        16세기 사림의 주자의례(朱子儀禮) 실천과 「분천강호가(汾川講好歌)」 - 제조에 맞는 몸과 마음은 어떻게 만들어지는가? -

        The practice into Chu Zi's Ceremonies & 'Bunchunganghoga' of Sarimin the 16th Century This article investigates how the Confucian ritual and morals was popularized in the society as Bunchunganghorok & Bunchunganghoga. The Sarim's intellectual class tried to renovate the transmissional life styles and manners to Cofucian rituals and canons in the 16th Century. Lee Suk-riang was the jaejisajok(在地士族) of Bunchun(汾川) in the 16th Century. His family had lived in Bunchun from generation to generation and exercised their's influence over that country. Lee Suk-riang carried out the 'Ceremony on the first & fifteenth days of the lunar month' and composed 'Bunchunganghorok' in Bunchun. 'Ceremony on the first & fifteenth days of the lunar month' was state plainly in 'Gugajabui'(居家儀禮) that was written by Samagwang. Suk-riang united this ceremony with hangyak(鄕約, the country's convention). This ceremony performed to band together Suk-riang's family and to eastablish order of his family. His ultimate aims was that enlighten the country's people by Cofucian rituals and canons. Suk-riang tried that his family fulfill 'filial duty & fraternity & family concord' by way of the moral rules to govern the Five Human Relations. His family fufilled 'filial duty & fraternity & family concord' by means of the compulsion that the observation & punishment' be attended and internalized 'the family's morals' by means of the influence as singing the poetry 'Bunchunganhoga'. Chu Zi's Family Ritual was popularized our life on grounds of this internalization.

        근현대 남성 농민 일기에 나타난 여성 표상

        이 글은 『아포일기』, 『창평일기』를 쓴 남성 농민이 여성을 어떻게 표상하고 있는지에 관한 연구이다. 첫 번째 장에서는 어머니에 대한 표상을 다루었다. 두 남성은 어머니를 ‘여성’으로서가 아닌, 효도해야 할 탈젠더화된 부모로 표상했다. 두 번째 장에서는 배우자와 딸에 대한 성역할 표상을 다루었다. 그들은 배우자에게는 가사와 육아 등의 재생산노동은 물론이고, 농업노동을 수행할 재생산노동자로 표상했으나, 딸 세대에는 전업 주부라는 성역할만을 표상했다. 마지막 장에서는 두 남성이 표상하는 바람직한 배우자상과 여성상을 다루었다. 두 남성에게 이상적인 여성상이란 헌신적이고 순종적인 배우자, 얌전하고 조신한 여성을 말했다. 필자는 조선조 유교 이데올로기뿐만 아니라, 근대화, 도시화 과정에서 진행된 가부장제의 자본주의적 변형이 두 농민의 여상 표상에 영향을 미치고 있는 것으로 해석했다. This paper aims to examine how women are represented in modern male farmers’ private journals, named Apo Diary and Chanpyong Diary. The first chapter analyzes how the mothers of the farmers were represented in the diaries. In both diaries, the mothers were described as a degendered parent rather than a woman, who was supposed to receive a filial piety from her son. The second chapter discusses how the gender roles of the farmers’ wives and daughters were represented in the diaries. The farmers perceived the multiple roles of their wives as a laborer for the domestic sector including housekeeping and childcare as well as for production labor in the farm, while limiting the role to a housewife for their daughters. The third chapter analyzes how the stereotypes of desirable women or wives were represented in the diaries. According to the farmers’ description, women are required to maintain calm and modest manners, and wives are supposed to be devoted and obedient to their spouses. Based on the analysis, this paper reaches a conclusion that the capitalist transformation of patriarchy traditionally stemming from the Confucian ideas of the Joseon Dynasty has influenced on the farmers’ perceptions on women around them.

        Renal papillary necrosis occurs most commonly in association with urinary tract infection, diabetes mellitus, sickle cell disease, vascular disease, and analgesic nephropathy. Clinical presentation may be related to symptoms of pyelonephritis such as flank pain, renal colic, hematuria, Proteinuria, recurrent fever. The necrotic tissue may be sloughed off, and the diagnosis can sometimes be made by finding piece of renal medullary tissue in the urine. Pyelography may demonstrate cavities and sinuses in the resion of papillae. Anuria & oliguria can lead to the acute renal failure, and especially prognosis and progress may be affect influenced by urinary infection. We report a case of renal papillary necrosis with ingestion of bear gallbladder. On pyelography, Persistent contrast is diagnostic clue of renal papillary necrosis. This case is not be related to urinary tract infection, but occurred acute renal failure. Expectant treatment was gone.

