        골절 및 탈구 없는 경추손상의 치료

        The authors analized the 98 cases of cervical spine injuries without fracture or dislocation by hyperflexion/hyperextension of the neck. The peak incidence of the age was in the second to fourth decades(69.4%). There was a male preponderance with a sex ratio of 1.8:1. Sources of trauma were motor vehicle accident, object falling on and motorcycle accident in the order of frequency. The common mechanisms of injury were flexion-extension, extension and flexion injury in the order of frequency. There were 10 cases of cervical cord injury without fracture or dislocation and operative management was given in 6 cases of them. The most common preexisting desease was congenital cervical stenosis. Surgical modality such as laminectomy, laminoplasty and anterior decompressive procedure made them improved. The authors discuss the mechanism of injury and surgical management of the patients with cervical cord injury in the abscence of concomittent fracture or dislocation.

      • 비글개에서 신원방우황청심원의 급성 및 아급성독성시험

        Single and 4 weeks oral administration of New wonbangwoohwangchungsimwon (NSCH) which was used l-muscone as substitutive material of musk, to beagle dogs of both sexes were performed to investigate both acute and subacute toxicity. Beagle dogs (3 males and 3 females) in acute experiments were administered orally with sinlge dose of 2,000 ㎎/㎏ and groups of 9 male and 9 female beagle dogs in subacute experiments were given daily different dosage of NSCH, 160 ㎎/㎏/day (low dosage group), 400 ㎎/㎏/day (middle dosage group), 1,000 ㎎/㎏/day (high dosage group) once a day for 4 weeks by oral route according to the Established Regulation of Korea Food and Drug Administration (1996. 4. 16). LD_50 value for beagle dogs was more than 2,000 ㎎/㎏ per oral for both male and females. In animals administered with NSCH, there were neither dead animals nor significant changes of body weights. In addition, no differences were found between control and treated groups in clinical sign, urinalysis, eye examination, hematology, serum chemistry, organ weight and other fingings. No histological lesions were observed in both control and treatment groups. Above data strongly suggset that NSCH in beagle dogs is considered to be safe.

      • 트랙터의 로터리 驅動을 위한 PTO 시스템과 油壓驅動 시스템의 性能 試驗에 關한 硏究

        엔진 회전속도 변동에 따른 PTO 회전수, PTO 토크, PTO 출력 및 연료 소비율을 측정하여 기계식과 유압식의 동력 특성을 비교, 분석한 결과는 다음과 같다. 1) 기계식, 유압식 모두 엔진의 회전 속도가 증가할수록 PTO 회전수는 증가하는 경향으로 나타났으며, 기계식이 변속단수 변동에 따른 엔진의 회전속도 폭이 안정적으로 나타난 대 반하여 유압식은 다소 크게 나타났다. 2) 엔진의 회전속도가 증가할수록 기계식, 유압식의 토크는 반비례 관계로 나타났다. 3) 최대 출력은 기계식의 경우 PTO 1, 2, 3단 순으로 나타난 데 비하여 유압식은 PTO 3, 2, 1단 순으로 나타나 PTO 변속 단수가 증가될수록 출력도 증가하는 것으로 나타났다. 4) 연료 소비율은 기계식, 유압식 모두 엔진의 회전수가 증가됨에 따라 연료 소비율은 어느 일정한 회전수 부근에서 최소가 된 후 다시 증가하는 경향으로 나타났다. 5) 유압구동 로터리의 효과적인 이용을 위해서는 유압펌프가 항상 고속으로 회전할 수 있도록 로터리는 어느 일정 회전속도를 유지시켜 작업하는 것이 필요할 것으로 판단되었다. 6) 유압식의 경우 유압펌프의 회전속도가 증가함에 따라 출력 및 연료 소비율이 향상되는 성으로 나타나 기계식에 비하여 효율이 크게 뒤떨어지지 않아 실용화의 가능성이 큰 것으로 생각되었다. The speed of revolution, the PTO torque, the PTO output power, and the specific fuel consumption caused by the variation in the revolution of an engine speed were measured. Therefore, the overall dynamic performance of the hydraulic PTO system was compared and analyzed with that of mechanical PTO system. The results are summarized as follows : 1) Both RPM of the hydraulic PTO and that of the mechanical one increased linearly as the engine speed increased. The engine speed from the variation of a mechanical type was saftly and small changed, but that of a hydraulic type was high changed. 2) Both torques of the hydraulic PTO system and those of the mechanical one decreased linearly as the engine speed increased. 3) The maximum power of the mechanical PTO system was the highest at the first gear of the mechanical one followed by the second one and the third one. However, that of the hydraulic PTO system was at the third gear followed by the second one and the first one. It means that the output power of a hydraulic PTO system increased as the variable PTO gear was increased. 4) After the specific fuel consumption decreased gradually to the specific point, which was required the lowest fuel consumption, then it increased as the engine speed increased. 5) To utilize effectively the hydraulic drive system, this system needs always to be kept revoluting constantly at an engine speed for hydraulic pump to rotate at the high speed. 6) The output power and the specific fuel consumption were improved by using the hydraulic system as the revolution of the engine speed was increased. The performance of hydraulic system was not significant of that of mechanical system. Therefore a hydraulic PTO system, which is convenient for a machine attached it to control automatically, would be utilized in future.

