        Nondaily dosing schedule of allergen-specific sublingual immunotherapy: efficacy and safety

        Purpose: Sublingual immunotherapy is currently promoted by various companies, with administration schedules variable in the different products even though almost all are standardized immunologically. So, this study was planned to examine the efficacy of simple nondaily dosing of sublingual immunotherapy instead of the widely used daily schedule. Materials and Methods: Fifty-two patients with allergic rhinitis and bronchial asthma were enrolled. Sublingual immunotherapy (manufactured at the allergen immunotherapy preparation unit at Mansoura University) was given in suitable bottles with a dropper mechanism that permits comfortable dosing under the tongue. The physician recommended that the patient put the drops under his/her tongue and leave the drops beneath the tongue for 2 minutes before swallowing. This was repeated every 3 days, with the drop number and concentration gradually rising. Results: After 2 months of follow-up, 65.8% responded partially to the symptom score and 26.3% responded completely to the medication score. There was a significant decline in the symptom and medication scores from the baseline scores (p<0.0001). After 4 months of follow-up, 95.8% responded partially to symptom scores and no one has not responded; 54.2% responded completely to medication scores; and 81% of studied patients had no side effects. However, the most frequent side effect was a sore throat. Conclusion: Our nondaily schedule of sublingual immunotherapy is tolerable, safe, and effective in patients with allergic rhinitis and bronchial asthma.

      • Cloning and Functional Characterization of Ptpcd2 as a Novel Cell Cycle Related Protein Tyrosine Phosphatase that Regulates Mitotic Exit

        Faithful transmission of genetic information depends on accurate chromosome segregation as cells exit from mitosis, and errors in chromosomal segregation are catastrophic and may lead to aneuploidy which is the hallmark of cancer. In eukaryotes, an elaborate molecular control system ensures proper orchestration of events at mitotic exit. Phosphorylation of specific tyrosyl residues is a major control mechanism for cellular proliferation and the activities of protein tyrosine kinases and phosphatases must be integrated. Although mitotic kinases are well characterized, phosphatases involved in mitosis remain largely elusive. Here we identify a novel variant of mouse protein tyrosine phosphatase containing domain 1 (Ptpcd1), that we named Ptpcd2. Ptpcd1 is a Cdc14 related centrosomal phosphatase. Our newly identified Ptpcd2 shared a significant homology to yeast Cdc14p (34.1%) and other Cdc14 family of phosphatases. By subcellular fractionation Ptpcd2 was found to be enriched in the cytoplasm and nuclear pellets with catalytic phosphatase activity. By means of immunofluorescence, Ptpcd2 was spatiotemporally regulated in a cell cycle dependent manner with cytoplasmic abundance during mitosis, followed by nuclear localization during interphase. Overexpression of Ptpcd2 induced mitotic exit with decreased levels of some mitotic markers. Moreover, Ptpcd2 failed to colocalize with the centrosomal marker ${\gamma}$-tubulin, suggesting it as a non-centrosomal protein. Taken together, Ptpcd2 phosphatase appears a non-centrosomal variant of Ptpcd1 with probable mitotic functions. The identification of this new phosphatase suggests the existence of an interacting phosphatase network that controls mammalian mitosis and provides new drug targets for anticancer modalities.

      • Static analysis of cutout microstructures incorporating the microstructure and surface effects

        This article develops a nonclassical model to analyze bending response of squared perforated microbeams considering the coupled effect of microstructure and surface stress under different loading and boundary conditions, those are not be studied before. The corresponding material and geometrical characteristics of regularly squared perforated beams relative to fully filled beam are obtained analytically. The modified couple stress and the modified Gurtin-Murdoch surface elasticity models are adopted to incorporate the microstructure as well as the surface energy effects. The differential equations of equilibrium including the Poisson’s effect are derived based on minimum potential energy. Exact closed form solution is obtained for bending behavior of the proposed model considering the classical and nonclassical boundary conditions for both uniformly distributed and concentrated loads. The proposed model is verified with results available in the literature. Influences of the microstructure length scale parameter, surface energy, beam thickness, boundary and loading conditions on the bending behavior of perforated microbeams are investigated. It is observed that microstructure and surface parameters are vital in investigation of the bending behavior of perforated microbeams. The obtained results are supportive for the design, analysis and manufacturing of perforated nanobeams that commonly used in nanoactuators, nanoswitches, MEMS and NEMS systems.

