        위절제술 환자의 표준진료지침 개발 및 적용 효과

        Background : Gastric cancer is the most common malignant tumor in Korea. surgical operation is one of the major treatment modalities for gastric cancer patients. Therefore, gastrectomy is one of the most common procedures in General Surgery. There were variation in length of hospital stay and medical treatment for gastrectomy between three surgeons at Asan Medical Center. Clinical pathways have received considerable attention as a tool for recucing the medical practice variation, increasing the efficiency of care process, and improving the quality of care. The aim of this study was to evaluate the effect of a clinical pathway for gastrectomy in gastric cancer patients. Methods : The clinical pathway for gastrectomy was developed and implemented by a multidisciplinary group in Asan Medical Center. A computerized clinical pathway program was developed and revised after a pilot test. A total of 145 patients underwent gastrectomy by three surgeons at Asan Medical Center. We compared the length of hospital stay, patient satisfaction, and unplanned readmission rate between the pre-pathway group(n=67) and the post-pathway group(n=78). We also investigated the degree of satisfaction among the physicians and nurses who were main end-users of the clinical pathway. Results : The clinical pathway was applied to all target patients. The average length of hospital stay was shortened from 12.7days to 10.6days(p<0.01). The degree of patient satisfaction with the care process changed from 90.3% to 89.2% after the implementation of the clinical pathway, but the difference was of satistically significant(p=0.761). Unplanned readmission rate was 2.9% in the pre-pathway group. More than 90% of physicians and nurses answered that the clinical pathway had been a useful tool in their medical practice. Conclusions : The findings of the study demonstrated that implementation of the clinical pathway for gastrectomy produced substantial reduction in the length of hospital stay while improving the quality of patient outcomes. The computerized clinical pathway program can be used as one of the powerful patient management tools for reducing the practice variations and increasing the efficiency of care process in Korea hospital settings.

      • 작동음의 분석을 이용한 펌프의 고장진단

        We should maintain the maximum operation capacity for production facilities and find properly out the fault of each equipment rapidly in order to decrease a loss caused by its failure. The acoustic signals of a machine always carry the dynamic information of the machine. These signals are very useful for the feature extraction and fault diagnosis. We performed a fundamental study which develops a system of fault diagnosis for a pump. We obtained noises by a microphone, analysed and compared the signals converted to frequency range for normal products, artificially deformed products. We tried to search a change of noise signals according to machine malfunctions and analyse the type of deformation or failure. The results showed that acoustic signals as well as vibration signals can be used as a simple method for a detection of machine malfunction or fault diagnosis.

        의복 생산시 마킹(Marking)공정에 관한 연구

        The purpose of this study is to help efficient use of marking system and more competitive products manufacture through the actual investigation into manufacturing process of apparel products. For lack of the previous study on marking, fundamental survey was performed on systematic research for pattern marking in ready-made production line Data were collected from 36 companies using Apparel CAD/CAM through questionnaire and interview. The data were analyzed by using Frequency and Crosstabs utilizing SPSS. The results were as follows: 1. Marking situation in clothing companies were examined; 24 companies(66.7%) use computer marking, and 12 compainies(33.3%) have both computer marking and hand marking. Besides only 9 companies out of 36(25.0%) have CAM in cutting process. 2. The efficiency in marking methods was also studied and the expense-saved ways were presented. we have found there is no difference in a way of the textural efficiency but the working methods of hand marking and computer marking. 3. Merits and demerits in marking methods were investigated. In case of marking two men's wear(Jacket and Pants), it takes 38 minutes by hand whereas it takes 15 minutes by computer; for two women's wear(Jacket and Skirt) 49 minutes by hand, and 16 minutes by computer. 4. Most of the markers have less than 5 years experiences and among them most women are less experienced. They are in the mid-twenties after college graduations.

      • Protoberberine 입체이성에 관한 연구

        We have synthesized protoberberine derivatives from tetra- hydroberberinium perchlorate. On quaternization of a several protoberberine derivativese with methyl iodide, the formation of the cis methiodides the corresponding trans methiodides increased according to the order of the bulkiness of the 14-substituent as H>CH₃>CN. The N-methyl signal of the cis 14-substituted quinolisidine methiodides was conformed to appear at higher field than those of the corresponding trans methiodides except for the 14-cyano methiodides in their NMR specta.

