Purpose: Lichen sclerosus (LS) in men is poorly understood. Though uncommon, it is often severe and leads to repeated surgical interventions and deterioration in quality of life. We highlight variability in disease presentation, diagnosis, and patient factors in male LS patients evaluated at a tertiary care center. Materials and Methods: We retrospectively reviewed charts of male patients presenting to our reconstructive urology clinic with clinical or pathologic diagnosis of LS between 2004 and 2014. Relevant clinical and demographic information was abstracted and descriptive statistics calculated. Subgroup comparisons were made based on body mass index (BMI), urethral stricture, and pathologic confirmation of disease. Results: We identified 94 patients with clinical diagnosis of LS. Seventy percent (70%) of patients in this cohort had BMI >30 kg/m2, and average age was 51.5 years. Lower BMI patients were more likely to suffer from urethral stricture disease compared to overweight counterparts (p=0.037). Patients presenting with stricture disease were more likely to be younger (p=0.003). Thirty percent (30%) of this cohort had a pathologic diagnosis of LS. Conclusions: Urethral stricture is the most common presentation for men with LS. Many patients endure skin scarring and have numerous comorbidities. Patient profile is diverse, raising the concern that not all patients with clinical diagnosis of LS are suffering from identical disease processes. The rate of pathologic confirmation at a tertiary care institution is alarmingly low. Our findings support a role for increased focus on pathologic confirmation and further delineation of the subtype of disease based on location and clinical manifestations.

The propagations of macro-scale cracks were visualized in Si3N4 and SiC ceramics using a mechano-luminescence (ML) of SrAl2O4:Eu,Dy. The bridging zones behind the crack tip were also clearly detected in the crack path within realistic time frames. The ML made it possible to detect cracks propagating within a speed range of 200 m/s to 250 m/s, thereby realizing so-called quasi-dynamic R-curves. The magnitudes and shapes of the bridging stress distributions were found to change with the advancing cracks, continuing to change as the applied load changes, even after the cessation of crack propagation. The effective toughening then commenced and the applied stress intensity factors increased dramatically up to ~120 MPa √m in Si3N4 and 150 MPa √m in SiC. The KTip values expected on the basis of the instantaneous bridging stress distributions obtained from ML observations deviated greatly from those measured using the conventional crack tip lengths; rather, the values support the results obtained by using bridging tips in quasi-dynamic crack propagations.
Capture of Volatile Organic Iodine Species Using Mordenites

The emission of off-gas streams from used fuel recycling is a concern in nuclear energy usage as they contain radioactive compounds, such as, 3H, 14C, 85Kr, 131I, and 129I that can be harmful to human health and environment. Radioactive iodine, 129I, is particularly troublesome as it has a half-life of more than 15 million years and is prone to accumulate in human thyroid glands. Organic iodides are hazardous even at very low concentrations, and hence the capture of 129I is extremely important. Dynamic adsorption experiments were conducted to determine the efficiency of sodium mordenite, partially exchanged silver mordenite, and fully exchanged silver mordenite for the removal of methyl iodide present at parts per billion concentrations in a simulated off-gas stream. Kinetic analysis of the system was conducted incorporating the effects of diffusion and mass transfer. The possible reaction mechanism is postulated and the order of the reaction and the values of the rate constants were determined from the experimental data. Adsorbent characterization is performed to investigate the nature of the adsorbent before and after iodine loading. This paper will offer a comprehensive understanding of the methyl iodide behavior when in contact with the mordenites.

Despite the increased frequency of analyst and management forecasts during earnings announcements, empirical evidence of their effects on investor responses to the earnings announcements is limited. We examine the implications of reinforcing and contradicting analyst forecasts issued during earnings announcements (days 0 and +1) on the market response to unexpected earnings. We classify forecasts as reinforcing when unexpected earnings are positive (negative) and concurrent analyst forecasts are higher (lower) than current year earnings, and as contradicting when unexpected earnings are positive (negative) and concurrent analyst forecasts are lower (higher) than current year earnings. We document greater earnings response coefficients for announcements accompanied by reinforcing analyst forecasts. Similarly, we find greater market responses to earnings announcements when management forecasts are reinforcing. These findings suggest that analyst forecasts at the time of earnings announcement provide incremental information regarding future earnings and that analyst and management forecasts convey complementary information about earnings announcements.

Population pharmacokinetics for gentamicin were compared with 24 American patients (16 male and 8 female) and 16 Korean-American appendicitis patients(12 male and 4 female). Two to six blood specimens were collected from all patients at the following times: just before a regularly scheduled infusion and at 1/2 hour after the end of a 1/2 hour infusion. Nonparametric expected maximum(NPEM) algorithm for population modeling was used. The estimated parameters were the elimination rate constants(K), the slope of the relationship between K versus creatinine clearance(KS), the apparent volume of distribution(V), the slope of the relationship between V versus weight(VS), gentamicin clearance(CL) and the slope of the relationship between CL versus creatinine clearance and the VS(CS). The output includes a 3-dimensional plot of the joint probability density function(PDF), two marginal PDF, means, medians, modes, variance, skewness, kurtosis, and CV%. The mean K(KS) were 0.424 +/- 0.139(0.00429 +/- 0.00164) and 0.411 +/- 0.135 hr-1 (0.00475 +/- 0.00180[hr.mL/min/1.73 m2]-1) for American and Korean-American populations, respectively. The mean V(VS) were not different at 15.6 +/- 4.77(0.233 +/- 0.0526) and 15.1 +/- 3.84L(O.239 +/- 0.0492L/kg) for American and Korean-American populations, respectively (P>0.2). The mean CL(CS) were 6.28 +/- 1.85(0.0634 +/- 0.0191) and 5.70 +/- 1.77L/hr(0.0701 +/- 0.0215L/kg[hr.mL/min/1.73m2]) for American and Korean-American populations, respectively. There are no differences in gentamicin pharmacokinetics between American and Korean-American appendicitis patients.