        Histopathological and genetic changes proved the anti‑cancer potential of free and nano‑capsulated sinapic acid

        Cancer is known to be a fierce disease that causes a large percentage of the deaths worldwide. The common cancer treatments; chemotherapy, radiotherapy and surgery are known for their severe side effects; therefore scientists are working on finding solutions to reduce these drawbacks. One of these treatment systems is the sustained released drugs formulations, these systems depend on the encapsulation of the chemotherapy within an emulsifying agent, in order to obtain a slow drug release of low doses over long time intervals. In this study, the anti-cancer effects of free and encapsulated sinapic acid was tested against lung (A549), and colon (CaCo2) cancer cell lines, along with normal fibroblast cells (HFB4) as a negative control. MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) assay was performed for IC50 evaluation, also cell cycle assay was performed to detect cell cycle arrest status and related anti-apoptotic and pro-apoptotic; Blc-2, BAX, and P53 gene profile fold changes post cellular treatment. Data recorded revealed that encapsulated SA showed a lower toxicity than the free form to both cell lines and also to the normal cells. The cell cycle analysis showed a cell cycle arrest at the G2/M phase post cell treatment with the free and encapsulated sinapic acid accompanied with up regulation of Bax and P53 and a down regulation of Blc-2 genes in both cell lines. The data suggest a promising anti-cancer and anti-proliferative potential of free and encapsulated sinapic acid. Also they show that the anti-cancer effect of free and encapsulated sinapic acid is quite close.

        Mechanical Thrombectomy in Patients with a Large Ischemic Volume at Presentation: Systematic Review and Meta-Analysis

        The benefits of mechanical thrombectomy (MT) for patients with acute ischemic stroke (AIS) and a large ischemic core (LIC) at presentation are uncertain. We aimed to obtain up-to-date aggregate estimates of the outcomes following MT in patients with volumetrically assessed LIC. We conducted a Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA)-conformed, PROSPERO-registered, systematic review and meta-analysis of studies that included patients with AIS and a baseline LIC treated with MT, reported ischemic core volume quantitatively, and included patients with a LIC defined as a core volume ≥50 mL. The search was restricted to studies published between January 2015 and June 2020. Random-effects-meta-analysis was used to assess the effect of MT on 90-day unfavorable outcome (i.e., modified Rankin Scale [mRS] 3–6), mortality, and symptomatic intracranial hemorrhage (sICH) occurrence. Sensitivity analyses were performed for imaging-modality (computed tomography-perfusion or magnetic resonance-diffusion weighted imaging) and LIC-definition (≥50 or ≥70 mL). We analyzed 10 studies (954 patients), including six (682 patients) with a control group, allowing to compare 332 patients with MT to 350 who received best-medical-management alone. Overall, after MT the rate of patients with mRS 3–6 at 90 days was 74% (99% confidence interval [CI], 67 to 84; Z-value=7.04; I2=92.3%) and the rate of 90- day mortality was 36% (99% CI, 33 to 40; Z-value=–7.07; I2=74.5). Receiving MT was associated with a significant decrease in mRS 3–6 odds ratio (OR) 0.19 (99% CI, 0.11 to 0.33; P<0.01; Z-value=–5.92; I2=62.56) and in mortality OR 0.60 (99% CI, 0.34 to 1.06; P=0.02; Z-value=–2.30; I2=58.72). Treatment group did not influence the proportion of patients experiencing sICH, OR 0.96 (99% CI, 0.2 to 1.49; P=0.54; Z-value=–0.63; I2=64.74). Neither imaging modality for core assessment, nor LIC definition influenced the aggregated outcomes. Using aggregate estimates, MT appeared to decrease the risk of unfavorable functional outcome in patients with a LIC assessed volumetrically at baseline.