        한국성인 남자의 하체 동작범위 연구 : 3D 동작분석 장치를 이용하여

        The purpose of this study: 1.The total 24 range measurements of active dynamic motion of 40 subjects(20's and 30's) were measured using 3-D dynamic motion analysis system. 2.Various comparisons were performed for the right and left side, male, age groups(20's, 30's, and 40's ∼60's) using previous studies. The results were compared with the other studies in the aspects of age. In this study, The 3-D motion analysis system consists of VICON 140, data acquisition system, and data analysis program called by KRISSMAS. VICON 140 is dynamic motion analyzer, and KRISSMAS is a program developed for analysis of the data captured from the VICON. The results of this study were as follows: 1.Comparing 20's∼30's and 40's∼60's the result shows that 40's∼60's have smaller ROM at the most joints(Thoratic and Lumber, Hip Joint, Knee Joint, Ankle Joint), which is inconsistent with the previous result. The reason is that most investigations were carried out using traditional measurement equipments, which have encountered difficulty in quantifying the exact motion of the body. 2.There are no significant mean difference between the right and the left side Most of the significances were not high(>0.05).

        많은 기업들이 고객, 공급업체, 경쟁업체 등과의 제휴를 확장하고 있다. 이들은 제품의 부가가치 증대, 원가감축, 고객서비스 증대, 시장접근 확장, 기술력 확장 등을 제휴의 근본적인 원인으로 제시하고 있다(Treacy, et al., 1992). 더불어 정보기술은 과거 조직 내의 컴퓨팅에서 조직과 조직을 연계시키는 컴퓨팅으로 발전되었다. 특히 인터넷의 성장, 개방시스템, 표준화에 따르는 정보기술은 조직과 조직의 연계를 과거 어느 때 보다도 더 수월하게 이룩할 수 있다. 과거에도 제휴는 중요했지만 이제는 가상기업을 표방하는 기업들이 증가함에 따라 다음과 같은 이유로 정보기술을 이용한 제휴는 더욱 중요해지고 있다. 첫째, 제휴를 성공적으로 이끌기 위해서는 업무방식의 통합이 필요하며 이를 정보기술로 구현하는 것이 중요하게 되었다. 시스코 시스템즈(Cisco Systems)는 공급업체와의 업무방식 통합을 통해 고객의 수요에 신속하고 정확하게 대응할 수 있었다. 즉, 공급업체들은 시스코의 정보시스템을 통해 고객의 주문을 확인하고 생산기획을 작성하고 있다. 또한, 공급업체들은 정보기술을 이용한 시스코의 품질관리 시스템을 사용함으로써 시스코 종업원의 감독 없이도 시스코가 추구하는 품질을 만족시킬 수 있었다. 둘째, 정보기술을 위한 제휴가 증가하고 있다. 최근 들어, 동선업체, 케이블 텔레비전 운영회사, 멀티 미디어 제조업체, 컨덴츠(Contents)제작업체, 컴퓨터 벤더 등의 제휴가 증가하고 있다. 최첨단 하이테크 업체들에서는 빠른 상품개발이 중요하며, 이를 위해서는 정보통신 인프라가 신속하게 구축되어야 한다. 단독으로 정보기술의 컨텐츠, 소프트웨어, 하드웨어 등을 구현하기 어렵기 때문에 제휴가 필요하게 되었다. 셋째, 아웃소싱과 가상기업을 표방하는 기업이 증가함에 따라 제휴는 업무방식의 일환으로 자리잡게 되었다. 게이트웨이는 유통을 신속하고 정확하게 하기 위해 택배업체인 UPS와 제휴해서 고객에게 컴퓨터를 배달하고 있다. 유통업무는 게이트웨이의 핵심업무가 아니기 때문에 유통업무를 더 탁월하게 수행하는 UPS에게 아웃소싱을 준 것이다. 고객은 UPS가 배달하는 것인가, 게이트웨이가 배달하는 것인가에는 관심이 없으며, 약속된 시간에 배달되는가에만 관심이 있으므로, 게이트웨이가 반드시 유통업무를 담당해야 하는 이유가 없다. Exponential Growth of Internet, open systems, standardized information technology has made strategic alliance more easily. Strategic alliance has been always important. However, as many corporations adopt virtual corporations as an emerging strategy, using information technology for strategic alliance becomes more important than ever. In past strategic alliance was deployed from the perspective of efficiency. But strategic alliance should be deployed from the perspective of the customer. That is, strategic alliance should be leveraged to improve the performance of the customer. In addition, information technology can be employed to automate transactions, to trigger other transactions, to integrate processes, and to establish virtual corporations.

      • 펌프의 작동음 주파수 분석에 의한 진단

        A fundamental study for developing a system of fault diagnosis of a pump is performed by using neural network. The acoustic signals were obtained and converted to frequency domain for normal products and artificially deformed products. The signals were obtained in various driving frequencies in order to obtain many types of data from a limited number of pumps. The acoustic data in frequency domain were managed to multiples of real driving frequency with the aim of easy comparison. The neural network model used in this study was 3-layer type composed of input, hidden, and output layer. The normalized amplitudes at the multiples of real driving frequency were chosen as units of input layer. Various sets of teach signals made from original data by eliminating some random cases were used in the training. The average errors were approximately proportional to the number of untaught data. The results showed neural network trained by acoustic signals can be used as a simple method for a detection of machine malfunction or fault diagnosis.
