        A Randomized Comparative Study of Unani Formulations in Abnormal Uterine Bleeding due to Endometrial Hyperplasia

        Objectives: To compare the efficacy of polyherbal Unani formulations in heavy menstrual bleeding due to endometrial hyperplasia. Methodology: A prospective, randomized comparative trial was conducted at Govt. Nizamia Tibbi College. Group A (n=20) received Itrifal Aftimoon 5g orally BID from menstruation day 3 to day 21 plus suprapubic Marham Dakhilyun application and per vaginally Marham Dakhilyun (5g) and Roghan Gul (10ml) application from menstruation day 5 to day 14. Group B (n=20) received Gulnar Farsi (2g), Phitakri Biryan (0.25g), Dammul Aqwain (0.25g), and Geru (2g), 2.5g powder orally BID, menstruation day 3 for 20 days plus Douche Bargh Sambhalu then Ḥamūl of Safuf Mazu (2g), Kalijiri (2g) and Roghan Gul (10ml) from menstruation day 3 to day 12 for 3 consecutive cycles. The primary outcome was pelvic ultrasound findings of endometrial thickness. The secondary outcome measures were improvement in haemoglobin percentage, change in menstrual flow and menstrual pattern. The level of significance was 5%. Results and conclusion: The intragroup comparison showed that the mean endometrial thickness at baseline and after treatment in groups A and B was extremely significantly different (P<0.0001). The intragroup comparison showed the mean haemoglobin percent at baseline and after treatment in group, A was significantly different (P<0.0001). After treatment, 50% and 60% of participants had normal duration and menstrual blood loss after treatment from baseline in Groups A and B respectively. However, further, phase II and III randomized standard controlled trials in larger samples are recommended to assess the efficacy of these group medicines.

        Case Report : Full term broad ligament pregnancy through a Cesarean scar

        A broad ligament pregnancy is an extremely rare condition. Diagnosis is often missed and finally made during laparotomy. We present a case which remained undiagnosed throughout her pregnancy till she reached term and unfortunately had intrauterine foetal demise when she reported to our hospital. On clinical suspicion, ultrasound and magnetic resonance imaging evaluation was done which picked up the diagnosis of abdominal pregnancy. On laparotomy a broad ligament pregnancy was found with a rent in the previous Cesarean scar towards the gestational sac in the broad ligament.

        Corporate Environmental Performance and Firm Value–Using Emission Metrics

        The study aims to examine and analyze the impact of corporate environmental performance (CEP) on firm value (FV) and the extent of disclosure of carbon emission by the sampled firms in India, where Corporate Social Responsibility (CSR) practices are mandatory. The analysis is based on secondary data collected from 40 Indian Carbonex-indexed firms from the period 2015–2019. The present study seeks to investigate the extent of disclosure by the firms and how this disclosure impacts the FV. The panel data regression model is employed to examine the impact of CEP on accounting and market-based firm value. This study uses accounting-based measure-ROA and market-based measure-Tobin’s Q proxies to measure firm value. CEP is captured by three emission metrics (TCO2 EQVE, TENUS, and EMC) extracted from the ESG Eikon database. The findings of the study show a positive link between CEP on FV. We also found that the data related to CSR/CEP disclosure of 34, out of 74 non-finance Carbonex companies are either not available or partially available, despite it being mandatory. Further, the study finds that, although the rest of the Carbonex firms are disclosing their environmental policy, some information pertaining to technology and resource efficiency is less disclosed.

      • An Energy Efficient Clustering Algorithm in Mobile Adhoc Network Using Ticket Id Based Clustering Manager

        Many emerging mobile ad-hoc network application communications are group-oriented. Multicast supports group-oriented applications efficiently, particularly in a mobile environment that has a limited bandwidth and limited power. Energy effectiveness along with safety are 2 key problem in MANET design. Within this paper, MANET is presented with a stable, energy-efficient clustering technique. In this proposed work advanced clustering in the networks with ticket ID cluster manager (TID-CMGR) has formed in MANET. The proposed routing scheme makes secure networking the shortest route possible. In this article, we propose a Cluster manager approach based on TICKET-ID to address energy consumption issues and reduce CH workload. TID-CMGR includes two mechanism including ticket ID controller, ticketing pool, route planning and other components. The CA (cluster agent) shall control and supervise the functions of nodes and inform to TID-CMGR. The CH conducts and transfers packets to the network nodes. As the CH energy level is depleted, CA elects the corresponding node with elevated energy values, and all new and old operations are simultaneously stored by CA at this time. A simulation trial for 20 to 100 nodes was performed to show the proposed scheme performance. The suggested approach is used to do experimental work using the NS- simulator. TIDCMGR is compared with TID BRM and PSO to calculate the utility of the work proposed. The assessment shows that the proposed TICKET-ID scheme achieves 90 percent more than other current systems.