      • Nonlocal strain gradient theory for buckling and bending of FG-GRNC laminated sandwich plates

        The bending and buckling behaviours of FG-GRNC laminated sandwich plates are investigated by using novel fivevariables quasi 3D higher order shear deformation plate theory by considering the modified continuum nonlocal strain gradient theory. To calculate the effective Young’s modulus of the GRNC sandwich plate along the thickness direction, and Poisson’s ratio and mass density, the modified Halpin-Tsai model and the rule of the mixture are employed. Based on a new field of displacement, governing equilibrium equations of the GRNC sandwich plate are solved using a developed approach of Galerkin method. A detailed parametric analysis is carried out to highlight the influences of length scale and material scale parameters, GPLs distribution pattern, the weight fraction of GPLs, geometry and size of GPLs, the geometry of the sandwich plate and the total number of layers on the stresses, deformation and critical buckling loads. Some details are studied exclusively for the first time, such as stresses and the nonlocality effect.

        Development of Cr cold spray – coated fuel cladding with enhanced accident tolerance

        Accident-tolerant fuels (ATFs) are currently of high interest to researchers in the nuclear industry and ingovernmental and international organizations. One widely studied accident-tolerant fuel concept ismultilayer cladding (also known as coated cladding). This concept is based on a traditional Zr-based alloy(Zircaloy-4, M5, E110, ZIRLO etc.) serving as a substrate. Different protective materials are applied to thesubstrate surface by various techniques, thus enhancing the accident tolerance of the fuel. This studyfocuses on the results of testing of Zircaloy-4 coated with pure chromium metal using the cold spray (CS)technique. In comparison with other deposition methods, e.g., Physical vapor deposition (PVD), lasercoating, or Chemical vapor deposition techniques (CVD), the CS technique is more cost efficient due tolower energy consumption and high deposition rates, making it more suitable for industry-scale production. The Cr-coated samples were tested at different conditions (500 C steam, 1200 C steam, andPressurized water reactor (PWR) pressurization test) and were precharacterized and postcharacterizedby various techniques, such as scanning electron microscopy, Energy-dispersive X-ray spectroscopy(EDX), or nanoindentation; results are discussed. Results of the steady-state fuel performance simulationsusing the Bison code predicted the concept's feasibility. It is concluded that CS Cr coating has highpotential benefits but requires further optimization and out-of-pile and in-pile testing

        Perfusion Imaging to Select Patients with Large Ischemic Core for Mechanical Thrombectomy

        Background and Purpose Patients with acute ischemic stroke, proximal vessel occlusion and a large ischemic core at presentation are commonly not considered for mechanical thrombectomy (MT). We tested the hypothesis that in patients with baseline large infarct cores, identification of remaining penumbral tissue using perfusion imaging would translate to better outcomes after MT. Methods This was a multicenter, retrospective, core lab adjudicated, cohort study of adult patients with proximal vessel occlusion, a large ischemic core volume (diffusion weighted imaging volume ≥70 mL), with pre-treatment magnetic resonance imaging perfusion, treated with MT (2015 to 2018) or medical care alone (controls; before 2015). Primary outcome measure was 3-month favorable outcome (defined as a modified Rankin Scale of 0–3). Core perfusion mismatch ratio (CPMR) was defined as the volume of critically hypo-perfused tissue (Tmax >6 seconds) divided by the core volume. Multivariable logistic regression models were used to determine factors that were independently associated with clinical outcomes. Outputs are displayed as adjusted odds ratio (aOR) and 95% confidence interval (CI). Results A total of 172 patients were included (MT n=130; Control n=42; mean age 69.0±15.4 years; 36% females). Mean core-volume and CPMR were 102.3±36.7 and 1.8±0.7 mL, respectively. As hypothesized, receiving MT was associated with increased probability of favorable outcome and functional independence, as CPMR increased, a difference becoming statistically significant above a mismatch-ratio of 1.72. Similarly, receiving MT was also associated with favorable outcome in the subgroup of 74 patients with CPMR >1.7 (aOR, 8.12;95% CI, 1.24 to 53.11; P=0.028). Overall (prior to stratification by CPMR) 73 (42.4%) patients had a favorable outcome at 3 months, with no difference amongst groups. Conclusions In patients currently deemed ineligible for MT due to large infarct ischemic cores at baseline, CPMR identifies a subgroup strongly benefiting from MT. Prospective studies are warranted.
