        First Isolates of OXA-48-Like Carbapenemase-Producing Enterobacteriaceae in A Specialized Cancer Center

        Background OXA-48-like carbapenemases have been found in a growing and varied number of carbapenemase-producing Enterobacteriaceae (CPE) isolates, and they are spreading to several countries. Although this oxacillinase leads to weak resistance to carbapenems without affecting broad-spectrum cephalosporin activity, when they are associated with other resistance mechanisms, the level of resistance to these antibiotics may be significantly higher. This weak resistance against carbapenems and cephalosporins, along with the absence of other resistance mechanisms, could render OXA-48-like harboring isolates undetected in the laboratory routine. In addition, the lack of a specific screening test for this enzyme complicates the detection of these isolates. This report characterizes the first isolates of OXA-48-like CPE detected in our laboratory. Materials and Methods The study was carried out at the Instituto Nacional de Enfermedades Neoplasicas, Lima - Peru, between March and December 2021. OXA-48-like CPE isolates were detected as part of the routine microbiological study, and clinical data were obtained by reviewing medical records. The automated microbiological system provides the bacterial identification and antimicrobial susceptibility profile by the dilution method. Additionally, the column chromatography test is used to detect carbapenemase enzymes, including OXA-48-like. Finally, the molecular identification of the OXA-48-like enzyme was carried out by Polymerase Chain Reaction PCR amplification for the blaOXA-48-like. Results Seven OXA-48-like CPE strains were isolated. Notably, in all cases, the automated system issued a minimum inhibitory concentration (MIC) of ≥1 ug/mL for ertapenem and a MIC of >64/4 ug/mL for piperacillin/tazobactam. In addition, resistance category to imipenem and meropenem was found (2/7), at least one indeterminate category for any of these carbapenems (5/7), and other serine β-lactamases such as Extended-spectrum beta-lactamases (3/7) and AmpC (3/7). The immunochromatographic study confirmed the presence of the OXA-48-like enzyme in all isolates, while class A and class B were ruled out for them. Finally, the multiplex PCR, for the five isolates that could be recovered, showed amplification for carbapenemase OXA-48-like, while none of the other carpabemases was amplified for class A or class B carbapenemase genes. Conclusion We confirm the emergence of OXA-48-like CPE isolates in our cancer center and highlight the need to implement surveillance and detection measures of these strains, for controlling their dissemination. We found practical and inexpensive methodologies for the detection of OXA-48-like CPE: (1) the finding of resistance to ertapenem and piperacillin/tazobactam in the antibiogram in the absence of class A and B carbapenemases, for screening and (2) immunochromatographic study, for confirmation.         Mckittrick-Wheelock syndrome: a case series

        McKittrick-Wheelock syndrome is a rare and life-threatening disease characterized by the triad of (1) chronic mucous diarrhea, (2) renal function impairment with hydroelectrolyte imbalance, and (3) a giant colorectal tumor. Often, the tumor is a rectal adenoma. With the mortality being certain, if left untreated, it is important to raise awareness on the presentation, diagnosis, and management of this disease entity. Here, we presented 3 cases of McKittrick-Wheelock syndrome that were successfully managed with surgical resection at the Philippine General Hospital from August 2018 to May 2019. Resolution of their symptoms, reversal of their renal impairment, and correction of their electrolyte depletion were noted after removal of the tumor with a sphincter-saving operation.

        Adipose tissue-derived mesenchymal stromal cells for treating chronic kidney disease: A pilot study assessing safety and clinical feasibility

        Background: Chronic kidney disease (CKD) is a growing public health concern, and available treatments are insufficient in limiting disease progression. New strategies, including regenerative cell-based therapies, have emerged as therapeutic alternatives. Results from several groups, including our own, have reported evidence of a supportive role for mesenchymal stromal cells (MSCs) in functional recovery and prevention of tissue damage in murine models of CKD. Prompted by these data, an open pilot study was conducted to assess the safety and efficacy of a single injection of autologous adipose tissue-derived MSCs (AT-MSCs) for treatment of CKD. Methods: AT-MSCs were infused intravenously into six CKD patients at a dose of 1 million cells/kg. Patients were stabilized and followed for one year prior to MSC infusion and one year following infusion. Results: No patients presented with adverse effects. Statistically significant improvement in urinary protein excretion was observed in AT-MSCs transplanted patients, from a median of 0.75 g/day (range, 0.15-9.57) at baseline to 0.54 g/day (range, 0.01-2.66) at month 12 (P = 0.046). The glomerular filtration rate was not significantly decreased post-infusion of AT-MSCs. Conclusion: Findings from this pilot study demonstrate that intravenous infusion of autologous expanded AT-MSCs into CKD patients was not associated with adverse effects and could benefit patients already undergoing standard medical treatment.