        텔레비전 역사드라마 〈이산〉의 정치적 성향 고찰

        2000년대 중반 이후 텔레비전 역사드라마는 군왕의 영웅적 면모가 아니라 인간적인 고뇌에 초점을 맞추면서 현대 사회가 필요로 하는 지도자의 덕목을 환기시키는 경향이 강해졌다. 그 결과 역사드라마에 내재될 수밖에 없는 역사 인식이 퇴조하는 대신에 방영 당시의 정치 상황에 따라 역사적 사실을 자의적으로 재구성하는 부작용이 나타나기 시작하였다. 개혁 담론이 풍성했던 2000년대 중반에 방영된, 조선왕조 제22대 임금 정조를 호출한 일련의 역사드라마들이 대표적인 경우였다. 특히 개혁군주 정조대왕의 일대기를 형상화한 〈이산〉은 노무현 정부에서 이명박 정부로 정권이 교체되던 시기에 방영되면서 정치 상황에 의해 극적 상황과 캐릭터가 변하는 문제를 드러냈다. 본고는 아버지 사도세자의 비극적인 죽음을 가슴에 묻은 세손 ‘이산’이 권력 투쟁에서 살아남아 임금의 자리에 올라 펼치는 개혁 정치를 다룬 〈이산〉을 중심으로 텔레비전 역사드라마의 정치적 성향을 고찰하였다. 그 결과 텔레비전 역사드라마의 정치적 성향은 방영 당시의 정치 상황과 밀접한 상관관계가 있음을 알 수 있었다. 정조대왕의 죽음과 함께 개혁의 동력이 멈추면서 그의 정책과 정신이 훼손되었던 것처럼, 역사드라마 <이산>이 환기시켰던 2000년대 중반 대한민국의 정치 개혁도 2008년 2월 정권 교체와 함께 동력을 상실한 채 좌초되고 말았음을 확인할 수 있었던 것이다. 이는 곧 〈이산〉의 극적 현실이 실제 현실로 전이되지 못한 것은 물론, 정조대왕의 정치 개혁 담론이 현실 정치의 영향을 받으면서 왜곡되었음을 의미한다. While focusing on human agonies, not a ruler‘s heroic aspects, since the 2000s when a historical TV drama arousing virtues of a leader needed in the modern society started gaining attention, the political traits of a historical TV drama have gotten closely engaged in the political environment at the time of being aired. A series of historical dramas which dealt with Jung Jo, the 22nd king of the Joseon Dynasty aired in the mid 2000s when there were plentiful discussions for renovation represented the political situation. In particular, the TV drama <Yi San> which dramatized the life of King Jung Jo spaning from his early years to death has been politically blamed for glorifying the President equipped with renovation traits by the conservative force and been engaged in the political debate concerning the issue of representing the dominating ideology of the ruling force since the change of government. The study was designed to find out political traits of a historical TV drama centering on 〈Yi San〉 dealing with the story of ‘Yi San’ who beared resentment about the tragic death of his father ‘Crown Prince Sado’ survived the battles for power to sworn in as king and unfolded the renovation of politics. As a result, it was found that the political traits of a historical TV drama were closely related to political environment at the time of being aired. In addition, it was confirmed that when the driving force for renovation stoped along with the death of the great king Jung Jo, his policies and spirits were also damaged, and the political renovation of Korea in the mid 2000s presented by the historical TV drama 〈Yi San〉 lost its driving force to be in trouble along with the change of government in February 2008. It meant that the dramatic reality of 〈Yi San〉 couldn’t spread to reality because discussions for Jung Jo’s political renovation were distorted while being affected by real politics.

