The use of critical pathways for a variety of clinical conditions has grown rapidly in recent years, particularly pathways for patients with acute myocardial infarction. We intend to determine the impact of a clinical pathway on ST-elevation myocardial infarction (STEMI) and to evaluate the efficacy and safety of facilitated percutaneous coronary intervention (PCI) compared with primary PCI. Low risk STEMI patients (ST elevation >0.1mV in more than 2 limb leads or ST elevation >0.2mV in contiguous precordial lead, chest pain lasting more than 30 min without response to nitroglycerin) will be included. All patients will be also treated medically according to critical pathway. STEMI is one of the common diseases in emergency medicine and so it is necessary to establish realistic treatment guidelines. The use of critical pathways will improve the quality of care.

Polyurethane diagnostic membranes were prepared to measure the concentration of glucose in blood. It was found that hematocrit did not show the serIous effects on the end-point results of K/S values.

The end-point results of K/S values with polyurethane diaganostic membranes had a linear relationship toward the glucose concentration. Testing Temperatures did not show the serious effects on each glucose concentration.

This study was performed to evaluate the usefulness of the electrical analogue model in porous medium. Experimental values by the electrical analogue models were compared with those by the sand models and theoretical values by the fragment method. The results obtained are as follows: 1. The flow lines into the sand model were changed according to rising the upstream water level but those of the electrical analogue model weren't changed, and the equipotential lines were revealed to be almost same in the two kinds of models above. 2. The seepage quantities by the fragment method and the electrical analogue model were a little more than those by the sand model. 3. The exit gradients by the fragment method were steeper than those by the sand model and the electrical analogue model. 4. The seepage quantities were increased in proportion to the increment of the exit gradients. By means of the results above, it may be given as a conclusion that the electrical analogue model test is more useful than the sand model test for appling to seepage flow in the field condition, because the electrical analogue model test is more accurate and simpler. Then, the electrical analogue model test will be able to apply to the three-dimension seepage flow problems.

Thermus caldophilus GK24 (Tca) DNA polymerase is highly useful enzyme for amplifying DNA fragments by polymerase chain reaction (PCR). A plasmid, pTCA, is expression vector for Tca DNA polymerase gene in Escherichia coli under the control of tac promoter and its expression level is low. For the high expression, the DNA sequence coding for NH_2-amino acid sequence of Tca DNA polymerase was changed by PCR, and then an expression vector pTCAM was constructed. The activity of Tca DNA polymerase in E. coli harboring for pTCAM has enhanced nearly 6-fold/ml than that for E. coli harboring pTCA.
악교정 환자의 악교정 수술전후 발음양상에 대한 비교연구

The purpose of this study was undertaken to determine the effects of orthognathic surgery on speech. The hyposis stated herein is that functional behaviors of the dentofacial complex, such as speech production, may be adversely affected by deviations of a structural nature(especially, Class III malocclusion) . Twenty adults with Class III malocclusion(13 female and 7 male) were studied preoperative, immediate postoperative and either 6 or 12 months postoperative lateral cephalograms. They had mandibular prognathism and had undergone mandible setback operation. The position of tongue, soft palate(Uvula), hyoid bone, respiratory track width, and pharyngeal depth were assessed on lateral cephalograms with 23 cephalometric variables, ANOVA, Paired t-tests and Pearson's product-moment correlation coefficient tests were used to evalute the operative changes in all cephalometric parameters. A experienced speech and language pathologists performed narrow phonetic transcriptions of tape-recorded words and sentences produced by each of the ninth patients and the recording tapes were analyzed by phonetic computer program(Computerized Speech Lab(CSL) Model 4300BI(U.S.A.)) These judges also recorded their ratings of each patient's overall consonants, hypernasality, hyponasality, and articulation proficiency. The results obtained are as follows ; 1.There were significant changes in distance of posterior pharyngeal wall to tongue (TI-TW2, TS-TW3) after the surgery at 6 months postoperatively(each p<0.01 p<0.05) 2.The posterior tongue point(TI, TS, PPT) moved posteriorly after surgery and remained to its changed position at 6 months postoperatively(p<0.05) . The displacement of tongue was correlated with the movement of mandibular setback amount(p<0.05). The hyoid bone moved posteriorly superiorly after immediate postoperative period. There was significant changes in hyoid bone movement after immediated post-operative period(p<0.05), but returned to its original position during the follow-up period(p>0.05) 3.The soft palate was displaced posteriorly superiorly after immediated operative period and remained to its changed position at 6 months postoperatively(p<0.05) ANS-PNS-SPT angle increasing, PPU-PPPo distance narrowing was showed after surgery, and remained its appearance 6 months postoperactively (p<0.05) 4.There were significant changes in formant value and squre diagram of vowel sound after the orthognathic surgery and the follow-up period. There were significant changes in /ㅅ/sound and posterior tongue sound. 5.The posterior movement of tongue and the posteriosuperior movement of soft palate was correlated with mandibular setback amount after orthognathic surgery. On the vowel squre diagram, the author found that the place of articulation after operation moved downward, backward, upward. 6.In assessing speech abnormalities, dental occlusion should be considered as a contributing factor. The vast majority of subjects with preoperactive misarticulations eliminated or reduced their errors following orthognathic surgery. There was significant difference in speech impovement between pre- and post-operation.
Thermus caldophilus GK24 DNA Polymerase를 이용한 Polymerase Chain Reaction의 최적조건

A thermostable DNA polymerase was used in an in vitro amplification procedure, the polymerase chain reaction(PCR). Thermus caldophilus GK24(Tca) DNA polymerase greatly simplifies the procedure and, by enabling the amplificatioin reaction to be performed at higher temperature, significantly improves the specificity, yield and sensitivity. The optimal conditions for PCR were investigated in terms of pH range, MgCl_2, KCl and DTT concentration using the purified Tca DNA polymerase. The optimal pH for DNA amplification was found to be between 8.6, and 9.2 in 50mM Tris-HCl buffer. The optimal concentrations of MgCl_2 and KCl for PCR were 1-1.5mM and 10-50mM, respectively. The concentration of DTT above 0.5mM was sufficient to amplify template DNA by PCR. These conditions can be used to amplify a wide range of target sequences with excellent specificity.