        Pathophysiology and Grading of the Ventral Displacement of Dorsal Spinal Cord Spectrum

        Study Design: A retrospective study of the ventral displacement of dorsal spinal cord (VDDSC) spectrum pathophysiology and grading. Purpose: This study aimed at examining the pathophysiology of VDDSC between D3 and D7, using magnetic resonance imaging (MRI) correlation and severity grading. Overview of Literature: The pathologies that lead to VDDSC were previously discussed in various articles. We attempted to group these pathological conditions under a single spectrum, and grade them according to their severity. Methods: We reviewed the MRI images of the dorsal spines of 1,350 patients over a period of 4 years (February 2013–February 2017); all MRI images were analyzed by two experienced radiologists. Results: Of the 1,350 patients, 28 exhibited VDDSC between D3 and D7. Additional findings included ventral transdural herniation of the spinal cord (n=10), anterior spinal cord adhesion (n=7), arachnoid web (n=6), and arachnoid cyst (n=5). Conclusions: We grouped the pathologies that lead to VDDSC at the thoracic level into a single spectrum of varying severity and graded VDDSC, from mild to severe.

        Dorsal Spinal Ligamentum Flavum Thickening: A Magnetic Resonance Imaging Study

        Study Design: A retrospective radiological study of the ligamentum flavum (LF). Purpose: We determined the relationship of dorsal spinal LF thickening with age and sex using magnetic resonance imaging (MRI). We also determined whether LF thickening has a predominant tendency to occur at a specific dorsal level and on a specific side. Overview of Literature: Many researchers have studied LF thickness at dorsal levels in patients with compressive myelopathy. However, there is a dearth of literature pertaining to the study of dorsal LF thickness in patients without myelopathy. Methods: LF thickness was measured at dorsal levels from T1 to T12 on both sides using MRI in 100 individuals. The patients were divided into three groups based on age: 20 to 40, 41 to 60, and >60 years. On axial T2-weighted imaging at the mid-disc level, LF thickness was measured perpendicular to the lamina border, either at half the length of LF or at maximum thickness, whichever was greater. Results: We found that LF thickness does not increase significantly with increasing age and there was no significant disparity in LF thickness between the sides and sexes. We also found that there was a significant increase in LF thickness at the T10–T11 level (mean value, 3.27±0.94 mm). Conclusions: LF thickness does not appear to have any side/sex dominance. LF thickening has a predominant tendency to occur specifically at the T10–T11 level. This may be due to maximum tensile strength and mobility at this level. Because there is an increased tendency for LF thickening at the T10–T11 level, this may be used as a reference point for counting the vertebral levels.

        Formulation optimization of PVA/HPMC cryogel of Diltiazem HCl using 3-level factorial design and evaluation for ex vivo permeation

        The aim of the present research was to optimize cryogel formulation of Diltiazem hydrochloride (DZ) with poly(vinyl alcohol) (PVA) and hydroxypropyl methylcellulose (HPMC) by utilizing response surface methodology followed by ex vivo permeation study on the optimized gel containing penetration enhancers. A 3-level factorial design was employed to formulate the experimental runs and to evaluate the effect of two independent variables such as the concentration of PVA and HPMC on characteristics of cryogel such as bioadhesive strength (BS) and in vitro drug release (dependent variables). Response surface plots such as contour and 3D plots were generated by the Design Expert software to analyze the effect of independent variables on dependent variables. Fourier transform infrared spectroscopy studies confirmed the absence of interaction between DZ and polymers. Among various models generated by the software, quadratic model was found to be best fit for both the responses. Both the formulation factors influenced BS synergistically. However, the effect of HPMC concentration was more pronounced compared to concentration of PVA. But, an opposite effect shown by both the formulation factors on cumulative percentage of drug release (CPR) in 8 h. The optimized batch of cryogel of DZ selected by the software was having composition of 5 %of PVA and 2 %of HPMC. Penetration enhancers such as 1,8-cineole, d-limonene and carvone were incorporated into the optimized gel and permeation study was carried out using abdominal skin of rat. The study demonstrated a highest flux of 118 ± 5.81 μg/cm²/h in case of 1,8-cineole containing gel followed by carvone and d-limonene.

        Though, the existence of Corporate Governance issues are always there since the existence of business organization, it has gained a significant attention in last three decades. The prevalence of bankruptcy and financial scams of large companies worldwide like world.Com, Enron, Tyco International, HIH Insurance, Paramalat and Satyam and recent saga of Infosys Limited in our own country becameexamples of laxity of good governance.In the entire corporate governance mechanism, board structure is the most important factor.It is the body, which is responsible to approve strategic decision, which in turn decides the future of the organization.As suggested by the recent studies, one of the significant ways to enhance the quality of corporate governance practices and performance of the company, is to include women directors in the board of directors. In this study, efforts are made to investigate the significance of Board Gender Diversity and to investigate the relationship between firm performance and board diversity. It is an analytical study, where BSE Sensex data are used. The period is of ten years and total 30 company’s data are analyzed. Board Gender Diversity used as proxy variables for corporate governance practices and Return on Asset as a measure for performance of selected firms.