        피로와 의료이용, 업무상 사고 및 질병결근

        Objectives: This study examined the relationship of fatigue to medical utilization, occupational accident and sickness absence. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project, 2002~2004). A total of 30,146 employees categorized within the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were recruited. A structured-questionnaire was used to assess the participants' sociodemo-graphics, job-related factors, health-related behaviors, personality trait, self=perceived fatigue (MFS) and the frequency of medical utilization, occupational accident and sickness absence. Data on medical utilization and occupational accident were confirmed from company health records. Chi-square test and logistic regression analysis were used to elucidate the relationship of self-perceived fatigue to medical utilization, occupational accident and sickness absence. Results: In logistic regression analyses, self-perceived fatigue was associated with medical utilization (outpatient), occupational accident and sickness absence although the strength of the associations was higher in the blue collar workers except for medical utilization. For medical utilization, the white collar worker's adjusted odds ratios of outpatient utilization for workers scoring in the third quartile and the highest quartile, compared to workers scoring in the lowest quartile were 1.50 (95% CI: 1.20-1.87) and 2.16 (95% CI: 1.69-2.75), respectively. The blue collar workers' adjusted odds ratios of outpatient utilization for workers scoring in the second (OR: 1.23, 95% CI: 1.06-1.43), third (OR: 1.42, 95% CI: 1.22-1.66) and highest quartile (OR:1.83, 95% CI: 1.55-2.17), respectively, compared to workers scoring in the lowest quartile were statistically significant. However, no associations were found between fatigue and inpatient utilization for either the white or blue collar workers. The blue collar workers' adjusted odds ratios of being injured for workers scoring in the second, third and highest quartile were 1.41 (95% CI: 1.00-2.00), 1.57 (95% CI: 1.10-2.22) and 2.41 (95% CI: 1.69-3.44), respectively, compared to those scoring in the lowest quartile, but, those of the white collar workers were not significant. There was more likely to an increased risk of sickness absence in workers scoring in the second, third and highest quartile compared to those scoring in the lowest quartile in both white and blue collar workers, but the magnitudes of risk were higher in the blue collar workers than the white collars workers. Conclusion: This result suggests that fatigue is a determinant predictor of medical utilization, occupational accident and sickness absence, and that the pattern of risks for the three outcomes differs according to the occupational type. Some limitations of this study and considerations for future study were also discussed.

        일반논문 : 노자상이주(老子想爾注) 양생윤리 -양생의 시대적 변용이라는 관점에서-

        본고는 『상이주』에 보이는 생명과 윤리의 논리를 양생윤리라는 개념으로 포착하고, 그것을 양생론의 시대적 변용이라는 관점에서 고찰한 연구의 결과물이다. 외척, 환관정치로 인해 생명이 피폐해지고 社의 권위가 흔들린 후한대의 상황은 개인적 차원에 머물러 있던 양생에 사회화를 요구했고, 그 결과 양생은 윤리와 결합하게 되었다. 감정을 중요한 생명의 단서로 생각하고, 감정의 유로를 생명의 소실로 보는 기존의 전통 윤리적 관념은, 양생윤리의 배경이 되었다. 『상이주』 양생윤리의 전모는 『正統道藏30冊』 543쪽에 실려 있는 ``道 德尊經戒``에 보인다. 도덕존경계에는 『도덕경』 등에 보이는 퇴양과 인순의 지혜가 담겨 있기 때문에, 『상이주』 도계는 지혜의 격언과 도덕규범 이 혼재된 것이었음을 알 수 있다. 이 점은 『상이주』가 『도덕경』 주석서 형태를 띠고 있다는 점, 또 『도덕경』의 가르침이 도덕규범으로 발전할 소지가 있었다는 점 등으로 설명될 수 있다. 『상이주』 양생윤리는 감정의 안정을 추구하던 전통적 양생론과 달랐다. 그러므로 마음의 안정을 지향하던 전통윤리와는 다른, 생명과 윤리를 연결하는 별도의 기제가 요구되었다. 『상이주』의 저자는 선악판단과 수명의 심판자인 天曹, 수명의 증감을 기록한 계약서라는 관념 그리고 尸解라는 독특한 죽음관을 적절히 이용해서 ``선악-천조의 관할-생명의 증감``이라는 논리를 만들었다. 이 배후에는 감정적 동요는 도덕적 악의 상태에 해당한다는 전통적 윤리관이 전제되어 있다. 결국 『상이주』 양생윤리는 감정의 안정을 추구하던 전통윤리, 그리고 전통윤리의 배후에 있던 생명과 윤리의 결합이라는 관념이 당시의 민중 신앙과 섞여 만들어진 양생론이자 민중종교 윤리였던 셈이다. Nurturing life is one of the most important thoughts in ancient china. Xiang Er Zhu is the bible of Wou Dou Mi Tu(五斗米徒). I investigated Xiang Er Zhu from the perspective of Nurturing Life in this treatise. Nurturing Life first shown in Zhuang Zi(莊子) and Guan Zi(管子) had developed through Qin Hin(秦漢) period and changed itself in accordance with the demand of age. This change can be told to be the development of Nurturing Life Theory. In post han period, Nurturing Life Theory had to fit itself into the age request, socialization. Nurturing Life Ethic in Xiang Er Zhu is the response to the request. China had some thoughts of the original form of Nurturing Life Ethic. Tai Ping Jing(太平經) is one of them. But the oldest model we can find would be in Guan Zi. In Guan Zi, we can find the thought that feeling stability cause health of mind and body. Nurturing Life in Xiang Er Zhu formed on the foundation of that thought. But it did not follow the same logic. It borrowed such concepts as Tai Yin(太陰), Tian Cao(天曹) from folk religion. Folk religion was familiar with the chinese and had the power to enforce moral behavior. People should keep Dao Jie(道誡) not to lessen his life by punishment of Tian Cao. But Zao Shen(조神) or San Shi(三尸) that report guilty of person to Tian Cao in Daoism Ethic is not in Xiang Er Zhu. Instead, Xiang Er Zhu proposed the technique of Shi Jie(尸解), revival from death. Nurturing Life Ethic in Xiang Er Zhu is folk religion ethic founded on the traditional ethic orienting feeling stability and the cohesion of life and ethic behind traditional ethic.