      • 유연구조물의 진동제어를 위한 압전작동기의 응용

        The paper presents the modeling and optimal control for the reduction of transverse vibration of simply supported beam under a moving mass. The equations of motion are derived by using assumed mode method. The coriolis and centripetal accelerations are accommodated in the equations of motion to account for the dynamic effect of the traveling mass. In order to reduce the transverse vibration of the beam, an optimal controller with full state feedback is designed based on the linearized equations of motion. The optimal actuator locations are determined with the evaluation of an optimal cost functional defined by the worst initial condition with the trade-off of controlled mode performance. Numerical simulations are performed with respect to various velocities and different traveling masses. Even if the velocity of the traveling mass reaches to the critical speed which can cause the resonance of the beam, the controller with two piezoelectric actuators shows the excellent performance under severe time-varying disturbances of the system.

        섬망 치료에 있어서 Quetiapine과 Risperidone의 효과 비교

        본 연구에서는 최근에 개발된 비전형 항정신병 약물인 quetiapine이 섬망의 치료에 효과가 있는지와 비전형 항정신병 약물인 risperidone과 효과의 차이가 있는지 평가하고자 하였다. 총 71명의 환자가 본 연구 대상에 포함되었고 CGI, K-DRS, K-DRS-R-98, K-MMSE를 평가 도구로 하여 치료 전과 치료 일 주일 후 측정한 전체 점수를 비교하였다. 부작용에 대한 평가도구로는 ESRS 점수를 치료 전후에 비교하였다. 연구 결과 두 군 모두에서 일주일 후 측정한 CGI-S, K-DRS, K-DRS-R-98, K-MMSE에서 통계학적으로 유의한 차이를 보였다. 두 군 간 치료전후의 통계학적인 차이는 보이지 않았다. 부작용은 두군 모두에서 나타나지 않았다. 이번 연구에서 quetiapine은 섬망 환자의 치료에 있어 risperidone과 동등한 효과를 보이고 부작용이 적어 섬망 환자의 치료에 안전하게 사용할 수 있을 것으로 기대된다. Objectives:It has been shown that risperidone, an atypical antipsychotics, has effectiveness for treatment of delirium as good as haloperidol and has less side effects than haloperidol. Quetiapine also has less extrapyramidal side effects and has effectiveness for treatment of delirium. The aim of this study was to investigate whether quetiapine is effective for the treatment of delirium compared with risperidone. Methods:All subjects(71 patients) were randomized to receive either risperidone(35 patients) or quetiapine(36 patients) with fixed dose at the first day of treatment. Then, all subjects received either medications with flexible doses according to judgement on clinical status of patients. The effectiveness was evaluated using Clinical Global Impression-Severity(CGI-S), Korean version of Delirium Rating Scale(K-DRS), Korean Mini Mental Status Examination(K-MMSE), and Korean version of Delirium Rating Scale-Revised-98. Results:There was no significant difference between both groups in the baseline K-DRS, K-DRS-R-98, K-MMSE and CGI scores. At the seventh day of the treatment, K-DRS, K-DRS-R-98 and CGI scores were significantly improved from the baseline both in the quetiapine and risperidone treatment group. Conclusion:Quetiapine is effective for treatment of delirium.
