year old pregnant woman after a cesarean section. The patient was induced with 300 mg of thiopental and 45 mg of rocuronium. The anesthesia was maintained with N2O/O2 and sevoflurane after endotracheal intubation. Five hours after surgery, the patient had a seizure at the ward. A MRI and CT scan of the brain showed a right intraventricular and basal ganglia hematoma. The CT 3-D brain angiography showed an unruptured small aneurysm on the right posterior communicating artery. (Korean J Anesthesiol 2006; 51: 257~60)