        소아청소년 복합만성질환자 수 및 진료비의 변화: 2011-2021년 건강보험청구자료 분석

        Background: The number of children and adolescents has decreased by more than 3 million over the past 10 years. We analyzed changes in the number of children and adolescents with complex chronic conditions (CCCs) and their medical expenditures between 2011 and 2021. Methods: We used National Health Insurance claims data of patients under the age of 20 from 2011 to 2021. CCCs were classified as pediatric CCCs by Feudtner et al. Patients who repeatedly utilize medical services due to CCCs were defined as patients who sought medical care twice or more within the past 3 years. Their sex, age, CCC categories, and the number of CCC categories were presented using descriptive analyses. Results: The number of children and adolescents with CCCs increased from 345,320 (3.04%) in 2011 to 451,238 (5.33%) in 2021. Patients who repeatedly utilized medical services due to CCCs increased by 71,889 during the same period. The proportion of patients with two or more CCC categories increased from 35.80% in 2011 to 43.67% in 2021. In 2021, patients with other congenital or genetic defect were most common. Medical expenditures associated with CCCs increased from 475,654 million won (12.34%) in 2011 to 768,467 million won (19.11%) in 2021. Conclusion: The number of patients with CCCs and their medical expenditures increased in Korea between 2011 and 2021. The results indicate that consistent national efforts are required to maintain the specialized pediatric care. Further studies are needed to identify pediatric patients who require specialized medical services through various methods.

        Methylation Status and Expression of E-cadherin in Oral Squamous Cell Carcinomas Compared to Benign Oral Epithelial Lesions

        Expression of invasion/metastasis suppressor, E-cadherin, is reduced in many types of human carcinomas. Although somatic and germline utations in the CDH1, which encodes the human E-cadherin, have frequently been reported in cases with diffuse gastric and lobular breast ancers, irreversible genetic inactivations are rare in other human carcinomas. Recently, it has been well documented that some genes in human cancers may be inactivated by altered CpG methylation. Herein, we determined the expression and methylation status of E-cadherin in oral squamous cell carcinoma (SCC) by immunohistochemistry and methylation-specific PCR. The expression of E-cadherin was significantly higher in the well-differentiated oral SCCs than the moderately or poorly differentiated ones. None of eight tested benign pithelial hyperplasias showed aberrant methylation, whereas five of 12 oral squamous cell carcinomas showed aberrant methylation. When we compared E-cadherin expression with methylation status, oral SCCs with normal methylation showed a higher expression of E-cadherin than those with methylation. These findings suggest that aberrant CpG methylation of CDH1 promoter region is closely associated with transcriptional inactivation and might be involved in tumor progression of the oral mucosa.