        Elementos Precinematograficos en El Quijote

        Desde una temprana aportacion del latinista Paul Leglise, estudioso de la Eneida (1958), el concepto de precinema, entendido en una dimension fundamentalmente estetica y artistica, se refiere al analisis de las estructuras y recursos formales que se encuentran en los filmes y que aparecen tambien en obras literarias escritas antes del descubrimiento de los hermanos Lumiere. Asi, el arte filmicode los escritores cinematograficos consiste en pintar cuadros animados, enriquecerlos mediante todos los artificios de la vision, situarlos segun planos diferentes y encadenarlos entre si conforme a una sintaxis artistica que asegure la continuidad de la accion con un ritmo agradable a la imaginacion visual del lector. El presente articulo representa una primera aproximacion a los elementos precinematograficos presentes en algunos episodios del Quijote de 1605, tales el de la venta en el capitulo XVI, el ataque de don Quijote a los rebanos (capitulo XVIII), el del acuchillamiento de los odres de vino (XXXV) o, entre otros, el de los molinos de viento y la pelea con el vizcaino (VIII y IX).

        Application of a novel approach to modelling the supercritical extraction kinetics of oil from two sets of chia seeds

        The kinetics of the supercriticalfluid extraction of edible and discarded chia seeds was studied andmodelled for thefirst time. The total oil was removed at 45 MPa and 60 C after 240 min. The extractionkinetics was simulated using a dynamic model in gPROMS ModelBuilder environment and the kineticparameters estimated. Triolein was chosen as a model compound of the chia oil. The agreement betweenthe experimental yields and those calculated by the model was good with deviations in the range(1.2–6.6) %, except at 25 MPa and 60 C (AARD = 9.5%).

        Influence of doping on chain-like TiO2 clusters: A DFT study

        DFT calculations were carried out to determine the electronic and structural properties of titanium dioxide clusters either pristine or doped. In this work, a maximum size of approximately 1.5 nm for the three cases was considered. There are no drastic changes concerning the geometry for larger clusters; however there is an important reduction of electronic gap for doped clusters compared to pristine ones in all range considered. The pristine clusters enhance the visible-light photocatalytic activity, whereas those of systems doped with one nitrogen and carbon atom exhibit a spectral shift in the near-infra-red region due to the introduction of additional electronic states originating from the N and C 2p orbitals in the band gap.

        The Calling of Asian Christian Scholars : A Biblical Scholar’s Perspective

        This article is an attempt to answer the question, what are we being called to do as biblical scholars in Asia today? What is our calling? Deriving inspiration from the story of the healing of the paralytic in Mark 2, this article critiques the traditional western biblical studies we inherited from the west. This approach sees the task of biblical studies as mere determination of “what the text meant in ancient times.” There is now acknowledgment that biblical studies is contextual. But this realization is also insufficient. There is a need to identify calls for a way of doing biblical studies that goes beyond the modern western approach to the discipline, which confines the text's meaning to the ancient time. Instead, as reflected in the view of the Bible as Scripture.

        The paper looked into the various adversities the students of Bulacan State University in studying Mandarin as a foreign language. Mandarin language has been included in the curriculum of education course of Bulacan State University. The various areas considered were reading, speaking, listening, writing, learning materials, medium of instruction and teaching methodology. The research instrument used was created by the researcher considering the four basic skills in communications and other areas in studying Mandarin where the researcher believed students are having adversities. The study revealed that students of Bulacan State University are having problem in various aspects of the study of Mandarin as foreign language more particularly on vocabulary enrichment. The researcher developed an instructional material in Mandarin that will help enrich the vocabulary of the students thereby enhancing the students competency in the mandarin language.
